Treatment-resistant hypertension is still common despite the availability of several types of antihypertensive agents acting by different mechanisms. The existence of refractory hypertension should lead to rule out "white-coat hypertension", poor adherence to prescribed drugs as well as classical causes of secondary hypertension such as renal artery stenosis, primary aldosteronism, pheochromocytoma and renal disease. It is also important to consider the possible existence of obstructive sleep apnea or the regular intake of vasopressive drugs or substances.

Die Kosten für Generika werden von der gesetzlichen Krankenversicherung übernommen, wenn sie die Bedingungen erfüllen, die das Bundesgesetz über die Krankenversicherung (KVG) und dessen Verordnungen vorgeben. Zu diesen Bedingungen gehören die Festsetzung eines Fabrikationspreises und die Festsetzung eines Maximalverkaufspreises. Diese Preise werden dann regelmässig über mehrere Jahre hinweg überprüft. Mehrere Revisionen der genannten Regelungen führten zu niedrigeren Preisen für Generika. Diese Preise sind allerdings umstritten, da sie immer noch höher sind als in den europäischen Nachbarländern. Der Beitrag beleuchtet die Regelungen des KVG für Generika. (sk) Les médicaments génériques sont pris en charge par l'assurance-maladie obligatoire s'ils remplissent les conditions imposées par la LAMal et ses ordonnances. Parmi ces conditions, figure la fixation d'un prix de fabrique (PF) et d'un prix public (PP) maximums. Ces prix sont ensuite régulièrement réexaminés au fil des ans. Les révisions successives des ordonnances susmentionnées ont conduit à une diminution des prix des génériques en Suisse. Leurs prix demeurent cependant controversés car ils sont globalement plus élevés que dans les pays européens voisins. L'article expose les règles LAMal applicables aux médicaments génériques.

The MDRD (Modification of diet in renal disease) equation enables glomerular filtration rate (GFR) estimation from serum creatinine only. Thus, the laboratory can report an estimated GFR (eGFR) with each serum creatinine assessment, increasing therefore the recognition of renal failure. Predictive performance of MDRD equation is better for GFR < 60 ml/min/1,73 m2. A normal or near-normal renal function is often underestimated by this equation. Overall, MDRD provides more reliable estimations of renal function than the Cockcroft-Gault (C-G) formula, but both lack precision. MDRD is not superior to C-G for drug dosing. Being adjusted to 1,73 m2, MDRD eGFR has to be back adjusted to the patient's body surface area for drug dosing. Besides, C-G has the advantage of a greater simplicity and a longer use.

When requesting a blood level measurement in the context of "Therapeutic drug monitoring" (TDM), numerous aspects have to be considered in the pre-analytical and analytical area, as in the integration of associated clinical data. This review presents therapeutic classes for which a clinical benefit of TDM is established or suggested, at least in some settings. For each class of drugs, the main pharmacokinetic, pre-analytical, analytical and clinical aspects are evaluated in the scope of such a monitoring. Each step of the TDM process is important and none should be neglected. Additional clinical trials are however warranted to better establish the exact conditions of use for such a monitoring.

Requesting a blood level measurement of a drug is part of the global approach known as "Therapeutic Drug Monitoring". Diverse situations require this monitoring approach, such as inadequate response to treatment or organ failure. Every drug however does not possess all the characteristics for a TDM program. The therapeutic range of a TDM drug has indeed to be narrow and its interindividual pharmacokinetic variability to be wide. As the development of new drugs is currently slowing down, the precise management of existing treatments certainly deserves progress, but needs however to be applied rationally, starting from a valid indication to blood sampling, and ending with a sound dosage adaptation decision.
