OBJECTIVE: A single course of antenatal corticosteroids (ACS) is associated with a reduction in respiratory distress syndrome and neonatal death. Multiple Courses of Antenatal Corticosteroids Study (MACS), a study involving 1858 women, was a multicentre randomized placebo-controlled trial of multiple courses of ACS, given every 14 days until 33+6 weeks or birth, whichever came first. The primary outcome of the study, a composite of neonatal mortality and morbidity, was similar for the multiple ACS and placebo groups (12.9% vs. 12.5%), but infants exposed to multiple courses of ACS weighed less, were shorter, and had smaller head circumferences. Thus for women who remain at increased risk of preterm birth, multiple courses of ACS (every 14 days) are not recommended. Chronic use of corticosteroids is associated with numerous side effects including weight gain and depression. The aim of this postpartum assessment was to ascertain if multiple courses of ACS were associated with maternal side effects. METHODS: Three months postpartum, women who participated in MACS were asked to complete a structured questionnaire that asked about maternal side effects of corticosteroid use during MACS and included the Edinburgh Postnatal Depression Scale. Women were also asked to evaluate their study participation. RESULTS: Of the 1858 women randomized, 1712 (92.1%) completed the postpartum questionnaire. There were no significant differences in the risk of maternal side effects between the two groups. Large numbers of women met the criteria for postpartum depression (14.1% in the ACS vs. 16.0% in the placebo group). Most women (94.1%) responded that they would participate in the trial again. CONCLUSION: In pregnancy, corticosteroids are given to women for fetal lung maturation and for the treatment of various maternal diseases. In this international multicentre randomized controlled trial, multiple courses of ACS (every 14 days) were not associated with maternal side effects, and the majority of women responded that they would participate in such a study again.

PURPOSE: To assess the chemotactic activity and phagocytic response of neutrophilic polymorphonuclear leukocytes among women in the first five days postpartum.METHODS: A prospective, cross-sectional clinical-laboratory study was conducted. Data of 31 postpartum women during the first five days after vaginal delivery were compared with those of 24 healthy non-pregnant non-postpartum women matched for age. The inclusion criteria were postpartum, clinically and obstetrically healthy women; vaginal delivery, singleton pregnancy carried to term; non-hypertensive, hyperglycemic, allergic, malnourished or with autoimmune or neoplastic diseases; not having received vaccines or blood products in the last three months. The Control Group was chosen according to the same inclusion criteria but involving non-pregnant non-postpartum women. The chemotactic activity of neutrophilic polymorphonuclear leukocytes was assessed by determining the distance from directed migration to bacterial lipopolysaccharide, in three Boyden chamber assays. The phagocytic response was identified by assessing the Zymosan particles' ingestion in three assays carried out in Leighton tubes. The Student's t-test was used in the statistical analysis, adopting a 5% level of significance.RESULTS: The chemotactic activity of neutrophilic polymorphonuclear leukocytes from postpartum women in the presence of homologous (73.2±6.9) and autologous (78.6±13.9) sera showed a significant increase compared to the values observed in the Control Group (64.1±4.1 and 66.6±5.4). Both chemotactic response and phagocytosis ingestion phase of neutrophilic polymorphonuclear leukocytes were significantly increased (p<0.05) in postpartum women compared to healthy non-pregnant and non-postpartum women.CONCLUSION: There was an increase in the chemotactic activity and phagocytic response of neutrophilic polymorphonuclear leukocytes during the first five days after vaginal delivery in women.

This research study was conducted as a descriptive study of prenatal care experiences of women enrolled in public and private managed care programs. The study's aim was to describe the demographic characteristics of the women in the study and to analyze and compare their prenatal care experiences. ^ The objective of this study was to examine the research question: Do pregnant women enrolled in Medicaid Managed Care receive the same level of care as women enrolled in other Managed Care Programs in Harris County, Texas? ^ The study population was a convenience sample of pregnant women enrolled in managed care programs who presented to one of the two hospital study sites for delivery of their infant. The study utilized a self administered survey to measure adequacy and content of prenatal care received by the women during this pregnancy. Adequacy of prenatal care utilization was determined based on the Kessner Index criteria of timing of initiation of care and number of visits. Content of care was measured by the number of different medical services the women reported they had received and the number of health information topics the women reported on which they had received information. Demographic characteristics were described with univariate and bivariate statistics of frequencies and cross tabulations. Associations were evaluated using measures of linear correlations. ^ Results from the study showed there is an association between enrollment in Medicaid Managed Care (public) and prenatal care received compared to women enrolled in other Managed Care Programs (private). The results were derived from statistical tests on data the postpartum women gave when they completed the self-administered survey. Provider type was a moderate predictor of quality and quantity of prenatal care. The results also indicate that in the study population, minority ethnicity, income and lower educational status were associated with intermediate and inadequate prenatal care. ^

To analyze the relationship between parity, pre-pregnancy body mass index (BMI), and gestational weight gain (GWG). This observational controlled study was conducted from November 2013 to April 2014, with postpartum women who started antenatal care up to 14 weeks and had full-term births. Data were collected from medical records and antenatal cards. Descriptive and bivariate analyses were performed. The significance level was 5%. Data were collected from 130 primiparous and 160 multiparous women. At the beginning of prenatal care, 54.62% of the primiparous were eutrophic, while the majority of multiparous were overweight or obese (62.51%). Multiparas are two times more likely to be obese at the beginning of their pregnancies, when compared to primiparas. The average pre-pregnancy weight and final pregnancy weight was significantly higher in multiparous, however, the mean GWG was higher among primiparous. We found an inverse correlation between parity and the total GWG, but initial BMI was significantly higher in multiparas. Nevertheless, monitoring of the GWG through actions that promote a healthier lifestyle is needed, regardless of parity and nutritional status, in order to prevent excessive GWG and postpartum weight retention and consequently inadequate pre-pregnancy nutritional status in future pregnancies.

A atenção primária visa garantir cobertura e acesso a cuidados de saúde abrangentes e aceitáveis pela população enfatizando a prevenção e a educação (STARFIELD, 2004). No Brasil a Estratégia Saúde da Família (ESF) é o modelo implantado pelo Ministério da Saúde (MS) com a finalidade de reestruturação da atenção primária e já abrange cerca de 50% da população. Segundo Starfield (2004) a atenção primária pode ser avaliada através de seus atributos essenciais: atenção de primeiro contato (utilização e acessibilidade), longitudinalidade, integralidade e coordenação de cuidados. No contexto da atenção à saúde brasileira, a mortalidade infantil é fato preocupante, principalmente a mortalidade que ocorre nos primeiros dias de vida. Este componente da mortalidade é intimamente relacionado à qualidade da atenção recebida no período gestacional. A atenção pré-natal deve incluir ações organizadas, amplas, integradas e com cobertura abrangente de promoção e prevenção da saúde, além de diagnóstico e tratamento adequado dos problemas que possam vir a ocorrer nesse período. O objetivo do presente estudo foi analisar o pré-natal de um grupo de puérperas quanto aos atributos da atenção primária acessibilidade e longitudinalidade. Para alcançar este objetivo foram realizadas 80 entrevistas com mulheres que realizaram pré-natal na atenção primária e 19 mulheres que realizaram pré-natal no Hospital Universitário Cassiano Antônio de Moraes (HUCAM). Utilizou-se parcialmente o questionário de avaliação da atenção primária PCATool. As puérperas também foram questionadas quanto à percepção da adequação do pré-natal realizado. A acessibilidade foi baixa para a maioria das entrevistadas. Nas puérperas que realizaram o pré-natal na Atenção primária a acessibilidade foi estatisticamente maior naquelas que realizaram pré-natal na ESF e naquelas que utilizaram as unidades de saúde do município de Vitória (ES). A longitudinalidade se relacionou ao tipo de unidade de saúde do pré-natal, ao número de consultas realizadas e ao índice de Ápgar no primeiro minuto. A percepção de um pré-natal adequado se relacionou com a utilização da ESF como serviço de pré-natal e à realização de seis ou mais consultas. Apesar de as puérperas do grupo que realizou o pré-natal no HUCAM perceberem o atendimento pré-natal como adequado, algumas questões revelaram fragilidades na assistência, principalmente quanto ao acesso ao serviço. A impossibilidade de receber atendimento pela equipe fora dos horários de funcionamento do ambulatório, a falta de um número de telefone no qual possam sanar dúvidas ou falar com o médico responsável pelo pré-natal, a percepção de dificuldade na marcação de consultas e a espera prolongada no dia do atendimento são pontos que devem ser melhorados nessa assistência.

A maternidade em meninas de 10 a 14 anos é um fenômeno que desafia as políticas públicas de saúde e a academia científica, visto que, sua ocorrência aumenta a cada ano, mesmo com a taxa de fecundidade decrescendo cada vez mais no Brasil. Procurando se aproximar dessa temática para contribuir com o aprofundamento do assunto nessa faixa etária, o presente estudo teve como objetivo compreender o significado da maternidade para mães de 10 a 14 anos de idade a partir de seus relatos de experiência. Parte-se de uma investigação epidemiológica realizada em uma região interiorana do Espírito Santo, no período de Julho de 2012 a Fevereiro de 2013, em que se avaliou a assistência pré-natal de 742 puérperas, através de informações registradas em formulário e contidas no Cartão da Gestante e no prontuário do recém-nascido. Aprofunda-se o estudo dos dez casos relativos às mães que possuíam entre 10 e 14 anos no momento do parto, mediante uma abordagem qualitativa que utilizou entrevistas individuais com roteiro semiestruturado. As falas foram gravadas, transcritas e analisadas através da técnica de análise de conteúdo, com enfoque na análise temática. Os resultados evidenciaram dois temas e cada um deles definiu quatro categorias de análise: 1) A experiência da maternidade: a) A vida familiar e conjugal; b) O que é ser mãe?; c) O apoio recebido após a maternidade e d) Reflexões sobre ser mãe adolescente e 2) A perspectiva após a maternidade: a) A escolaridade; b) A inserção no mercado de trabalho; c) Conselhos para jovens da mesma faixa etária e d) Os planos para o futuro. O estudo considera que o comportamento da maternidade poderá ter características e significados distintos de acordo com o contexto social em que vivem essas meninas. O arcabouço sexual, reprodutivo e familiar possui faces e formas estruturais que diferem dentro de cada grupo e a raiz desse assunto pode estar sendo baseada em uma relação que não depende da idade cronológica das mães.

RESUMO: A Dissertação teve por objectivos comparar e estudar as relações entre os fenómenos psicológicos observados no puerpério, nomeadamente a ansiedade-estado e ansiedade-traço, os sintomas depressivos e a sensibilidade ao stresse. A amostra foi composta de 200 puérperas, com idades compreendidas entre os 18 e os 42 anos (M = 28,88; DP = 5,87), que sabiam ler e falar Língua Portuguesa, possuíam pelo menos quatro anos de escolaridade, residiam em Portugal há mais de um ano e tinham tido um parto de termo. As participantes responderam a um protocolo de avaliação constituído por um Questionário de dados sociodemográficos, um Questionário de dados clínicos e os instrumentos DASS e STAI. Com a DASS pretendia-se avaliar os construtos ansiedade hiperfisológica, depressão e stresse; através da STAI observou-se a ansiedade-traço e ansiedade-estado. Os resultados demonstraram que as primíparas, quando comparadas com as multíparas, possuíam maior ansiedade-estado. Constatou-se que as mulheres que tiveram um parto distócico mostraram mais sensibilidade ao stresse. Também se concluiu que as puérperas que manifestaram patologias médicas durante a gestação apresentaram, no puerpério, ansiedade-traço, sintomas depressivos e sensibilidade ao stresse. Por último, confirmou-se que as mulheres com um trabalho de parto mais longo revelaram maior ansiedade-estado. Espera-se, com esta investigação, contribuir para um maior conhecimento psicológico das puérperas. ABSTRACT: The aims of this Dissertation were to compare and to ascertain the relationships between postpartum psychological phenomena, namely state-anxiety, trait-anxiety, depressive symptoms and stress sensitivity. The sample encompassed 200 puerperas aged 18 to 42 (M = 28,88; SD = 5,87), who were able to read and write in Portuguese, attended at least four years at school, lived in Portugal for at least one year and had a term delivery. Participants were requested to answer an evaluation protocol composed by a sociodemographic Questionnaire, clinical data Questionnaire, DASS and STAI instruments. DASS enabled the evaluation of hyperphysiological anxiety, depression and stress constructs; through STAI state-anxiety and trait-anxiety were scrutinized. Results revealed that primiparas had greater state-anxiety than multiparas. Women who had a dystocic delivery showed increased stress sensitivity. Postpartum women who suffered medical intercurrences during pregnancy exhibited traitanxiety, depressive symptoms and stress sensitivity. Women who underwent protracted labour had greater state-anxiety. This research is expected to attain a greater psychological knowledge regarding postpartum women.

OBJECTIVE Determine the coverage rate of syphilis testing during prenatal care and the prevalence of syphilis in pregnant women in Brazil. METHODS This is a national hospital-based cohort study conducted in Brazil with 23,894 postpartum women between 2011 and 2012. Data were obtained using interviews with postpartum women, hospital records, and prenatal care cards. All postpartum women with a reactive serological test result recorded in the prenatal care card or syphilis diagnosis during hospitalization for childbirth were considered cases of syphilis in pregnancy. The Chi-square test was used for determining the disease prevalence and testing coverage rate by region of residence, self-reported skin color, maternal age, and type of prenatal and child delivery care units. RESULTS Prenatal care covered 98.7% postpartum women. Syphilis testing coverage rate was 89.1% (one test) and 41.2% (two tests), and syphilis prevalence in pregnancy was 1.02% (95%CI 0.84;1.25). A lower prenatal coverage rate was observed among women in the North region, indigenous women, those with less education, and those who received prenatal care in public health care units. A lower testing coverage rate was observed among residents in the North, Northeast, and Midwest regions, among younger and non-white skin-color women, among those with lower education, and those who received prenatal care in public health care units. An increased prevalence of syphilis was observed among women with < 8 years of education (1.74%), who self-reported as black (1.8%) or mixed (1.2%), those who did not receive prenatal care (2.5%), and those attending public (1.37%) or mixed (0.93%) health care units. CONCLUSIONS The estimated prevalence of syphilis in pregnancy was similar to that reported in the last sentinel surveillance study conducted in 2006. There was an improvement in prenatal care and testing coverage rate, and the goals suggested by the World Health Organization were achieved in two regions. Regional and social inequalities in access to health care units, coupled with other gaps in health assistance, have led to the persistence of congenital syphilis as a major public health problem in Brazil.

ABSTRACT OBJECTIVE To identify the factors that interfere with the access of adolescents and young people to childbirth care for in the Northeast region of Brazil. METHODS Cross-sectional study with 3,014 adolescents and young people admitted to the selected maternity wards to give birth in the Northeast region of Brazil. The sample design was probabilistic, in two stages: the first corresponded to the health establishments and the second to women who had recently given birth and their babies. The data was collected by means of interviews and consulting the hospital records, from pre-tested electronic form. Descriptive statistics were used for the univariate analysis, Pearson’s Chi-square test for the bivariate analysis and multiple logistic regressions for the multivariate analysis. Sociodemographic variables, obstetrical history, and birth care were analyzed. RESULTS Half of the adolescents and young people interviewed had not been given guidance on the location that they should go to when in labor, and among those who had, 23.5% did not give birth in the indicated health service. Furthermore, one third (33.3%) had to travel in search of assisted birth, and the majority (66.7%) of the postpartum women came to maternity by their own means. In the bivariate analysis, the variables marital status, paid work, health insurance, number of previous pregnancies, parity, city location, and type of health establishment showed a significant association (p < 0.20) with inadequate access to childbirth care. The multivariate analysis showed that married adolescents and young people (p < 0.015), with no health insurance (p < 0.002) and from the countryside (p < 0.001) were more likely to have inadequate access to childbirth care. CONCLUSIONS Adolescents and young women, married, without health insurance, and from the countryside are more likely to have inadequate access to birth care. The articulation between outpatient care and birth care can improve this access and, consequently, minimize the maternal and fetal risks that arise from a lack of systematic hospitalization planning.

A serological survey of Toxoplasma gondii infection in population groups in Fortaleza, Brazil, was conducted, in order to identify seroprevalence rates according to age and risk factors associated with acquired infection. A Toxoplasma IgG-antibody enzyme immunoassay (Sanofi Pasteur Diagnostics, Marnes la Coquette, France) was employed to assess the immunity. Public day-care centers and schools were randomly selected, while three large antenatal clinics and maternity units were choosen by its importance. Population groups and results of 997 blood tests were: 227 children (mean age 3.8 years), 22.8% seropositives; 584 students (mean 11.4 years), 58.4%, and pregnant and postpartum women (mean 24 years), 71.5% seropositives (p < 0.001). Of 256 participants reporting close contact with cats, 59.8% were seropositive, in contrast with 51% seropositives without contact (relative risk 1.17; 95% confidence interval 1.04-1.33; p = 0.01). Having three or more siblings at home was related to higher seroprevalence in children and students (relative risk 1.39; 95% confidence interval 1.21-1.60; p < 0.01). In conclusion, toxoplasmosis seroprevalence showed a rapid increase during the first ten years of life, in association with close contact with cats and larger households, probably related to inappropriate hygiene and child-care practices.

Objective To assess the efficacy of an intervention designed to improve the mother-infant relationship and security of infant attachment in a South African peri-urban settlement with marked adverse socioeconomic circumstances. Design Randomised controlled trial. Setting Khayelitsha, a peri-urban settlement in South Africa. Participants 449 pregnant women. Interventions The intervention was delivered from late pregnancy and for six months postpartum. Women were visited in their homes by previously untrained lay community workers who provided support and guidance in parenting. The purpose of the intervention was to promote sensitive and responsive parenting and secure infant attachment to the mother. Women in the control group received no therapeutic input from the research team. Main outcome measures Primary outcomes: quality of mother-infant interactions at six and 12 months postpartum; infant attachment security at 18 months. Secondary outcome: maternal depression at six and 12 months. Results The intervention was associated with significant benefit to the mother-infant relationship. At both six and 12 months, compared with control mothers, mothers in the intervention group were significantly more sensitive (6 months: mean difference=0.77 (SD 0.37), t=2.10, P<0.05, d=0.24; 12 months: mean difference=0.42 (0.18), t=-2.04, P<0.05, d=0.26) and less intrusive (6 months: mean difference=0.68 (0.36), t=2.28, P<0.05, d=0.26; 12 months: mean difference=-1.76 (0.86), t=2.28, P<0.05, d=0.24) in their interactions with their infants. The intervention was also associated with a higher rate of secure infant attachments at 18 months (116/156 (74%) v 102/162 (63%); Wald=4.74, odds ratio=1.70, P<0.05). Although the prevalence of maternal depressive disorder was not significantly reduced, the intervention had a benefit in terms of maternal depressed mood at six months (z=2.05, P=0.04) on the Edinburgh postnatal depression scale). Conclusions The intervention, delivered by local lay women, had a significant positive impact on the quality of the mother-infant relationship and on security of infant attachment, factors known to predict favourable child development. If these effects persist, and if they are replicated, this intervention holds considerable promise for use in the developing world. Trial registration Current Controlled Trials ISRCTN25664149.

Este estudo descritivo coletou informações sociodemográficas, obstétricas e relacionadas ao diagnóstico e tratamento da gestante/puérpera e parceiro das 67 gestantes/puérperas notificadas no Sistema Nacional de Agravos de Notificação, usuárias de maternidades públicas do Distrito Federal, Brasil, entre 2009 e 2010. As informações do acompanhamento clínico e laboratorial recebido pela criança vieram do prontuário médico hospitalar, fichas de notificação compulsória e Cartão da Criança. Das gestantes, 41,8% foram adequadamente tratadas, o principal motivo para a inadequação foi a ausência (83,6%) ou inadequação do tratamento do parceiro (88,1%). Mais de um terço necessitou de novo tratamento na maternidade por falta de documentação terapêutica no pré-natal. Dos recém-nascidos com sífilis congênita, 48% fizeram estudo radiográfico, 42% passaram por punção liquórica e 36% deles não receberam qualquer tipo de intervenção. Nota-se, assim, que a qualidade do pré-natal recebido pela gestante não é suficiente para garantir o controle da sífilis congênita e o alcance da meta de incidência da doença.

Objective: the proposal was to study the presence of immunoglobulin A (IgA) in the chorioamniotic membrane of healthy postpartum women with premature rupture of the chorioamniotic membrane (FROM). Method: A single radial immunodiffusion technique was used to quantify the IgA in the chorioamniotic membrane tissues. Results: the level of IgA was approximately 10 times higher in patients whose membranes had been ruptured for > 10 h (24.58 mg/dl). These results were compared with those of a previously published study where the mean of amount of IgA was 2.52 mg/dl in membranes of patients with rupture < 10 h. Our results show that IgA began to rise after 10-15 h following rupture. Conclusion: Although more studies need to be performed our data indicate that the increasing IgA in our patients after 10 h of latency probably represents the beginning of an ascending colonization of bacteria which could be the source of future infection.
