O atrito existente entre bráquetes e os fios ortodônticos é um fator importante a ser considerado quanto à eficiência do tratamento ortodôntico nas mecânicas de deslize. Em função disto, sempre se procurou maneiras de se diminuir os níveis de atrito durante a aplicação deste tipo de mecânica. Com o surgimento dos bráquetes autoligados, observou-se que uma das características principais destes dispositivos seria uma redução dos níveis de atrito entre os bráquetes e fios, e com isso poderia se obter um movimento de deslizamento eficiente com a aplicação de forças menores, quando comparado com o uso de bráquetes que tenham sistemas de amarração convencionais. O propósito deste trabalho foi comparar, por meio de ensaios laboratoriais, os níveis de atrito estático de um bráquete autoligado estético, de um bráquete autoligado metálico e dois tipos de bráquetes convencionais metálicos, com a utilização de fios ortodônticos de diferentes ligas, secção e diâmetros, para determinar quais bráquetes apresentam os menores coeficientes de atrito, em situações sem angulação, em angulações de primeira ordem e angulações de segunda ordem entre os diferentes fios e bráquetes. Os resultados demonstraram que os bráquetes autoligados apresentaram um atrito estático significativamente mais baixo nas situações onde não existiu angulação entre o fio e o bráquete e quando se utilizou fios de calibres menores e com material mais flexível, quando comparados com os bráquetes convencionais. O bráquete autoligado metálico apresentou maior atrito, semelhante aos bráquetes convencionais, com o uso dos fios de maior calibre, nas situações sem angulação. Pesquisas sobre aplicação de borracha de pneus descartados em pavimentação asfáltica vêm sendo desenvolvidas desde a década de 1960, especialmente nos Estados Unidos e, desde o início da década de 1990, no Brasil, como alternativa para diminuição dos problemas ambientais gerados por estes resíduos sólidos. Esta pesquisa tem por objetivo avaliar o desempenho de três misturas asfálticas do tipo concreto asfáltico usinado à quente: duas empregando teores diferentes de borracha moída e óleo de xisto (CAP 40 + 12% de borracha + 10% de óleo de xisto e CAP 40 + 20% de borracha + 15% de óleo de xisto) e uma convencional. Este estudo faz parte de um programa de pesquisa coordenado pela Escola de Engenharia de São Carlos da Universidade de São Paulo (EESC-USP), Universidade Estadual de Maringá (UEM) e Petrobrás, envolvendo a execução de misturas com borracha e óleo de xisto em trechos experimentais. Foram realizadas dosagens Marshall e ensaios de resistência à tração, módulo de resiliência e fluência por compressão uniaxial estática. Os corpos-de-prova empregados nos ensaios foram moldados no teor ótimo de cimento asfáltico referente a cada uma das misturas. Constatou-se a viabilidade técnica da adição de óleo extensor para a incorporação de borracha de pneus descartados em concreto asfáltico, permitindo a obtenção de resultados satisfatórios quanto aos projetos de dosagem das misturas pelo método Marshall. Com base nos resultados dos ensaios realizados, acredita-se ser viável a execução de trechos experimentais empregando as duas misturas modificadas.

OBJECTIVES Sleep-disordered breathing (SDB) is very common in acute stroke patients and has been related to poor outcome. However, there is a lack of data about the association between SDB and stroke in developing countries. The study aims to characterize the frequency and severity of SDB in Brazilian patients during the acute phase of ischemic stroke; to identify clinical and laboratorial data related to SDB in those patients; and to assess the relationship between sleep apnea and functional outcome after six months of stroke. METHODS Clinical data and laboratorial tests were collected at hospital admission. The polysomnography was performed on the first night after stroke symptoms onset. Functional outcome was assessed by the modified Rankin Scale (mRS). RESULTS We prospectively evaluated 69 patients with their first-ever acute ischemic stroke. The mean apnea-hypopnea index (AHI) was 37.7 ± 30.2. Fifty-three patients (76.8%) exhibited an AHI ≥ 10 with predominantly obstructive respiratory events (90.6%), and thirty-three (47.8%) had severe sleep apnea. Age (OR: 1.09; 95% CI: 1.03-1.15; p= 0.004) and hematocrit (OR: 1.18; 95% CI: 1.03-1.34; p= 0.01) were independent predictors of sleep apnea. Age (OR: 1.13; 95% CI: 1.03-1.24; p= 0.01), body mass index (OR: 1.54; 95% CI: 1.54-2.18; p= 0.01), and hematocrit (OR: 1.19; 95% CI: 1.01-1.40; p= 0.04) were independent predictors of severe sleep apnea. The National Institutes of Health Stroke Scale (NIHSS; OR: 1.30; 95% CI: 1.1-1.5; p= 0.001) and severe sleep apnea (OR: 9.7; 95% CI: 1.3-73.8; p= 0.03) were independently associated to mRS >2 at six months, after adjusting for confounders. CONCLUSION Patients with acute ischemic stroke in Brazil have a high frequency of SDB. Severe sleep apnea is associated with a poor long-term functional outcome following stroke in that population.

Brazil is a country in development, rich in natural resources. In order to grow sustainably, it is necessary to Brazil to preserve its environment, which is an expressive challenge, especially to industries, such as those producing ceramic materials. This study was developed using Porcelain Tile Polishing Residue (RPP) in blends with soil to build compacted fills. This residue is a slurry generated during the polishing process of porcelain tiles and contains powdery material from the polished tile, the abrasives used during the process and cooling water. The RPP was collected from a private company located in Conde/PB and it was mixed with a sandy-clayey soil, to build the fills. Laboratorial tests were conducted with pure soil, pure RPP and blends in proportions of 5%, 10%, 15% and 20% of RPP in addition to the dry mass of pure soil. The Chemical and Physical Characterization tests performed were: specific solid weight, grain size distribution, laser analysis of grain size distribution, Atterberg limits, X ray fluorescence, X ray diffraction, scanning electron microscopy and soil compaction,. The materials and blends were also compacted and direct shear tests and plate load tests were performed. Plate load tests were conducted using a circular plate with 30 cm diameter, on specimens of pure soil and 5% blend, compacted in a metallic box inside the Soil Mechanics Laboratory of the Federal University of Rio Grande do Norte, Brazil. Both mechanical tests performed were conducted under inundated conditions, willing to reduce the influence of soil suction. An evaluation of the results of the tests performed shows that RPP is a fine material, with grain size distribution smaller than 0,015mm, composed mainly of silica and alumina, and particles in angular shape. The soil was characterized as a clayey sand, geologically known as a lateritic soil, with high percentages of alumina and iron oxide, and particles with rounded shape. Both the Soil and the blends presented low plasticity, while the residue showed a medium plasticity. Direct shear tests showed that the addition of RPP did not cause major changes into blends’ friction angle data, however, it was possible to note that, for the proportions studied, that is a tendency of obtain lower shear stresses for higher percentages of RPP in the blends. Both pure soil and 5% mixture showed a punching disruption for the Plate load test. For this same test, the allowable stress for 5% mixture was 44% higher than the pure soil, and smaller vertical settlement results for all stresses.

MENEZES, Patrick Lourenço. Erros pré-analíticos em medicina laboratorial: uma revisão sistemática. 2013. 98 f. Dissertação (Mestrado em Saúde, Medicina Laboratorial e Tecnologia Forense) - Instituto de Biologia Roberto Alcântara Gomes, Universidade do Estado do Rio de Janeiro, Rio de Janeiro, 2013. A relevância evidente dos erros pré-analíticos como problema de saúde pública fica patente tanto no dano potencial aos pacientes quanto nos custos ao sistema de saúde, ambos desnecessários e evitáveis. Alguns estudos apontam que a fase pré-analítica é a mais vulnerável a erros, sendo responsável por, aproximadamente, 60 a 90% dos erros laboratoriais em consequência da falta orientação aos pacientes sobre os procedimentos que serão realizados no laboratório clínico. Objetivos: Sistematizar as evidências científicas relacionadas aos erros pré-analíticos dos exames laboratoriais de análises clínicas. Método: Uma revisão sistemática foi realizada, buscando as bases de dados do Medical Literature Analysis and Retrieval System Online (MEDLINE), Scopus(que inclui MEDLINE e Embase), ISI Web of Knowledge, SciFinder, Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) (que inclui a Scientific Electronic Library Online SciELO) e o Índice Bibliográfico Espanhol de Ciências de Saúde (IBECS), para artigos publicados entre janeiro de 1990 e junho de 2012 sobre erros de exames laboratoriais que possam ocorrer na fase pré-analítica. Os estudos foram incluídos de acordo com os seguintes exames laboratoriais: hemograma, análise bioquímica do sangue total ou do soro, exames de coagulação sanguínea,uroanálise e exames hematológicos ou bioquímicos em outros materiais e categorizados pelo tipo de erro pré-analítico e pela frequência dos incidentes. Resultados: A busca nas bases de dados bibliográficas resultou no seguinte número de artigos recuperados: 547 na MEDLINE, 229 na Scopus, 110 na ISI, 163 na SciFinder, 228 na Lilacs e 64 na IBECS, perfazendo um total de 1.341 títulos. Ao fim da revisão sistemática, obteve-se um conjunto de 83 artigos para leitura de texto completo, dos quais 14 foram incluídos na revisão. Os estudos abrangeram diferentes tipos de laboratórios, setores técnicos e origem de erros, segundo a fase do processo laboratorial. Discussão: Sete artigos demonstraram erros de pedidos médicos, com uma alta variabilidade nos valores de incidência. Os seis artigos que estudaram erros de coleta de amostra observaram redução deste desfecho. As proporções de eventos adversos relatados e os impactos clínicos variaram, levando a consequências descritas como: erros decorrentes da flebotomia, recoleta de amostras, repetições de exames, atrasos na liberação de resultados de exames e possíveis danos ao paciente. Conclusões: O laboratório deve ter instruções por escrito para cada teste, que descreva o tipo de amostra e procedimento de coleta de amostra. Meios de identificação por código de barras, sistemas robóticos e analíticos reduzem os erros pré-analíticos. A melhoria da fase pré-analítica de testes laboratoriais permanece um desafio para muitos laboratórios clínicos.

Canine Visceral Leishmania (CVL) is an important zoonotic disease that has a world wide distribution and has a large impact on public health on the American Continent, especially in Brazil, where the nature of endemic diseases in humans affects a large part of the nation. The influence of the prevalence of CVL in the increased rate of human cases in endemic areas and in the unleashing of epidemic outbreaks shows the need for a more profound understanding, that would generate significant advances in the current measures used to control the reservoirs of sickness that are practiced by the Programa Nacional de Vigilância e Controle da Leishmaniose Visceral. The present work describes and compares the clinical-laboratorial and histopathological findings of twenty-three dogs that were naturally infected by Leishmania chagasi, from endemic areas in metropolitan Natal, Rio Grande do Norte, Brazil. These animals, that were selected and given physical and serological exams (IFI and ELISA rK-39), were classified according to the degree of clinical severity and had blood samples drawn (whole blood and serum) for a complete hemogram and a coagulogram to be done as well as biochemical tests for kidney and liver function. The confirmation of infection by L. chagasi was done after the euthanasia of the animals, through the direct demonstration of the parasite in the impression of the spleen and liver crowned with GIEMSA and through a cultivation by means of NNN/Schneider. According to the clinical evaluation, the animals were classified as asymptomatic (7), oligosymptomatic (7) and polysymptomatic (9). Among the animals that were chosen to be autopsied, there were 2 asymptomatic, 3 oligosymptomatic and 3 polysymptomatic, for the purpose of studying their histopathology, having collected fragments of the spleen, liver, kidneys and skin and were fixed in 10% tamponed formol. The comparison between the average parameters of the clinical-laboratory tested animals in the groups was done through the Student t test (a<0.05). The main clinical signals observed were lymphadenomegaly, alopecy, dermatitis, exfoliation, cutaneous ulcers, onicogriphosis and emaciation. The main clinical-laboratorial alterations established, mainly in the polysymptomatic group, were anemia, hyperproteinemia, hyperglobulinemia, alterations in the albumin/globulin ratio and increased ALT activity. Renal alterations were not verified (urea and creatinine levels were normal). Thrombocytopenia was observed in three clinical groups. However, the other indicators of coagulation function (TAP and TTPA) did not have abnormal variations. There were inflammatory infiltrations and leishmania amastigotes in the skin of polysymptomatic dogs, however, they were not found in the skin of asymptomatic animals. Hypertrophy and hyperplasia of the phagocyte mononuclear system, leishmania amastigote parasites were found in the macrophages, extramedullary hematopoiesis and degenerative alterations were detected in the spleen and liver of 8 of the animals submitted to histopathological exams. In accord with these results, it was demonstrated that the expected alterations in the hematological and biochemical parameters in function of their viscerotropic nature of CVL are mainly observed in the more advanced stages of the disease. The absence of inflammatory infiltration and parasite load in the skin suggest that infected animals without symptoms may have an importance irrelevant to the infectiousness of the vector

The identification of appropriate laboratory measures to confirm clinical hypotheses is important in routine paracoccidioidomycosis medical care. The clinical records and laboratory reports of 401 paracoccidioidomycosis patients attended at the Tropical Diseases Area, Faculdade de Medicina de Botucatu, from 1974 to 2008 were reviewed. Direct mycological (DM), cell block (CB), histopathological (HP), and double immunodiffusion (DID) tests were evaluated before treatment. Typical Paracoccidioides brasiliensis yeast forms were observed in clinical specimens of 86% of the patients, but 14% were detected only by serological test. DM of 51 different tissue specimens produced 74.5% sensitivity, and 62.5% sensitivity was observed in 112 sputum samples. CB in 483 sputum samples generated 55.3% sensitivity. HP performed in 239 samples from different tissues revealed 96.7% sensitivity. Serology carried out in 351 patients and 200 healthy controls provided 90.0% sensitivity, 100.0% specificity, 100.0% positive predictive value, 85.1% negative predictive value and 93.6% accuracy. Comparisons of laboratory measurements performed in the same patient showed that sensitivity decreases from HP to DID to CB and DM, with the last two assays providing similar sensitivities. This study demonstrated that P. brasiliensis identification by HP, CB, and/or DM associated with DID is sufficient to establish the laboratorial diagnosis of paracoccidioidomycosis in practically all cases. A hemoglobina S (HbS) está presente na população brasileira com prevalência variável, dependente dos grupos raciais formadores de cada região. A migração eletroforética em pH alcalino apresenta similaridade com outras hemoglobinas, e estudos complementares para sua correta caracterização são necessários. No presente estudo objetivamos traçar um fluxograma com as metodologias disponíveis para a caracterização da hemoglobina S e das hemoglobinas que apresentam migração semelhante em pH alcalino. No período de janeiro a junho de 2000, analisamos amostras de sangue com suspeita de hemoglobina S encaminhadas ao Laboratório de Hemoglobinas da Unesp. Caracterizamos diferentes mutantes e formas interativas com hemoglobina S, por procedimentos eletroforéticos, em variados pH, análises citológicas e testes bioquímicos específicos. Os procedimentos de análise aplicados resultaram em orientação fornecida aos laboratórios de rotina sobre como proceder no diagnóstico laboratorial destas alterações de hemoglobina. Desta forma contribuímos para um melhor conhecimento sobre a variabilidade genética das hemoglobinas em nossa população, auxiliando no acompanhamento clínico e no aconselhamento genético das hemoglobinopatias com fisiopatologia relacionada à alteração.

The protozoan parasite Giardia lamblia is responsible for a common intestinal infection in all regions of the world. In this study, four laboratory tests were evaluated for diagnostic reproducibility in the detection of this infection: Coprotest®, Direct modified method, Faust method and iron-hematoxylin staining. Positive diagnoses were tested for association with factors such as the age group and gender of the subject and the month when the sample was taken. Feces of 200 children in the Araraquara region (SP, Brazil) were examined by all four methods and the results compared. G. lamblia was found to be the most frequent parasite and 8.0% of the children showed giardiasis. There was no apparent correlation with gender, but most of the parasites were found in three- to five-year-olds. The highest frequency of infection occurred in January. The most reliable diagnostic results for G. lamblia were achieved by combining at least two methods of good reproducibility, i.e. Coprotest-Faust, Direct-Faust or Coprotest-Direct (A > 0.81).

Combination Antiretroviral Therapy (cART) aims to inhibit viral replication, delay immunodeficiency progression and improve survival in AIDS patients. The objective of this study was to compare two different schemes of cART, based on plasma viral load (VL) and CD4+ T lymphocyte count, during 48 weeks of treatment. For this purpose, 472 medical charts of a Specialized Outpatient Service were reviewed from 1998 to 2005. Out of these, 58 AIDS patients who had received a triple drug scheme as the initial treatment were included in the study and two groups were formed: Group 1 (G1): 47 individuals treated with two nucleoside reverse-transcriptase inhibitors (NRTI) and one non-nucleoside reverse-transcriptase inhibitor; Group 2 (G2): 11 patients treated with two NRTI and one protease inhibitor. In G1 and G2, 53.2% and 81.8% respectively were patients with an AIDS-defining disease. The T CD4+ lymphocyte count increased progressively up until the 24th week of treatment in all patients, while VL became undetectable in 68.1% of G1 and in 63.6% of G2. The study concluded that the evolutions of laboratory tests were similar in the two treatment groups and that both presented a favorable clinical evolution.
