The novel multidomain organization in the multimeric Escherichia coli AHAS I (ilvBN) enzyme has been dissected to generate polypeptide fragments. These fragments when cloned, expressed and purified reassemble in the presence of cofactors to yield a catalytically competent enzyme. Structural characterization of AHAS has been impeded due to the fact that the holoenzyme is prone to dissociation leading to heterogeneity in samples. Our approach has enabled the structural characterization using high-resolution nuclear magnetic resonance methods. Near complete sequence specific NMR assignments for backbone H-N, N-15, C-13 alpha and C-13(beta) atoms of the FAD binding domain of ilvB have been obtained on samples isotopically enriched in H-2, C-13 and N-15. The secondary structure determined on the basis of observed C-13(alpha) secondary chemical shifts and sequential NOEs indicates that the secondary structure of the FAD binding domain of E. coli AHAS large Subunit (ilvB) is similar to the structure of this domain in the catalytic subunit of yeast AHAS. Protein-protein interactions involving the regulatory subunit (ilvN) and the domains of the catalytic subunit (ilvB) were studied using circular dichroic and isotope edited solution nuclear magnetic resonance spectroscopic methods. Observed changes in circular dichroic spectra indicate that the regulatory subunit (ilvN) interacts with ilvB alpha and ilvB beta domains of the catalytic subunit and not with the ilvB gamma domain. NMR chemical shift mapping methods show that ilvN binds close to the FAD binding site in ilvB beta and proximal to the intrasubunit ilvB alpha/ilvB beta domain interface. The implication of this interaction on the role of the regulatory subunit oil the activity of the holoenzyme is discussed. NMR studies of the regulatory domains show that these domains are structured in solution. Preliminary evidence for the interaction of ilvN with the metabolic end product of the pathway, viz., valine is also presented.

Myotonic dystrophies type 1 (DM1) and type 2 (DM2) are the most common forms of muscular dystrophy affecting adults. They are autosomal dominant diseases caused by microsatellite tri- or tetranucleotide repeat expansion mutations in transcribed but not translated gene regions. The mutant RNA accumulates in nuclei disturbing the expression of several genes. The more recently identified DM2 disease is less well known, yet more than 300 patients have been confirmed in Finland thus far, and the true number is believed to be much higher. DM1 and DM2 share some features in general clinical presentation and molecular pathology, yet they show distinctive differences, including disease severity and differential muscle and fiber type involvement. However, the molecular differences underlying DM1 and DM2 muscle pathology are not well understood. Although the primary tissue affected is muscle, both DMs show a multisystemic phenotype due to wide expression of the mutation-carrying genes. DM2 is particularly intriguing, as it shows an incredibly wide spectrum of clinical manifestations. For this reason, it constitutes a real diagnostic challenge. The core symptoms in DM2 include proximal muscle weakness, muscle pain, myotonia, cataracts, cardiac conduction defects and endocrinological disturbations; however, none of these is mandatory for the disease. Myalgic pains may be the most disabling symptom for decades, sometimes leading to incapacity for work. In addition, DM2 may cause major socio-economical consequences for the patient, if not diagnosed, due to misunderstanding and false stigmatization. In this thesis work, we have (I) improved DM2 differential diagnostics based on muscle biopsy, and (II) described abnormalities in mRNA and protein expression in DM1 and DM2 patient skeletal muscles, showing partial differences between the two diseases, which may contribute to muscle pathology in these diseases. This is the first description of histopathological differences between DM1 and DM2, which can be used in differential diagnostics. Two novel high-resolution applications of in situ -hybridization have been described, which can be used for direct visualization of the DM2 mutation in muscle biopsy sections, or mutation size determination on extended DNA-fibers. By measuring protein and mRNA expression in the samples, differential changes in expression patterns affecting contractile proteins, other structural proteins and calcium handling proteins in DM2 compared to DM1 were found. The dysregulation at mRNA level was caused by altered transciption and abnormal splicing. The findings reported here indicate that the extent of aberrant splicing is higher in DM2 compared to DM1. In addition, the described abnormalities to some extent correlate to the differences in fiber type involvement in the two disorders.

Structural Support Vector Machines (SSVMs) have recently gained wide prominence in classifying structured and complex objects like parse-trees, image segments and Part-of-Speech (POS) tags. Typical learning algorithms used in training SSVMs result in model parameters which are vectors residing in a large-dimensional feature space. Such a high-dimensional model parameter vector contains many non-zero components which often lead to slow prediction and storage issues. Hence there is a need for sparse parameter vectors which contain a very small number of non-zero components. L1-regularizer and elastic net regularizer have been traditionally used to get sparse model parameters. Though L1-regularized structural SVMs have been studied in the past, the use of elastic net regularizer for structural SVMs has not been explored yet. In this work, we formulate the elastic net SSVM and propose a sequential alternating proximal algorithm to solve the dual formulation. We compare the proposed method with existing methods for L1-regularized Structural SVMs. Experiments on large-scale benchmark datasets show that the proposed dual elastic net SSVM trained using the sequential alternating proximal algorithm scales well and results in highly sparse model parameters while achieving a comparable generalization performance. Hence the proposed sequential alternating proximal algorithm is a competitive method to achieve sparse model parameters and a comparable generalization performance when elastic net regularized Structural SVMs are used on very large datasets.

O objetivo do presente trabalho foi avaliar in vivo a detecção de cárie através do exame visual ICDAS, transiluminação por fibra ótica combinado ao ICDAS e exame radiográfico. Um total de 2.279 superfícies proximais e cicatrículas e fissuras em incisivos superiores, pré-molares e molares permanentes e 272 superfícies em molares decíduos em72 escolares (8 a 18 anos) foram avaliadas por um examinador treinado. Os sete escores para detecção de cárie primária do sistema visual ICDAS foram aplicados. Dois equipamentos de transiluminação por fibra ótica foram avaliados: FOTI Schott (SCH), com ponta de fibra ótica com 0,5mm de diâmetro, e FOTI Microlux (MIC), com diâmetro da ponta 3 mm. Durante o exame combinado FOTI/ICDAS, a fibra ótica era utilizada tanto para iluminar quanto para transiluminar a superfície sob avaliação. O exame radiográfico (RX) consistiu de radiografias interproximais posteriores e periapicais anteriores. Os exames foram realizados em consultório odontológico após escovação supervisionada. No primeiro dia de exame, o exame visual utilizando o ICDAS era realizado e em seguida, o exame combinado ao MIC ou SCH. Logo após era realizado o exame radiográfico. Após uma semana, novamente o ICDAS era realizado, e em seguida o exame combinado com o equipamento de FOTI não utilizado na semana anterior. Os exames foram repetidos em 10 pacientes após intervalo mínimo de uma semana para avaliação da reprodutibilidade intra-examinador, a qual apresentou valores de 0,95 (ICDAS), 0,94 (MIC), 0,95 (SCH) e 0,99 (RX) pelo kappa ponderado. Em cicatrículas e fissuras de permanentes, o RX julgou que um número maior de superfícies apresentava lesão em dentina (53) do que os outros métodos (34 a 36); porém não detectou nenhuma lesão em esmalte, as quais foram identificadas pelo ICDAS (94), SCH (107) e MIC (91). Em proximais permanentes, a transiluminação por fibra ótica identificou maior número de proximais como lesão em esmalte - 150 (SCH) e 139 (MIC) - do que o exame visual (106), enquanto o RX identificou somente 43. Em oclusais de decíduos, os quatro métodos julgaram um número aproximadamente similar de superfícies sem lesão (52 a 59) ou com lesão em dentina (21 a 26), assim como para lesões proximais em dentina (31 a 36). Entretanto um número reduzido de lesões proximais decíduas em esmalte foi julgado pelo exame radiográfico (3) em comparação com os outros métodos (15 a 16). Em decíduos, o ICDAS e o FOTI combinado ao exame visual julgaram maior número de lesões proximais em esmalte que o exame radiográfico, sendo que número similar de lesões em dentina foram classificadas pelos quatro métodos em oclusais e proximais de molares decíduos. Em cicatrículas e fissuras de permanentes, tanto o exame visual ICDAS quanto sua combinação aos dois equipamentos de transiluminação apresentaram maior similaridade de superfícies julgadas como lesão em esmalte ou como lesão em dentina, enquanto o exame radiográfico classificou mais superfícies como lesão em dentina e nenhuma como lesão em esmalte. A adição da transiluminação por fibra ótica ao exame visual aumentou em um terço a detecção das lesões cariosas proximais julgadas em dentina pelo ICDAS isoladamente e aproximadamente quadruplicou o número daquelas assim classificadas pela avaliação radiográfica em permanentes.

Background: There has been an explosion of interest in methods of exogenous brain stimulation that induce changes in the excitability of human cerebral cortex. The expectation is that these methods may promote recovery of function following brain injury. To assess their effects on motor output, it is typical to assess the state of corticospinal projections from primary motor cortex to muscles of the hand, via electromyographic responses to transcranial magnetic stimulation. If a range of stimulation intensities is employed, the recruitment curves (RCs) obtained can, at least for intrinsic hand muscles, be fitted by a sigmoid function.

Objective/hypothesis: To establish whether sigmoid fits provide a reliable basis upon which to characterize the input–output properties of the corticospinal pathway for muscles proximal to the hand, and to assess as an alternative the area under the (recruitment) curve (AURC).

Methods: A comparison of the reliability of these measures, using RCs obtained for muscles that are frequently the targets of rehabilitation.

Results: The AURC is an extremely reliable measure of the state of corticospinal projections to hand and forearm muscles, which has both face and concurrent validity. Construct validity is demonstrated by detection of widely distributed (across muscles) changes in corticospinal excitability induced by paired associative stimulation (PAS).

Conclusion(s): The parameters derived from sigmoid fits are unlikely to provide an adequate means to assess the effectiveness of therapeutic regimes. The AURC can be employed to characterize corticospinal projections to a range of muscles, and gauge the efficacy of longitudinal interventions in clinical rehabilitation.

Introduction : La néphrotoxicité est une complication majeure de la gentamicine, qui est largement utilisée dans le traitement des infections bactériennes, en particulier celles provoquées par des bactéries à Gram-négatif. La gentamicine induit l'apoptose tubulaire, mais les mécanismes moléculaires impliqués demeurent mal compris. Dans l’étude présente, nous avons examiné le rôle des espèces réactives de l'oxygène (ROS) , des proteins Bax, Bmf et Caspase-12 (Csp-12) dans le mécanisme d’action de la gentamicine sur l'apoptose des tubules proximaux rénaux (RPT) et les dommages rénaux induits par ce médicament chez la souris. Méthode: Des souris adultes (âgées 18-19 semaines) mâles non-Tg et des souris transgéniques (CAT-Tg) qui surexpriment la catalase spécifiquement dans leurs cellules des RPT ont été traitées par injections intra-péritonéales de gentamicine (20 mg/kg/jour) pour 5 jours consécutifs, puis euthanasiés. Les reins ont été examinés et analysés par histologie, immunohistochimie pour presansance de la stress oxidative, expression des proteins Bax, Bmf et Csp-12 et essai TUNEL pour étudier de l’apoptose . Nous avons aussi examiné l'effet de la gentamicine sur génération des ROS et l’apoptose dans les cellules RPTC immortalisées de rat (IRPTC) in vitro. Résultats: In vivo, chez les souris non-Tg, la gentamicine induit une tubulopathie et l'apoptose des RPT , stimule la production de ROS et induit une augmentation de Bax et Bmf détectée par immunohistochimie et augmont activité du caspase-12. Ces changements sont atténués chez les souris Cat-Tg. In vitro, la gentamicine induit l’apoptos des cellulles. Le co-traitement avec la catalase normalise ces effets dans les IRPTC. Conclusion : Ces données démontrent que l'apoptose des RPTC induite par la gentamicine s’effectue, au moins en partie, par l'intermédiaire de la génération des ROS.

Background/Aims: It has been widely accepted that chloride ions moving along chloride channels act to dissipate the electrical gradient established by the electrogenic transport of H(+) ions performed by H(+)-ATPase into subcellular vesicles. Largely known in intracellular compartments, this mechanism is also important at the plasma membrane of cells from various tissues, including kidney. The present work was performed to study the modulation of plasma membrane H(+)-ATPase by chloride channels, in particular, CFTR and ClC-5 in kidney proximal tubule. Methods and Results: Using in vivo stationary microperfusion, it was observed that ATPase-mediated HCO(3)(-) reabsorption was significantly reduced in the presence of the Cl(-) channels inhibitor NPPB. This effect was confirmed in vitro by measuring the cell pH recovery rates after a NH(4)Cl pulse in immortalized rat renal proximal tubule cells, IRPTC. In these cells, even after abolishing the membrane potential with valinomycin, ATPase activity was seen to be still dependent on Cl(-). siRNA-mediated CFTR channels and ClC-5 chloride-proton exchanger knockdown significantly reduced H(+)-ATPase activity and V-ATPase B2 subunit expression. Conclusion: These results indicate a role of chloride in modulating plasma membrane H(+)-ATPase activity in proximal tubule and suggest that both CFTR and ClC-5 modulate ATPase activity. Objective: The present study evaluated morphometrically bone loss percentages in experimental periodontitis in rats, comparing different locations (lingual mandible, palatal maxilla and buccal maxilla) and two evaluation methods (distance and area methods). Material and Methods: Ligatures were placed around the maxillary right second molar and around the mandibular right first molar in 14 female Wistar rats. The contralateral molars served as intragroup control. After 4 weeks, the rats were sacrificed and their mandible and maxilla were removed. The specimens were dissected and stained with methylene blue dye. Bone loss was evaluated by two different methods on the surfaces of the defleshed jaw. In the first method, the distance from the cementoenamel junction (CEJ) to the alveolar bone crest was measured in the roots of teeth associated with ligature. In the second method, the area of bone loss was determined using the alveolar tissue bone, CEJ and the proximal region of roots associated with the ligature as reference. The data were converted to bone loss percentages caused by ligature: (ligated - unligated) x 100/ligated. Results: When comparing the distance and area methods, no statistically significant difference was observed (p>0.05). Both methodologies indicated that the maxilla presented greater bone loss than the mandible and it was more accentuated on the buccal side than on the palatal side (p<0.05). Conclusions: The findings of this study show that both the area and the distance methods can be used to evaluate bone loss caused by ligature placement in rats, and suggest applying the morphometric methodology to the maxilla on the buccal side.

Background: Treatment of deep-vein thrombosis (DVT) with a once-daily regimen of enoxaparin, rather than a continuous infusion of unfractionated heparin (UFH) is more convenient and allows for home care in some patients. This study was designed to compare the efficacy and safety of these two regimens for the treatment of patients with proximal lower limb DVT. Methods: 201 patients with proximal lower limb DVT from 13 centers in Brazil were randomized in an open manner to receive either enoxaparin [1.5 mg/kg subcutaneous (s.c.) OD] or intravenous (i.v.) UFH (adjusted to aPTT 1.5-2.5 times control) for 5-10 days. All patients also received warfarin (INR 2-3) for at least 3 months. The primary efficacy endpoint Was recurrent DVT (confirmed by venography or ultrasonography), and safety endpoints included bleeding and serious adverse events. The rate of pulmonary embolism (PE) was also collected. Hospitalization was at the physician's discretion. Results: Baseline patient characteristics were comparable between groups. The duration of hospital stay was significantly shorter with enoxaparin than with UFH (3 versus 7 days). In addition, 36% of patients receiving enoxaparin did not need to be hospitalized, whereas all of the patients receiving UFH were! hospitalized. The treatment duration was slightly longer with enoxaparin (8 versus 7 days). There was a nonsignificant trend toward a reduction in the rate of recurrent DVT with enoxaparin versus UFH, and similar safety. Conclusions: A once-daily regimen of enoxaparin 1.5 mg/kg subcutaneous is at least as effective and safe as conventional treatment with a continuous intravenous infusion of UFH. However, the once daily enoxaparin regimen is easier to administer (subcutaneous versus intravenous), does not require aPTT monitoring, and leads to both a reduced number of hospital admissions and an average 4-day-shorter hospital stay.
