Eye screening for diabetics - prevents blindness

Diabetes is a common condition affecting around 69,000 people in Northern Ireland. One of the possible complications of diabetes is a condition called diabetic retinopathy, which can cause sight loss and blindness. Retinopathy causes damage to the tiny blood vessels (capillaries) that nourish the retina, the tissues in the back of the eye that deal with light. This can seriously affect vision.Research shows that if retinopathy is identified early, for example through retinal screening, and treated appropriately, blindness can be prevented in the majority of people with diabetes, both type 1 and type 2.Screening programmeIn Northern Ireland, a diabetic retinopathy screening programme (DRSP), run by the Public Health Agency, has been put in place to screen all eligible people with diabetes aged 12 years and over. Dr Bernadette Cullen, Consultant in Public Health Medicine, PHA, said: "Screening detects problems early and allows appropriate treatment to be offered. It is vital that everyone with diabetes attends diabetic retinopathy screening when it is offered. Early detection of potential problems offers a very real opportunity to intervene and, with appropriate treatment, can prevent blindness in the majority of those at risk."The screening testThe screening test involves photographs being taken of the back of each eye, using a special camera. The test is painless and takes about 15 minutes. If the person is over 50 years of age, they will need to have drops put in their eyes about 15 minutes before the test to dilate their pupils.The photographs are sent to the regional screening centre for analysis by trained graders. Results will show whether patients require further referral for assessment or treatment by hospital eye services (HES). If this is not required, screening will be offered again the following year.GPs are informed of all results and if the patient is under the care of a diabetologist, they too will be informed. Patients are informed of results by their GP and if they need an urgent referral, protocols are in place to ensure this happens.Many people with diabetes attend their optometrist (optician) on a regular basis to have a sight test for glasses. It is important they continue to do this - this test is free to people with diabetes. It is also vital that people with diabetes attend for diabetic retinopathy screening when invited, regardless of how or where their diabetes is treated, or whether they visit a hospital consultant/GP for their diabetic care.Patients are invited to screening via their GP practice. An information leaflet to help patients make an informed decision to attend for screening is also sent. This can be accessed via the PHA website: www.publichealth.hscni.net.

Jyväskylän kaukolämpöverkon kehittäminen

Kaukolämmitys on yleisin Suomessa käytetty lämmitysmuoto ja sen osuus koko lämmitysmarkkinoista on noin 50 %. Kaukolämmön varsinainen tuotanto ja jakelu alkoivat Suomessa vuonna 1957 ja Jyväskylässä vuonna 1960. Jyväskylässä kaukolämmön piiriin kuuluivuoden 2006 loppuun mennessä noin 3200 asiakasta. Suurin tuntiteho laitoksilta oli noin 375 MW. Vuonna 2020 suurimman tuntitehon on ennustettu olevan noin 480 MW, mikä tarkoittaa asiakasmäärän ja verkon pituuden merkittävää kasvua. Jyväskylän kaukolämpöverkon nykytilanne on hyvä. Verkostolaskennan tulosten perusteella lämpö saadaan siirtymään nykyisillä laitoksilla ja verkon laitteilla jokaiselle asiakkaalle niin normaalitilanteessa kuin päälaitoksen häiriötilanteessa. Häiriötilanteessa paikallisia ongelmia voi esiintyä, mutta niihin voidaan vaikuttaa parantavasti esimerkiksi verkon staattisen paineen nostolla. Jyväskylän kaukolämpöverkon tulevaisuuden näkymät ovat erinomaiset. Laskennan perusteella verkkoa voidaan laajentaa kaikille laskennassa mukana olleille alueille ilman suurempia ongelmia. Uuden tuotantokapasiteetin, verkonlaajentumisen ja asiakasmäärän kasvun aiheuttamat kehitystarpeet saatiin selvitetyksi ja tulokset olivat aikaisempien tutkimusten suuntaisia. Tulevaisuutta ajatellen verkkoon joudutaan tekemään muutamia muutoksia suunniteltujen uusien yhteyksien lisäksi. Uuteen linjaan, joka yhdistää Palokan verkon toisen yhteyden kautta Jyväskylän pääverkkoon, rakennetaan mahdollisesti pumppaamo. Lisäkuristusventtiilejä tarvitaan viiteen eri paikkaan turvaamaan riittävän suuret kaukolämpöveden paluupaineet. Osa venttiileistä olisi suositeltavaa asentaa jo nykytilanteessa, sillä niiden hankinta parantaisi verkon nykytilaa.

rDock: A Fast, Versatile and Open Source Program for Docking Ligands to Proteins and Nucleic Acids

Identification of chemical compounds with specific biological activities is an important step in both chemical biology and drug discovery. When the structure of the intended target is available, one approach is to use molecular docking programs to assess the chemical complementarity of small molecules with the target; such calculations provide a qualitative measure of affinity that can be used in virtual screening (VS) to rank order a list of compounds according to their potential to be active. rDock is a molecular docking program developed at Vernalis for high-throughput VS (HTVS) applications. Evolved from RiboDock, the program can be used against proteins and nucleic acids, is designed to be computationally very efficient and allows the user to incorporate additional constraints and information as a bias to guide docking. This article provides an overview of the program structure and features and compares rDock to two reference programs, AutoDock Vina (open source) and Schrodinger's Glide (commercial). In terms of computational speed for VS, rDock is faster than Vina and comparable to Glide. For binding mode prediction, rDock and Vina are superior to Glide. The VS performance of rDock is significantly better than Vina, but inferior to Glide for most systems unless pharmacophore constraints are used; in that case rDock and Glide are of equal performance. The program is released under the Lesser General Public License and is freely available for download, together with the manuals, example files and the complete test sets, at http://rdock.sourceforge.net/

Le comportement coopératif à la lumière de deux conceptions de la moralité : devoir et choix

Nous nous proposons dans ce papier d'étudier la question de la coopération à la lumière de la moralité. Cela nous conduit à discerner deux formes de moralité, celle conçue comme un devoir (impératif catégorique de Kant) et celle conçue comme un choix (l'altruisme). Néanmoins, ces deux représentations se recoupent dans leur traitement de la coopération. Dans la mesure où elles sont formalisées dans le cadre d'un jeu statique, les résultats qui en ressortent ne nous disent pas plus qu'il est dans l'intérêt d'un individu de s'y soumettre lorsque les autres en ont fait autant. Toutefois, il n'est pas expliqué comment les autres en sont venus à adopter un comportement moral. Cela soulève en substance le problème de l'outil utilisé, celui-ci définissant les comportements à l'équilibre sans nous livrer la moindre information sur la situation qui prédominait avant d'arriver à l'équilibre.
