BACKGROUND The Endoscopic Release of Carpal Tunnel Syndrome (ECTR) is a minimal invasive approach for the treatment of Carpal Tunnel Syndrome. There is scepticism regarding the safety of this technique, based on the assumption that this is a rather "blind" procedure and on the high number of severe complications that have been reported in the literature. PURPOSE To evaluate whether there is evidence supporting a higher risk after ECTR in comparison to the conventional open release. METHODS We searched MEDLINE (January 1966 to November 2013), EMBASE (January 1980 to November 2013), the Cochrane Neuromuscular Disease Group Specialized Register (November 2013) and CENTRAL (2013, issue 11 in The Cochrane Library). We hand-searched reference lists of included studies. We included all randomized or quasi-randomized controlled trials (e.g. study using alternation, date of birth, or case record number) that compare any ECTR with any OCTR technique. Safety was assessed by the incidence of major, minor and total number of complications, recurrences, and re-operations.The total time needed before return to work or to return to daily activities was also assessed. We synthesized data using a random-effects meta-analysis in STATA. We conducted a sensitivity analysis for rare events using binomial likelihood. We judged the conclusiveness of meta-analysis calculating the conditional power of meta-analysis. CONCLUSIONS ECTR is associated with less time off work or with daily activities. The assessment of major complications, reoperations and recurrence of symptoms does not favor either of the interventions. There is an uncertain advantage of ECTR with respect to total minor complications (more transient paresthesia but fewer skin-related complications). Future studies are unlikely to alter these findings because of the rarity of the outcome. The effect of a learning curve might be responsible for reduced recurrences and reoperations with ECTR in studies that are more recent, although formal statistical analysis failed to provide evidence for such an association. As part of our continuing organic geochemical studies of sediments recovered by the Deep Sea Drilling Project, we have analyzed the types, amounts, and thermal alteration indices of organic matter in samples collected from the California continental margin on Leg 63. Some of the samples were frozen core; others were canned on site. Canned samples were analyzed for gas content using methods described by Mclver (1972). Our main objective was to see if the changes in surface circulation that had occurred through time off the California coast were reflected in changes in the type and amount of organic matter accumulating on the sea floor.

Three sustainable projects were studied under a sustainable park model for humid climates to determine where their costs lie in terms of installation, maintenance or both. These projects included the use of solar lighting to replace every configuration of conventional lighting, inclusion of a water garden/bog filter and Riparian Buffer System for the purposes of filtering sediments and nutrients out of runoff to prevent contaminated runoff from reaching the river that was adjacent to the park model location and construction of a LEED-inspired building to serve as the concession stand/restrooms building. The aggregate cost savings of instituting all three projects over ten years was $74,120 and the entire project paid itself off in approximately four years.

Long traffic queues on off-ramps significantly compromise the safety and throughput of motorways. Obtaining accurate queue information is crucial for countermeasure strategies. However, it is challenging to estimate traffic queues with locally installed inductive loop detectors. This paper deals with the problem of queue estimation with the interpretation of queuing dynamics and the corresponding time-occupancy distribution over motorway off-ramps. A novel algorithm for real-time queue estimation with two detectors is presented and discussed. Results derived from microscopic traffic simulation validated the effectiveness of the algorithm and revealed some of its useful features: (a) long and intermediate traffic queues could be accurately measured, (b) relatively simple detector input (i.e., time occupancy) was required, and (c) the estimation philosophy was independent with signal timing changes and provided the potential to cooperate with advanced strategies for signal control. Some issues concerning field implementation are also discussed.

The approach adopted for investigating the relationship between rainfall characteristics and pollutant wash-off process is commonly based on the use of parameters which represent the entire rainfall event. This does not permit the investigation of the influence of rainfall characteristics on different sectors of the wash-off process such as first flush where there is a high pollutant wash-off load at the initial stage of the runoff event. This research study analysed the influence of rainfall characteristics on the pollutant wash-off process using two sets of innovative parameters by partitioning wash-off and rainfall characteristics. It was found that the initial 10% of the wash-off process is closely linked to runoff volume related rainfall parameters including rainfall depth and rainfall duration while the remaining part of the wash-off process is primarily influenced by kinetic energy related rainfall parameters, namely, rainfall intensity. These outcomes prove that different sectors of the wash-off process are influenced by different segments of a rainfall event.

Under the New Labour Governments in the UK, successive reforms of the tax and benefit system sought to improve the financial benefits of paid work. Drawing on two waves of qualitative interviews with low-income working families this article examines the role of the UK tax credit system in shaping decisions about employment and unpaid care work. The article suggests that the financial support provided for lone parent participants by the tax credit system enhanced their temporal autonomy, permitting participation in paid work to align more closely with temporally situated notions of parental responsibility for caring. For couple families however, parental perceptions of responsibility for pre-school children, along with childcare constraints and the structure of the tax credit system served to constrain the autonomy of the main carer and implicitly encourage a gendered specialisation in caring or employment.

Spectroscopic studies of line emission intensities and ratios offer an attractive option in the\r\ndevelopment of non-invasive plasma diagnostics. Evaluating ratios of selected He I line\r\nemission profiles from the singlet and triplet neutral helium spin systems allows for simultaneous\r\nmeasurement of electron density (ne) and temperature (Te) profiles. Typically, this powerful\r\ndiagnostic tool is limited by the relatively long relaxation times of the 3S metastable term of helium\r\nthat populates the triplet spin system, and on which electron temperature sensitive lines are based.\r\nBy developing a time dependent analytical solution, we model the time evolution of the two spin\r\nsystems. We present a hybrid time dependent/independent line ratio solution that improves the\r\nrange of application of this diagnostic technique in the scrape-off layer (SOL) and edge plasma\r\nregions when comparing it against the current equilibrium line ratio helium model used at\r\nTEXTOR.

La multiplication dans le corps de Galois à 2^m éléments (i.e. GF(2^m)) est une opérations très importante pour les applications de la théorie des correcteurs et de la cryptographie. Dans ce mémoire, nous nous intéressons aux réalisations parallèles de multiplicateurs dans GF(2^m) lorsque ce dernier est généré par des trinômes irréductibles. Notre point de départ est le multiplicateur de Montgomery qui calcule A(x)B(x)x^(-u) efficacement, étant donné A(x), B(x) in GF(2^m) pour u choisi judicieusement. Nous étudions ensuite l'algorithme diviser pour régner PCHS qui permet de partitionner les multiplicandes d'un produit dans GF(2^m) lorsque m est impair. Nous l'appliquons pour la partitionnement de A(x) et de B(x) dans la multiplication de Montgomery A(x)B(x)x^(-u) pour GF(2^m) même si m est pair. Basé sur cette nouvelle approche, nous construisons un multiplicateur dans GF(2^m) généré par des trinôme irréductibles. Une nouvelle astuce de réutilisation des résultats intermédiaires nous permet d'éliminer plusieurs portes XOR redondantes. Les complexités de temps (i.e. le délais) et d'espace (i.e. le nombre de portes logiques) du nouveau multiplicateur sont ensuite analysées: 1. Le nouveau multiplicateur demande environ 25% moins de portes logiques que les multiplicateurs de Montgomery et de Mastrovito lorsque GF(2^m) est généré par des trinômes irréductible et m est suffisamment grand. Le nombre de portes du nouveau multiplicateur est presque identique à celui du multiplicateur de Karatsuba proposé par Elia. 2. Le délai de calcul du nouveau multiplicateur excède celui des meilleurs multiplicateurs d'au plus deux évaluations de portes XOR. 3. Nous determinons le délai et le nombre de portes logiques du nouveau multiplicateur sur les deux corps de Galois recommandés par le National Institute of Standards and Technology (NIST). Nous montrons que notre multiplicateurs contient 15% moins de portes logiques que les multiplicateurs de Montgomery et de Mastrovito au coût d'un délai d'au plus une porte XOR supplémentaire. De plus, notre multiplicateur a un délai d'une porte XOR moindre que celui du multiplicateur d'Elia au coût d'une augmentation de moins de 1% du nombre total de portes logiques.

This thesis is an attempt to Provenence, Sedimentetion and Geochemistry of the Modern Sediments of the Mud Banks off the Central Kerela Coast, India. In the present doctoral work, an attempt has been made to study in detail the mud banks of central Kerala, i.e. of Narakkal, Saudi and Purakkad areas which are reported as permanent mud banks, since olden days. The studies have been conducted during the years 1985 and 1986. The important findings of the study is stated as clay mineralogical studies of the rivers, lake and mud bank sediments reveal that the dominant clay mineral is kaolinite followed by montmorillonite, illite and gibbsite. Geochemical analysis of the Vembanad lake and mud bank sediments show that the iron and manganese are widely distributed both in the lake and mud bank sediments

Various methods of assessment have been applied to the One Dimensional Time to Explosion (ODTX) apparatus and experiments with the aim of allowing an estimate of the comparative violence of the explosion event to be made. Non-mechanical methods used were a simple visual inspection, measuring the increase in the void volume of the anvils following an explosion and measuring the velocity of the sound produced by the explosion over 1 metre. Mechanical methods used included monitoring piezo-electric devices inserted in the frame of the machine and measuring the rotational velocity of a rotating bar placed on the top of the anvils after it had been displaced by the shock wave. This last method, which resembles original Hopkinson Bar experiments, seemed the easiest to apply and analyse, giving relative rankings of violence and the possibility of the calculation of a “detonation” pressure.

This paper deals with a system that describes an electrical circuitcomposed by a linear system coupled to a nonlinear one involving a tunneldiode in a flush-and-fill circuit. One of the most comprehensive models for thiskind of circuits was introduced by R. Fitzhugh in 1961, when taking on carebiological tasks. The equation has in its phase plane only two periodic solutions,namely, the unstable singular point S0 and the stable cycle Γ. If the system isat rest on S0, the natural flow of orbits seeks to switch-on the process by going- as time goes by - toward its steady-state, Γ. By using suitable controls it ispossible to reverse such natural tendency going in a minimal time from Γ toS0, switching-off in this way the system. To achieve this goal it is mandatorya minimal enough strength on controls. These facts will be shown by means ofconsiderations on the null control sets in the process.

Low-frequency "off-line" repetitive transcranial magnetic stimulation (rTMS) over the course of several minutes has attained considerable attention as a research tool in cognitive neuroscience due to its ability to induce functional disruptions of brain areas. This disruptive rTMS effect is highly valuable for revealing a causal relationship between brain and behavior. However, its influence on remote interconnected areas and, more importantly, the duration of the induced neurophysiological effects, remain unknown. These aspects are critical for a study design in the context of cognitive neuroscience. In order to investigate these issues, 12 healthy male subjects underwent 8 H(2)(15)O positron emission tomography (PET) scans after application of long-train low-frequency rTMS to the right dorsolateral prefrontal cortex (DLPFC). Immediately after the stimulation train, regional cerebral blood flow (rCBF) increases were present under the stimulation site as well as in other prefrontal cortical areas, including the ventrolateral prefrontal cortex (VLPFC) ipsilateral to the stimulation site. The mean increases in rCBF returned to baseline within 9 min. The duration of this unilateral prefrontal rTMS effect on rCBF is of particular interest to those who aim to influence behavior in cognitive paradigms that use an "off-line" approach.
