Este trabajo tiene como propósito presentar y valorar, desde la perspectiva del alumnado participante, un proyecto de investigación-formación puesto en marcha durante el curso 2003-2004 en la elaboración del trabajo de tesina, fin de carrera, en la Escuela de Enfermería de Vitoria, dentro del programa de Licenciatura Europea de Enfermería. Constituye el punto de partida de un proyecto a largo plazo, iniciado con la intención de desarrollar principios teóricos y procedimientos prácticos que nos permitan sistematizar procesos formativos que, centrados en la investigación, articulen la teoría y la práctica e integren una perspectiva comunicativa y cooperativa.

El present treball analitza l'ètica de la cura i l'atenció d'infermeria. Es descriuen els seus antecedents i la influència del feminisme en les cures d'infermeria. Així mateix, s'analitza mitjançant unes entrevistes en profunditat, la dimensió ètica de les cures segons els models de Watson i Roach a un centre hospitalari del nostre pais.

Background: The lack of adequate instruments prevents the possibility of assessing the competence of health care staff in evidence-based decision making and further, the identification of areas for improvement with tailored strategies. The aim of this study is to report about the validation process in the Spanish context of the Evidence-Based Practice Questionnaire (EBPQ) from Upton y Upton. Methods: A multicentre, cross-sectional, descriptive psychometric validation study was carried out. For cultural adaptation, a bidirectional translation was developed, accordingly to usual standards. The measuring model from the questionnaire was undergone to contrast, reproducing the original structure by Exploratory Factorial Analysis (EFA) and Confirmatory Factorial Analysis (CFA), including the reliability of factors. Results: Both EFA (57.545% of total variance explained) and CFA (chi2=2359,9555; gl=252; p<0.0001; RMSEA=0,1844; SRMR=0,1081), detected problems with items 7, 16, 22, 23 and 24, regarding to the original trifactorial version of EBPQ. After deleting some questions, a reduced version containing 19 items obtained an adequate factorial structure (62.29% of total variance explained), but the CFA did not fit well. Nevertheless, it was significantly better than the original version (chi2=673.1261; gl=149; p<0.0001; RMSEA=0.1196; SRMR=0.0648). Conclusions: The trifactorial model obtained good empiric evidence and could be used in our context, but the results invite to advance with further refinements into the factor “attitude”, testing it in more contexts and with more diverse professional profiles.

Fear of achieving higher levels of nurse autonomy must be overcome for a new environment of relationships, change, communication and respect within the multidisciplinary team. The progressive achievement of new skills, the handling of evidence and the implementation of models and ways of working based on the nursing process, through encouraged training courses into the company and by the possibility of resource management in line and technology, foster as the ideal setting for Emergency Nursing, Emergency and Disaster acquires its own identity and development.

The creation and establishment within the healthcare team of the Primary Healthcare Centre of Baena of a Protocol and Procedures for healthcare that provides a communication tool for nurses enabled the healthcare team to develop a dynamic circuit and at the same time communication with nurses of the Critical Care and Emergency Team (DCCU) assigned to that centre. The created work environment for healthcare included Case Management Nurse, Primary Healthcare Nurse and Critical Care and Emergency Nurse. Thus, nursing assessment and actions for programmed and urgent healthcare, provided the first contact with DCCU nurses and reoriented them for the proposed healthcare plan for the patients, in addition to communicate with the rest of the healthcare team. This article presents the results of continuous nurse healthcare over nine months, applying this protocol.

Asthma is a chronic disease and as such, is inexorably tied to periods of exacerbations in which quality of life is affected. This represent not only a threat to physical integrity of the sick person but also an increase in both workloads and expenses supported by health services. The key point of this case is that if we make our patients to get a major implication in their own disease control, we are going to reduce umber and severity of episodes, to optimize resources and the most important to improve the quality of life of our patients

The aim of this research is to know the barriers and opportunities that nursing professionals detect in their clinical practice in order to develop the culture of patient safety and to identify future research lines. This qualitative study is based on the DELPHI method, with a group of 19 nursing professionals from education and care practice, involving both primary and specialized care. Weaknesses and threats revolve around five categories: profession, organization and infrastructure; indicators; communication and safety culture; and safety training. Opportunities to improve safety cover six categories: organizational change; promotion of the safety culture, professional training and development; relationship with the patients; research; and strategic planning. Work is needed to improve safety and nursing should be ready to assume this leadership.

Bipolar disorder is a chronic disease that causes abnormal shifts in mood. Although it may be infra diagnosed, a prevalence of 1-2.5% is estimated. As treatment, there is a wide range of medicines but the bests goals are achieved with a mix of psicotherapy and medicines.The following is the case of AF, Who was diagnosed of bipolar disorder 20 years ago. Along this time, he has been amitted to hospital several times. This case runs the normal course of bipolar disorder and shows that treating severe mental illness may be really complex.

There is currently around one million people receiving oral anticoagulants in Spain. The drug most used is acenocoumarol, which requires coagulation monitoring to ensure that the patient is within its normal therapeutic range. Patients usually start this treatment in a hospital clinic and, when they are stabilised, they are referred to primary care, where they are followed-up by their community nurses. The usual practice is that nurses are responsible for changes in the dose when the patients are outside the range. This practice is not performed by hospital nurses, despite having sufficient experience and knowledge to adequately manage these types of patients. An Advanced Nursing Practice model has been introduced into the Haematology management unit of the Hospital Universitario Virgen de la Victoria, Málaga. This involves various aspects of attention and care of patients on anticoagulant therapy, and includes adjusting the doses of their treatment following a catalogue of therapeutic and diagnostic ranges.

BACKGROUND Job satisfaction of nurses is a determinant factor in the quality and organizational adaptation of clinical management models in the current socio-economic context. The aim of this study was to construct and validate a questionnaire to measure job satisfaction of nurses in the Clinical Management Units in the Andalusian Public Health System. METHODS Clinimetric and cross-sectional study with a sample of 314 nurses of two university hospitals from Seville. Nurses were surveyed in 2011, from March to June. We used the Font Roja questionnaire adapted to our study variables. We performed analyses of correlations, reliability and construct validity, using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) to test the a priori model. RESULTS The end questionnaire consists of 10 items, whose internal consistency was 0.75, with a percentage of variance explaining of 63.67%. CFA confirmed 4 dimensions (work environment, work relationships, motivation, and recognition): significant χ2 (p < .001); χ2/gl = 2.013; GFI= 0.958, RMR = 0.055 y RMSEA = 0.057; AGFI = 0.927, NFI = 0.878, TLI = 0.902, CFI =0.933 e IFI = 0.935; AIC = 132.486 y ECVI = 0.423. CONCLUSION This new questionnaire (G_Clinic) improves clinimetric values of the Font Roja questionnaire, because it reduces the number of items, improves the reliability of the dimensions, increases the value of variance explained, and allows knowing job satisfaction of nurses in clinical managementt.

El objeto de estudio parte de un proceso de investigación acción (McKernan, 1999 y Eliot, 1993), el cual estamos llevando a cabo desde hace dos cursos, en el marco del plan piloto para la adaptación de los estudios de la UdG al Espacio Europeo deEducación Superior (EEES). La justificación se enmarca en la demanda del Espacio Europea de Educación Superior, mediante el desarrollo de una educación basada en competencias con el objetivo de partir de una información que responda a unas necesidades reales basadas del Proyecto Tunning. Esta aportación se refleja en una asignatura de libre elección de nuevaincorporación. Esta propuesta, da un énfasis especial en la interacción entre los dos estudios: educación y enfermería. La cooperación interdisciplinar nos ha permitido compartir, interactuar y dialogar de forma crítica para poder seguir avanzando en nuestra propuesta. Estamos plenamente convencidos que la Educación Física bien dosificada, la alimentación cualitativamente equilibrada y unas condiciones higiénicas correctas en un entorno favorable, son pilares de una buena educación para la salud

La promoción de la salud mental es una faceta importante de la promoción general de la salud. Teniendo en cuenta los niveles de estrés y de conflicto que soportan las personas y las comunidades, es preciso redoblar esfuerzos para promover la salud mental. Uno de los objetivos de la promoción de la salud en este ámbito es identificar las conductas inadaptadas de los pacientes y promover las actividades personales y psicológicas más eficaces para poder planificar los cuidados de enfermería en un entorno terapéutico favorable. La OMS está examinando los datos disponibles sobre la eficacia de las actividades de fomento de la salud mental y ha propuesto nuevas políticas y modelos de cambio de la asistencia: enfoque comunitario y desinstitucionalización, integración de la atención en salud mental con los servicios generales de salud, la colaboración intersectorial, enfoques multidisciplinarios, reorientar las funciones de los profesionales y evitar el estigma de las personas con problemas mentales. Así mismo también se han propuesto diez áreas de acción: Apoyo a los padres y a los primeros años de vida, promover la salud mental en las escuelas, promover la salud mental en el lugar de trabajo, apoyo en el envejecimiento mentalmente sano, en grupos de riesgo de trastornos mentales, prevenir la depresión y el suicidio, prevenir la violencia y el uso de sustancias nocivas, involucrar a la atención primaria y secundaria, reducir la situación de desventaja y evitar el estigma, trabajo intersectorial. Los profesionales de enfermería de salud mental han de proporcionar los cuidados teniendo en cuenta a los nuevos modelos de atención en salud mental y las acciones propuestas

La trayectoria de este estudio se dirigió para la comprensión de la vivencia de ser enfermera clínica que actúa con estudiantes en los campos clínicos. Tuvo como objetivo comprender la experiencia vivida de las enfermeras clínicas al estar con los estudiantes de enfermería en cuanto ellos desarrollan su experiencia clínica en su servicio. Se optó por realizar una investigación cualitativa con enfoque fenomenológico. Se entrevistaron nueve enfermeras clínicas y de sus discursos se identificaron cuatro categorías concretas de lo vivido, que fueron interpretadas según el referente filosófico de Martín Heidegger. De las categorías que emergieron de los discursos se destacaron: Siendo enfermera con estudiantes en el proceso de formación y La posibilidad de estar con estudiantes de universidades privadas y universidades tradicionales, que se presentaron en el intento de estimular reflexiones acerca de la importante participación de este profesional en la formación de los futuros profesionales de enfermería.
