Au cours des dernières décennies, l’augmentation du nombre de guerres irrégulières place les armées modernes dans une situation délicate. Pour pouvoir relever ce nouveau défi, ces organisations militaires, avant tout destinées à mener des guerres interétatiques, se doivent d’innover. Plusieurs cas historiques montrent comment, face à de nouvelles formes de conflit, certaines armées se sont lancées dans un processus d’innovation. Le cas de l’armée française en Algérie en est un. L’objectif de ce mémoire est de comprendre comment l’innovation a émergé lors du conflit, et comment elle s’est diffusée au sein de l’organisation militaire. Un survol de la stratégie française en Algérie va permettre de dégager trois principales innovations : La mise en place de la doctrine de la guerre révolutionnaire, la création des Sections Administratives Spécialisées et l’approche de David Galula. L’étude de ses trois cas nous montre que, durant la Guerre d’Algérie, l’innovation provenait principalement des militaires, plus précisément du sommet de la hiérarchie. Cependant, certains exemples d’innovation par le bas existent. Dans ces cas-là, la diffusion se fera via la doctrine informelle. L’exemple de Galula nous prouve néanmoins qu’une innovation se diffuse beaucoup plus difficilement lorsqu’elle émane des hommes de terrain.

L’eau, l’air, la nourriture, le sol, le travail sont autant de « milieux d’exposition » à des contaminants chimiques et microbiologiques, ainsi qu’à des agents physiques dont on continue de découvrir, et à tenter de minimiser, les effets sur la santé humaine. La santé environnementale, autrefois appelée « hygiène des milieux », couvre un très vaste domaine de disciplines, de connaissances, de pratiques et de recherches que nul ne saurait maîtriser dans sa totalité. D’où l’intérêt d’un ouvrage exhaustif qui prend le parti de décrire la relation environnement-santé dans ses diverses dimensions : principes, méthodes, milieux, effets, pratiques. Fruit d’une complexe, mais enrichissante collaboration entre les meilleurs spécialistes francophones des deux côtés de l’Atlantique, cet ouvrage, avec ses trente-neuf chapitres regroupés en cinq parties, servira aussi bien de livre de référence pour les professionnels que de manuel d’étude universitaire.

Persistent atrial fibrillation (AF) ablation may lead to partial disconnection of the coronary sinus (CS). As a result, disparate activation sequences of the local CS versus contiguous left atrium (LA) may be observed during atrial tachycardia (AT). We aimed to evaluate the prevalence of this phenomenon and its impact on activation mapping.

OBJECTIVES: In this population-based study, reference values were generated for renal length, and the heritability and factors associated with kidney length were assessed. METHODS: Anthropometric parameters and renal ultrasound measurements were assessed in randomly selected nuclear families of European ancestry (Switzerland). The adjusted narrow sense heritability of kidney size parameters was estimated by maximum likelihood assuming multivariate normality after power transformation. Gender-specific reference centiles were generated for renal length according to body height in the subset of non-diabetic non-obese participants with normal renal function. RESULTS: We included 374 men and 419 women (mean ± SD, age 47 ± 18 and 48 ± 17 years, BMI 26.2 ± 4 and 24.5 ± 5 kg/m(2), respectively) from 205 families. Renal length was 11.4 ± 0.8 cm in men and 10.7 ± 0.8 cm in women; there was no difference between right and left renal length. Body height, weight and estimated glomerular filtration rate (eGFR) were positively associated with renal length, kidney function negatively, age quadratically, whereas gender and hypertension were not. The adjusted heritability estimates of renal length and volume were 47.3 ± 8.5 % and 45.5 ± 8.8 %, respectively (P < 0.001). CONCLUSION: The significant heritability of renal length and volume highlights the familial aggregation of this trait, independently of age and body size. Population-based references for renal length provide a useful guide for clinicians. KEY POINTS: • Renal length and volume are heritable traits, independent of age and size. • Based on a European population, gender-specific reference values/percentiles are provided for renal length. • Renal length correlates positively with body length and weight. • There was no difference between right and left renal lengths in this study. • This negates general teaching that the left kidney is larger and longer.

AIMS: Currently available devices for transcatheter closure of patent foramen ovale (PFO) which rely on a permanent implant have limitations, including late complications. The study objective was to evaluate the safety, feasibility, and effectiveness of the PFx Closure System, the first transcatheter technique for PFO closure without an implantable device. METHODS AND RESULTS: A prospective study of 144 patients was conducted at nine clinical sites from October 2005 through August 2007. All patients had a history of cryptogenic stroke, transient ischemic attack, migraines, or decompression illness. The mean balloon stretched diameter of the PFO was 7.9 +/- 2.5 mm. Technical success (successful application of radiofrequency energy) was achieved in 130 patients. One patient required a transfusion as a result of blood loss during the procedure. There were no other major procedural complications. There were no recurrent strokes, deaths, conduction abnormalities, or perforations following the procedure. At a mean follow-up of 6 months, successful closure was achieved in 79 patients (55%). In PFOs with balloon sized or stretched diameters less than 8 mm, the closure rate was 72% (53/74). CONCLUSION: This study demonstrates that transcatheter closure of a PFO without a permanent implant is technically feasible and safe. Further technique and device modifications are required to achieve higher closure rates.
