Alterações específicas do desenvolvimento da linguagem (AEDL) devem ser identificadas precocemente, pois tais alterações podem interferir nos aspectos sociais e escolares da criança. O objetivo dessa pesquisa foi verificar o desempenho de crianças com diagnóstico de AEDL, em comparação com o de crianças normais, por meio da Escala de Desenvolvimento Comportamental de Gesell e Amatruda (EDCGA). Foram selecionadas 25 crianças de 3 a 6 anos com o diagnóstico de AEDL (grupo estudado - GE) e 50 crianças normais da mesma faixa etária (grupo controle - GC). As crianças do GC apresentaram desempenho satisfatório e melhor que as crianças do GE, em todos os campos da escala. O valor da mediana do GE foi limítrofe nos comportamentos adaptativo e social-pessoal, já no de linguagem foi discrepantemente rebaixado. Concluímos que as alterações de linguagem interferiram na avaliação dos outros campos do desenvolvimento (adaptativo e pessoal-social). Apesar da interferência, a escala pode ser instrumento útil no diagnóstico de AEDL.

Comportamentos de pais e de filhos influenciam-se mutuamente. Crianças com alterações de linguagem relacionadas ou não à perda auditiva, podem apresentar dificuldades de relacionamento com os irmãos e seus pares. Assim, a investigação das práticas educativas parentais e do repertório comportamental infantil é fundamental para a busca de intervenções efetivas para essas crianças. O estudo tem como objetivos: (a) comparar o repertório positivo e negativo de mães e crianças com deficiência auditiva (DA) e distúrbio de linguagem (DL); (b) comparar cada uma das deficiências com grupo não clinico; (c) correlacionar comportamentos para cada uma das deficiências. Participaram deste estudo 72 mães, cujos filhos apresentavam da (n = 27), DL (n = 19) ou compunha uma população não clínica (n = 26). O instrumento utilizado foi o Roteiro de Entrevista de Habilidades Sociais Educativas Parentais, que avalia a ocorrência de habilidades sociais aplicáveis às práticas educativas. Os resultados evidenciaram a associação entre práticas positivas e habilidades sociais, bem como entre práticas negativas e problemas de comportamento. O grupo de DL não apresentou mais problemas que as crianças não clínicas, sugerindo a participação de intervenções de caráter preventivo, facilitando a inclusão social. Por outro lado as crianças com da apresentaram menos habilidades sociais, bem como suas mães, menos habilidades sociais educativas. Este estudo evidencia a importância da metodologia empregada na reabilitação de crianças com distúrbios da comunicação, sobretudo para aquelas com da visando o beneficio de programas educativos de promoção do repertório parental positivo integrados aos objetivos da fonoterapia.

This study, performed under a linguistic-discoursive prism, aimed to: (1) describe the moments in which occurred hesitations in the utterances of a psychotic child diagnosed with Language Disorder; and (2) determine the extent to which these moments (also) can indicate aspects of a subjectivity trying to emerge in this child. Data were extracted from a speech therapy session with a ten year old female child. Concerning the fi rst goal, from a total of 362 utterances produced by the child, only 74 (20%) had hesitation traces, while 288 (80%) did not. Concerning the second goal, the utterances with hesitation traces occurred in situations of: topic development, especially in the form of complementarity; introduction of new topic; return to the previous topic; refuses to the topic; enunciative incompleteness. The high percentage of utterances without hesitation traces (80%) is explained because they are highly predictable from the context, mostly in situations of ritualized adjacent pairs, oftentimes in situations of immediate specularity. The reduced percentage of utterances with hesitation traces are explained precisely by the fact that, unlike those without traces, in these ones, signs of a subjectivity that tries to emerge and show itself in the discourse production are detected. With the development of this study, we tried to emphasize the view at the hesitations as marks of subjectivity – in other words, evidences of confl icting relationships between the subject and the others that constitute the utter. The concern was also about bringing to the fi eld of Speech Pathology discoursive linguistic refl ections based on data extracted from symptomatic contexts of language.

Las Tecnologías de la Información y las Comunicaciones han propiciado avances en el contexto de la salud tanto en la gestión efectiva de información socio‐sanitaria de forma electrónica, como en la provisión de servicios de e‐salud y telemedicina. Los antecedentes de investigación publicados en esta área corroboran este hecho presentando las mejoras experimentadas en la atención de la población y en la provisión de servicios sanitarios. La atención temprana, cuyos principios científicos se fundamentan en los campos de la pediatría, neurología, psicología, psiquiatría, pedagogía, fisiatría y lingüística, entre otros, tiene como finalidad ofrecer a los niños con déficit o con riesgo de padecerlos un conjunto de acciones optimizadoras y compensadoras, que faciliten su adecuada maduración en todos los ámbitos y que les permita alcanzar el máximo nivel de desarrollo personal y de integración social. La detección de posibles alteraciones en el desarrollo infantil es un aspecto clave de la atención temprana en la medida en que puede posibilitar la puesta en marcha de diversos mecanismos de actuación disponibles en las entidades implicadas, valiosos para la calidad de vida de la persona. Cuanto antes se realice la detección, existen mayores garantías de prevenir patologías añadidas, lograr mejoras funcionales y posibilitar un ajuste más adaptativo entre el niño y su entorno. El objetivo de la investigación presentada en esta tesis doctoral es analizar, diseñar, verificar y validar un sistema de información abierto, basado en conocimiento, que facilite efectivamente a los profesionales que trabajan con la población infantil entre 0 y 6 años la detección precoz de posibles trastornos del lenguaje. Desde el punto de vista metodológico, la Ingeniería del Conocimiento ofrece un marco conceptual sólido que permite desarrollar y validar Sistemas de Ayuda a la Toma de Decisiones distribuidos y escalables, capaces de ayudar al pediatra de Atención Primaria y al educador infantil en la detección precoz de posibles trastornos del lenguaje en niños. La evaluación del sistema se ha realizado de forma incremental mediante el diseño y validación de pruebas de campo experimentales consistentes en la evaluación de niños en dos escenarios distintos: la escuela infantil y el centro de atención temprana. Los experimentos realizados en poblaciones distintas con alrededor de 344 niños durante 2 años, han permitido contrastar la buena adecuación del sistema propuesto a las necesidades de detección de los profesionales que trabajan con niños entre 0 y 6 años. La tesis resultante ha permitido caracterizar el uso del sistema en entornos reales, conocer la aceptación entre los usuarios y su impacto en la provisión de un servicio de atención temprana como el descrito para el correcto seguimiento del desarrollo del lenguaje en los niños, además de proponer un nuevo modelo de atención y evaluación cooperativa que permita incrementar el conocimiento experimental existente al respecto. ABSTRACT The Information and Communication Technology have led to advances in the context of health both in the effective management of socio‐health information electronically, and in the provision of e‐health and telemedicine. The history of research published in this area confirm this fact by presenting the improvements in the care of the population and the provision of health services. Early attention, whose scientific principles are based on the fields of pediatrics, neurology, psychology, psychiatry, pedagogy, physical medicine and linguistics, among others, aims to provide children with deficits or risk of suffering a set of enhancer actions, which facilitate adequate maturation in all areas and allow them to achieve the highest level of personal development and social integration. The detection of possible changes in child development is a key aspect of early intervention to the extent that it can enable the implementation of different mechanisms of action available to the entities involved, valuable to the quality of life of the person. The earlier the detection is made, there are more guarantees added to prevent diseases, achieving functional improvements and enable a more adaptive fit between the child and his environment. The aim of the research presented is to analyze, design, verify and validate an open information system, based on knowledge, which effectively provide professionals working with the child population between 0 and 6 years, in processes of early detection of language disorders. From the methodological point of view, Knowledge Engineering provides a solid conceptual framework to develop and validate a distributed and scalable decision support systems aim to assist pediatricians and language therapists at early identification and referral of language disorder in childhood. The system evaluation was performed incrementally with the design and validation of consistent experimental field tests in the assessment of children in two different scenarios: the nursery and early intervention center. Experiments in different populations with about 344 children over 2 years, allowed to testing the adequacy of the proposed good detection needs of professionals working with children between 0 and 6 years old system. The resulting thesis has allowed to formalizing the system at real environments and to identifying the acceptance by users as well as its impact on the provision of an early intervention service, such as the one described for the proper monitoring of language development in children. In addition, it proposes a new model of care and cooperative evaluation that lets to increase the existing experimental knowledge about it.

Syftet med detta arbete är att studera vilka olika arbetssätt läraren använder för att introducera, befästa och använda nya begrepp i undervisningen i samhällsorienterande ämnen (SO) samt hur undervisningen uppfattas av elever med språkstörningsdiagnos i en klass i årskurs 4. Utifrån ett specialpedagogiskt perspektiv är fokus att hitta arbetssätt som stöder lärandet för elever med språkstörningsdiagnos. I min studie ingår lärarintervju, lektionsobservationer samt två elevintervjuer. Jag analyserade data utifrån Grundad teori. Analysprocessen var uppbyggd av flera steg och mynnade ut i en egen grundad teoretisk modell med utgångspunkt i mitt syfte och mina frågeställningar. Den teoretiska modellen representeras av sex trappsteg, tydlighet, varierade arbetssätt, repetition, progression, förståelse och förmåga att använda ord och begrepp där det personliga stödet utgör förutsättningen - grunden - för att eleverna med språkstörningsdiagnos ska kunna tillgodogöra sig den klassgemensamma undervisningen. Läraren hade samtalet i helklass som stomme i sin undervisning. Hen använde arbetssätt baserade på forskning: tydliga instruktioner uppdelade i steg, visuellt stöd, elevernas tidigare kunskaper och erfarenheter samt vardagsspråk är utgångspunkt, många tillfällen för repetitioner och uppgifter där eleverna får använda de nya orden och begreppen. Eleverna beskrev att de behövde många repetitioner för att ta till sig nya ord och begrepp. De upplevde de största svårigheterna när många nya begrepp presenterades samtidigt och om de själva skulle söka förklaringar i skriven text. Det bästa sättet att ta till sig ny kunskap var att lyssna, gärna i kombination med bilder av olika slag. De var överens om att Ipad var ett bra verktyg som gav möjlighet att lyssna och tala istället för att läsa och skriva. Studien visar på nödvändigheten av funktionella metoder och uppgifter i klassrummet i kombination med extra anpassningar och personligt stöd utifrån elevens individuella behov.

Background: Neurocognitive impairment is known to occur in euthymic bipolar patients, but language alterations have not been thoroughly investigated. The aim of this study is to examine the performance in language tests of a sample of elderly patients with bipolar disorder. Methods: We studied 33 eurthymic elderly patients with bipolar disorder but no dementia and 33 healthy individuals, matched for age and education, who were compared in terms of their CAMCOG global score and its subitems. Results: The scores obtained in language-related abilities for patients and controls, respectively, were: language (total): 27.3 (1) and 28.5 (1), p < 0.0001)comprehension: 8.6 (0.5) and 8.9 (0.3), p = 0.006; production: 18.7 (1) and 19.6 (0.9), p = < 0.0001; abstraction: 6.8 (1.1) and 7.3 (0.7), p = 0.016; verbal fluency: 16.3 (4.3) and 19.6 (4.1), p = 0.003. Conclusion: A mild but significant impairment in language-related ability scores was detected when comparing patients and controls.

Despite ample research into the language skills of children with specific reading disorder no studies so far have investigated whether there may be a difference between expressive and receptive language skills in this population. Yet, neuro-anatomical models would predict that children who have specific reading disorder which is not associated with movement or attention difficulties, would have lower receptive language skills than expressive. This study investigates the difference between expressive and receptive language skills in a sample of 17 children with specific reading difficulty aged between 7 and 12 years. They were administered a battery of two receptive and two expressive language measures. The results showed that as the neuro-anatomical model would predict, the children scored significantly lower on tests of receptive than on tests of expressive language skills.

Background: Autism spectrum disorders (ASD) and specific language impairment (SLI) are common developmental disorders characterised by deficits in language and communication. The nature of the relationship between them continues to be a matter of debate. This study investigates whether the co-occurrence of ASD and language impairment is associated with differences in severity or pattern of autistic symptomatology or language profile. Methods: Participants (N = 97) were drawn from a total population cohort of 56,946 screened as part of study to ascertain the prevalence of ASD, aged 9 to 14 years. All children received an ICD-10 clinical diagnosis of ASD or No ASD. Children with nonverbal IQ 80 were divided into those with a language impairment (language score of 77 or less) and those without, creating three groups: children with ASD and a language impairment (ALI; N = 41), those with ASD and but no language impairment (ANL; N = 31) and those with language impairment but no ASD (SLI; N = 25). Results: Children with ALI did not show more current autistic symptoms than those with ANL. Children with SLI were well below the threshold for ASD. Their social adaptation was higher than the ASD groups, but still nearly 2 SD below average. In ALI the combination of ASD and language impairment was associated with weaker functional communication and more severe receptive language difficulties than those found in SLI. Receptive and expressive language were equally impaired in ALI, whereas in SLI receptive language was stronger than expressive. Conclusions: Co-occurrence of ASD and language impairment is not associated with increased current autistic symptomatology but appears to be associated with greater impairment in receptive language and functional communication.

This study investigated whether there are differences in the Speech-Evoked Auditory Brainstem Response among children with Typical Development (TD), (Central) Auditory Processing Disorder (C) APD, and Language Impairment (LI). The speech-evoked Auditory Brainstem Response was tested in 57 children (ages 6-12). The children were placed into three groups: TD (n = 18), (C)APD (n = 18) and LI (n = 21). Speech-evoked ABR were elicited using the five-formant syllable/da/. Three dimensions were defined for analysis, including timing, harmonics, and pitch. A comparative analysis of the responses between the typical development children and children with (C)APD and LI revealed abnormal encoding of the speech acoustic features that are characteristics of speech perception in children with (C)APD and LI, although the two groups differed in their abnormalities. While the children with (C)APD might had a greater difficulty distinguishing stimuli based on timing cues, the children with LI had the additional difficulty of distinguishing speech harmonics, which are important to the identification of speech sounds. These data suggested that an inefficient representation of crucial components of speech sounds may contribute to the difficulties with language processing found in children with LI. Furthermore, these findings may indicate that the neural processes mediated by the auditory brainstem differ among children with auditory processing and speech-language disorders. BACKGROUND: The role of the language network in the pathophysiology of formal thought disorder has yet to be elucidated. AIMS: To investigate whether specific grey-matter deficits in schizophrenic formal thought disorder correlate with resting perfusion in the left-sided language network. METHOD: We investigated 13 right-handed patients with schizophrenia and formal thought disorder of varying severity and 13 matched healthy controls, using voxel-based morphometry and magnetic resonance imaging perfusion measurement (arterial spin labelling). RESULTS: We found positive correlations between perfusion and the severity of formal thought disorder in the left frontal and left temporoparietal language areas. We also observed bilateral deficits in grey-matter volume, positively correlated with the severity of thought disorder in temporoparietal areas and other brain regions. The results of the voxel-based morphometry and the arterial spin labelling measurements overlapped in the left posterior superior temporal gyrus and left angular gyrus. CONCLUSIONS: Specific grey-matter deficits may be a risk factor for state-related dysfunctions of the left-sided language system, leading to local hyperperfusion and formal thought disorder.
