Au Québec, les tentatives de suicide chez les adolescents prennent une ampleur considérable dans le réseau public de la santé et un nombre élevé de ces adolescents suicidaires sont épris d’un trouble de personnalité limite (TPL). Ces adolescents TPL présentent de nombreuses crises associées aux conduites suicidaires et aux comportements d’automutilation les amenant à consulter des professionnels de la santé de manière récurrente due, entre autres, aux abandons prématurés de la thérapie. Considérant cette problématique préoccupante, ce mémoire s’est intéressé aux facteurs impliqués dans la complexité de l’intervention auprès des adolescents atteints d’un TPL selon la perception des thérapeutes. Ce projet exploratoire de type qualitatif a été effectué sous forme d’entrevues semi-structurées auprès de onze thérapeutes de troisième ligne en pédopsychiatrie. Dans ce contexte de soins psychiatrique au Québec, les résultats de ce projet confirme la complexité de l’intervention auprès de ces jeunes et font émerger plusieurs facteurs pouvant expliquer les difficultés liées au processus d’intervention en lien, entre autres, à la symptomatologie, au risque suicidaire élevé, au risque d’abandons prématurés, à leur grande instabilité émotionnelle, relationnelle et comportementale influençant, par le fait même, le thérapeute dans son savoir-faire et son savoir-être.

Recension des écrits. L’autisme est un désordre neurodévelopemental qui peut présenter plusieurs défis pour le dentiste traitant en cabinet. Il n’existe aucune donnée publiée sur l’accessibilité et la dispensation de soins dentaires pour les enfants autistes dans le régime actuel des soins de santé au Québec. Matériels et Méthodes. Un questionnaire composé pour les besoins de l’étude et approuvé par le Comité d’éthique à la recherche du CHU Sainte-Justine a été distribué à des parents d’enfants visitant des cliniques externes du CHU Sainte-Justine. Les enfants devaient avoir entre 18 mois et 18 ans, avoir un grade ASA inférieur ou égal à II et, pour le groupe cas, avoir été diagnostiqué autiste ou TED par un professionnel compétent. Résultats. Vingt-sept parents de patients autistes et 37 parents de patients en bonne santé ont été sondé. Quarante-quatre pourcent du groupe cas rapportait qu’il était difficile de trouver un dentiste pour leur enfant contre 5% du groupe témoin. Seulement 35% des patients autistes sont suivis par un dentiste généraliste contre 79% des patients en bonne santé. Soixante-dix pourcent des parents du groupe cas doivent brosser les dents de leur enfant autiste et 83% considèrent la tâche difficile par un manque de coopération de l’enfant. Conclusion. Il est significativement plus difficile de trouver un dentiste pour un enfant autiste au Québec que pour un enfant en bonne santé. Pour cette population, il faudrait faciliter l’accès à un dentiste dans notre province. De plus, il faut épauler les parents dans la délivrance des mesures préventives.

La présente étude vise à examiner d’une part le lien entre les pratiques éducatives rapportées par la mère et celles perçues par son adolescent ayant un trouble déficitaire de l’attention avec ou sans hyperactivité (TDAH) et examiner le lien avec l’adoption de comportements à risque chez l’adolescent (délinquance et association aux pairs déviants). Cinq pratiques éducatives ont été étudiées de manière distincte, soit l’engagement parental, les pratiques positives, la discipline inconstante, la faible supervision ainsi que la punition corporelle. Chacune d’entre elles a été mesurée par la mère, son adolescent et une variable de désaccord a ensuite été créée. L’échantillon est composé de 32 dyades mère-adolescent ayant participé cinq ans auparavant au programme Ces Années Incroyables. Les adolescents ont tous un diagnostic de TDAH et sont âgés entre 10 ans et 15 ans. Les résultats démontrent une absence de corrélation entre chacune des pratiques éducatives rapportées par la mère et celles perçues par l’adolescent. La mère tend à avoir une perception plus positive des pratiques éducatives que son adolescent. Les résultats démontrent aussi que la perception de l’adolescent sur les pratiques éducatives est associée à la compréhension des comportements à risque. Finalement, la perception de l’adolescent s’est avérée être un facteur aggravant de la relation entre l’ampleur du désaccord mère adolescent et l’association aux pairs déviants. La discussion explique ces résultats et aborde les importantes implications cliniques.

Ce mémoire dresse un portrait des pratiques concernant le programme d’hébergement avec encadrement intensif en centre jeunesse au moment de l’étude. Depuis l’ajout, lors de la réforme en 2007, de l’article 11.1.1 dans la Loi sur la protection de la jeunesse, les motifs pour recourir à l’encadrement intensif pour les jeunes pris en charge pour leur protection sont désormais balisés. Cette réforme résulte de controverses à savoir que cette mesure ne respectait pas les droits des jeunes et brimait leur liberté. L’encadrement intensif était perçu par les juristes comme étant illégal et sans fondement juridique. Afin de connaître les effets de la réforme dans les pratiques des intervenants, des entretiens qualitatifs ont été effectués avec vingt-cinq employés des centres jeunesse de Montréal-Institut universitaire et de Laval. Malgré certaines difficultés d’implantation ainsi que certains signes de résistance au changement, les résultats laissent entendre que cette réforme permet d’accroître le respect des droits des jeunes. Du côté des conditions d’hébergement, les changements majeurs se trouvent dans la lourdeur du profil de la clientèle desservie, et la surpopulation des unités d’encadrement intensif. La mixité de la clientèle sur la réadaptation des jeunes est discutée et analysée en fonction du concept de l'inertie socioculturelle et celui du respect des droits de l’enfant.

À la suite d’un crime, les victimes vont ressentir différents besoins et celui d’être informé serait fondamental (Baril, 1984). Les recherches ont permis d’établir que les policiers ont un rôle important à jouer dans la transmission des informations aux victimes puisqu’ils sont les premiers acteurs du système pénal avec lesquels celles-ci sont en contact (Laxminarayan, 2013). De plus, la perception des victimes quant à la façon dont elles ont été traitées par les policiers peut avoir un impact significatif sur leur rétablissement psychologique. Cette étude a pour but de mesurer l’effet thérapeutique des interactions entre les victimes et les policiers ainsi que de la transmission des informations aux victimes à la suite d’un crime sur l’état de stress post-traumatique (ÉSPT). Pour ce faire, l’instrument qui a été administré aux participants (n=188) est l’Échelle modifiée des symptômes du trouble de stress post-traumatique. Cet instrument est une version adaptée et validée en français (Guay, Marchand, Iucci et Martin, 2002) de l’échelle MPSS-SR élaborée par Falsetti, Resnick, Resick et Kilpatrick (1993). Cette échelle permet d’évaluer la présence des symptômes de stress post-traumatique en terme de fréquence et de sévérité. Les résultats de l’étude montrent que l’information est un déterminant important d’un traitement juste et la qualité de ce traitement à un effet thérapeutique sur les symptômes de stress post-traumatique. Une discussion des résultats dans une perspective plus globale concernant les théories en victimologie sur la perception de justice, la problématique de la victimisation secondaire et les implications pour les politiques en matière de droits des victimes sera présentée à la fin de cette recherche.

Autistic spectrum disorder (ASD) is characterised by qualitative alterations in reciprocal social interactions. Some recent studies show alterations in gaze patterns during social perception and rest-functional abnormalities in the ‘social brain network’. This study investigated: i) social perception gaze patterns in children with ASD and controls, ii) the relationship between autism clinical severity and social perception gaze patterns, iii) the relationship between resting cerebral blood flow (rCBF) and social perception gaze patterns. Methods: Nine children with ASD and 9 children with typical development were studied. Eye-tracking was used to detect gaze patterns during presentation of stimuli depicting social scenes. Autism clinical severity was established using the Autism Diagnostic Interview Revised (ADI-R). Arterial spin labelling MRI was used to quantify rCBF. Results: The ASD group looked less at social regions and more at non-social regions than controls. No significant correlation was found between ASD clinical severity and social perception gaze patterns. In the ASD group, gaze behaviour was related to rCBF in the temporal lobe regions at trend level. Positive correlations were found between temporal rCBF and gaze to the face region, while negative correlations were found between temporal rCBF and gaze to non-social regions. Conclusions: These preliminary results suggest that social perception gaze patterns are altered in children with ASD, and could be related to temporal rCBF.
Abnormal anterior cingulum integrity in bipolar disorder determined through diffusion tensor imaging

Background Convergent evidence implicates white matter abnormalities in bipolar disorder. The cingulum is an important candidate structure for study in bipolar disorder as it provides substantial white matter connections within the corticolimbic neural system that subserves emotional regulation involved in the disorder. Aims To test the hypothesis that bipolar disorder is associated with abnormal white matter integrity in the cingulum. Method Fractional anisotropy in the anterior and posterior cingulum was compared between 42 participants with bipolar disorder and 42 healthy participants using diffusion tensor imaging. Results Fractional anisotropy was significantly decreased in the anterior cingulum in the bipolar disorder group compared with the healthy group (P=0.003); however, fractional anisotropy in the posterior cingulum did not differ significantly between groups. Conclusions Our findings demonstrate abnormalities in the structural integrity of the anterior cingulum in bipolar disorder. They extend evidence that supports involvement of the neural system comprising the anterior cingulate cortex and its corticolimbic gray matter connection sites in bipolar disorder to implicate abnormalities in the white matter connections within the system provided by the cingulum.

Aims: Obsessivecompulsive disorder (OCD) also generates emotional burden in the patient's family members, but no study has evaluated the specific dimensions of burden. The objectives were to evaluate the dimensions of the Zarit Burden Interview (ZBI) and possible correlates. Methods: This was a cross-sectional study involving 47 patients and 47 caregivers, using a sociodemographic questionnaire; the ZBI; the Self Reporting Questionnaire; the Family Accommodation Scale; and the YaleBrown ObsessiveCompulsive Scale. The ZBI factor analysis was conducted using Varimax Rotation. Results: Six factors were identified, explaining 74.2% of the total variance: factor 1, interference in the caregiver's personal life (36.6% of the variance); factor 2, perception of patient's dependence (10.8%); factor 3, feelings of irritation or intolerance (9.2%); factor 4, guilt (7.2%); factor 5, insecurity (5.6%); and factor 6, embarrassment (4.8%). The six ZBI factors were associated with greater OCD severity and with greater accommodation to the patient's symptoms, and factors 1, 2, 5 and 6 with caregiver's psychological morbidity. Caregiver's sex (female) was associated with factors 5 and 6, relationship with the patient (being a parent or son/daughter) with factor 5, higher educational level with factor 6, living with the patient with factor 3, worse self-evaluation of health with factors 1, 5 and 6, and occupational status (not working) with factors 1, 2, 5 and 6. Conclusion: The dimensions of burden identified indicate the most affected aspects of a caregiver's life and could guide the planning of more specific interventions. Thus, the caregiver could participate more effectively in the OCD patient's treatment, with a lower impact on his/her life.

Background: Cross-sectional studies have associated poor insight in patients with obsessive-compulsive disorder (OCD) with increased OCD symptom severity, earlier age of onset, comorbid depression, and treatment response. The goal of this current study was to examine the relationship between dimensions of OCD symptomatology and insight in a large clinical cohort of Brazilian patients with OCD. We hypothesized that poor insight would be associated with total symptom severity as well as with hoarding symptoms severity, specifically. Methods: 824 outpatients underwent a detailed clinical assessment for OCD, including the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS), the Dimensional Yale-Brown Obsessive-Compulsive Scale (DY-BOCS), the Brown Assessment of Beliefs Scale (BABS), a socio-demographic questionnaire, and the Structured Clinical Interview for axis I DSM-IV disorders (SCID-P). Tobit regression models were used to examine the association between level of insight and clinical variables of interest. Results: Increased severity of current and worst-ever hoarding symptoms and higher rate of unemployment were associated with poor insight in OCD after controlling for current OCD severity, age and gender. Poor insight was also correlated with increased severity of current OCD symptoms. Conclusion: Hoarding and overall OCD severity were significantly but weakly associated with level of insight in OCD patients. Further studies should examine insight as a moderator and mediator of treatment response in OCD in both behavioral therapy and pharmacological trials. Behavioral techniques aimed at enhancing insight may be potentially beneficial in OCD, especially among patients with hoarding. © 2011.

Introduction: Impairments in facial emotion recognition (PER) have been reported in bipolar disorder (BD) during all mood states. FER has been the focus of functional magnetic resonance imaging studies evaluating differential activation of limbic regions. Recently, the alpha 1-C subunit of the L-type voltage-gated calcium channel (CACNA1C) gene has been described as a risk gene for BD and its Met allele found to increase CACNA1C mRNA expression. In healthy controls, the CACNA1C risk (Met) allele has been reported to increase limbic system activation during emotional stimuli and also to impact on cognitive function. The aim of this study was to investigate the impact of CACNA1C genotype on FER scores and limbic system morphology in subjects with BD and healthy controls. Material and methods: Thirty-nine euthymic BD I subjects and 40 healthy controls were submitted to a PER recognition test battery and genotyped for CACNA1C. Subjects were also examined with a 3D 3-Tesla structural imaging protocol. Results: The CACNA1C risk allele for BD was associated to FER impairment in BD, while in controls nothing was observed. The CACNA1C genotype did not impact on amygdala or hippocampus volume neither in BD nor controls. Limitations: Sample size. Conclusion: The present findings suggest that a polymorphism in calcium channels interferes FER phenotype exclusively in BD and doesn't interfere on limbic structures morphology. This study investigated whether there are differences in the Speech-Evoked Auditory Brainstem Response among children with Typical Development (TD), (Central) Auditory Processing Disorder (C) APD, and Language Impairment (LI). The speech-evoked Auditory Brainstem Response was tested in 57 children (ages 6-12). The children were placed into three groups: TD (n = 18), (C)APD (n = 18) and LI (n = 21). Speech-evoked ABR were elicited using the five-formant syllable/da/. Three dimensions were defined for analysis, including timing, harmonics, and pitch. A comparative analysis of the responses between the typical development children and children with (C)APD and LI revealed abnormal encoding of the speech acoustic features that are characteristics of speech perception in children with (C)APD and LI, although the two groups differed in their abnormalities. While the children with (C)APD might had a greater difficulty distinguishing stimuli based on timing cues, the children with LI had the additional difficulty of distinguishing speech harmonics, which are important to the identification of speech sounds. These data suggested that an inefficient representation of crucial components of speech sounds may contribute to the difficulties with language processing found in children with LI. Furthermore, these findings may indicate that the neural processes mediated by the auditory brainstem differ among children with auditory processing and speech-language disorders. Introduction: The perception of comfort during air trips is determined by several factors. External factors like cabin design and environmental parameters (temperature, humidity, air pressure, noise, and vibration) interact with individual characteristics (anxiety traits, fear of flying, and personality) from arrival at the airport to landing at the destination. In this study, we investigated the influence of space and motion discomfort (SMD), fear of heights, and anxiety on comfort perception during all phases of air travel. Methods: We evaluated 51 frequent air travelers through a modified version of the Flight Anxiety Situations Questionnaire (FAS), in which new items were added and where the subjects were asked to report their level of discomfort or anxiety (not fear) for each phase of air travel (Chronbach's alpha = 0.974). Correlations were investigated among these scales: State-Trait Anxiety Inventory (STAB, Cohen's Acrophobia Questionnaire, and the Situational Characteristics Questionnaire (SitQ, designed to estimate SMD levels). Results: Scores of SitQ correlated with discomfort in situations involving space and movement perception (Pearson's rho = 0.311), while discomfort was associated with cognitive mechanisms related to scores in the anxiety scales (Pearson's rho = 0.375). Anxiety traits were important determinants of comfort perception before and after flight, while the influence of SMD was more significant during the time spent in the aircraft cabin. Discussion: SMD seems to be an important modulator of comfort perception in air travel. Its influence on physical well being and probably on cognitive performance, with possible effects on flight safety, deserves further investigation.

Objective: To characterize the PI component of long latency auditory evoked potentials (LLAEPs) in cochlear implant users with auditory neuropathy spectrum disorder (ANSD) and determine firstly whether they correlate with speech perception performance and secondly whether they correlate with other variables related to cochlear implant use. Methods: This study was conducted at the Center for Audiological Research at the University of Sao Paulo. The sample included 14 pediatric (4-11 years of age) cochlear implant users with ANSD, of both sexes, with profound prelingual hearing loss. Patients with hypoplasia or agenesis of the auditory nerve were excluded from the study. LLAEPs produced in response to speech stimuli were recorded using a Smart EP USB Jr. system. The subjects' speech perception was evaluated using tests 5 and 6 of the Glendonald Auditory Screening Procedure (GASP). Results: The P-1 component was detected in 12/14 (85.7%) children with ANSD. Latency of the P-1 component correlated with duration of sensorial hearing deprivation (*p = 0.007, r = 0.7278), but not with duration of cochlear implant use. An analysis of groups assigned according to GASP performance (k-means clustering) revealed that aspects of prior central auditory system development reflected in the P-1 component are related to behavioral auditory skills. Conclusions: In children with ANSD using cochlear implants, the P-1 component can serve as a marker of central auditory cortical development and a predictor of the implanted child's speech perception performance. This study had the aim to investigate the auditory and communicative abilities of children diagnosed with Auditory Neuropathy Spectrum Disorder due to mutation in the Otoferlin gene. It is a descriptive and qualitative study in which two siblings with this diagnosis were assessed. The procedures conducted were: speech perception tests for children with profound hearing loss, and assessment of communication abilities using the Behavioral Observation Protocol. Because they were siblings, the subjects in the study shared family and communicative context. However, they developed different communication abilities, especially regarding the use of oral language. The study showed that the Auditory Neuropathy Spectrum Disorder is a heterogeneous condition in all its aspects, and it is not possible to make generalizations or assume that cases with similar clinical features will develop similar auditory and communicative abilities, even when they are siblings. It is concluded that the acquisition of communicative abilities involves subjective factors, which should be investigated based on the uniqueness of each case.
