Foi avaliado o emprego de metanosulfonato de hycanthone em dose única via intramuscular em pacientes esquistossomóticos crônicos, nas condições de ambulatório da Seção de Parasitoses da Prefeitura Municipal de Belo Horizonte. Estudaram-se 790 pacientes submetidos ao tratamento com 2,5 mg/k peso corporal, quantificando-se as reações colaterais referidas em função da forma clínica da esquistossomose, ao sexo e à idade dos pacientes, bem como em relação ao tempo de surgimento e à duração de cada reação. Os pacientes foram tratados após exame clínico e, em alguns casos, provas laboratoriais, sendo vetados ao tratamento ambulatorial cerca de 4% dos pacientes que buscaram o tratamento. De modo geral esta rotina tem-se mostrado bastante simples e destituída de maiores riscos, com mais de 12.000 tratamentos realizados sem uma única reação grave evidenciada. Dos 790 pacientes avaliados, 37,8% não referiram nenhuma reação colateral. Dentre as reações detectadas, as mais freqüentes foram as náuseas, os vômitos e as tonturas, geralmente surgindo no mesmo dia da injeção e com desaparecimento espontâneo nas 24 ou 48 horas subseqüentes. Relativamente às formas clínicas, não houve diferenças estatisticamente significativas entre o grupo hépato-esplênico e os demais, talvez por problemas de ordem amostra. Foi significativamente maior a freqüência de reações colaterais entre o sexo feminino, acima dos 15 anos de idade. Com relação aos grupos etários, verificou-se a ocorrência significativamente maior de vômitos entre os pacientes de 5 a 14 anos, contrastando com a predominância de tonturas e anorexia nas faixas superiores.

A localização muscular dos cistos hidáticos é incomum (0,5 a 5,4%) e geralmente secundária à doença hepática ou pulmonar. Reporta-se um caso de hidatidose intramuscular glútea, chamando-se a atenção para essa localização atípica da doença, que deve ser considerada na prática cirúrgica. A inexistência de cistos concomitantes nas sedes mais comuns confere interesse ainda maior ao caso apresentado.

BACKGROUND: In this study we compared the immunogenicity of influenza vaccine administered intradermally to the standard intramuscular vaccination in lung transplant recipients. METHODS: Patients were randomized to receive the trivalent inactivated seasonal 2008-9 influenza vaccine containing either 6 μg (intradermal) or 15 μg (intramuscular) of hemagglutinin per viral strain. Immunogenicity was assessed by measurement of geometric mean titer of antibodies using the hemagglutination-inhibition (HI) assay. Vaccine response was defined as a 4-fold or higher increase of antibody titers to at least one vaccine antigen. RESULTS: Eighty-five patients received either the intradermal (n = 41) or intramuscular (n = 44) vaccine. Vaccine response was seen in 6 of 41 patients (14.6%) in the intradermal vs 8 of 43 (18.6%) in the intramuscular group (p = 0.77). Seroprotection (HI ≥1:32) was 39% for H1N1, 83% for H3N2 and 29% for B strain in the intradermal group vs 28% for H1N1, 98% for H3N2 and 58% for B strain in the intramuscular group (p = 0.36 for H1N1, p = 0.02 for H3N2, p < 0.01 for B). Mild adverse events were seen in 44% of patients in the intradermal group and 34% in the intramuscular group (p = 0.38). CONCLUSIONS: Immunogenicity of the 2008-9 influenza vaccine given intradermally or intramuscularly was overall poor in lung transplant recipients. Novel strategies for influenza vaccination in this population are needed.

Complicações advindas da aplicação incorreta das injeções intramusculares podem acarretar sérios prejuízos orgânicos, favorecer o aparecimento de infecções e aumentar a permanência do paciente na instituição ou prejudicar as funções no trabalho. Estudo anterior detectou vários casos de pacientes com complicações locais após aplicação intramuscular de diclofenaco sódico em farmácias e concluiu que a falta de conhecimento da técnica de administração desse medicamento pode ter sido uma das causas das complicações. Diante disso o estudo atual teve o objetivo de identificar e avaliar o nível de conhecimentos de ocupacionais que administram medicamentos injetáveis intramusculares à população, em farmácias de Ribeirão Preto - SP. O estudo se constituiu num levantamento, a partir de um questionário contendo 20 questões abertas e fechadas, aplicado a uma população constituída por ocupacionais atuando em farmácias, selecionadas através do método da amostragem simples casual estratificada -partilha proporcional, resultando num total de 41 sujeitos. Concluímos que a população vem recebendo medicamento s injetáveis de ocupacionais com déficit de informações a respeito da técnica de aplicação de injetáveis. Os sujeitos referiram não receber treinamento adequado para exercer esse procedimento, terminando por aprendê-lo com outro funcionário ou observando alguém que o realize. Disto resulta lacunas de conhecimento em uma amostra de ocupacionais que podem, realmente, levar ao aparecimento de complicações pós injeções intramusculares na população que utiliza os serviços desses estabelecimentos.

Utilizando-se questionário contendo questões abertas e fechadas, o presente estudo teve como objetivos: identificar entre profissionais de enfermagem quais as regiões mais utilizadas para aplicação de medicamentos intramuscular; identificar o método que utilizam para delimitação das regiões e local de punção, identificar seu conhecimento acerca das complicações e contra-indicações para uso das regiões e averiguar a utilização da região ventro-glútea. Participaram do estudo 32 profissionais que referiram utilizar as regiões dorsoglútea (65,62%) e deltóidea (31,25%). Evidenciou-se que há necessidade de atualização desses profissionais, especialmente quanto a anatomia, utilização adequada de terminologia para denominação das regiões e conhecimentos acerca de complicações e contra-indicações. É essencial o investimento em treinamento desses profissionais para a utilização da região ventroglútea.

Triglycerides are transported in plasma by specific triglyceride-rich lipoproteins; in epidemiological studies, increased triglyceride levels correlate with higher risk for coronary artery disease (CAD). However, it is unclear whether this association reflects causal processes. We used 185 common variants recently mapped for plasma lipids (P < 5 × 10(-8) for each) to examine the role of triglycerides in risk for CAD. First, we highlight loci associated with both low-density lipoprotein cholesterol (LDL-C) and triglyceride levels, and we show that the direction and magnitude of the associations with both traits are factors in determining CAD risk. Second, we consider loci with only a strong association with triglycerides and show that these loci are also associated with CAD. Finally, in a model accounting for effects on LDL-C and/or high-density lipoprotein cholesterol (HDL-C) levels, the strength of a polymorphism's effect on triglyceride levels is correlated with the magnitude of its effect on CAD risk. These results suggest that triglyceride-rich lipoproteins causally influence risk for CAD.

Background: Immunogenicity of standard infl uenza vaccine is suboptimal in lung transplant recipients. Intradermal vaccine may elicit stronger responses due to recruitment of local dendritic cells. We compared the immunogenicity of the infl uenza vaccine administered intradermally (ID) to the standard intramuscular (IM) vaccination. Methods: In this investigator-blinded, two-center, prospective trial, lung transplant patients were randomized to receive intradermal (6ug) or intramuscular (15ug) 2008/9 trivalent inactivated infl uenza vaccine. Immunogenicity was evaluated using a standard hemagglutination inhibition assay (HIA). Response to the vaccine was defi ned as a fourfold increase of the HIA levels for any of the 3 viral strains in the vaccine. Geometric mean titers (GMT) and seroprotection rate (HIA ≥32) were also analyzed. Patients were followed during 6 months for the development of infl uenza or acute rejection. Results: We randomized 84 patients to receive the ID (n=41) vs. IM (n=43) vaccine, respectively. Baseline characteristics were similar between groups. Median time from transplantation was 3.4 yrs (ID) vs. 3.3 yrs (IM) (p=0.84). Vaccine response to at least one antigen was seen in 6/41 (14.6%) patients in the ID vs. 8/43 (18.6%) in the IM group (p=0.77). In the ID group, GMTs (95% CI) after vaccination were 15.7 (11.1-22.3) for H1N1, 84.0 (52.0-135.7) for H3N2, and 14.5 (9.6-21.8) for B strains vs. in the IM group 17.5 (11.8-25.9) for H1N1, 108.9 (77.5-153.2) for H3N2, and 20.2 (12.8-31.9) for B (p=NS, all 3 strains). Seroprotection was 39% (H1N1), 82.9% (H3N2) and 29.3% (B strain) in the ID group vs. 27.9% (H1N1), 97.7% (H3N2) and 58.1% (B strain) in the IM group. No factors associated with vaccine response were identifi ed. Mild adverse events were seen in 44% of patients (ID) vs. 34% (IM) (p=0.38). Two patients (4.8%) in the ID group developed infl uenza infection compared to none in the IM group. Two patients in each group developed biopsy-proven acute rejection during follow-up. Conclusions: Immunogenicity of the 2008/09 infl uenza vaccine was poor in lung transplant recipients. ID administration of the vaccine elicited similar immune responses to standard IM vaccination. Novel strategies of vaccination are needed to protect lung transplant recipients from infl uenza.

Se presentan los resultados obtenidos en la determinación de ácidos grasos trans en una serie de muestras de tejidos subcutáneo y muscular, procedentes de canales de cerdo, mediante cromatografía en fase gaseosa. Ambos tipos de grasas presentan valores similares y relativamente bajos, con un valor medio del 0.6% de 018:1 t. Se detecta una marcada influencia de la alimentación, puesta de manifiesto por las diferencias significativas obtenidas para los contenidos de C18:1 trans, en función del origen de las muestras. Igualmente, se recogen las correlaciones obtenidas entre los contenidos de C18:1 t. y los de ácidos grasos saturados, monoinsaturados y poliinsaturados, así como con el contenido graso intramuscular.

Se presentan los resultados obtenidos para la determinación de ácidos grasos en una serie de muestras de tejidos subcutáneo y muscular, procedentes de canales de vacuno, por aplicación de la cromatografía en fase gaseosa, para los que se obtuvieron unos valores medios de 58.7% de ácidos saturados, 39.1 % de monoinsaturados y 2.7% de polinsaturados, en el tejido adiposo, y de 44.7% de saturados, 46.1% de monoinsaturados y 9.4% de polinsaturados, en el tejido muscular. Los contenidos de ácidos grasos trans muestran diferencias significativas entre ambos tejidos (medias del 7% de ácidos trans totales en grasa intramuscular y 10.5% en grasa de depósito). El C18: 1t presenta una distribución paralela a la del total de ácidos trans, mientras que para el C16: 1 trans se observa un comportamiento claramente diferente, ya que no se presentan estas diferencias significativas entre ambos tejidos. En cuanto a los factores estudiados que pueden influir en el contenido de isómeros trans, cabe destacar que la raza fue aquél que ofrecía más diferencias, mientras que entre los diversos orígenes (explotaciones ganaderas) y entre categorías de canal se presentaron menos diferencias en relación a los contenidos de estos isómeros. También es importante destacar que las correlaciones que se han observado entre los contenidos de ácidos trans y los totales de ácidos saturados, mono y polinsaturados presentan un signo contrario, según el tipo de tejido. Así, un aumento del % de ácidos trans va aparejado con un aumento de saturados y una disminución de polinsaturados en el tejido muscular, mientras que va aparejado con una disminución de saturados y un aumento de mono y polinsaturados en el tejido adiposo.

Previous studies showed a fetal sheep liver extract (FSLE), in association with LPS, injected into aged (>20 months) mice reversed the altered polarization (increased IL-4 and IL-10 with decreased IL-2 and IFN-gamma) in cytokine production seen from ConA stimulated lymphoid cells of those mice. Aged mice show a >60% decline in numbers and suppressive function of both CD4(+)CD25(+)Foxp3(+)Treg and so-called Tr3 (CD4(+)TGFbeta(+)). Their number/function is restored to levels seen in control (8-week-old) mice by FSLE. We have reported at length on the ability of a novel pair of immunoregulatory molecules, members of the TREM family, namely CD200:CD200R, to control development of dendritic cells (DCs) which themselves regulate production of Foxp3(+) Treg. The latter express a distinct subset of TLRs which control their function. We report that a feature of the altered Treg expression following combined treatment with FSLE and monophosphoryl lipid A, MPLA (a bioactive component of lipid A of LPS) is the altered gene expression both of distinct subsets of TLRs and of CD200Rs. We speculate that this may represent one of the mechanisms by which FSLE and MPLA alter immunity in aged mice.

OBJECTIVE-Chronic exercise and obesity both increase intra-myocellular triglycerides (IMTGs) despite having opposing effects on insulin sensitivity. We hypothesized that chronically exercise-trained muscle would be characterized by lower skeletal muscle diacylglycerols (DAGs) and ceramides despite higher IMTGs and would account for its higher insulin sensitivity. We also hypothesized that the expression of key skeletal muscle proteins involved in lipid droplet hydrolysis, DAG formation, and fatty-acid partitioning and oxidation would be associated with the lipotoxic phenotype.RESEARCH DESIGN AND METHODS-A total of 14 normal-weight, endurance-trained athletes (NWA group) and 7 normal-weight sedentary (NWS group) and 21 obese sedentary (OBS group) volunteers were studied. Insulin sensitivity was assessed by glucose clamps. IMTGs, DAGs, ceramides, and protein expression were measured in muscle biopsies.RESULTS-DAG content in the NWA group was approximately twofold higher than in the OBS group and similar to 50% higher than in the NWS group, corresponding to higher insulin sensitivity. While certain DAG moieties clearly were associated with better insulin sensitivity, other species were not. Ceramide content was higher in insulin-resistant obese muscle. The expression of OXPAT/perilipin-5, adipose triglyceride lipase, and stearoyl-CoA desaturase protein was higher in the NWA group, corresponding to a higher mitochondrial content, proportion of type 1 myocytes, DAGs, and insulin sensitivity.CONCLUSIONS-Total myocellular DAGs were markedly higher in highly trained athletes, corresponding with higher insulin sensitivity, and suggest a more complex role for DAGs in insulin action. Our data also provide additional evidence in humans linking ceramides to insulin resistance. Finally, this study provides novel evidence supporting a role for specific skeletal muscle proteins involved in intramyocellular lipids, mitochondrial oxidative capacity, and insulin resistance. Influenza vaccines are recommended for administration by the intramuscular route. However, many physicians use the subcutaneous route for patients receiving an oral anticoagulant because this route is thought to induce fewer hemorrhagic side effects. Our aim is to assess the safety of intramuscular administration of influenza vaccine in patients on oral anticoagulation therapy. Methods: Design: Randomised, controlled, single blinded, multi-centre clinical trial. Setting: 4 primary care practices in Barcelona, Spain. Participants: 229 patients on oral anticoagulation therapy eligible for influenza vaccine during the 20032004 season. Interventions: intramuscular administration of influença vaccine in the experimental group (129 patients) compared to subcutaneous administration in the control group (100 patients). Primary outcome: change in the circumference of the arm at the site of injection at 24 hours. Secondary outcomes: appearance of local reactions and pain at 24 hours and at 10 days; change in INR (International Normalized Ratio) at 24 hours and at 10 days. Analysis was by intention to treat using the 95% confidence intervals of the proportions or mean differences. Results: Baseline variables in the two groups were similar. No major side effects or major haemorrhage during the follow-up period were reported. No significant differences were observed in the primary outcome between the two groups. The appearance of local adverse reactions was more frequent in the subcutaneous administration group (37,4% vs. 17,4%, 95% confidence interval of the difference 8,2% to 31,8%). Conclusion: This study shows that the intramuscular administration route of influenza vaccine in patients on anticoagulant therapy does not have more side effects than the subcutaneous administration route

Bioorganic and biological chemistry have been found to be highly motivating to undergraduate students and in this context, biochemical blood parameter analysis emerges as highly attractive content. In this proposal, several aspects related to analyses of glucose, cholesterol and triglycerides using the enzymatic colorimetric method were involved, and the findings have at least two relevant implications: i) introducing students to connections between organic chemistry and biology based on enzymatic processes, including reactivity and mechanistic aspects; ii) performing a micro scale bioassay analysis. The proposal requires two theoretical classes (2 h per class) and one practical class (4 h).
