Au Canada, la proportion de la population âgée de 65 ans et plus a augmenté depuis 1980. Bien que la dénutrition ne soit pas inévitable avec le vieillissement, certains changements et facteurs physiopathologiques, environnementaux et psycho socio-économiques peuvent entraîner une détérioration des choix alimentaires et donc, de la qualité de vie des aînés [1]. Plusieurs études font le lien entre l’état nutritionnel et la morbidité ainsi qu’avec les capacités fonctionnelles [2]. Ces observations expliquent l'intérêt de la prévention et du traitement de la dénutrition afin d’optimiser la prise alimentaire et un meilleur état de santé de cette population. Objectifs :1) Explorer les barrières individuelles et socio-environnementales, réelles et perçues, qui peuvent mener à la détérioration des choix et de la qualité alimentaires et entraîner une dénutrition chez les personnes âgées vivant à domicile. 2) Examiner la distribution de ces facteurs dans la population à l’étude. 3) Étudier la relation entre ces facteurs afin de dresser un portrait plus éclairé des déterminants négatifs de l’alimentation chez les adultes âgés pour mieux comprendre les barrières à la prise alimentaire saine. Méthodologie : Il s'agit d'une analyse secondaire réalisée à partir des données recueillies auprès des participants (n=1 602), âgés entre 67 et 84 ans,de l’Étude longitudinale québécoise sur la nutrition et le vieillissement réussi (NuAge) débutée en 2003 et dont le suivi était prévu sur cinq ans [3]. Le but principal de NuAge était de déterminer le rôle de la nutrition dans l’accomplissement d’un vieillissement réussi. Les données comprennent des mesures socio-démographiques, nutritionnelles, fonctionnelles, sociales de même que biologiques et médicales. À partir d'un modèle théorique des déterminants de la prise alimentaire chez la population âgée, ces données ont été mises en lien avec la qualité alimentaire. Cette dernière a été déterminée selon l’adaptation canadienne de l’indice d’alimentation saine (C-HEI), calculé à partir des données alimentaires et nutritionnelles obtenues par le questionnaire de fréquence alimentaire administré aux participants au T1, soit à leur entrée dans l'étude. Résultats : Les barrières qui pourraient freiner la qualité alimentaire des femmes incluent un statut affectif fragile et un fonctionnement social limité. Ce qui ressort, comme étant des barrières au C-HEI chez les hommes, est un revenu perçu comme étant insuffisant pour satisfaire les besoins, le port de prothèses dentaires et le fait de manger souvent au restaurant. Étonnamment, le nombre d’attitudes positives relatives à l’alimentation et un score plus élevé de la composante mentale du SF-36 prédisent un C-HEI plus faible. La nature des réponses auto rapportées pourrait expliquer ces résultats. Conclusion : Les résultats de cette recherche permettent de mieux comprendre les barrières d’une saine alimentation au sein d’une population bien-portante. Il est souhaité que les résultats contribueront au développement d’interventions efficaces ciblant les personnes âgées pour favoriser un apport nutritionnel et un état de santé optimal.

Foram estudados parâmetros relacionados ao estado nutricional de 151 adultos sadios, pertencentes à classe média e residindo em Botucatu, SP, Brasil. Valores antropométricos foram maiores nos homens, com exceção da prega tricipital e da área adiposa do braço. O aumento da idade associou-se a aumento dos valores da massa muscular (homens e mulheres) e do peso do corpo, da prega tricipital e da área adiposa do braço (mulheres). Os resultados antropométricos aproximaram-se dos valores referenciais internacionais, mas não foram inteiramente concordantes com eles, sendo inferiores para o peso corpóreo e circunferência e área musculares do braço. Nos indivíduos de menos de 50 anos, os valores da ingestão energética foram ligeiramente inferiores aos níveis recomendados. A ingestão protéica foi adequada. Os valores médios das proteínas e lípides do soro foram similares aos valores de referência. Testes de hipersensibilidade cutânea são apresentados como uma prova funcional para avaliação do estado nutricional.

Introducción: analizar la asociación entre la frecuencia de consumo de alimentos y la presencia de sobrepeso-obesidad a partir de datos de la Encuesta Nacional de Salud Española 2006 (ENS-06). Material y métodos: estudio transversal retrospectivo de fuentes secundarias de la ENS-06. Se seleccionaron 29.321 individuos que respondieron el Cuestionario de Frecuencia de Consumo de Alimentos, compuesto por una lista de 12 grupos, sobre los que se preguntaba su frecuencia de consumo en 5 opciones de respuesta y que se categorizaron en 3 para el análisis (consumo diario, semanal y ocasional). Se realizó un análisis descriptivo del consumo de grupos de alimentos categorizados por frecuencia de consumo, y calculando la diferencia (brecha) entre hombres y mujeres. La asociación entre frecuencia de consumo de alimentos y sobrepeso-obesidad se realizó mediante regresión logística. Resultados: el 40,7% de la población consume a diario verduras-hortalizas y el 65,2% consume frutas a diario. El 69,8%, y el 46,3% de las mujeres cumplen las recomendaciones de consumo de frutas (brecha -7,2), y de verduras-hortalizas (brecha -14,0) respectivamente. Consumir fruta ocasionalmente, disminuye la probabilidad de padecer obesidad (OR=0,72 IC95%: 0,63-0,82), frente a consumirla a diario. Consumir embutidos a diario, disminuye la probabilidad de padecer obesidad (OR=0,75 IC95%: 0,68-0,83), frente a un consumo ocasional. Conclusiones: Los resultados muestran contradicciones científicas por la presencia de sesgos en el instrumento utilizado en la recogida de la información, sugiriendo la necesidad de que la ENS incluya un método de recogida de información dietética-nutricional validado y adoptado, que permita mediciones de mayor calidad y exactitud.

Since the 1980s, numerous studies conducted in the United States have attempted to estimate the prevalence of supplement use among the population (e.g., the National Health and Nutrition Survey and the National Health Interview Surveys). Despite these efforts, the true extent of supplement use is unclear. The literature pertaining to the prevalence of supplement use refers to a confusing array of ambiguous terms. Forming accurate conclusions about supplement use is confounded by differences in terminology and methodology between studies. Direct comparisons between studies are therefore inherently problematic. The emphasis in future investigations should be on standardizing the study design; recording data on daily, weekly, or even monthly use in order to establish the safety and efficacy of supplement use; and adopting a consistent, uniform definition of the term supplement.

Contemporary nutrition policies and plans call for focussing efforts to improve nutrition through a closer connection with food and the everyday practicalities of how people live and eat. Various words have been used to articulate what this might mean in practice. More recently, the term “food literacy” has emerged to explain this gap between the policy aims the (in)ability of people to know, understand and use food to meet nutrition recommendations. Despite its increasing use, there is no common understanding of this term or its components. Once established, food literacy could be measured in order to examine its association with nutritional outcomes. A Delphi study of 43 Australian food experts from diverse sectors and settings explored their understanding of the term “food literacy”, the likely components and possible relationship with nutrition. The three round Delphi study began with a semi-structured telephone interview and was followed by two online surveys. Constructivist grounded theory was used to analyse data, from which a conceptual model of the relationship between food literacy and nutrition was developed. The model was then tested and refined following a phenomenological study of 37 young people aged 16-25 years who were responsible for feeding themselves. They were interviewed about their food intake, day-to-day food decision making, the knowledge and skills used and their perceptions of someone who is “good with food”. Analysis from the Delphi study identified, eighty components of food literacy and these were grouped into eight domains: 1)access, 2)planning and management, 3)selection, 4)knowing where food comes from, 5)preparation, 6)eating, 7)nutrition and 8)food related language. When these were compared to results of the Young People’s study it was found that while specific components of food literacy were largely contextual, the importance of all eight domains continued to be relevant. The results of these qualitative studies have set the boundaries and scope of meaning of food literacy and will be used to inform the development of measurable variables to be tested in a quantitative cross-sectional study. This prospective study will examine the relationship between food literacy and nutrition. This research is useful in guiding government strategy and investment, and informing the planning, implementation and evaluation of interventions by practitioners.

“Food literacy” is an emerging term used to describe the relative ability to understand the nature of food and how it is important. It also describes the ability to gather, process, analyse and act upon information about food and to apply it in individual settings. A Delphi study of 43 Australian food experts from diverse sectors and settings in all states and territories explored the meaning of food literacy, its constitutive components and how they relate to nutrition. The three-round Delphi began with a semi-structured telephone interview and was followed by two online surveys. Grounded theory was used to develop a conceptual model of the relationship between food literacy and nutrition. It is proposed that food literacy influences nutrition through three related mechanisms of security, choice and pleasure. These mechanisms will be mediated by the local food supply and individual values. The relative importance of components of food literacy will depend upon these mediators. The level of nutrition outcome being sought (for example, dietary guidelines versus food group serves) will also influence the relative importance of these components. This model will be useful in informing program planning and evaluation and will be tested and refined following a phenomenological study of consumers.

This report is a literature review on Food and Nutrition Security in Solomon Islands, based on data from surveys conducted by Solomon Islands National Statistical Office, as well as from national and international organizations working in Solomon Islands. The purpose of the report is to present information outlining the current food and nutrition situation in Solomon Islands before implementation of the CGIAR Research Program on Aquatic Agricultural Systems (AAS), led by WorldFish. The aim of the AAS program is to enhance production in natural freshwater and/or coastal ecosystems to improve household livelihood, including income and food security. This report summarizes national statistics and also focuses in more detail on a subset of provinces: Guadalcanal, Malaita and Western. In 2012, the AAS program was rolled out in Guadalcanal, Central and Malaita Provinces, designated the Central Hub. In 2013, roll out is beginning in the Western Hub (Western and Isabel Provinces). The priority province for the Central Hub has been identified as Malaita.

This report is a literature review on Food and Nutrition Security in Timor-Leste based on data from surveys conducted by the Timor-Leste National Statistics Directorate, as well as from national and international organizations working in Timor-Leste. This review was supported by the Australian Centre for International Agricultural Research (ACIAR)-funded project “Strategy for Investment in Fisheries in East Timor”. This report describes the current food and nutrition situation in Timor-Leste for the purpose of planning and implementing interventions aimed at improving food and nutrition security, especially within aquatic agricultural systems. The potential role of aquaculture in improving food and nutrition security is considered, with reference to the recently endorsed Timor-Leste National Aquaculture Development Strategy (2012-2030) developed by the National Directorate of Fisheries and Agriculture, Ministry of Agriculture and Fisheries.

This research investigated the micronutrient intakes of Irish pre-school children (1-4 years) and adults (18-64 years) and the role that fortified foods (FFs) play in the diets of these population groups. Dietary intake data were collected as part of the National Pre-school Nutrition Survey (NPNS) (2010-2011) and the National Adult Nutrition Survey (NANS) (2008-2010) using 4-day food and beverage records. Nutrient intakes were estimated using WISP©, which encompasses McCance and Widdowson’s The Composition of Foods and the Irish Food Composition Database. A FF is one in which one or more micronutrients are added. Key dietary sources of micronutrients in NPNS and NANS were “milk”, “meat & meat products”, “breakfast cereals”, “fruit & fruit juices” and “breads”. In general, intakes of most micronutrients were adequate with the exception of iron (1 year old children and adult women) and vitamin D (in all population groups). Small proportions of the pre-school population had intakes which exceeded the upper level (UL) (zinc: 11%, folic acid: 5%, retinol: 4%, copper: 2%). Less than 2% of adults had intakes of iron, copper, zinc and vitamin B6 which exceeded the UL. FFs were consumed by 97% of pre-school children and 82% of adults, representing 17% and 9% of mean daily energy intake respectively. Relative to energy intake, FFs contributed substantially greater proportions to intakes of key micronutrients, such as iron and vitamin D. FFs were effective in reducing the prevalence of inadequate micronutrient intakes in these population groups, particularly for iron in women and 1 year old children. FFs made a significant contribution to folate intake in women of childbearing age (72µg). FFs contributed greater proportions of carbohydrate and lower proportions of fat to the diets of consumers. Voluntary addition of nutrients to foods did not contribute appreciably to intakes exceeding the UL in these population groups.

Field and laboratory studies were conducted from 1998 - 2005 to examine the relationship between nutritional status and mycobacteriosis in Chesapeake Bay striped bass (Morone saxatilis). A review of DNA from archived tissue blocks indicated that the disease has been present since at least 1984. Field surveys and feeding trials were conducted from 1998-1999 to determine the nutritional condition of striped bass and the association with disease state. Proximate composition revealed elevated moisture (~ 80%) and low storage lipids (< 0.5% ww), characteristic of a poorly nourished population. These findings were not consistent with data collected in 1990-1991, or with experimentally fed fish. Mycobacteriosis explained little of the variance in chemical composition (p > 0.2); however elevated moisture and low lipid concentration were associated with fish with ulcerative lesions (p < 0.05). This suggests that age 3 and 4 striped bass were in poor nutritional health in 1998-1999, which may be independent from the disease process. Challenge studies were performed to address the hypothesis that disease progression and severity may be altered by nutritional status of the host. Intraperitoneal inoculation of 104 CFU M. marinum resulted in high mortality, elevated bacterial density, and poor granuloma formation in low ration (0.15% bw/d) groups while adequately fed fish (1% bw/d) followed a normal course of granulomatous inflammation with low mortality to a steady, equilibrium state. Further, we demonstrated that an active inflammatory state could be reactivated in fish through reductions in total diet. The energetic demand of mycobacteriosis, was insignificant in comparison to sham inoculated controls in adequately fed fish (p > 0.05). Declines in total body energy were only apparent during active, inflammatory stages of disease. Overall, these findings suggest that: 1) mycobacteriosis is not a new disease of Chesapeake Bay striped bass, 2) the disease has little energetic demand in the normal, chronic progression, and 3) poor nutritional health can greatly enhance the progression and severity, and reactivation of disease. The implications of this research are that management strategies focused on enhancing the nutritional state of striped bass could potentially alter the disease dynamics in Chesapeake Bay.

Objectives: Multimorbidity is common in the older population, but the impact of combinations of chronic conditions on disability and quality of life (QoL) is not well known. This analysis explores the effect of specific combinations of chronic diseases on disability, QoL and self-rated health (SRH).

Design: We used data from two population representative cross-sectional studies, the Northern Ireland Health and Social Wellbeing Survey (NIHSWS) 2005 and the Survey of Lifestyle, Attitudes and Nutrition (SLAN) 2007 (conducted in the Republic of Ireland).

Setting: Randomly selected community-living participants were interviewed at home.

Participants: A total of 6159 participants aged 50 years and older were included in the analysis.

Outcome measures: Chronic conditions were classified as cardiovascular disease, chronic pain,diabetes or respiratory disease. Interaction terms estimated by logistic regression were used to examine the effects of multiple chronic conditions on disability, SRH and QoL.

Results: Each chronic condition group was correlated with each of the others after adjusting for sociodemographic factors. Those from Northern Ireland were more likely to report a limitation in daily activities (45%) compared to those from the Republic of Ireland (21%). Each condition had an independent effect on disability, SRH and QoL, and those with multiple chronic conditions reported the worst outcomes. However, there were no statistically significant positive interactions between chronic condition groups with respect to any outcome.

Conclusions: Chronic conditions affect individuals largely independent of each other with respect to their effect on disability, SRH and QoL. However, a significant proportion of the population aged 50 years and over across the island of Ireland lives with multimorbidity,and this group is at the highest risk of disability, poor SRH and poor QoL.

OBJECTIVE: To develop a model of the psychological factors which predict people's intention to adopt personalised nutrition. Potential determinants of adoption included perceived risk and benefit, perceived self-efficacy, internal locus of control and health commitment.

METHODS: A questionnaire, developed from exploratory study data and the existing theoretical literature, and including validated psychological scales was administered to N=9381 participants from 9 European countries (Germany, Greece, Ireland, Poland, Portugal, Spain, the Netherlands, the UK, and Norway).

RESULTS: Structural equation modelling indicated that the greater participants' perceived benefits to be associated with personalised nutrition, the more positive their attitudes were towards personalised nutrition, and the greater their intention to adopt it. Higher levels of nutrition self-efficacy were related to more positive attitudes towards, and a greater expressed intention to adopt, personalised nutrition. Other constructs positively impacting attitudes towards personalised nutrition included more positive perceptions of the efficacy of regulatory control to protect consumers (e.g. in relation to personal data protection), higher self-reported internal health locus of control, and health commitment. Although higher perceived risk had a negative relationship with attitude and an inverse relationship with perceived benefit, its effects on attitude and intention to adopt personalised nutrition was less influential than perceived benefit. The model was stable across the different European countries, suggesting that psychological factors determining adoption of personalised nutrition have generic applicability across different European countries.

CONCLUSION: The results suggest that transparent provision of information about potential benefits, and protection of consumers' personal data is important for adoption, delivery of public health benefits, and commercialisation of personalised nutrition.
