Based on the exponential aging of the population and the increasing life expectancy in industrialized western countries, prostate cancer (PCa) in elderly men is becoming a disease of increasing significance. Consensus exists that men over the age of 75 years should not be screened for PCa; however, higher age as a single parameter should not exclude men from being diagnosed with prostate cancer and treated accordingly. It is well-known that overdiagnosis and overtreatment are frequent in this age group. Competing mortality risks of men older than 75 years may supersede the risk of dying from PCa several fold. Both the treating physician and the patient himself should therefore balance the possible risks and benefits of diagnosing and treating prostate cancer concerning the impact on quality of life. This is of special importance when taking into account that the complication rates of curative treatment modalities are higher in older patients than in younger men and that hormonal treatment might have negative effects especially in older men.Age, existing comorbidities, cognitive and physical status in combination with specific tumor parameters are useful tools for an individualized treatment.Therapy should be considered for healthy, active men aged 75 years or older who present with high-risk PCa and/or with a PSA doubling time <12 months. Elderly men who are unfit or have low to intermediate risk PCa will most likely not benefit from treatment.

Sarcopenia is the age-related loss of muscle mass and strength and has been associated with an increased risk of falling and the development of metabolic diseases. Various training protocols, nutritional and hormonal interventions have been proposed to prevent sarcopenia. This study explores the potential of continuous eccentric exercise to retard age-related loss of muscle mass and function. Elderly men and women (80.6 +/- 3.5 years) were randomized to one of three training interventions demanding a training effort of two sessions weekly for 12 weeks: cognitive training (CT; n = 16), conventional resistance training (RET; n = 23) and eccentric ergometer training (EET; n = 23). Subjects were tested for functional parameters and body composition. Biopsies were collected from M. vastus lateralis before and after the intervention for the assessment of fiber size and composition. Maximal isometric leg extension strength (MEL: +8.4 +/- 1.7%) and eccentric muscle coordination (COORD: -43 +/- 4%) were significantly improved with EET but not with RET (MEL: +2.3 +/- 2.0%; COORD: -13 +/- 3%) and CT (MEL: -2.3 +/- 2.5%; COORD: -12 +/- 5%), respectively. We observed a loss of body fat (-5.0 +/- 1.1%) and thigh fat (-6.9 +/- 1.5%) in EET subjects only. Relative thigh lean mass increased with EET (+2.5 +/- 0.6%) and RET (+2.0 +/- 0.3%) and correlated negatively with type IIX/type II muscle fiber ratios. It was concluded that both RET and EET are beneficial for the elderly with regard to muscle functional and structural improvements but differ in their spectrum of effects. A training frequency of only two sessions per week seems to be the lower limit for a training stimulus to reveal measurable benefits.

La condición física, o como mejor se la conoce hoy en día el “fitness”, es una variable que está cobrando gran protagonismo, especialmente desde la perspectiva de la salud. La mejora de la calidad de vida que se ha experimentado en los últimos años en las sociedades desarrolladas, conlleva un aumento de la esperanza de vida, lo que hace que cada vez más personas vivan más años. Este rápido crecimiento de la población mayor de 60 años hace que, un grupo poblacional prácticamente olvidado desde el punto de vista de la investigación científica en el campo de la actividad física y del deporte, cobre gran relevancia, con el fin de poder ayudar a alcanzar el dicho “no se trata de aportar años a la vida sino vida a lo años”. La presente memoria de Tesis Doctoral tiene como principal objetivo valorar los niveles de fitness en población mayor española, además de analizar la relación existente entre el fitness, sus condicionantes y otros aspectos de la salud, tales como la composición corporal y el estado cognitivo. Entendemos que para poder establecer futuras políticas de salud pública en relación a la actividad física y el envejecimiento activo es necesario conocer cuáles son los niveles de partida de la población mayor en España y sus condicionantes. El trabajo está basado en los datos del estudio multicéntrico EXERNET (Estudio Multi-céntrico para la Evaluación de los Niveles de Condición Física y su relación con Estilos de Vida Saludables en población mayor española no institucionalizada), así como en los datos de dos estudios, llevados a cabo en población mayor institucionalizada. Se han analizado un total de 3136 mayores de vida independiente, procedentes de 6 comunidades autónomas, y 153 mayores institucionalizados en residencias de la Comunidad de Madrid. Los principales resultados de esta tesis son los siguientes: a) Fueron establecidos los valores de referencia, así como las curvas de percentiles, para cada uno de los test de fitness, de acuerdo a la edad y al sexo, en población mayor española de vida independiente y no institucionalizada. b) Los varones obtuvieron mejores niveles de fitness que las mujeres, excepto en los test de flexibilidad; existe una tendencia a disminuir la condición física en ambos sexos a medida que la edad aumenta. c) Niveles bajos de fitness funcional fueron asociados con un aumento en la percepción de problemas. d) El nivel mínimo de fitness funcional a partir del cual los mayores perciben problemas en sus actividades de la vida diaria (AVD) es similar en ambos sexos. e) Niveles elevados de fitness fueron asociados con un menor riesgo de sufrir obesidad sarcopénica y con una mejor salud percibida en los mayores. f) Las personas mayores con obesidad sarcopénica tienen menor capacidad funcional que las personas mayores sanas. g) Niveles elevados de fuerza fueron asociados con un mejor estado cognitivo siendo el estado cognitivo la variable que más influye en el deterioro de la fuerza, incluso más que el sexo y la edad. ABSTRACT Fitness is a variable that is gaining in prominence, especially from the health perspective. Improvement of life quality that has been experienced in the last few years in developed countries, leads to an expanded life expectancy, increasing the numbers of people living longer. This population consisting of people of over 60 years, an almost forgotten population group from the point of view of scientific research in the field of physical activity and sport, is becoming increasingly important, with the main aim of helping to achieve the saying “do not only add years to life, but also add life to years”. The principal aim of the current thesis was to assess physical fitness levels in Spanish elderly people, of over 65 years, analyzing relationship between physical fitness, its determinants, and other aspects of health such as body composition and cognitive status. In order to establish further public health policies in relation to physical activity and active ageing it is necessary to identify the starting physical fitness levels of the Spanish population and their determinants. The work is based on data from the EXERNET multi-center study ("Multi-center Study for the Evaluation of Fitness levels and their relationship to Healthy Lifestyles in noninstitutionalized Spanish elderly"), and on data from two studies conducted in institutionalized elderly people: a total of 3136 non-institutionalized elderly, from 6 Regions of Spain, and 153 institutionalized elderly in nursing homes of Madrid. The main outcomes of this thesis are: a) sex- and age-specific physical fitness normative values and percentile curves for independent and non-institutionalized Spanish elderly were established. b) Greater physical fitness was present in the elderly men than in women, except for the flexibility test, and a trend toward decreased physical fitness in both sexes as their age increased. c) Lower levels of functional fitness were associated with increased perceived problems. d) The minimum functional fitness level at which older adults perceive problems in their ADLs, is similar for both sexes e) Higher levels of physical fitness were associated with a reduced risk of suffering sarcopenic obesity and better perceived health among the elderly. f) The elderly with sarcopenic obesity have lower physical functioning than healthy counterparts. g) Higher strength values were associated with better cognitive status with cognitive status being the most influencing variable in strength deterioration even more than sex and age.

Background. It is uncertain whether accepted associations between health behaviors and mortality are pertinent to elderly people. No previous studies have examined the patterns of lifestyle in elderly men with and without clinically evident vascular disease by using a lifestyle score to predict survival. Methods. We measured prevalence of a healthy lifestyle (four or more healthy behaviors out of eight) and examined survival in 11,745 men aged 65-83 years participating in a randomized population-based trial of screening for abdominal aortic aneurysm in Perth, Western Australia. After stratifying participants into five groups according to history and symptoms of vascular disease, we compared survival of men in each subgroup with that of 'healthy' men with no history or symptoms of vascular disease. Results. Invitations to screening produced a corrected response of 70.5%. After adjusting for age and place of birth, having an unhealthy lifestyle was associated with an increase of 20% in the likelihood of death from any cause within 5 years (95% CI: 10-30%). This pattern was consistently evident across subgroups defined by history of vascular disease, but was less evident for deaths from vascular disease. Conclusions. Our results highlight the importance of maintaining a healthy lifestyle through to old age, regardless of history of vascular disease. (c) 2005 Elsevier Inc. All rights reserved.

The purpose of this article is to examine firearm suicide in Queensland. In 2006, statistical data were gathered from all closed paper coronial files for the 12-month period of December 2003—December 2004. Of the 567 people who committed suicide in Queensland during this period, 48 (8.5%) used firearms. The following results emerge from this data: first, gun suicides are continuing to decrease in Queensland, most likely as a function of ongoing gun controls, a decrease accompanied by a lesser increase in other methods of suicide, thereby providing little support for substitution theory; second, men continue to be more likely to shoot themselves, particularly elderly men; third, firearms are more likely to be used in rural settings, and by those with no known history of mental illness or previous suicide attempts. Finally, in spite of otherwise very high suicide rates, Aborigines rarely employ firearms, using instead the culturally significant method of hanging.

Herbal Fructus Corni is a folk medicine with a long history of safe use for treating osteoporosis in postmenopausal women or elderly men in Asia. Sweroside is a bioactive herbal ingredient isolated from Fructus Corni, which has been widely used for the treatment of osteoporosis in traditional Chinese medicine (TCM). Unfortunately, the working mechanisms of this compound are difficult to determine and thus remain unclear. The aim of the study was performed to determine the potential molecular mechanism of the anti-osteoporotic effect of sweroside on the human MG-63 cells and rat osteoblasts. 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) test was used to observe the effect of sweroside on cell proliferation. The activity of alkaline phosphatase (ALP) and the amount of osteocalcin were also assayed the cell differentiation. Sweroside significantly increased the proliferation of human MG-63 cells and rat osteoblasts (P<0.01). It increased the activity of ALP, and osteocalcin was also elevated in response to sweroside (P<0.05). Of note, flowcytometer assay showed that sweroside can attenuate and inhibit apoptosis. Sweroside has a direct osteogenic effect on the proliferation and differentiation of cultured human MG-63 cells and rat osteoblasts in vitro. These data will help in understanding the molecular mechanisms of therapeutic efficacy of sweroside, and highlight insights into drug discovery. In the current study, sweroside has been suggested to be a promising osteoporosis therapeutic natural product.

Kaposi's sarcoma (KS) is a relatively low grade neoplasm, classically occurring in the skin of elderly men. A more virulent and invasive form of Kaposi's sarcoma has been described in patients with acquired immune deficiency syndrome (AIDS). The origin and identification of the tumor cells in these lesions is controversial. Here we have studied the behavior of cells derived from KS lesions in an in vitro assay which measures the ability of cells to invade through a reconstituted basement membrane. In agreement with previous work, KS cells obtained under selective culture conditions were invasive showing activity comparable to that of malignant tumor cells. Normal fibroblasts, smooth muscle cells, and endothelial cells did not demonstrate invasive behavior under the same experimental conditions. To characterize further the nature of the KS cells we tested the chemotactic response of cells from the most invasive line to a variety of growth factors and compared their response to those of fibroblasts, smooth muscle, and endothelial cells. These studies suggest that normal cells respond to a unique repertoire of chemotactic factors. The chemotactic response of the KS cells most closely resembled that of smooth muscle cells and was quite distinct from endothelial cells. These results indicate that the KS-derived cultures contain invasive cells with a smooth muscle cell-like phenotype.

Background Osteoporosis is a common cause of disability and death in elderly men and women. Until 2007, Australian Government-subsidized use of oral bisphosphonates, raloxifene and calcitriol (1α,25-dihydroxycholecalciferol) was limited to secondary prevention (requiring x-ray evidence of previous low-trauma fracture). The cost to the Pharmaceutical Benefits Scheme was substantial (164 million Australian dollars in 2005/6). Objective To examine the dispensed prescriptions for oral bisphosphonates, raloxifene, calcitriol and two calcium products for the secondary prevention of osteoporosis (after previous low-trauma fracture) in the Australian population. Methods We analysed government data on prescriptions for oral bisphosphonates, raloxifene, calcitriol and two calcium products from 1995 to 2006, and by sex and age from 2002 to 2006. Prescription counts were converted to defined daily doses (DDD)/1000 population/day. This standardized drug utilization method used census population data, and adjusts for the effects of aging in the Australian population. Results Total bisphosphonate use increased 460% from 2.19 to 12.26 DDD/1000 population/day between June 2000 and June 2006. The proportion of total bisphosphonate use in June 2006 was 75.1% alendronate, 24.6% risedronate and 0.3% etidronate. Raloxifene use in June 2006 was 1.32 DDD/1000 population/day. The weekly forms of alendronate and risedronate, introduced in 2001 and 2003, respectively, were quickly adopted. Bisphosphonate use peaked at age 80–89 years in females and 85–94 years in males, with 3-fold higher use in females than in males. Conclusions Pharmaceutical intervention for osteoporosis in Australia is increasing with most use in the elderly, the population at greatest risk of fracture. However, fracture prevalence in this population is considerably higher than prescribing of effective anti-osteoporosis medications, representing a missed opportunity for the quality use of medicines.

O cotidiano dos projetos sociais sobre esporte e lazer revela grupos de participantes que se identificam com as atividades lá desenvolvidas. Por outro lado, há pessoas avessas às atividades dos projetos, porque normalmente alguma característica as deixa desconfortáveis. Considerando essa realidade, o estudo procurou identificar os fatores que interferem na participação de homens idosos em programas de esporte e lazer. O estudo foi aplicado no bairro de Piedade, zona norte da cidade do Rio de Janeiro, onde foram entrevistados 10 idosos que residem próximo do projeto SUDERJ em FORMA. Para a coleta, processamento e apresentação dos dados utilizamos a abordagem radical das representações sociais proposta por Votre (2009), a partir do Discurso do Sujeito Coletivo (DSC) de Lefebvre et al (2007). O modelo de análise delineado valoriza tanto as pessoas individuais, em suas falas singulares, quanto a reunião dessas pessoas, que falam como grupo sobre tópicos que lhes são relevantes. Os resultados obtidos apontam que os fatores que contribuem para a ausência dos homens idosos nas atividades desenvolvidas pelo projeto são: a figura do professor e o método por ele aplicado; receio, vergonha e discriminação, em participar com mulheres e jovens; gosto por outras práticas não oferecidas pelo projeto. Embora toda a boa premissa relacionada aos benefícios da participação dos idosos em programas de esporte e lazer e perante a garantia de acesso a estes programas, no estudo ficou constatado que tais condições não são suficientes para atingir o público masculino de idosos. Consideramos que no processo de criação de novas estratégias e propostas de intervenção, é de extrema importância que ocorra um planejamento cuidadoso que identifique as dificuldades sociais e individuais que reduzam ou impeçam a participação do sujeito idoso nas atividades propostas. Concluímos, também, que reforçar os benefícios da prática da atividade física, oferecer atividades de lazer não ligadas à prática esportiva, oferecer atividades adaptadas e atividades exclusivas para os homens, são alternativas viáveis para atrair o homem idoso para o projeto.

Abstract Objective: Study the relationship between sex hormones and cognitive function in aged male. Methods: The serum sex hormones including Free Testosterone(FT), Total Testosterone(TT), Estradiol(E2), Prolactin(PRL) and Luteotropic Hormone(LH) of 74 elderly men(47-75 years old, mean 58.73) were measured by radioimmunoassay. They were all applied the cognitive tests of episodic memory and verbal fluency. For the episodic memory, four experiments examined picture, digit and words memory separately. Four verbal fluency tests were performed in the middle of each episodic memory. The 74 men were divided into two groups according to the levels of their sex hormones respectively, with each group had 37 subjects. Data was analyzed by the software of SPSS 11.0. Results: Mean age and the mean years of formal education had no significant difference between the groups; Men in the group with higher levels of FT had significantly higher scores in the tests of picture memory and words memory;Men in the group with higher levels of TT had significantly higher scores in the tests of verbal fluency4, picture memory and words memory;Men in the group with higher levels of E2 had significantly higher scores in the tests of picture memory and verbal fluency3,4;There were no correlations between PRL or LH and the cognitive tests. Conclusions: 1 Sex hormones (especially FT, TT, E2) did have effect on the cognitive function in aged male, higher levels of hormones related with higher scores of some cognitive tests. 2 Among the cognitive functions, picture memory and words memory performance seemed being effected by the sex hormones most.

Chez les personnes âgées, la dépression est un problème important en santé publique, à cause de sa prévalence élevée et de son association avec les incapacités fonctionnelles, la mortalité et l’utilisation des services. La plupart des études ont montré que le manque de relations sociales était associé à la dépression, mais les résultats ne sont pas clairs. Au Québec et au Canada, on possède peu de données sur la prévalence de la dépression chez les personnes âgées et de son association avec les relations sociales. Peu d’études ont examiné le rôle des relations sociales sur l’utilisation des services de santé par les personnes âgées déprimées. Le but de cette recherche était d’examiner le rôle des relations sociales dans la présence de la dépression et dans la consultation chez un professionnel de la santé des personnes âgées déprimées, au Québec. Plus spécifiquement, ce travail visait à : 1) examiner les associations entre les relations sociales et les troubles dépressifs selon la région de résidence; 2) examiner les associations différentielles des relations sociales sur la dépression des femmes et des hommes âgés; 3) examiner le rôle des relations sociales dans la consultation auprès d’un professionnel de la santé des personnes âgées déprimées. Pour répondre à ces objectifs, nous avons utilisé les données de l’enquête ESA (Enquête sur la Santé des Aînés), réalisée en 2005 -2006 auprès d’un échantillon de 2670 personnes âgées résidant à domicile au Québec, qui nous ont permis de rédiger trois articles. Les troubles dépressifs (incluant la dépression majeure et mineure) ont été mesurés, selon les critères du DSM-IV, en excluant le critère de l’altération du fonctionnement social, professionnel ou dans d’autres domaines importants, à l’aide du questionnaire ESA développé par l’équipe de recherche. Les relations sociales ont été mesurées à l’aide de cinq variables : (1) le réseau social; (2) l’intégration sociale; (3) le soutien social, (4) la perception d’utilité auprès des proches et (5) la présence de relations conflictuelles avec le conjoint, les enfants, les frères et sœurs et les amis. Des modèles de régression logistique multiple ont été ajustés aux données pour estimer les rapports de cote et leur intervalle de confiance à 95 %. Nos résultats ont montré des prévalences de dépression plus élevées chez les personnes qui résident dans les régions rurales et urbaines, comparées à celles qui résident dans la région métropolitaine de Montréal. La pratique du bénévolat, le soutien social et les relations non conflictuelles avec le conjoint sont associés à une faible prévalence de dépression, indépendamment du type de résidence. Comparés aux hommes, les femmes ont une prévalence de dépression plus élevée. L’absence de confident est associée à une prévalence de dépression élevée, tant chez les hommes que chez les femmes. La probabilité de dépression est plus élevée chez les hommes veufs et chez ceux qui ne pratiquent pas d’activités de bénévolat, comparativement à ceux qui sont mariés et font du bénévolat. Chez les femmes, aucune association significative n’a été observée entre le statut marital, le bénévolat et la dépression. Cependant, la présence de relations conflictuelles avec le conjoint est associée avec la dépression, seulement chez les femmes. Les relations avec les enfants, les frères et sœurs et les amis ne sont pas associées avec la dépression dans cette population de personnes âgées du Quebec. En ce qui concerne la consultation chez un professionnel de la santé, nos résultats ont révélé que presque la moitié des personnes âgées dépressives n’ont pas consulté un professionnel de la santé, pour leurs symptômes de dépression, au cours des 12 derniers mois. Par ailleurs, notre étude a montré que les personnes âgées qui disposent de tous les types de soutien (confident, émotionnel et instrumental) consultent plus pour leurs symptômes de dépression que ceux qui ont moins de soutien. Comparativement aux hommes mariés, les femmes mariées consultent plus les professionnels de la santé, ce qui laisse supposer que le réseau de proches (épouse et enfants) semble agir comme un substitut en réduisant la fréquence de consultation chez les hommes. Vu la rareté des études canadiennes sur la prévalence de la dépression chez les personnes âgées et les facteurs psychosociaux qui y sont associés, les résultats de ce travail seront utiles pour les cliniciens et pour les responsables des politiques à l’échelle nationale, provinciale et locale. Ils pourront guider des interventions spécifiques, selon la région de résidence et pour les hommes et les femmes âgées, dans le domaine de la santé mentale.

L’Étude sur la santé des aînés au Québec montre que les femmes âgées de 65 ans et plus ont un risque deux fois plus élevé de dépression et de troubles anxieux que les hommes âgés. La littérature scientifique des 40 dernières années suggère que le stress quotidien et le soutien social sont associés à la présence de ces troubles mentaux dans la communauté. L’objectif de la présente étude est de vérifier l’effet médiateur du stress quotidien et du soutien social sur l’association entre le genre et la dépression et les troubles anxieux. L’échantillon était composé d’individus de 65 ans et plus vivant en résidence privée au Québec. Des analyses de régression logistiques ont été menées afin d’identifier l’effet médiateur du stress et du soutien social en contrôlant pour des variables démographiques, socioéconomiques et de santé potentiellement confondantes. Les résultats de ces analyses ne permettent pas de conclure que le stress quotidien et le soutien social sont des variables médiatrices dans la relation entre le genre et la dépression ou entre le genre et les troubles anxieux chez les aînés. Puisque ces variables n’expliquent pas les différences de genre observées dans les prévalences de ces troubles psychiatriques d’autres avenues devront être étudiées.

Background: Isometric grip strength, evaluated with a handgrip dynamometer, is a marker of current nutritional status and cardiometabolic risk and future morbidity and mortality. We present reference values for handgrip strength in healthy young Colombian adults (aged 18 to 29 years). Methods: The sample comprised 5.647 (2.330 men and 3.317 women) apparently healthy young university students (mean age, 20.6±2.7 years) attending public and private institutions in the cities of Bogota and Cali (Colombia). Handgrip strength was measured two times with a TKK analogue dynamometer in both hands and the highest value used in the analysis. Sex- and age-specific normative values for handgrip strength were calculated using the LMS method and expressed as tabulated percentiles from 3 to 97 and as smoothed centile curves (P3, P10, P25, P50, P75, P90 and P97). Results: Mean values for right and left handgrip strength were 38.1±8.9 and 35.9±8.6 kg for men, and 25.1±8.7 and 23.3±8.2 kg for women, respectively. Handgrip strength increased with age in both sexes and was significantly higher in men in all age categories. The results were generally more homogeneous amongst men than women. Conclusions: Sex- and age-specific handgrip strength normative values among healthy young Colombian adults are defined. This information may be helpful in future studies of secular trends in handgrip strength and to identify clinically relevant cut points for poor nutritional and elevated cardiometabolic risk in a Latin American population. Evidence of decline in handgrip strength before the end of the third decade is of concern and warrants further investigation

La osteoporosis es una de las condiciones patológicas en mayor crecimiento a medida que la población de tercera edad aumenta, esto se traduce en fracturas por fragilidad como lo son las fracturas de radio distal y las fracturas de cadera, actualmente no se cuentas con datos de la población a estudio que correlacione este tipo de fracturas. Es un estudio retrospectivo de casos y controles donde se obtuvo un grupo de pacientes con fractura de cadera que consultaron a un hospital universitario de alta complejidad en la ciudad de Bogotá, se evaluó la presencia de antecedente de fractura de radio distal y se comparó con un grupo control de trauma en cadera. Se obtuvo un total de 325 casos (72,5%) y 123 (25%) controles. El promedio de edad fue de 81 años, el 70% de los pacientes en ambos grupos correspondió a mujeres. No hubo diferencia en cuanto a la prevalencia de tabaquismo, hipertensión arterial o diabetes en los grupos. No se encontraron diferencias significativas en cuanto a niveles de glicemia, calcio, vitamina D. La presencia de antecedente de fractura de radio distal en grupo con fractura de cadera fue del 7,1% encontrando un OR de 3,91 IC 95%(1,17– 13,10). La presencia de fractura de radio distal como antecedente es un predictor para la fractura de cadera en pacientes mayores. Se necesitan más estudios que correlacionen otras variables que pueden influir en la asociación para fractura de cadera y radio, para así identificar una población específica que se beneficie de un tratamiento temprano.
