Introduction : L’inconfort causé par les appareils orthodontiques peut significativement affecter la coopération des patients dans leur traitement. La douleur, ainsi que la détérioration de la fonction (mastication, élocution) sont reconnus comme les déterminants majeurs de la coopération des patients traités par appareils amovibles. Invisalign® se positionne comme une alternative esthétique aux multiples inconforts observés lors des traitements fixes avec boîtiers. À ce jour, peu d’études ont cherché à comparer la douleur (quantitativement et qualitativement) perçue entre cette technique et celle avec boîtiers fixes sur une longue période. Objectif : L’objectif de la présente étude est d’évaluer la douleur ressentie par les patients qui suivent un traitement orthodontique avec coquilles correctrices Invisalign® et de la comparer avec celle des patients qui suivent un traitement orthodontique conventionnel avec des boîtiers fixes. Matériels et Méthodes: L’étude compte 70 patients (29 garçons, 41 filles), moyenne d’âge de 16 ans [11 à 30]. Les trois sous-groupes sont Invisalign® (n=31), boîtiers Damon (n=19) et boîtiers Speed (n=20). Les groupes avec boîtiers (Damon et Speed) sont les 2 groupes de l’étude menée au sein de la clinique d’Orthodontie de l’Université de Montréal en 2011 qui comparait la perception de la douleur durant le traitement orthodontique entre boîtiers auto-ligaturants passifs et actifs. L’étude a été organisée en 4 phases correspondant à l’insertion des 4 premiers fils pour les groupes avec boîtiers (Phase 1: 0,016" Supercable, Phase 2: 0,016" CuNiTi, Phase 3: 0,016"x0,022" CuNiTi, Phase 4: 0,019"x0,025" CuNiTi) et à l’insertion des coquilles 1, 4, 7 et 10 pour le groupe Invisalign®. À l’aide d’un questionnaire, l’étude évalue pour chaque phase l’ampleur (grâce à une échelle visuelle analogue EVA), la durée et la localisation de la douleur à 6 différents points (T1: immédiatement après l’insertion, T2: 5h après, T3: 24h après, T4: 3 jours après, T5: une semaine après, T6: 2 semaines après). Résultats: À T1Ph3 le pourcentage de patients rapportant de la douleur était plus élevé avec Damon qu’avec Invisalign® (p=0,032) (Damon=55,6% ; Invisalign®=23,3%) mais il n’y avait pas de différence avec le groupe Speed (p=0,114). Les patients avec Invisalign® rapportaient significativement moins d’irritation des tissus mous (muqueuses, gencives) que les patients avec des boîtiers. Pour les résultats des EVA, les différences étaient statistiquement significatives à 2 temps : T3Ph1 (Médiane Invisalign®=33,31, Médiane Speed=49,47; p=0,025) et T3Ph4 (Médiane Invisalign®=13,15, Médiane Damon=27,28; p=0,014). Pour la majorité des patients la douleur ne nécessitait pas la prise de médicament et il n’y avait pas de différence significative entre les groupes sur ce point. La qualité de vie était légèrement affectée lors de la première phase et moindrement pour le groupe Invisalign® que pour les groupes avec boîtiers. Pour les patients Invisalign®, la douleur atteignait son niveau le plus élevé entre 5 et 24 heures après l’insertion de la première coquille, et diminuait en intensité et en durée à chaque phase. Conclusion: La perception de la douleur lors d’un traitement orthodontique avec Invisalign® est inférieure à celle ressentie lors d’un traitement avec des boîtiers fixes. Cette méthode de traitement est donc une thérapie attirante pour les patients désirant un traitement esthétique et relativement confortable.

The aim of this study was to compare the in situ and in vitro performances of a laser fluorescence (LF) device (DIAGNOdent 2095) with visual inspection for the detection of occlusal caries in permanent teeth. Sixty-four sites were selected, and visual inspection and LF assessments were carried out, in vitro, three times by two independent examiners, with a 1-week interval between evaluations. Afterwards, the occlusal surfaces were mounted on the palatal portion of removable acrylic orthodontic appliances and placed in six volunteers. Assessments were repeated and validated by histological analysis of the tooth sections under a stereomicroscope. For both examiners, the highest intra-examiner values were observed for the visual inspection when in vitro and in situ evaluations were compared. The inter-examiner reproducibility varied from 0.61 to 0.64, except for the in vitro assessment using LF, which presented a lower value (0.43). The methods showed high specificity at the D(1) threshold (considering enamel and dentin caries as disease). In vitro evaluations showed the highest values of sensitivity for both methods when compared to the in situ evaluations at D(1) and D(2) (considering only dentinal caries as the disease) thresholds. For both methods, the results of sensitivity (at D(1) and D(2)) and accuracy (at D(1)) showed significant differences between in vitro and in situ conditions. However, the sensitivity (at D(1) and D(2)), specificity and accuracy (both at D(1)) of the methods were not significantly different when the same condition was considered. It can be concluded that visual inspection and LF showed better performance in vitro than in situ.

Purpose: the effect of orthodontic movement on the periodontal tissues of maxillary second pre-molars, after regenerative treatment for class II furcations, was evaluated in four mongrel dogs.Material and Methods: Class II furcation lesions were created. After 75 days they were treated with bovine bone mineral matrix and guided tissue regeneration with absorbable membrane. After 2 months of daily plaque control, each of the dog's furcation pre-molars was randomly assigned to a test or control group. Orthodontic appliances were placed on both sides of the maxilla using third pre-molars and canines as anchorages. In the test group, bodily orthodontic movement of the second pre-molars was performed in the mesial direction for 3 months while control pre-molars remained unmoved. The dogs were sacrificed for histometric and histologic analyses.Results: There were no statistically significant differences between the two groups in total bone and biomaterial areas or linear extension of periodontal regeneration on the radicular surfaces. In the test group, however, there was a tendency to a greater quantity of bone and a lesser quantity of biomaterial.Conclusion: the orthodontic movement was not pre-judicial to the results obtained with the regenerative periodontal treatment.

Background: Patients with fixed orthodontic appliances often experience an absolute increase in the number of Streptococci mutans colony-forming units (cfu). The aim of this investigation was to study the development of biofilm and S. mutans cfu in connection with stainless steel ligatures and elastomeric rings in orthodontic patients treated with and without 0.4% stannous fluoride gel (SFG). Material: Forty-seven patients were divided into 2 groups: those treated with 0.4% SFG for 4 minutes (experimental) and those without 0.4% SFG (control). In each patient, elastomeric rings were used for ligation on 1 side of the dental arch midline, and stainless steel ligatures were used on the opposite side. Saliva samples were collected before and after appliance placement. At 15 and 30 days after appliance placement, biofilm samples from the stainless steel ligatures and the elastomeric rings were collected and subjected to microbiologic procedures and scanning electron microscopy (SEM) analysis. Results: The numbers of S. mutans cfu in the saliva and biofilm were not statistically different between the teeth fitted with elastomeric rings and stainless steel ligatures, or between the experimental and control groups. SEM analysis showed biofilm formation on both ligature ties. Conclusions: Topical application of 0.4% SFG in orthodontic patients with elastomeric rings or stainless steel ligatures does not cause a significant decrease in S. mutans cfu in the saliva and biofilm. This in vivo experimental study evaluated the efficacy of fluoride-releasing elastomers in the control of Streptococcus mutans levels in the oral cavity. Forty orthodontic patients were recruited and divided into two groups of 20. Fluoride-releasing elastomeric ligature ties (Fluor-I-Ties, Ortho Arch Co. Inc., USA) were used in the experimental group, and conventional elastomeric ligature ties (D. Morelli, Brazil), in the control group. Two initial samples of saliva were collected at a 14-day interval to determine the number of colony forming units (CFU) of Streptococcus mutans. Immediately after collecting the second sample, fluoride-releasing elastomeric ligature ties were placed in the patients of the experimental group, and conventional ligature ties, in the patients of the control group. Seven, 14 and 28 days after placement of the elastomeric ligature ties, saliva and plaque surrounding the orthodontic appliance were collected for microbiologic analysis. There were no significant differences in the number of Streptococcus mutans CFUs in saliva or plaque in the area surrounding the fluoride-releasing or conventional elastomeric ligature ties. Thus, fluoride-releasing elastomeric ligature ties should not be indicated to reduce the incidence of enamel decalcification in orthodontic patients. Since there was no significant reduction in S. mutans in saliva or plaque, other means of prevention against enamel decalcification should be indicated for these patients.

Esthetic orthodontic appliances continue to appeal to more patients, which results in objections to extraction spaces that remain for several months during orthodontic therapy. This has led orthodontists to design temporary pontics that fill extraction sites and that can be reduced as the spaces close. This report describes a simple, efficient, and expeditious technique for making such pontics. © 2010 Quintessence Publishing Co, Inc.

Permanent teeth impaction is highly prevalent among brazilian people. Its etiology is related to local and general factors association. Permanent teeth retention compromises dental occlusion and when anterior teeth are involved, it also brings esthetics impairments which lead to psychological disturbance. Early diagnosis and adequate treatment are extremely important to solve not only occlusal problems but also psychological aspects. Orthodontic traction of impacted teeth can be conducted by using fixed or removable appliances. Although it depends on patient compliance the use of removable appliances provides an anchorage based on the teeth and the palate reducing undesirable side effects. This paper describes the case of a fourteen years old female patient whose right maxillary central incisor was adequately tractioned with a removable orthodontic appliance. Removable orthodontic devices were used at first to reposition teeth in maxillary anterior area what provided adequate space to allow the placement of the impacted incisor and after were also used to traction and position this tooth. The procedure described seemed to be effective, non expensive and a viable treatment to be performed even on the scope of public health services, extending orthodontic treatment to a higher number of patients.

The increased interest in orthodontics by adults has led companies to develop new treatment alternatives that use less visible and more aesthetic orthodontic appliances. The aesthetic aligners stand out among the treatment options that attend these expectations. This paper presented some relevant aspects of these devices, as well as two cases treated with Essix MTM (Dentsply) system, in which all aesthetic and functional objectives proposed treatment have been achieved. The results of this study support the idea that, when properly indicated, aesthetic aligners are a viable treatment option within the clinical routine of the orthodontist.

Introduction: Knowing the microbiota that colonizes orthodontic appliances is important for planning strategies and implementing specific preventive measures during treatment. The purpose of this clinical trial was to evaluate in vivo the contamination of metallic orthodontic brackets with 40 DNA probes for different bacterial species by using the checkerboard DNA-DNA hybridization (CDDH) technique. Methods: Eighteen patients, 11 to 29 years of age having fixed orthodontic treatment, were enrolled in the study. Each subject had 2 new metallic brackets bonded to different premolars in a randomized manner. After 30 days, the brackets were removed and processed for analysis by CDDH. Data on bacterial contamination were analyzed descriptively and with the Kruskal-Wallis and Dunn post tests (alpha = 0.05). Forty microbial species (cariogenic microorganisms, bacteria of the purple, yellow, green, orange complexes, "red complex + Treponema socranskii," and the cluster of Actinomyces) were assessed. Results: Most bacterial species were present in all subjects, except for Streptococcus constellatus, Campylobacter rectus, Tannerella forsythia, T socranskii, and Lactobacillus acidophillus (94.4%), Propionibacterium acnes I and Eubacterium nodatum (88.9%), and Treponema denticola (77.8%). Among the cariogenic microorganisms, Streptococcus mutans and Streptococcus sobrinus were found in larger numbers than L acidophillus and Lactobacillus casei (P < 0.001). The periodontal pathogens of the orange complex were detected in larger numbers than those of the "red complex + T socranskii" (P < 0.0001). Among the bacteria not associated with specific pathologies, Veillonella parvula (purple complex) was the most frequently detected strain (P < 0.0001). The numbers of yellow and green complex bacteria and the cluster of Actinomyces were similar (P > 0.05). Conclusions: Metallic brackets in use for 1 month were multi-colonized by several bacterial species, including cariogenic microorganisms and periodontal pathogens, reinforcing the need for meticulous oral hygiene and additional preventive measures to maintain oral health in orthodontic patients. O tratamento da má oclusão de Classe II sem extrações dentárias vem ganhando popularidade na comunidade ortodôntica já há três décadas. Aparelhos funcionais fixos vêm sendo utilizados por profissionais, de maneira crescente, para promover compensações dentoalveolares e corrigir a má oclusão de Classe II. Os efeitos mais significativos são observados em pacientes com padrão de crescimento horizontal. Um caso clínico será relatado com o uso do aparelho fixo Twin Force Bite Corrector em uma paciente do sexo feminino, para a correção da Classe II. Esse dispositivo de ancoragem fixa dispensa o uso de aparelhos funcionais removíveis e não necessita da cooperação do paciente.
