BACKGROUND: Treatment of patients with severe liver dysfunction including hyperbilirubinemia secondary to liver metastases of gastrointestinal (GI) cancer is challenging. Regimen of oxaliplatin and fluoropyrimidine (FP)/folinic acid (FA) ± a monoclonal antibody (moAb), represents a feasible option considering the pharmacokinetics. Clinical data on the respective dosage and tolerability are limited and no recommendations are available. METHODS: Consecutive patients with severe hyperbilirubinemia [>2 × upper limit of the normal range (ULN) and >2.4 mg/dl] due to liver metastases of GI cancer without options for drainage receiving oxaliplatin, FP/FA ± moAb were analyzed. To collect further data a review of the literature was performed. RESULTS: A total of 12 patients were identified between 2011 and 2015. At treatment start, median bilirubin level was 6.1 mg/dl (>5 × ULN, range 2.7-13.6). The majority of patients (n = 11) received dose-reduced regimen with oxaliplatin (60-76%) and FP/FA (0-77%), rapidly escalating to full dose regimen. During treatment, bilirubin levels dropped more than 50% within 8 weeks or normalized within 12 weeks in 6 patients (responders). Median overall survival was 5.75 months (range 1.0-16.0 months) but was significantly prolonged in responders compared to nonresponders [9.7 and 3.0 months, p = 0.026 (two-sided test); 95% confidence interval (CI): 1.10-10.22]. In addition, case reports or series comprising a further 26 patients could be identified. Based on the obtained data a treatment algorithm was developed. CONCLUSION: Treatment with oxaliplatin, FP/FA ± moAb is feasible and may derive relevant benefits in patients with severe liver dysfunction caused by GI cancer liver metastases without further options of drainage.

Knowledge-based radiation treatment is an emerging concept in radiotherapy. It
mainly refers to the technique that can guide or automate treatment planning in
clinic by learning from prior knowledge. Dierent models are developed to realize
it, one of which is proposed by Yuan et al. at Duke for lung IMRT planning. This
model can automatically determine both beam conguration and optimization ob-
jectives with non-coplanar beams based on patient-specic anatomical information.
Although plans automatically generated by this model demonstrate equivalent or
better dosimetric quality compared to clinical approved plans, its validity and gener-
ality are limited due to the empirical assignment to a coecient called angle spread
constraint dened in the beam eciency index used for beam ranking. To eliminate
these limitations, a systematic study on this coecient is needed to acquire evidences
for its optimal value.
To achieve this purpose, eleven lung cancer patients with complex tumor shape
with non-coplanar beams adopted in clinical approved plans were retrospectively
studied in the frame of the automatic lung IMRT treatment algorithm. The primary
and boost plans used in three patients were treated as dierent cases due to the
dierent target size and shape. A total of 14 lung cases, thus, were re-planned using
the knowledge-based automatic lung IMRT planning algorithm by varying angle
spread constraint from 0 to 1 with increment of 0.2. A modied beam angle eciency
index used for navigate the beam selection was adopted. Great eorts were made to assure the quality of plans associated to every angle spread constraint as good
as possible. Important dosimetric parameters for PTV and OARs, quantitatively
re
ecting the plan quality, were extracted from the DVHs and analyzed as a function
of angle spread constraint for each case. Comparisons of these parameters between
clinical plans and model-based plans were evaluated by two-sampled Students t-tests,
and regression analysis on a composite index built on the percentage errors between
dosimetric parameters in the model-based plans and those in the clinical plans as a
function of angle spread constraint was performed.
Results show that model-based plans generally have equivalent or better quality
than clinical approved plans, qualitatively and quantitatively. All dosimetric param-
eters except those for lungs in the automatically generated plans are statistically
better or comparable to those in the clinical plans. On average, more than 15% re-
duction on conformity index and homogeneity index for PTV and V40, V60 for heart
while an 8% and 3% increase on V5, V20 for lungs, respectively, are observed. The
intra-plan comparison among model-based plans demonstrates that plan quality does
not change much with angle spread constraint larger than 0.4. Further examination
on the variation curve of the composite index as a function of angle spread constraint
shows that 0.6 is the optimal value that can result in statistically the best achievable
plans.

Better outcomes of the patients receiving liver transplantation for viral hepatitis and hepatocellular carcinoma (HCC) are achieved by improved patient selection and perioperative treatment with antiviral agents including lamivudine, ribavirin and interferon. Patient selection is accomplished by high-quality imaging as well as exclusion of patients with large tumors, obvious extrahepatic disease or macroscopic vascular invasion. Using such criteria, a 5-year survival of 92% has been reached in the Queensland Liver Transplant Service on a small number of highly selected patients with HCC. The treatment algorithm of Makuuchi has guided us in recommending resection, estimating to what extent the liver resection can be performed safely, and timing liver transplantation when it is the only option. Adult-to-adult living-donor liver transplantation is being performed safely in many centers worldwide. The transplantation of liver from living donors to HCC patients, when standard criteria for the likelihood of good outcomes are fulfilled, will increase in Japan in the near future. FUNDAMENTO: Estados congestivos podem ser identificados e manejados através de algoritmos como o Diuretic Treatment Algorithm (DTA) para ajuste de diurético por telefone, com enfoque na avaliação clínica. Porém, o DTA está disponível somente em língua inglesa. OBJETIVO: Adaptar o DTA e testar sua aplicabilidade para uso no Brasil em pacientes ambulatoriais com IC. MÉTODOS: Seguiram-se as etapas de tradução, síntese, retrotradução, avaliação por comitê de especialistas e pré-teste (aplicabilidade clínica por meio de ensaio clínico randomizado). O DTA foi denominado, na versão para o Brasil, algoritmo de ajuste de diurético (AAD). Os pacientes foram randomizados para grupo intervenção (GI) - ajuste de diurético conforme o AAD - ou grupo controle (GC) - ajuste convencional. Foi avaliado o escore clínico de congestão (ECC) e o peso para ambos os grupos. RESULTADOS: Foram realizadas 12 modificações no DTA. Incluíram-se 34 pacientes. Para aqueles congestos, o aumento de diurético guiado pelo AAD resultou em maior resolução da congestão, com redução de dois pontos no ECC para 50% da amostra -2 (-3,5; -1,0), enquanto a mediana para o GC foi 0 (-1,25; -1,0), (p < 0,001). A mediana de variação de peso foi maior no GI -1,4 (-1,7; -0,5) quando comparado ao GC 0,1 (1,2; -0,6), p = 0,001. CONCLUSÕES: O ADD mostrou-se aplicável na prática clínica após adaptação e parece resultar em melhor controle da congestão em pacientes com IC. A efetividade clínica da ferramenta merece ser testada em amostra maior de pacientes visando sua validação para uso no Brasil (Universal Trial Number: U1111-1130-5749) (Arq Bras Cardiol. 2013; [online]. ahead print, PP.0-0).

INTRODUCTION: Trialing for intrathecal pump placement is an essential part of the decision-making process in placing a permanent device. In both the United States and the international community, the proper method for trialing is ill defined. METHODS: The Polyanalgesic Consensus Conference (PACC) is a group of well-published experienced practitioners who meet to update the state of care for intrathecal therapies on the basis of current knowledge in the literature and clinical experience. Anexhaustive search is performed to create a base of information that the panel considers when making recommendations for best clinical practices. This literature, coupled with clinical experience, is the basis for recommendations and for identification of gaps in the base of knowledge regarding trialing for intrathecal pump placement. RESULTS: The panel has made recommendations for the proper methods of trialing for long-term intrathecal drug delivery. CONCLUSION: The use of intrathecal drug delivery is an important part of the treatment algorithm for moderate to severe chronic pain. It has become common practice to perform a temporary neuroaxial infusion before permanent device implantation. On the basis of current knowledge, the PACC has developed recommendations to improve care. The need to update these recommendations will be very important as new literature is published.

Background: In recent years, an increasing number of auto-antibodies (AB) have been detected in the CSF and serum of patients with new onset epilepsy. Some of these patients develop convulsive or nonconvulsive status epilepticus (AB-SE), necessitating intensive medical care and administration of multiple antiepileptic and immunomodulatory treatments of uncertain effectiveness. Objectives: In this retrospective multicenter survey we aimed to determine the spectrum of gravity, the duration and the prognosis of the disorder. In addition, we sought to identify the antibodies associated with this condition, as well as determine whether there is a most effective treatment regime. Methods: 12 European Neurology University Clinics, with extensive experience in the treatment of SE patients, were sent a detailed questionnaire regarding symptoms and treatment of AB-SE patients. Seven centers responded positively, providing a total of 13 patients above the age of 16. Results: AB-SE affects mainly women (12/13, 92%) with a variable age at onset (17-69 years, median: 25 years). The duration of the disease is also variable (10 days to 12 years, median: 2 months). Only the 3 oldest patients died (55-69 years). Most patients were diagnosed with anti NMDAR encephalitis (8/13) and had oligoclonal bands in the CSF (9/13). No specific treatment regimen (antiepileptic, immunomodulatory) was found to be clearly superior. Most of the surviving 10 patients (77%) recovered completely or nearly so within 2 years of index poststatus. Conclusion: AB-SE is a severe but potentially reversible condition. Long duration does not seem to imply fatal outcome; however, age older than 50 years at time of onset appears to be a risk factor for death. There was no evidence for an optimal antiepileptic or immunomodulatory treatment. A prospective multicenter study is warranted in order to stratify the optimal treatment algorithm, determine clear risk factors of unfavorable outcome and long-term prognosis.

[The causal treatment of a residual hip dysplasia consists of an acetabular reorientation osteotomy, the results of which, however, strongly depend on concomitant intra-articular lesions. The most important prognostic factor is the cartilage status. However, tears in the labrum and the ligament of the head of the femur may also be responsible for symptoms. Hip arthroscopy is a valuable tool for a precise diagnosis and simultaneous treatment of these lesions. However, as yet no results supporting this treatment algorithm have been published.]

Elbow arthroplasty is increasingly performed in patients with rheumatic and post-traumatic arthritis. Data on elbow periprosthetic joint infection (PJI) are limited. We investigated the characteristics and outcome of elbow PJI in a 14-year cohort of total elbow arthroplasties in a single centre. Elbow prosthesis, which were implanted between 1994 and 2007 at Schulthess Clinic in Zurich, were retrospectively screened for infection. PJI was defined as periprosthetic purulence, the presence of sinus tract or microbial growth. A Kaplan-Meier survival method and Cox proportional hazard analysis were performed. Of 358 elbow prostheses, PJI was identified in 27 (7.5%). The median patient age (range) was 61 (39-82) years; 63% were females. Seventeen patients (63%) had a rheumatic disorder and ten (37%) had osteoarthritis. Debridement and implant retention was performed in 78%, followed by exchange or removal of the prosthesis (15%) or no surgery (7%).The relapse-free survival (95% CI) was 79% (63-95%) after 1 year and 65% (45-85%) after 2 years. The outcome after 2 years was significantly better when patients were treated according to the algorithm compared to patients who were not (100% vs. 33%, p <0.05). In 21 patients treated with debridement and retention, the cure rate was also higher when the algorithm was followed (100% vs. 11%, p <0.05). The findings of the present study suggest that the treatment algorithm developed for hip and knee PJI can be applied to elbow PJI. With proper patient selection and antimicrobial therapy, debridement and retention of the elbow prosthesis is associated with good treatment outcome.

OPINION STATEMENT: • In acute spinal cord ischemia syndrome (ASCIS), treatment recommendations are derived from data of cerebral ischemic stroke, atherosclerotic vascular disease and acute spinal cord injury. Besides acute management, secondary prevention is of major importance. Pathologies affecting the aorta as well as underlying cerebrovascular conditions should be treated whenever possible.• ASCIS may occur after aortic surgery, less often after thoracic endovascular aortic repair (TEVAR). Protocols are proposed.• Acute spinal cord hemorrhage can be treated surgically and/or pharmacologically.• Symptomatic treatment in patients with a spinal cord lesion is of major importance. Depending on level and extension of the lesion, there is a risk for systemic and neurological complications, which may be life-threatening.• Each spinal vascular malformation is a unique lesion that needs an individualized treatment algorithm. In case of a symptomatic vascular malformation, endovascular intervention is the primary treatment option.• Spinal dural Arteriovenous fistula (AVF) may be treated endovascularly or surgically. If preoperative localization of the fistula is possible, surgery is feasible with a low complication rate. In comparison, endovascular approaches are less invasive.• Spinal AVM are rather treated endovascularly than surgically or in a stepwise multidisciplinary approach.• Symptomatic and exophytic spinal cavernous angiomas should be treated surgically. Deep spinal cavernous angiomas that are asymptomatic or only show mild symptoms can be observed.

In contrast to the treatment of avulsion lesions of the anterior cruciate ligament (ACL) the management of intrasubstance ACL tears in the skeletally immature patient remains controversial. Prospective studies could show that conservative treatment results in severe instability with concomitant intraarticular damage and poor function of the knee. Reconstruction of a torn ACL always carries the risk of damaging the open growth plates; with consecutively affecting the longitudinal or axial growth of the lower extremity either on the femoral or the tibial side. Thus, several surgical procedures are available to prevent adverse events mentioned above. The purpose of this study is to review the recent literature regarding the treatment algorithm for ACL injuries in skeletally immature patients. This review will (1) investigate the indications for ACL surgery in children; (2) determine if a surgical procedure is clinically superior in skeletally immature patients; and (3) correlate the adverse events with the surgical technique.

The minimally invasive approach for hysterectomy with proven benefits and lower morbidity has become the gold standard, even in women with large uterine masses. Most women with a malignant condition present with abnormal vaginal bleeding and/or suspicious imaging such that few are diagnosed by final histopathology after surgery. However, if a malignancy is not diagnosed preoperatively, intraabdominal morcellation for uterus extraction has an increased risk for potential tumor spread and peritoneal metastases, especially in cases of unexpected leiomyosarcoma. We describe a simple method to wrap the uterus in a contained environment with a plastic bag through the posterior vaginal fornix prior to conventional coring morcellation for vaginal extraction in total laparoscopic hysterectomy. We further describe our experience with a risk stratification and treatment algorithm to implement this procedure in daily routine. A video and an illustrating sketch demonstrate the simplicity and safety of the procedure.

BACKGROUND & AIMS European and American guidelines have endorsed the Barcelona Clinic Liver Cancer (BCLC) staging system. The aim of this study was to assess the performance of the recently developed Hong Kong Liver Cancer (HKLC) classification as a staging system for hepatocellular carcinoma (HCC) in Europe. METHODS We used a pooled set of 1693 HCC patients combining three prospective European cohorts. Discrimination ability between the nine substages and five stages of the HKLC classification system was assessed. To evaluate the predictive power of the HKLC and BCLC staging systems on overall survival, Nagelkerke pseudo R2, Bayesian Information Criterion and Harrell's concordance index were calculated. The number of patients who would benefit from a curative therapy was assessed for both staging system. RESULTS The HKLC classification in nine substages shows suboptimal discrimination between the staging groups. The classification in five stages shows better discrimination between groups. However, the BCLC classification performs better than the HKLC classification in the ability to predict OS. The HKLC treatment algorithm tags significantly more patients to curative therapy than the BCLC. CONCLUSIONS The BCLC staging system performs better for European patients than the HKLC staging system in predicting OS. Twice more patients are eligible for a curative therapy with the HKLC algorithm, whether this translates in survival benefit remains to be investigated. This article is protected by copyright. PURPOSE: Patients with chronic depression (CD) by definition respond less well to standard forms of psychotherapy and are more likely to be high utilizers of psychiatric resources. Therefore, the aim of this guidance paper is to provide a comprehensive overview of current psychotherapy for CD. The evidence of efficacy is critically reviewed and recommendations for clinical applications and research are given. METHODS: We performed a systematic literature search to identify studies on psychotherapy in CD, evaluated the retrieved documents and developed evidence tables and recommendations through a consensus process among experts and stakeholders. RESULTS: We developed 5 recommendations which may help providers to select psychotherapeutic treatment options for this patient group. The EPA considers both psychotherapy and pharmacotherapy to be effective in CD and recommends both approaches. The best effect is achieved by combined treatment with psychotherapy and pharmacotherapy, which should therefore be the treatment of choice. The EPA recommends psychotherapy with an interpersonal focus (e.g. the Cognitive Behavioural Analysis System of Psychotherapy [CBASP]) for the treatment of CD and a personalized approach based on the patient's preferences. DISCUSSION: The DSM-5 nomenclature of persistent depressive disorder (PDD), which includes CD subtypes, has been an important step towards a more differentiated treatment and understanding of these complex affective disorders. Apart from dysthymia, ICD-10 still does not provide a separate entity for a chronic course of depression. The differences between patients with acute episodic depression and those with CD need to be considered in the planning of treatment. Specific psychotherapeutic treatment options are recommended for patients with CD. CONCLUSION: Patients with chronic forms of depression should be offered tailored psychotherapeutic treatments that address their specific needs and deficits. Combination treatment with psychotherapy and pharmacotherapy is the first-line treatment recommended for CD. More research is needed to develop more effective treatments for CD, especially in the longer term, and to identify which patients benefit from which treatment algorithm.

%WL: porcentaje de pérdida de peso; %FL: porcentaje de pérdida de grasa. Objetivo: describir la metodología en la gestión del tratamiento dietético cuali-cuantitativo en el sobrepeso y la obesidad. Método: se llevaron a cabo 4.625 consultas con 616 pacientes mayores de 25 años con sobrepeso y obesidad, en el sur-este de España, durante los años 2006-12. Se utilizó una dieta equilibrada, hipocalórica, cuali-cuantitativa con alimentos de la zona. Se describió la metodología del tratamiento dietético en la pérdida y el mantenimiento. Se trató qué unidades de medida son adecuadas para la expresión del éxito en la pérdida; una nueva visión del asesoramiento individualizado y el tratamiento multidisciplinar. Resultados: el 80% de los pacientes obtienen un %FL ≥ 5% (22,6 ± 11,8 - 11,2 ± 7,4), y asistieron a la consulta más de mes y medio. Conclusión: se presenta un esquema de la metodología del tratamiento dietético; se han recomendado las unidades de medidas a utilizar en consulta y en la publicación de ensayos clínicos, creando un precedente con grado de evidencia de cómo determinar la pérdida exitosa; se recomienda medir los perímetros de cadera y cintura, e incorporar el estudio de la imagen corporal; se presenta una nueva visión del asesoramiento individualizado y una atención multidisciplinar avanzada, independiente de la edad, el estado de gestación y las minusvalías físicas. La postura del sanitario debe de ser considerada como el gestor encargado de determinar qué técnicas podrían ser más efectivas en dicha pérdida.
