Challenging environmental conditions, including heat and humidity, cold, and altitude, pose particular risks to the health of Olympic and other high-level athletes. As a further commitment to athlete safety, the International Olympic Committee (IOC) Medical Commission convened a panel of experts to review the scientific evidence base, reach consensus, and underscore practical safety guidelines and new research priorities regarding the unique environmental challenges Olympic and other international-level athletes face. For non-aquatic events, external thermal load is dependent on ambient temperature, humidity, wind speed and solar radiation, while clothing and protective gear can measurably increase thermal strain and prompt premature fatigue. In swimmers, body heat loss is the direct result of convection at a rate that is proportional to the effective water velocity around the swimmer and the temperature difference between the skin and the water. Other cold exposure and conditions, such as during Alpine skiing, biathlon and other sliding sports, facilitate body heat transfer to the environment, potentially leading to hypothermia and/or frostbite; although metabolic heat production during these activities usually increases well above the rate of body heat loss, and protective clothing and limited exposure time in certain events reduces these clinical risks as well. Most athletic events are held at altitudes that pose little to no health risks; and training exposures are typically brief and well-tolerated. While these and other environment-related threats to performance and safety can be lessened or averted by implementing a variety of individual and event preventative measures, more research and evidence-based guidelines and recommendations are needed. In the mean time, the IOC Medical Commission and International Sport Federations have implemented new guidelines and taken additional steps to mitigate risk even further.

The Spanish Society of Nursing Emergency (SEEUE) has several lines documentaries that can be consulted on its web page and that have been spread through various publications: Statutes, scientific recommendations, professional recommendations, statements and allegations, grounds for nursing emergency, guarantees and rules, documents of interest and legislation. Set this that composed the regulatory environment, legal and recommendations which society poses to the collective nurse from the area of the emergency, as well as the rest of actors associated with urgent assistance (institutional and administratively) and through what has been the work of conceptualization and definition in our area the past few years. Part of this documentation offer possibilities for scientific endorsement and professional and invites to continue building knowledge and evidence. It is in this sense in which this work can and should be defined from a literature review approach and under the scheme of "review article". The working Group in Primary Care (PC) of the SEEUE, decided to build a Professional Recommendation (PR) in one of the areas of "uncertainty/variability" in the employment context and historical demand of nurses and other emergency care team on security issues: "The uniform and personal protective equipment for professional teams on prehospital emergency areas”.

OBJECTIVE Identify the direct cost of reprocessing double and single cotton-woven drapes of the surgical LAP package. METHOD A quantitative, exploratory and descriptive case study, performed at a teaching hospital. The direct cost of reprocessing cotton-woven surgical drapes was calculated by multiplying the time spent by professionals involved in reprocessing the unit with the direct cost of labor, adding to the cost of materials. The Brazilian currency (R$) originally used for the calculations was converted to US currency at the rate of US$0.42/R$. RESULTS The average total cost for surgical LAP package was US$9.72, with the predominance being in the cost of materials (US$8.70 or 89.65%). It is noteworthy that the average total cost of materials was mostly impacted by the cost of the cotton-woven drapes (US$7.99 or 91.90%). CONCLUSION The knowledge gained will subsidize discussions about replacing reusable cotton-woven surgical drapes for disposable ones, favoring arguments regarding the advantages and disadvantages of this possibility considering human resources, materials, as well as structural, environmental and financial resources.

AIM: To describe outdoor activities, sun protection behaviours and the experience of sunburn in a sample of New Zealanders during summer weekends of 1994. METHODS: 1243 respondents to a telephone survey provided information regarding their outdoor activities for the 5 hour period around midday of the previous Saturday and Sunday. The sample was drawn from those aged 15 to 65 years in the five centres of Auckland, Hamilton, Wellington, Christchurch and Dunedin. Respondents provided information on sun exposure, sunburn, sun protection and beliefs about tanning, as well as background demographic information, skin type and previous experience of sunburn. RESULTS: 12% of the sample (or 17% of all those outdoors) reported being sunburned on the preceding weekend, and those sunburned tended to be men, and to be under age 35 years. The face, neck and limbs were the areas most frequently reported as burned. Sporting activities and beach or water activities were associated with the highest number of episodes of burning. Overall 38% of those outside reported wearing a hat and 32% reported the use of a sunscreen. Positive attitudes to tanning were quite common and probably present the main target for change in the community. CONCLUSION: On any sunny weekend in summer about three-quarters of adult New Zealanders will be out in the sun for relatively long periods of time, and many will get sunburned. The reduction of such harmful sun exposures remains an important public health goal.

Tässä diplomityössä kehitettiin mittaus- ja analyysimenetelmät suojavaatemateriaalien kemikaaliläpäisevyyden testaamiseksi sekä neste- että kaasumaisella metyylisalisylaatilla. Kehitystyö tehtiin standardin SFS-EN ISO 6529 pohjalta. Tämä standardi kuvaa kuitenkin varsin yleisellä tasolla suojavaatemateriaalien kemikaaliläpäisevyyden testimenetelmät, eikä se ole mikään tarkka työohje. Näytteenotto- ja analyysimenetelmät joudutaan siis valitsemaan sekä validoimaan jokaiselle kemikaalille erikseen, tässä tapauksessa metyylisalisylaatille. Työn tarkoituksena oli kehittää toimivat menetelmät ja laitteistot siten, että olisi mahdollista suorittaa metyylisalisylaatin määritys on-line mittauksena läpäisytestauksessa. Läpäisytestaustoiminnan nopeuttamiseksi työssä myös suunniteltiin ja rakennettiin laitteistot kolmen rinnakkaisen testikennon yhdenaikaista testausta varten sekä kaasumaisen että nestemäisen metyylisalisylaatin ollessa testikemikaalina. Havaittiin, että UV/VIS-spektrofotometri varustettuna läpivirtauskyvetillä ja näyteenottopumpulla on toimiva ja käyttökelpoinen analyysimenetelmä metyylisalisylaatin määrittämiseen nesteistä on-line mittauksena läpäisyn testauksessa. Kaasumaisen metyylisalisylaatin tapauksessa määritysmenetelmänä päädyttiin käyttämään kokonaishiilivetyanalysaattoria, joka on varustettu liekki-ionisaatiodetektorilla. Molemmissa tapauksissa saatiin aikaan toimivat ja on-line mittauksiin kykenevät näytteenotto- ja määritysmenetelmät, joilla loppukäyttäjä voi suorittaa laajempaa läpäisytestausta.

La maladie de Lyme est la maladie vectorielle la plus fréquente dans les pays tempérés et est en émergence dans plusieurs régions du monde. Plusieurs stratégies de prévention existent et comprennent des interventions qui visent les individus, comme le port de vêtements protecteurs, et d’autres qui sont implantées au niveau collectif, dont des interventions de contrôle des tiques dans l’environnement. L’efficacité de ces stratégies peut être influencée par divers facteurs, dont des facteurs sociaux tels que les connaissances, les perceptions et les comportements de la population ciblée. Elles peuvent également avoir des impacts parallèles non désirés, par exemple sur l’environnement et l’économie, et ces derniers peuvent s’opposer aux bénéfices des interventions jusqu’à remettre en cause la pertinence de leur mise en œuvre. Aussi, ces facteurs sociaux et les impacts des interventions sont susceptibles de varier selon la population ciblée et en fonction du contexte épidémiologique et social. L’objectif de cette thèse était donc d’étudier les principaux facteurs sociaux et enjeux d’importance à considérer pour évaluer l’efficacité et prioriser des interventions de prévention pour la maladie de Lyme dans deux populations exposées à des contextes différents, notamment en ce qui concerne leur situation épidémiologique, soient au Québec, où l’incidence de la maladie de Lyme est faible mais en émergence, et en Suisse, où elle est élevée et endémique depuis plus de trois décennies. L’approche choisie et le devis général de l’étude sont basés sur deux modèles théoriques principaux, soient le modèle des croyances relatives à la santé et celui de l’aide à la décision multicritère. Dans un premier temps, les facteurs associés à la perception du risque pour la maladie de Lyme, c’est-à-dire l’évaluation cognitive d’une personne face au risque auquel elle fait face, ont été étudiés. Les résultats suggèrent que les facteurs significatifs sont différents dans les deux régions à l’étude. Ensuite, l’impact des connaissances, de l’exposition, et des perceptions sur l’adoption de comportements préventifs individuels et sur l’acceptabilité des interventions de contrôle des tiques (acaricides, modifications de l’habitat, contrôle des cervidés) a été comparé. Les résultats suggèrent que l’impact des facteurs varierait en fonction du type du comportement et des interventions, mais que la perception de l’efficacité est un facteur commun fortement associé à ces deux aspects, et pourrait être un facteur-clé à cibler lors de campagnes de communication. Les résultats montrent également que les enjeux relatifs aux interventions de contrôle des tiques tels que perçus par la population générale seraient communs dans les deux contextes de l’étude, et partagés par les intervenants impliqués dans la prévention de la maladie de Lyme. Finalement, un modèle d’analyse multicritère a été développé à l’aide d’une approche participative pour le contexte du Québec puis adapté pour le contexte suisse et a permis d’évaluer et de prioriser les interventions préventives selon les différentes perspectives des intervenants. Les rangements produits par les modèles au Québec et en Suisse ont priorisé les interventions qui ciblent principalement les populations humaines, devant les interventions de contrôle des tiques. L’application de l’aide à la décision multicritère dans le contexte de la prévention de la maladie de Lyme a permis de développer un modèle décisionnel polyvalent et adaptable à différents contextes, dont la situation épidémiologique. Ces travaux démontrent que cette approche peut intégrer de façon rigoureuse et transparente les multiples perspectives des intervenants et les enjeux de la prévention relatifs à la santé publique, à la santé animale et environnementale, aux impacts sociaux, ainsi qu’aux considérations économiques, opérationnelles et stratégiques. L’utilisation de ces modèles en santé publique favoriserait l’adoption d’une approche « Une seule santé » pour la prévention de la maladie de Lyme et des zoonoses en général. Introducción: la fotoprotección constituye las actividades preventivas que minimizan los efectos deletéreos de la exposición solar; estos comportamientos de salud pueden estar relacionados con actitudes y conocimientos adquiridos. Objetivo: Identificar estas asociaciones en Estudiantes de Medicina de la Escuela de Medicina y Ciencias de la Salud de la Universidad del Rosario, quienes se encargarán de transmitir educación y ejemplo de comportamiento en su contexto personal y profesional. Metodología: estudio de corte transversal. Se implementó una encuesta voluntaria por correo institucional y físicamente entre estudiantes de 1-8 semestre matriculados en el segundo semestre de 2009; n= 122 estudiantes, la mayoría menores de 20 años y de género femenino; factores de estudio analizados: biológico – demográficos, informador, actitudes, conocimientos, personas modelo y comportamiento, expresados en frecuencias, analizados con pruebas y fuerzas de asociación con intervalo de confianza del 95%. Resultados: factores asociados a fotoprotección: ser de 1-4 semestre (p=0,008), ser =19 años (p=0,028), reconocer como consecuencias las alteraciones en los ojos y la visión (p=0,043) y las alteraciones producidas en el sistema inmune (p=0,021), uso de la pareja de ropa protectora (p=0,019), permanencia de un amigo a la sombra (p=0,055), conocimiento de la posibilidad de quemadura independiente al clima (p=0,001) y conocimiento de la posibilidad de quemadura sin sentir los rayos calientes del sol (p=0,049). Conclusiones: es posible reforzar comportamientos preventivos, favorecer el seguimiento de modelos positivos afines a los jóvenes, incrementar el conocimiento en salud y afirmar la educación primaria en salud desde la Medicina General y mejorar así la fotoprotección.

Los bomberos aeronáuticos son los encargados de atender todas las emergencias en los aeropuertos y sus cercanías. Estas emergencias incluyen emergencias aéreas, en tierra, eventos con materiales peligros e incendios, entre otros. Su trabajo tiene como características la realización de actividades durante periodos largos de baja intensidad y periodos cortos de alta intensidad. De acuerdo con estas características, es necesario que los bomberos aeronáuticos tengan una buena condición física. El consumo máximo de oxígeno (VO2 máx) como indicador de capacidad aeróbica resulta indispensable para conocer el desempeño de los bomberos en su trabajo. El objetivo de este estudio es determinar la capacidad aeróbica de los bomberos aeronáuticos y sus factores determinantes. Por tanto se desarrolló un estudio transversal de tipo descriptivo en una muestra de 23 hombres bomberos aeronáuticos. Se obtuvo información acerca de sus variables socio-demográficas, se determinó el VO2 máx y umbral ventilatorio mediante análisis de gases espirados durante un protocolo de ejercicio máximo sobre tapiz rodante, se evaluó la composición corporal mediante adipometría y se determinó el nivel de actividad física mediante el cuestionario internacional de actividad física IPAQ. Se encontró que la muestra tenia una edad de 32,6 ± 4,8 años, peso de 78,4 ± 9,8 kg, porcentaje de grasa de 14,8 ± 3,8 %, índice de masa corporal de 25,7 ± 2,7 y VO2máx de 44,6 ± 6. No se encontraron cambios significativos del VO2máx con la edad, pero si con la actividad física, porcentaje de grasa e índice de masa corporal. Se sugiere que el entrenamiento de los bomberos aeronáuticos durante su jornada laboral sea de intervalos de alta intensidad y que se monitorice su nivel de actividad física y composición corporal.

The objectives of the present study were to evaluate the safety of mixer/loaders and applicators of paraquat to maize crop by knapsack sprayers and to determine the efficacy of safety measures applied to the sprayers. Potential dermal exposure (PDE) was evaluated in 22 worker body parts. The Cu2+ cation of a copper-based fungicide was used as tracer in the spray solution. Sanitary pads and cotton gloves were used to collect the pesticide solution on the sampled body parts. It was observed that paraquat application in front of the applicator's body (0.5 and 1.0 m lance) is unsafe because PDE was 1,979.8 ml/day (for 0.5 m lance) and 1,290.4 ml/day (for 1.0 m lance) and needs 50-80% and 37-69% control of PDE respectively. Control can be achieved by the use of protective garment on the legs and feet only, which received 92-93% of the PDE. Switching the spray nozzle to the back of the operator reduced the PDE by 98% and was sufficient to make working conditions safe, while maintaining the efficiency of application and making the work lighter and more comfortable. Mixer/loaders received 86% of the PDE to the hands and the work condition was safe (MOS > 1), however impermeable gloves could be used as a further safety measure.

The objectives of this study were to verify the effects of wet suits (WS) on the performance during 1500m swimming (V1500), on the velocity corresponding to the anaerobic threshold (VAT) and on the drag force (AD) as well as its coefficient (Cx). 19 swimmers randomly completed the following protocols on different days (with and without WS): 1) maximal performance of 1500m swimming; 2) VAT in field test, with fixed concentration of blood lactate (4 mM) and 3) determination of hydrodynamic indices (AD and Cx). The results demonstrated significant differences (p < 0.05) in the VAT (1.27±0.09; 1.21±0.06 m.s-1), and in the V1500 (1.21±0.08; 1.17±0.08 m.s-1), with and without WS, respectively. However the AD, and its Cx did not present significant differences (p>0.05) for the respective maximal speeds of swimming. In summary, we can conclude that WS allows swimmers to reach greater speeds in both, long- and short-course swims. This improvement can be related to the decrease of the AD, since with higher speeds (with WS) the subjects presented the same resistance, as they did when compared to speeds without a WS. Moreover, these data suggest that the methodology used in this study to determine the Cx is unable to detect the improvement caused by WS.
