Case Report. An 8-year-old girl with amelogenesis imperfecta (AI) reported unsatisfactory aesthetics, difficulty in mastication, and dental hypersensitivity. The intraoral examination observed mixed dentition, malocclusion in anteroposterior relationships, anterior open bite, and dental asymmetry. A hypoplastic form of AI was diagnosed in the permanent dentition. A multidisciplinary planning was performed and divided into preventive, orthopedic, and rehabilitation stages. Initially, preventive treatment was implemented, with fluoride varnish applications, in order to protect the fragile enamel and reduce the dental sensitivity. In the second stage, the patient received an interceptive orthopedic treatment to improve cross-relationship of the arches during six months. Finally, the rehabilitation treatment was executed to establish the vertical dimension. In the posterior teeth, indirect composite resin crowns were performed with minimally invasive dental preparation. Direct composite resin restorations were used to improve the appearance of anterior teeth. Follow-Up. The follow-up was carried out after 3, 6, 12, and 18 months. After 18 months of follow-up, The restoration of integrity, oral hygiene, and patient satisfaction were observed . Conclusion. Successful reduction of the dental hypersensitivity and improvement of the aesthetic and functional aspects as well as quality of life were observed.

The diet of early human ancestors has received renewed theoretical interest since the discovery of elevated d13C values in the enamel of Australopithecus africanus and Paranthropus robustus. As a result, the hominin diet is hypothesized to have included C4 grass or the tissues of animals which themselves consumed C4 grass. On mechanical grounds, such a diet is incompatible with the dental morphology and dental microwear of early hominins. Most inferences, particularly for Paranthropus, favor a diet of hard or mechanically resistant foods. This discrepancy has invigorated the longstanding hypothesis that hominins consumed plant underground storage organs (USOs). Plant USOs are attractive candidate foods because many bulbous grasses and cormous sedges use C4 photosynthesis. Yet mechanical data for USOs—or any putative hominin food—are scarcely known. To fill this empirical void we measured the mechanical properties of USOs from 98 plant species from across sub-Saharan Africa. We found that rhizomes were the most resistant to deformation and fracture, followed by tubers, corms, and bulbs. An important result of this study is that corms exhibited low toughness values (mean = 265.0 J m-2) and relatively high Young’s modulus values (mean = 4.9 MPa). This combination of properties fits many descriptions of the hominin diet as consisting of hard-brittle objects. When compared to corms, bulbs are tougher (mean = 325.0 J m-2) and less stiff (mean = 2.5 MPa). Again, this combination of traits resembles dietary inferences, especially for Australopithecus, which is predicted to have consumed soft-tough foods. Lastly, we observed the roasting behavior of Hadza hunter-gatherers and measured the effects of roasting on the toughness on undomesticated tubers. Our results support assumptions that roasting lessens the work of mastication, and, by inference, the cost of digestion. Together these findings provide the first mechanical basis for discussing the adaptive advantages of roasting tubers and the plausibility of USOs in the diet of early hominins.

TEMA: o desenvolvimento do controle motor oral depende em parte das experiências sensoriais e motoras. OBJETIVO: analisar a relação entre a duração do aleitamento natural, artificial e da sucção e destas com o desempenho motor orofacial. MÉTODO: cento e setenta e seis crianças, de 6 a 12 anos de idade, passaram por avaliação miofuncional orofacial, empregando o protocolo com escores, e os responsáveis foram entrevistados a respeito do aleitamento e hábitos de sucção de suas crianças. As correlações foram calculadas pelo teste de Spearman. RESULTADOS: na amostra estudada, a média de duração do aleitamento natural foi de 10,30 meses (variando de zero a 60 meses), do aleitamento artificial 44,12 (zero a 122 meses) e dos hábitos de sucção de 39,32 meses (zero a 144 meses). Houve correlação negativa da duração do aleitamento natural com a duração do aleitamento artificial e a duração dos hábitos de sucção (p < 0,001). A maior duração do aleitamento artificial correspondeu à maior duração dos hábitos de sucção, apresentando, assim, correlação positiva (p < 0,001). A duração do aleitamento natural foi correlacionada positivamente com a mobilidade orofacial (p = 0,05). Houve correlação negativa da duração do aleitamento artificial e da duração dos hábitos de sucção com, respectivamente, o desempenho na mastigação e na deglutição, bem como da duração de ambos os tipos de sucção com a prova de diadococinesia (p = 0,05). CONCLUSÃO: a duração do aleitamento natural mostrou efeito positivo sobre a mobilidade das estruturas orofaciais. Os efeitos deletérios da duração dos hábitos de sucção no controle motor orofacial foram confirmados.
Masticatory muscle function three years after surgical correction of class III dentofacial deformity

Individuals with dentofacial deformities have masticatory muscle changes. The objective of the present study was to determine the effect of interdisciplinary treatment in patients with dentofacial deformities regarding electromyographic activity (EMG) of masticatory muscles three years after surgical correction. Thirteen patients with class III dentofacial deformities were studied, considered as group PI (before surgery) and group P3 (3 years to 3 years and 8 months after surgery). Fifteen individuals with no changes in facial morphology or dental occlusion were studied as controls. The participants underwent EMG examination of the temporal and masseter muscles during mastication and biting. Evaluation of the amplitude interval of EMG activity revealed a difference between P1 and P3 and no difference between P3 and the control group. In contrast, evaluation of root mean square revealed that, in general, P3 values were higher only when compared with PI and differed from the control group. There was an improvement in the EMG activity of the masticatory muscles, mainly observed in the masseter muscle, with values close to those of the control group in one of the analyses.
Partial cricotracheal resection for pediatric subglottic stenosis: long-term outcome in 57 patients.

OBJECTIVE: We sought to assess the long-term outcome of 57 pediatric patients who underwent partial cricotracheal resection for subglottic stenosis. METHODS: Eighty-one pediatric partial cricotracheal resections were performed in our tertiary care institution between 1978 and 2004. Fifty-seven patients had a minimal follow-up time of 1 year and were included in this study. Evaluation was based on the last laryngotracheal endoscopy, the responses to a questionnaire, and a retrospective review of the patient's data. The following parameters were analyzed: decannulation rates, breathing, voice quality, and deglutition. RESULTS: A single-stage partial cricotracheal resection was performed in 38 patients, and a double-stage procedure was performed in 19 patients. Sixteen patients underwent an extended partial cricotracheal resection (ie, partial cricotracheal resection combined with another open procedure). At a median follow-up time of 5.1 years, the decannulation rates after a single- or double-stage procedure were 97.4% and 95%, respectively. Two patients remained tracheotomy dependent. One patient had moderate exertional dyspnea, and all other patients had no exertional dyspnea. Voice quality was found to improve after surgical intervention for 1 +/- 1.34 grade dysphonia (P < .0001) according to the adapted GRBAS grading system (Grade, Roughness, Breathiness, Asthenia, and Strain). CONCLUSIONS: Partial cricotracheal resection provides good results for grades III and IV subglottic stenosis as primary or salvage operations. The procedure has no deleterious effects on laryngeal growth and function. The quality of voice significantly improves after surgical intervention but largely depends on the preoperative condition.

Background and Objective: Mucositis is the most common oral complication of cancer chemotherapy, which causes pain on mastication and swallowing, impairs patients` ability to eat and take oral drugs and may determine interruption of the treatment. The aim of this study was to evaluate the effect of light-emitting diode (LED) therapy on chemotherapy-induced mucositis in hamsters. Study Design/Materials and Methods: Animals of both experimental (Group 1; n = 32) and positive control (Group II; n = 32) groups received intraperitoneal injections of 5-fluorouracil on days 0 and 2. All animals had their right and left cheek pouch irritated by superficial scratching on days 3 and 4. In Group I, LED irradiation (630 nm +/- 10 nm, 160 mW, 12 J/cm(2)) was applied during 37.5 seconds at days 3, 4, 6, 8, 10, 12, and 14. In Group II, mucositis was induced, but LED therapy was not performed. The oral mucosa was photographed from day 4 to 14 at 2-day intervals. Photographs were randomly scored according to the severity of induced mucositis (0 to 5). In the negative control group (Group III; n = 6), no mucositis was induced. Biopsies of the cheek pouches of 8 animals (Group I and Group II) were surgically obtained on days 5, 9, 13 and 15 and processed for histological examination. Results: The statistical analysis showed significant differences between irradiated and non-irradiated groups (P < 0.05). However, muscular degeneration was observed in 18% of the samples of Group I. Conclusion: It may be concluded that the LED therapy protocol established for this in vivo study was effective in reducing the severity of oral mucositis, although the oral lesions were not completely prevented. The authors studied the lateral positional desviations of the mandible, in relation to the facial median line in 30 (thirty) full edentulous patients, with the purpose to verify the influence of the unstrained guided (chinpoint guidance) and deglutition methods for the determination of centric relation. According the results obtained they conclude that the unstrained guided method produced a mandibular lateral desviation with a mean value of the 0.752 mm and in the deglutition method the mean value observed was 1.109 mm. The statistical analysis of the results revealed for the unstrained guided method a proportion of the points to the right and left of the median line not statistically significant, while for the deglutition method the difference was statistically significant at a level of 5%.

This study aimed to evaluate the effect of simulated chewing in the laboratory on the survival of seeds of four tropical forage legumes (butterfly pea, Clitorea ternatea; estilosantes, Stylosanthes capitata/S. macrocephala 'Campo Grande; archer, Macrotyloma axillare and perennial soybean, Neonotonia wightii) submitted to different periods of acid enzymatic digestion in vitro. Three trials were conducted to observe the percentage of destroyed seeds by the mastication; to compare the germination of the seeds (intact seeds, simulated mastication, scarification with sandpaper, mastication and scarification with sandpaper). And, finally the seeds were incubated at 39oC with hydrochloric acid and pepsin for: 0, 2, 4, 8, 12 and 24 hours. The percentages of not destroyed seeds in mastication (archer, 91,5; perennial soybean, 88.0; butterfly pea, 82.1, and estilo, 81.1), associated with the beneficial effects of scarification on germination (64.7, 60.0, 92.0 e 87.3%, respectively) and the effects of time of acid-enzymatic digestion (75% higher if they stay 24 hours in HCl + pepsin) associated to the hard and not permeable coats of legume seeds, allow a high potential for resistance, and to pass intact through the digestive tract of cattle, being able to germinate when defecated in the pastures. However, estilo should not be included in the feeding of cattle for this purpose, because it do not resists the acid-enzyme digestion.

Para medir possíveis influências da mastigação e do estilo de vida sedentário em modelo murino senil, impusemos um de três regimes de dieta aos diferentes grupos experimentais do 21º dia pós-natal até 6 ou 18 meses de vida: dieta sólida tipo pellet; dieta em pellet seguida por uma em pó, farelada; ou dieta peletizada seguida de pó e novamente pellet, com intervalos de tempo iguais em cada dieta. Para mimetizar o estilo de vida sedentário ou ativo, os animais foram criados, respectivamente, em gaiolas-padrão (ambiente empobrecido-AP) ou em gaiolas enriquecidas (ambiente enriquecido-AE). Para medir os efeitos da dieta, do ambiente e da idade sobre a atividade exploratória, realizamos o teste do campo aberto, onde camundongos jovens de AP que sofreram alteração da atividade mastigatória demonstraram maior preferência pela zona periférica, mas no envelhecimento e no AE essas diferenças foram minimizadas. Nos velhos de AE, essas diferenças reapareceram. Já sobre as influências na aprendizagem e memória espacial, aplicamos o labirinto aquático de Morris e vimos que a redução da atividade mastigatória, independente do ambiente, diminuiu a taxa média de aprendizado espacial e sua reabilitação recuperou as perdas associadas em animais jovens e, quando combinada ao AE, melhorou a taxa de aprendizado em velhos. Não se encontrou correlação entre taxa de aprendizado e velocidade de nado dos camundongos sugerindo que os déficits são de natureza cognitiva. Concluímos assim, que a alteração da atividade mastigatória influencia o padrão de exploração por zonas no campo aberto e a estimulação ambiental acentua os seus efeitos no envelhecimento, privilegiando a preferência pela zona periférica e a redução da atividade mastigatória prejudica a memória espacial durante o teste do labirinto aquático de Morris e a sua reabilitação é capaz de recuperar as habilidades espaciais, mas em idosos é necessária a combinação com um AE.

Although one cannot prevent the birth of human beings with cleft palate lesions one can adopt attitudes and various procedures entirely viable to rehabilitate and recuperate these persons through a team work composed of the facilities of a hospital, physician, dentist, psychotherapist, epeech pathologist, pedagogue and a social worker. In order to reduce the suffering of these patients we took up this work to prove that even if not in an ideal manner nevertheleses in an acceptable one we can reintegrate these persons to their families, friends and social milieu, offering them more security in their social, psychic and human relationships. We have made a review of the literature during the last three decades where we found many possibilities of prosthetic recuperations which concerns esthetics, mastication and maxillomandibular relationships beyond allowing these pacients conditions to perform satisfactory the functions of deglution and speech. We have selected and executed 5 clinical cases out of 15 pacients formely choosen looking to different prosthetic plannings. We have also shown the major contribution that the osseous integrated implants can offer to the rehabilitation and recuperation of pacients whith congenitall labio palatal lesions. To simplify the understanding we tried to discuss the most varied types of prosthesis in well defined subjects having a separate approach for each of them and we still showed that the specificity of each case leads to a specific type of rehabilitation founded om esthetics, occlusion, osseous suport, edentulous space, and above all based in the common sense and Professional integrity

O objetivo da presente revisão de literatura foi analisar artigos científicos internacionais publicados sobre a fisiologia da deglutição de alimentos líquidos nas fases oral e faríngea. A metodologia empregada envolveu a formulação da pergunta; localização e seleção dos estudos; avaliação crítica dos artigos; conforme os preceitos do Cochrane Handbook. Foram identificados 185 artigos, dos quais se excluiu 141 por não relacionarem-se diretamente ao tema e analisou-se 29 estudos. As pesquisas estão fortemente relacionadas às formas de identificação de disfagia e não as características proporcionadas pela deglutição de diferentes consistências. Quanto à metodologia empregada nos artigos analisados observa-se que na maioria dos estudos não há grupo-controle. Os grupos estudados são heterogêneos, principalmente quando considerando indivíduos com alterações neurológicas, além disso, não há pareamento de idade na maioria dos estudos. Dessa forma, os achados desta revisão demonstram que há dificuldade na aplicabilidade clínica dos achados científicos, dificultando a prática baseada evidências.
Vergleichende computergestützte funktionsmorphologische Analyse an Molaren cercopithecoider Primaten

Die Analyse funktioneller Zusammenhänge zwischen Ernährung und Zahnmorphologie ist ein wichtiger Aspekt primatologischer und paläontologischer Forschung. Als überdauernder Teil des Verdauungssystems geben Zähne die bestmöglichen Hinweise auf die Ernährungsstrategien (ausgestorbener) Arten und eine Fülle weiterer Informationen. Aufgrund dessen ist es für die wissenschaftliche Arbeit von größter Bedeutung, die Zähne so detailliert und exakt wie möglich in ihrer gesamten Struktur zu erfassen. Bisher wurden zumeist zweidimensionale Parameter verwendet, um die komplexe Kronenmorphologie von Primatenmolaren vergleichend zu untersuchen. Die vorliegende Arbeit hatte das Ziel, Zähne verschiedener Arten von Altweltaffen mittels computerbasierter Methoden dreidimensional zu erfassen und neue Parameter zu definieren, mit denen die Form dieser Zähne objektiv erfasst und funktionell interpretiert werden kann. Mit einem Oberflächen-Scanner wurden die Gebisse einer Stichprobe von insgesamt 48 Primaten von fünf verschiedenen Arten eingescannt und mit Bildverarbeitungsmethoden so bearbeitet, dass dreidimensionale digitale Modelle einzelner Backenzähne zur Analyse vorlagen. Es wurden dabei sowohl Arten ausgewählt, die eine für ihre Gattung typische Ernährungsweise besitzen - also Frugivorie bei den Cercopithecinen und Folivorie bei den Colobinen - als auch solche, die eine davon abweichende Alimentation bevorzugen. Alle Altweltaffen haben sehr ähnliche Molaren. Colobinen haben jedoch höhere und spitzere Zahnhöcker, dünneren Zahnschmelz und scheinen ihre Zähne weniger stark abzukauen als die Meerkatzen. Diese Beobachtungen konnten mit Hilfe der neuen Parameter quantifiziert werden. Aus der 3D-Oberfläche und der Grundfläche der Zähne wurde ein Index gebildet, der die Stärke des Oberflächenreliefs angibt. Dieser Index hat bei Colobinen deutlich höhere Werte als bei Cercopithecinen, auch bei Zähnen, die schon stark abgekaut sind. Die Steilheit der Höcker und ihre Ausrichtung wurden außerdem gemessen. Auch diese Winkelmessungen bestätigten das Bild. Je höher der Blätteranteil an der Ernährung ist, desto höher sind die Indexwerte und umso steiler sind die Höcker. Besonders wichtig war es, dies auch für abgekaute Zähne zu bestätigen, die bisher nicht in funktionelle Analysen miteinbezogen wurden. Die Ausrichtung der Höckerseiten gibt Hinweise auf die Kaubewegung, die zum effizienten Zerkleinern der Nahrung notwendig ist. Die Ausrichtung der Höcker der Colobinen deutet darauf hin, dass diese Primaten flache, gleitende Kaubewegungen machen, bei denen die hohen Höcker aneinander vorbei scheren. Dies ist sinnvoll zum Zerschneiden von faserreicher Nahrung wie Blättern. Cercopithecinen scheinen ihre Backenzähne eher wie Mörser und Stößel zu verwenden, um Früchte und Samen zu zerquetschen und zu zermahlen. Je nachdem, was neben der hauptsächlichen Nahrung noch gekaut wird, unterscheiden sich die Arten graduell. Anders als bisher vermutet wurde, konnte gezeigt werden, dass Colobinen trotz des dünnen Zahnschmelzes ihre Zähne weniger stark abkauen und weniger Dentin freigelegt wird. Dies gibt eindeutige Hinweise auf die Unterschiede in der mechanischen Belastung, die während des Kauvorgangs auf die Zähne wirkt, und lässt sich gut mit der Ernährung der Arten in Zusammenhang bringen. Anhand dieser modellhaften Beobachtungen können in Zukunft ausgestorbene Arten hinsichtlich ihrer Ernährungsweise mit 3D-Techniken untersucht werden.
