Governmental programmes should be developed to collect and analyse data on healthcare associated infections (HAIs). This study describes the healthcare setting and both the implementation and preliminary results of the Programme for Surveillance of Healthcare Associated Infections in the State of Sao Paulo (PSHAISP), Brazil, from 2004 to 2006. Characterisation of the healthcare settings was carried out using a national database. The PSHAISP was implemented using components for acute care hospitals (ACH) or long term care facilities (LTCF). The components for surveillance in ACHs were surgical unit, intensive care unit and high risk nursery. The infections included in the surveillance were surgical site infection in clean surgery, pneumonia, urinary tract infection and device-associated bloodstream infections. Regarding the LTCF component, pneumonia, scabies and gastroenteritis in all inpatients were reported. In the first year of the programme there were 457 participating healthcare settings, representing 51.1% of the hospitals registered in the national database. Data obtained in this study are the initial results and have already been used for education in both surveillance and the prevention of HAI. The results of the PSHAISP show that it is feasible to collect data from a large number of hospitals. Healthcare in developing countries is affected by severe poverty, political instability and diseases that may be of lesser importance in industrialized countries. The aim of this paper was to present two cases and histories of physicians working in hospitals in developing countries and to discuss the opportunities for clinical investigation and collaboration. Cases of patients in Phnom Penh, Cambodia, with histoplasmosis, cryptococcal meningitis, crusted scabies, cerebral lesions and human immunodeficiency virus and of patients in Kabul, Afghanistan, with liver cirrhosis, nephrotic syndrome and facial ulcer are discussed. Greater developmental support is required from industrialized nations, and mutually beneficial cooperation is possible since similar clinical problems exist on both sides (e.g. opportunistic cardiovascular infections). Examples for possible support of hospital medicine include physician interchange visits with defined objectives (e.g. infection control or echocardiography training) and collaboration with clinical investigations and projects developed locally (e.g. epidemiology of cardiovascular diseases or nosocomial bloodborne infections).

INTRODUCTION: This study was developed to evaluate the situation of leprosy in the general population of the municipality of Buriticupu, State of Maranhão, Brazil. METHODS: We used the method of active search to identify new cases from 2008 to 2010. Bacilloscopy of intradermal scrapings was performed in all patients with skin lesions compatible with leprosy, and histopathological examination in those who had doubts on the definition of the clinical form. RESULTS: The study included 19,104 individuals, with 42 patients diagnosed with leprosy after clinical examination, representing a detection rate of 219.84 per 100,000 inhabitants. The predominant clinical presentation was tuberculoid with 24 (57.1%) cases, followed by borderline with 11, indeterminate with four, and lepromatous with three cases. The study also allowed the identification of 81 patients with a history of leprosy and other skin diseases, such as pityriasis versicolor, dermatophytosis, scabies, vitiligo, and skin carcinoma. The binomial test showed that the proportion of cases in the headquarters was significantly higher than that in the villages (p = 0.04), and the generalized exact test showed that there was no association between age and clinical form (p = 0.438) and between age and gender (p = 0.083). CONCLUSIONS: The elevated detection rate defines the city as hyperendemic for leprosy; the active search for cases, as well as the organization of health services, is an important method for disease control.

The writers report results on the application of four fumigants (D. D., E. D. B.-40, C. B. P. and Vapam) for control of root-knot and meadow nematodes attacking potato in beds filled with soil artificially inoculated. The data obtained were as follows: a) as reported by previous authors, potato is sensitive to C.B.P., the toxical effects of which disapearing only about 6 and half months after application. On the other hand, C.B.P. proved to have a significative residual nematicidal value, protecting the seeds from root-knot nematodes for a period of two years; b) D. D., E. D. B., and Vapam were effective for controling root-knot but with no residual value, having to be used prior to each planting; c) at the rates used, no nematicide was effective to control meadow nematodes; d) in the conditions of the experiments, all nematicides incited attacks bv Streptomyces scabies. Actually, in some cases scab did not affect any tuber from the check while the entire production from the treated beds was heavily desfigured. The writers assume that as the nematicides killed protozoa and too many bacteria-eating nematodes, they destroyed the biological equilibrium existing in the soil, thus allowing the S. scabies population to reach a high level.

A cross-sectional survey of 19 dairy sheep farms in Argentina was carried out with the purpose to know farm management, health practices, and occurrence and mortality of diseases. The survey comprised 40% of all sheep milking farms in Argentina. A questionnaire was conducted by way of personal interviews with sheep owners during farm visits. The proportions of farms reporting routine vaccination for clostridial diseases, contagious ecthyma, pneumonia and mineral and vitamin parenteral administration were 63%, 47.3%, 16.6% and 42.1% respectively. Regular treatment against lice was used in 37.5% of the farms, and 89.5% o the farmers treated against gastrointestinal nematodes (GIN). The mean number of GIN drenches per farm was 2.26±1.78 annually. In 68.4% of the milking flocks the California Mastitis Test was regularly done and 55.6% of the farmer managers had sampled their flocks once a year for Brucella ovis antibodies. During the pre-mating period respectively 68.4% and 50% of farmers clinically examined their rams and ewe for general health and teeth condition. The udders of ewes were frequently inspected at the start of each milking period. The most important parasite problems noticed were GIN (reported by 57.9% of farmers), lice (57.9%) and scabies (10.5%) and the most frequent infectious diseases were ecthyma (73.7%), pneumonia and other respiratory problems (57.9%), clinical mastitis (55.6%), clostridial diseases (36.9%) and foot lameness (35.2%). Photosensitivity (47.4%) and ruminal acidosis (42.1%) were reported as other frequent toxic or metabolic disorders. Owners mentioned that the mean lifespan or milk productive time per ewe was 4.5±1.4 years. Perinatal lamb mortality was 8.5% and the total flock mortality rates, above the first 24 h of life was 6.9%. The high rates of lamb mortality during the pre-weaning (10.3%) and post-weaning (5.9%) periods indicate that this problem, as well as the most prevalent diseases, should be the subject of further studies.

Les pyréthrinoïdes et les pyréthrines sont des insecticides neurotoxiques auxquels on attribue également des effets néfastes sur les systèmes immunitaire, hormonal et reproducteur. Ils sont abondamment utilisés en agriculture, mais aussi en horticulture, en extermination et dans le traitement d’infestations parasitaires humaines et animales (gale, poux). Il y a donc un intérêt en santé environnementale à connaître l’ampleur de l’exposition humaine à ces pesticides. La mesure des métabolites urinaires des pyréthrinoïdes et des pyréthrines apparaît une approche de choix pour arriver à cette fin puisqu’elle permet, en théorie, d’obtenir un portrait global de l’exposition. Or,traditionnellement et par soucis de simplicité les concentrations volumiques ou ajustées pour la créatinine) de ces biomarqueurs dans des urines ponctuelles sont déterminées, mais l’effet de l’utilisation de ces unités sur la validité des estimations de dose quotidienne absorbée n’a jamais été vérifié. L’objectif général de cette thèse était donc de développer, appliquer et valider une nouvelle stratégie de mesure et d’analyse de biomarqueurs pour améliorer la précision et la fiabilité des évaluations de l’exposition individuelles et populationnelles aux pyréthrinoïdes et pyréthrines. Les objectifs spécifiques étaient : i) de caractériser l’exposition humaine à ces contaminants en région urbaine et rurale au Québec et ii) de comparer la validité de la nouvelle stratégie de mesure et d’analyse de biomarqueurs urinaires proposée avec les autres méthodes plus usuelles utilisées pour estimer l’exposition individuelle et populationnelle et les doses absorbées de pyréthrinoïdes. Des adultes et des enfants, recrutés dans la population de l’Île de Montréal et de la Montérégie ont recueilli leurs urines pendant une période d’au moins douze heures et complété un questionnaire documentant les sources potentielles d’exposition. Les quantités de métabolites de pyréthrinoïdes et pyréthrines (pmol) mesurées dans les urines ont été ajustées pour une période de douze heures exactement et pour le poids corporel. Les quantités excrétées en région urbaine ont été comparées à celles excrétées en région rurale et les données individuelles et populationnelles ont été comparées à celles qui auraient été obtenues si des concentrations volumiques ou ajustées pour la créatinine avaient été mesurées. Les résultats montrent que l’exposition à ces pesticides est ubiquiste puisque plus de 90% des participants excrétaient les principaux métabolites de pyréthrinoïdes et pyréthrines à un niveau supérieur au seuil de détection analytique. Les résultats suggèrent que l’alimentation pourrait être à l’origine de ce niveau de base puisque les autres sources d’exposition connues n’ont été que rarement rapportées. Au Québec, l’exposition en milieu rural apparaissait légèrement plus importante qu’en milieu urbain et certains facteurs d’exposition, comme l’utilisation de pesticides domestiques, ont été rapportés plus fréquemment en milieu rural. Enfin, il a été démontré que la mesure des concentrations volumiques ou ajustées pour la créatinine de biomarqueurs est une approche qui peut entraîner des biais importants (jusqu’à 500% d’erreur et plus) en particulier lors de l’évaluation de l’exposition individuelle. Il est évident que les autorités de santé publique et de santé environnementale employant des biomarqueurs urinaires afin d’évaluer l’exposition aux pyréthrinoïdes et aux pyréthrines (ainsi qu’à d’autres molécules ayant des demi-vies d’élimination similaire)devraient diriger leurs efforts vers la mesure des quantités excrétées pendant une période d’au moins douze heures pour obtenir un portrait le plus valide possible de l’exposition. Il serait aussi souhaitable de mieux documenter la toxicocinétique de ces molécules chez l’humain afin d’établir avec une plus grande confiance la relation entre les quantités excrétées de biomarqueurs et les doses absorbées de ces contaminants.

Recent studies have demonstrated that essential oils, and in particular, pennyroyal, tea tree and anise, have potent insecticidal and acaricidal (mite-killing) activity. The individual components of essential oils are now being investigated in order to give a rational basis to discover which essential oils may prove to be the most effective all-round agents for killing headlice and their eggs, and treating scabies, and for eliminating house dust mites, a major cause of asthma.

Irrigation is a major husbandry tool, vital for world food production and security. The purpose of this review is twofold:- firstly drawing attention to the beneficial and deleterious aspects of irrigation resulting from interactions with the microbial world; secondly, forming a basis for encouraging further research and development. Irrigation is for example, a valuable component in the control of some soil borne pathogens such as Streptomyces scabies, the cause of potato common scab and Fusarium cubense, a cause of banana wilt. By contrast, applying irrigation encourages some foliar pathogens and factors such as splash dispersal of propagules and the retention of leaf wetness are important elements in the successful establishment of disease foci. Irrigation applied at low levels in the canopy directly towards the stem bases and root zones of plants also provides means encouraging disease development. Irrigation also offers means for the direct spread of microbes such as water borne moulds, Oomycetes, and plasmodial pathogens coming from populations present in the water supply. The presence of plant disease causing microbes in sources of irrigation has been associated with outbreaks of diseases such as clubroot (Plasmodiophora brassicae). Irrigation can be utilised as a means for applying agrochemicals, fungigation. The developing technologies of water restriction and root zone drying also have an impact on the success of disease causing organisms. This is an emerging technology and its interactions with benign and pathogenic microbes require consideration.

Behind the high walls of a prison, there is suffering! Suffering from those who atone to their punishment, from those who work on behalf of an alleged regeneration, suffering that has remained for over two centuries. For those who had their conducts characterized as a crime, many studies have been developed. On the other hand, little is known about the prison servants. This research focused mainly on the identification, from the perspective of the correctional officers from two prisons in different states of Brazil, of the negative influence that their jobs have on them, on the factors that serve as protection, as well as the ones that offer them risk. It was observed that hygiene and security conditions are precarious in both prisons. From the reports, scabies, tuberculosis, hepatitis, H1N1, constant violence, politicization of the working environment, doubled working hours, lack of career planning and salaries emerge as evidence of the place where the bodies who work there are subdued, not taking the differences into account. From the above, and as of the many things that still need to be said about this category, it is reiterated the importance in developing studies about the conditions and the work organization and its implications on the mental health of those workers, who lack of public policies committed to their working realities

The purpose of this study was to collect data on skin diseases in dogs evaluated at the Veterinary Hospital of the Northern Parana State University (UENP), in Bandeirantes, Parana, Brazil. Dermatological conditions amounted to 31.38% of all consultations in dogs, and the most common skin conditions observed were, from the most to the least common: bacterial, parasitic, fungal, immune, and keratinization disorders; general skin diseases and psychogenic conditions. The highest scoring diseases, in number of occurrences were, in descending order: Superficial folliculitis, demodicidosis, dermatophytosis, bacterial otitis, deep folliculitis, dry dekeratinization, allergic dermatitis caused by flea bites, fungal ear infections, scabies, impetigo, malassezia dermatitis, parasitical otitis, pad dermatitis, and atopic dermatitis.

Neste trabalho é apresentado um estudo das perdas de água no sistema de abastecimento e nas residências do Conjunto Benjamim Sodré. O trabalho foi dividido em duas etapas, sendo a primeira experimental e a segunda de resultados. Posteriormente os dados foram tratados e avaliados e os principais resultados foram: a caracterização e avaliação do cadastro de consumidores, o volume faturado e medido das subcategorias do Conjunto, bem como, as vazões de todo o Sistema de Abastecimento de Água (SAA). Além disso, foram determinados alguns indicadores básicos de desempenho, como índice de perda na distribuição (70,04%), índice de perda no faturamento (70,61%), índice linear bruto de perda (157,96 L/m.dia), índice de perda por ligação (1.446 L/ligação.dia), índice de hidrometração (72,27%) e o Índice Global de Perdas (70,93%). Além disso, foi possível ter o conhecimento do nível sócio-econômico de 30% população residente, das subcategorias R2 e R3 no Conjunto (176 imóveis), onde os resultados mostraram que cerca de 94% dos entrevistados que pagam somente uma taxa para consumo afirmaram praticar desperdícios de água em suas residências. Procurou-se analisar a qualidade da água fornecida a população e cerca de 71,50% dos entrevistados considerou de má qualidade, o que de certa forma pode estar contribuindo com ocorrências de doenças de veiculação hídrica, pois nos últimos anos foi verificado que 24% da população tiveram problemas como diarréias, verminoses, hepatite A e escabiose. Além disso, foi feita uma avaliação do uso da água nas diversas atividades domésticas das subcategorias R2 e R3. Há uma perda de R$ 426,38 ao mês na conta de 15 consumidores que alegaram ter vazamentos na residência. Foi mostrado que para a atividade de lavar louça o consumidor teria uma redução no valor da conta de água de 89,28% em média, para tomar banho 43,66%, lavar roupa na máquina 34,23%, para lavar o pátio ou a calçada 84,80%, e para escovar os dentes 17,06%, referentes as subcategorias R2 e R3.
