Graft-versus-host disease is observed mainly in recipients of hematopoietic cell transplantation and is expressed by cutaneous or systemic signals and symptoms. Graft-versus-host disease is clinically classified as acute or chronic. Chronic Graft-versus-host disease occurs in up to 70% of hematopoietic cell transplanted patients and its clinical manifestations have important impact on morbidity and quality of life. The authors report an expressive cutaneous, oral and adnexal involvement in a patient with chronic Graft-versus-host disease with multiple lesions of lichenoid and atrophic pattern.

The graft-versus-host disease is the major cause of morbidity and mortality in patients who have undergone hematopoietic stem cell transplantation. Aiming at contributing to the understanding of the role of myeloid and plasmacytoid dendritic cells, and natural killer cells in chronic graft-versus-host disease, we examined biopsies of jugal mucosa of 26 patients with acute myeloid leukemia who had undergone allogenic hematopoietic stem cell transplantation. Half of these patients developed oral chronic graft-versus-host disease. Microscopic sections were immunohistochemically stained for anti-CD1a, anti-CD123 and anti-CD56. We calculated the number of immunostained cells in the corium per square millimeter and applied the Mann-Whitney test. Results showed a statistically significant increase of myeloid dendritic cells (CD1a+; p=0,02) and natural killer cells (CD56; p=0,04) in patients with oral chronic graft-versus-host disease. CD123 immunostaining showed no statistical difference between groups. It was concluded that myeloid dendritic cells and natural killer cells participate in the development of oral chronic graft-versus-host disease.

Die Transplantation von allogenen hämatopoetischen Stammzellen stellt für viele Patienten mit hämatologischen Erkrankungen, wie beispielsweise akuter Leukämie, oftmals die einzige kurative Therapieoption dar. Die Erkennung von Empfängerantigenen durch immunkompetente Zellen des Spenders bietet dabei die Basis für erwünschte Graft-versus-Tumor-Effekte, verursacht jedoch häufig außerdem die unerwünschte Graft-versus-Host Disease (GvHD), eine mitunter schwerwiegende Komplikation. In der vorliegenden Arbeit wurden potentielle Mechanismen zur Hemmung alloreaktiver CD4+ und CD8+ T-Zellen (TZ) und folglich zur Hemmung der akuten GvHD in einem experimentellen GvHD-Modell untersucht, welches auf dem Transfer von allogenen Zellen zwischen MHC-inkompatiblen Mausstämmen basiert. Die vorliegende Arbeit weist zum Einen darauf hin, dass das Fehlen MyD88- und TRIF-vermittelter Toll-like-Rezeptor-Signale zumindest im Rahmen des hier verwendeten Transplantationsmodells nicht zwingend zu einer Hemmung der akuten GvHD führt. Zum Anderen konnte belegt werden, dass CD4+ CD25+ regulatorische T-Zellen (Tregs) kompetente Suppressoren der durch alloreaktive CD4+ und CD8+ TZ ausgelösten akuten GvHD darstellen. In weiterführenden Experimenten ist gezeigt worden, dass die Tregs sich verschiedener Mechanismen bedienen, um ihre Zielzellen zu inhibieren. Das suppressive Zytokin Interleukin-10 kann als löslicher Mediator zumindest in vitro offenbar eine Rolle bei der Treg-vermittelten Suppression alloreaktiver TZ spielen. Da jedoch auch Tregs aus Interleukin-10-defizienten Spendern die GvHD-Entstehung in den Empfängern abschwächen konnten, müssen noch weitere Mechanismen involviert sein. Es konnte in einer gemischten Leukozyten Reaktion in vitro eine zellkontaktabhängige Kommunikation mittels gap junctions hauptsächlich zwischen den Tregs und den allogenen Dendritischen Zellen (DCs) nachgewiesen werden, welche prinzipiell den Transfer von cAMP möglich macht. Die Kommunikation zwischen Tregs und DCs resultierte in einem supprimierten Phänotyp der DCs, gekennzeichnet durch eine verminderte Expression kostimulatorischer Moleküle auf ihrer Oberfläche. Solche supprimierten DCs können als Folge die alloreaktiven Spender-TZ vermutlich nicht aktivieren. Das cAMP-erhöhende Rolipram konnte in einer gemischten Leukozyten Reaktion in vitro die Proliferation alloreaktiver CD4+ und CD8+ TZ hemmen. Daneben konnte die Treg-vermittelte Suppression alloreaktiver TZ und der GvHD in vivo durch die zusätzliche Verabreichung von Rolipram noch gesteigert werden. Im letzten Kapitel dieser Arbeit wurde beschrieben, dass die alleinige Aktivierung alloreaktiver CD8+ TZ ausreichend ist, um eine akute GvHD auszulösen. In diesem Zusammenhang konnte nachgewiesen werden, dass CD4+ CD25+ Tregs die akute GvHD auch in einer scheinbar MHC-II-unabhängigen Weise hemmen können. Zusammenfassend belegt die vorliegende Arbeit, dass Tregs in einem MHC-inkompatiblen Transplantationsmodell alloreaktive CD4+ und CD8+ TZ und folglich die Entstehung einer GvHD effizient hemmen können. Bei der Hemmung der GvHD kommen wahrscheinlich verschiedene Mechanismen zum Tragen. Zumindest in vivo scheint von Tregs produziertes Interleukin-10 eine untergeordnete Rolle bei der Suppression alloreaktiver TZ und der GvHD zu spielen, hierbei steht vermutlich vielmehr der cAMP-abhängige Suppressionsmechanismus im Vordergrund.

Die allogene hämatopoetische Stammzelltransplantation ist bereits seit mehreren Jahrzehnten zur Therapie von Leukämien und anderen malignen Erkrankungen etabliert, aber ihre Effektivität wird durch Graft-versus-Host Reaktionen weiterhin deutlich eingeschränkt. Um die zu Grunde liegenden Mechanismen besser zu verstehen und Möglichkeiten zur Modulation zu untersuchen, wurden in dieser Arbeit verschiedene Ansätze verfolgt.rnRegulatorische T-Zellen sind in der Lage allogene T-Zell-Antworten, wie sie auch bei einer GvH-Erkrankung auftreten zu supprimieren. Es konnte gezeigt werden, dass dies unabhängig von Interleukin-10 geschieht, dafür jedoch ein kontaktabhängiger Mechanismus eine wichtige Rolle spielt. Dabei wird cAMP von Treg über Gap-Junctions in allogene Dendritische Zellen übertragen und deren Aktivierung dadurch verhindert. Versuche zur Modulation dieses Mechanismus mithilfe von Phosphodiesterase-Inhibitoren haben gezeigt, dass diese nicht nur die suppressiven Fähigkeiten von Treg verbessern, sondern ebenfalls direkt auf die T-Zellen einwirken, die schließlich die GvH-Erkrankung auslösen. Diese Ergebnisse konnten in vivo bestätigt werden und zeigen somit einen möglichen Ansatz hin zu einer kombinierten zellulären und pharmakologischen Therapie von GvH-Erkrankungen. Ein großer Vorteil dabei wäre, dass bereits eine Palette an PDE-Inhibitoren in der Klinik zur Verfügung steht.rnInterleukin-10 ist ein immunsuppressives und anti-inflammatorisches Zytokin, dem bei der Regulation des Immunsystems eine wichtige Rolle zukommt. Wie in dieser und anderen Arbeiten gezeigt, ist diese Funktion von IL-10 auch bei GvH-Erkrankungen essentiell. Ein Ziel war es daher, die Zellpopulationen, die für die Produktion des Zytokins verantwortlich sind, zu identifizieren. Mittels einer IL-10 Reporter-Maus konnten B-Zellen vom Spender, wie auch vom Empfänger als IL-10 Produzenten ausgemacht werden. Darüberhinaus zeigen die so gefundenen Zellen auch einen typischen Phänotyp für sog. immunregulatorische B-Zellen. Transplantationsexperimente mit Mäusen, die einen B-Zell-spezifischen Knock-out für IL-10 tragen, konnten die Relevanz der B Zellen als IL-10 Produzenten in vivo belegen.rnDendritische Zellen sind sehr potente Antigenpräsentierende Zellen und somit in der Lage GvH-Reaktionen zu induzieren. Überraschenderweise ist das Überleben von Versuchsmäusen, denen alle DC oder auch nur die BATF3-abhängige Subpopulation der CD8α+ DC fehlt, nicht besser als das des WT, sondern sogar deutlich schlechter. Dies geht einher mit entsprechenden Veränderungen im Zytokinmilieu der peripheren lymphatischen Organe. Bei Abwesenheit der CD8α+ DC sind die Zellen der mesenterialen Lymphknoten nach dem Konditionierungsprotokoll stärkere Stimulatoren für allogene T-Zell-Proliferation, was eine Erklärung für die stärkere GvH-Erkrankung ist. Eine Erklärung für diese Befunde liefert die verringerte Anzahl an Treg, die nach einer Transplantation in Abwesenheit der CD8α+ DC zu beobachten ist.rnDie aufgezeigten immunsupressiven Mechanismen stellen gute Ansatzpunkte dar, um GvH-Erkrankungen besser zu verstehen und damit die Effektivität der allogenen hämatopoetischen Stammzelltransplantation zu verbessern.rn

Chronic graft-versus-host disease (cGVHD), a common complication after stem cell transplant (SCT), has an impact on morbidity and survival. Previous classification of cGVHD has not been reproducible or prognostic for nonrelapse mortality (NRM). Recently the National Institutes of Health (NIH) consensus criteria were proposed, but the ability of this classification to predict outcome of various subtypes of cGVHD is unknown. Patients (N = 110) undergoing an SCT for a hematologic malignancy and surviving until day 100 posttransplant from 2001 to 2003 were studied. The overall survival (OS) using a landmark analysis at day 100 was 44% versus 66% (no GVHD vs. GVHD, P = .026). The OS of patients with various types of GVHD as proposed by the NIH criteria were significantly different (P < .0001). In a univariate analyses, this was more apparent when patients with any acute features of GVHD were compared to classic cGVHD (3-year OS 46% vs. 68%, P = .033). The 3-year NRM for the entire cohort was 21%, and was not affected by presence or absence of GVHD or subtypes of GVHD. In a multivariable analysis, extensive cGVHD (hazard ratio [HR] 0.35, P = .015) and having any acute feature of GVHD after day 100 (HR 3.36, P = .0144) were significant independent predictors of survival. The OS with different NIH subtypes of GVHD after day 100 from SCT varies, and is superior for patients with classic cGVHD.

RATIONALE: Pulmonary complications of hematopoietic stem cell transplantation include infections and graft-versus-host diseases, such as idiopathic pneumonia syndrome (IPS). Conflicting data exist regarding the role of the interferon (IFN)-gamma-producing Th1 CD4(+) T-cell subset and IL-17A in IPS. OBJECTIVES: To determine the role of IFN-gamma and IL-17A in the establishment of pulmonary graft-versus-host disease. METHODS: A semiallogeneic murine model based on C57BL/6 x BALB/c as recipients with transplantation of BALB/c RAG2(-/-) bone marrow and transfer of different genetic knockout T cells (T-bet(-/-), IFN-gamma(-/-), IFN-gammaR(-/-)) on a BALB/c background. Lung tissue was examined for parenchymal changes and infiltrating cells by histology and fluorescence-activated cell sorter analysis. MEASUREMENTS AND MAIN RESULTS: After transfer of semiallogeneic bone marrow together with donor CD4(+) T cells lacking IFN-gamma or T-bet-a T-box transcription factor controlling Th1 commitment-we found severe inflammation in the lungs, but no enhancement in other organs. In contrast, wild-type donor CD4(+) T cells mediated minimal inflammation only, and donor CD8(+) T cells were not required for IPS development. Mechanistically, the absence of IFN-gamma or IFN-gamma signaling in pulmonary parenchymal cells promoted expansion of IL-17A-producing CD4(+) T cells and local IL-17A release. In vivo depletion of IL-17A reduced disease severity. CONCLUSIONS: One mechanism of IFN-gamma protection against IPS is negative regulation of the expansion of pathogenic IL-17A-producing CD4(+) T cells through interaction with the IFN-gamma receptor on the pulmonary parenchymal cell population.

In this single-center, cross-sectional study, we evaluated 44 very long-term survivors with a median follow-up of 17.5 years (range, 11-26 years) after hematopoietic stem cell transplantation. We assessed the telomere length difference in human leukocyte antigen-identical donor and recipient sibling pairs and searched for its relationship with clinical factors. The telomere length (in kb, mean +/- SD) was significantly shorter in all recipient blood cells compared with their donors' blood cells (P < .01): granulocytes (6.5 +/- 0.9 vs 7.1 +/- 0.9), naive/memory T cells (5.7 +/- 1.2 vs 6.6 +/- 1.2; 5.2 +/- 1.0 vs 5.7 +/- 0.9), B cells (7.1 +/- 1.1 vs 7.8 +/- 1.1), and natural killer/natural killer T cells (4.8 +/- 1.0 vs 5.6 +/- 1.3). Chronic graft-versus-host disease (P < .04) and a female donor (P < .04) were associated with a greater difference in telomere length between donor and recipient. Critically short telomeres have been described in degenerative diseases and secondary malignancies. If this hypothesis can be confirmed, identification of recipients at risk for cellular senescence could become part of monitoring long-term survivors after hematopoietic stem cell transplantation.

The National Institutes of Health (NIH) classification of graft-versus-host disease (GVHD) is a significant improvement over prior classifications, and has prognostic implications. We hypothesized that the NIH classification of GVHD would predict the survival of patients with GVHD treated with extracorporeal photopheresis (ECP). Sixty-four patients with steroid refractory/dependent GVHD treated with ECP were studied. The 3-year overall survival (OS) was 36% (95% confidence interval [CI] 13-59). Progressive GVHD was seen in 39% of patients with any acute GVHD (aGVHD) (classic acute, recurrent acute, overlap) compared to 3% of patients with classic chronic GVHD (cGVHD) (P=.002). OS was superior for patients with classic cGVHD (median survival, not reached) compared to overlap GVHD (median survival, 395 days, 95% CI 101 to not reached) and aGVHD (delayed, recurrent or persistent) (median survival, 72 days, 95% CI 39-152). In univariate analyses, significant predictors of survival after ECP included GVHD subtype, bilirubin, platelet count, and steroid dose. In multivariate analyses overlap plus classic cGVHD was an independent prognostic feature predictive of superior survival (hazard ratio [HR] 0.34, 95% CI 0.14-0.8, p=.014). This study suggests that NIH classification can predict outcome after ECP for steroid refractory/dependent GVHD.

IMMUNOLOGICAL MECHANISMS OF EXTRACORPOREAL PHOTOPHERESIS IN CUTANEOUS T CELL LYMPHOMA AND GRAFT VERSUS HOST DISEASE Publication No.___________ Lisa Harn-Ging Shiue, B.S. Supervisory Professor: Madeleine Duvic, M.D. Extracorporeal photopheresis (ECP) is an effective, low-risk immunomodulating therapy for leukemic cutaneous T cell lymphoma (L-CTCL) and graft versus host disease (GVHD), but whether the mechanism(s) of action in these two diseases is (are) identical or different is unclear. To determine the effects of ECP in vivo, we studied regulatory T cells (T-regs), cytotoxic T lymphocytes (CTLs), and dendritic cells (DCs) by immunofluorescence flow cytometry in 18 L-CTCL and 11 GVHD patients before and after ECP at Day 2, 1 month, 3 months, and 6 months. In this study, ECP was effective in 12/18 L-CTCL patients with a 66.7% overall response rate (ORR) and 6/11 GVHD patients with a 54.5% ORR. Prior to ECP, the percentages of CD4+Foxp3+ T cells in 9 L-CTCL patients were either lower (L-CTCL-Low, n=2) or higher (L-CTCL-High, n=7) than normal. Five of the 7 GVHD patients had high percentages of CD4+Foxp3+ T cells (GVHD-High). Six of 7 L-CTCL-High patients had >80% CD4+Foxp3+ T cells which were correlated with tumor cells, and were responders. Both L-CTCL-High and GVHD-High patients had decreased percentages of CD4+Foxp3+ and CD4+Foxp3+CD25- T cells after 3 months of treatment. CD4+Foxp3+CD25+ T cells increased in GVHD-High patients but decreased in L-CTCL-High patients after 3 months of ECP. In addition, numbers of CTLs were abnormal. We confirmed that numbers of CTLs were low in L-CTCL patients, but high in GVHD patients prior to ECP. After ECP, CTLs increased after 1 month in 4/6 L-CTCL patients whereas CTLs decreased after 6 months in 3/3 GVHD patients. Myeloid (mDCs) and plasmacytoid DCs (pDCs) were also low at baseline in L-CTCL and GVHD patients confirming the DC defect. After 6 months of ECP, numbers and percentages of mDCs and pDCs increased in L-CTCL and GVHD. MDCs were favorably increased in 8/12 L-CTCL responders whereas pDCs were favorably increased in GVHD patients. These data suggest that ECP is favorably modulating the DC subsets. In L-CTCL patients, the mDCs may orchestrate Th1 cell responses to overcome immune suppression and facilitate disease regression. However, in GVHD patients, ECP is favorably down-regulating the immune system and may be facilitating immune tolerance to auto-or allo-antigens. In both L-CTCL and GVHD patients, DCs are modulated, but the T cell responses orchestrated by the DCs are different, suggesting that ECP modulates depending on the immune milieu. _______________

Graft-versus-host disease (GVHD) is a T-cell-mediated disease of transplanted donor T cells recognizing host alloantigens. Data presented in this report show, to our knowledge, for the first time that a synthetic copolymer of the amino acids L-Glu, L-Lys, L-Ala, and L-Tyr (molecular ratio, 1.9:6.0:4.7:1.0; Mr, 6000-8500) [corrected], termed GLAT, with promiscuous binding to multiple major histocompatibility complex class II alleles is capable of preventing lethal GVHD in the B10.D2 --> BALB/c model (both H-2d) across minor histocompatibility barriers. Administration of GLAT over a limited time after transplant significantly reduced the incidence, onset, and severity of disease. GLAT also improved long-term survival from lethal GVHD: 14/25 (56%) of experimental mice survived > 140 days after transplant compared to 2/26 of saline-treated or to 1/10 of hen egg lysozyme-treated control mice (P < 0.01). Long-term survivors were documented to be fully chimeric by PCR analysis of a polymorphic microsatellite region in the interleukin 1beta gene. In vitro, GLAT inhibited the mixed lymphocyte culture in a dose-dependent fashion across a variety of major barriers tested. Furthermore, GLAT inhibited the response of nylon wool-enriched T cells to syngeneic antigen-presenting cells presenting minor histocompatibility antigens. Prepulsing of the antigen-presenting cells with GLAT reduced the proliferative response, suggesting that GLAT inhibits antigen presentation.

The granulocyte colony-stimulating factor (G-CSF) and Fit-3 receptor agonist progenipoietin-1 (ProGP-1) has potent effects on dendritic cell (DC) expansion and may be an alternative to G-CSF for the mobilization of stem cells for allogeneic stem cell transplantation (SCT). We studied the ability of stem cell grafts mobilized with this agent to induce graft-versus-host disease (GVHD) to minor and major histocompatibility antigens in the well-described B6 --> B6D2F1 SCT model. ProGP-1, G-CSIF, or control diluent was administered to donor B6 mice. ProGP-1 expanded all cell lineages in the spleen, and unseparated splenocytes from these animals produced large amounts of interleukin 10 (IL-10) and transforming growth factor beta (TGFbeta) whereas the expression of T-cell adhesion molecules was diminished. Transplantation survival was 0%, 50%, and 90% in recipients of control-, G-CSF-, and ProGP-1-treated allogeneic donor splenocytes, respectively (P < .0001). Donor pretreatment with ProGP-1 allowed a 4-fold escalation in T-cell dose over that possible with G-CSF. Donor CD4 T cells from allogeneic SCT recipients of ProGP-1 splenocytes demonstrated an anergic response to host antigen, and cytokine production (interferon gamma [IFNγ], IL-4, and IL-10) was also reduced while CD8 T-cell cytotoxicity to host antigens remained intact. Neither CD11c(hi) DCs nor CD11c(dim)/B220(hi) DCs from ProGP-1-treated animals conferred protection from GVHD when added to control spleen. Conversely, when equal numbers of purified T cells from control-, G-CSF-, or ProGP-1-treated allogeneic donors were added to allogeneic T-cell-depleted control spleen, survival at day 60 was 0%, 15%, and 90%, respectively (P < .0001). The improved survival in recipients of ProGP-1 T cells was associated with reductions in systemic tumor necrosis factor alpha generation and GVHD of the gastrointestinal tract. We conclude that donor pretreatment with ProGP-1 is superior to G-CSIF for the prevention of GVHD after allogeneic SCT, primarily due to incremental affects on T-cell phenotype and function

The use of granulocyte colony-stimulating factor (G-CSF)-mobilized peripheral blood as a source of stem cells has resulted in a high incidence of severe chronic graft-versus-host disease (cGVHD), which compromises the outcome of clinical allogeneic stem cell transplantation. We have studied the effect of G-CSF on both immune complex and fibrotic cGVHD directed to major (DBA/2 --> B6D2F1) or minor (B10.D2 --> BALB/c) histocompatibility antigens. In both models, donor pretreatment with G-CSF reduced cGVHD mortality in association with type 2 differentiation. However, after escalation of the donor T-cell dose, scleroderma occurred in 90% of the recipients of grafts from G-CSF-treated donors. In contrast, only 11% of the recipients of control grafts developed scleroderma, and the severity of hepatic cGVHD was also reduced. Mixing studies confirmed that in the presence of high donor T-cell doses, the severity of scleroderma was determined by the non-T-cell fraction of grafts from G-CSF-treated donors. These data confirm that the induction of cGVHD after donor treatment with G-CSF is dependent on the transfer of large numbers of donor T cells in conjunction with a putatively expanded myeloid lineage, providing a further rationale for the limitation of cell dose in allogeneic stem cell transplantation. Transfusion-associated graft-versus-host disease (TA-GVHD) is a rare complication of transfusion of nonirradiated blood components. It usually affects children in high-risk groups, including those who have primary immunodeficiencies (PIDs). It usually presents with skin, hepatic, digestive, and hematologic involvement and is normally fatal.
