Partout dans le monde, la formation en gestion des cadres infirmiers est devenue une nécessité. Afin de doter la profession infirmière de chefs de file capables de faire face aux changements, plusieurs pays ont conçu des programmes de formation de deuxième cycle en administration des services de soins infirmiers. Plusieurs programmes sont développés en ce sens dont peu ou prou sont évalués. Plusieurs auteurs croient que l’évaluation de ces programmes permettrait de mieux apprécier l’atteinte des objectifs pédagogiques et ultérieurement, d’améliorer la formation et les pratiques de gestion. Afin de contribuer à ce projet, la présente étude vise à évaluer le programme de formation Administration et enseignement de l'École nationale de développement sanitaire et social (ENDSS) du Sénégal. Cette évaluation prend en compte les points de vue des diplômés, des enseignants, des concepteurs du programme et des directions d’hôpitaux. Il s’agit d’une recherche évaluative de type rétrospectif. Une enquête par questionnaire et par entrevue a été réalisée auprès des diplômés, des enseignants, des concepteurs de programme et des directions d'hôpitaux. Également, des bases de données sur les étudiants finissants entre les années 2003 et 2009 ont été consultées pour recueillir les notes obtenues à l’examen de certification, les effectifs d’admission et d’inscrits et les renseignements sociodémographiques. Cette évaluation porte sur les principales composantes du programme de formation Administration et enseignement de l’ENDSS qui sont examinées à l’aide du modèle systémique d’évaluation de programme éducatif de Stufflebeam (1981) communément appelé modèle CIPP. Les résultats de l’analyse s’appuient sur 70 questionnaires et 37 entrevues. En vue de répondre aux questions de recherche, les données recueillies sont analysées et interprétées à partir d’indicateurs et selon des critères définis. La démarche d’analyse des résultats s’est effectuée en deux étapes. D'abord, les données des questionnaires et des entrevues fournies par les diplômés, les enseignants et les concepteurs ont été analysées ainsi que celles du dossier scolaire des étudiants inscrits au programme. Ensuite, l’examen a porté sur les données des questionnaires recueillies auprès des directions d’hôpitaux. Les avis de tous les répondants sur les améliorations à apporter au programme sont analysés pour rendre compte de leurs recommandations à ce sujet. Tout au long de l’analyse des résultats, les données des questionnaires et des entrevues sont présentées à la fois. Le jugement sur l’efficacité du programme de formation Administration et enseignement de l’ENDSS est établi par l’analyse d’indicateurs reliés aux quatre types d'évaluation proposés par Stufflebeam (1981). Les diplômés et les concepteurs jugent que la formation est adéquate pour l’exercice de la fonction de chef de services des soins infirmiers. Par contre, les enseignants sont très partagés sur cette question, puisque près de la moitié d'entre eux expriment un avis favorable tandis que les autres ont une opinion contraire. Les diplômés apparaissent très partagés par rapport à la cohérence apparente entre les objectifs de stage d’administration hospitalière et les compétences à développer, alors que les enseignants et les concepteurs expriment plus largement une opinion négative. La cohérence entre les notions du module de Management des services de santé et l’exercice de la fonction de chef de services des soins infirmiers est jugée plus forte en ce qui concerne plusieurs notions, par les diplômés, les enseignants et les concepteurs. La majorité des diplômés et des enseignants jugent que la supervision offerte aux étudiants est insatisfaisante. Les diplômés et les enseignants croient que la disponibilité des ressources didactiques à la bibliothèque ne facilite pas les apprentissages. Tandis que les concepteurs, en proportion plus élevée, expriment le même jugement. Les diplômés et les enseignants sont divisés sur la question de l’équilibre entre le nombre d’heures alloué à la théorie et à la formation pratique. Tandis que la majorité des concepteurs croient qu’il n’y a pas d'équilibre entre le nombre d'heures consacré aux cours et à la formation pratique. Les diplômés et les enseignants expriment une opinion très positive sur la qualité des rapports sociaux des étudiants entre eux, avec les enseignants et le personnel administratif. Tandis que les concepteurs ont une opinion plutôt négative à ce sujet. Les diplômés, de même que les enseignants jugent en majorité, que la supervision des stages offerte aux étudiants est insatisfaisante. Ces opinions convergent avec celles de la grande majorité des concepteurs. Selon les diplômés, le travail de groupe, de même que les études de cas sont les méthodes les plus pertinentes dans la formation de chef de service des soins infirmiers. Pour les enseignants et les concepteurs c'est le travail de groupe qui est la méthode la plus pertinente pour faire acquérir des connaissances. Parmi l’ensemble des inscrits, incluant les étudiants sénégalais et étrangers, 104 d’entre eux ont obtenu un diplôme. Subséquemment, seuls 2 étudiants sur les 106 inscrits au programme ont échoué. Les enseignants, les diplômés et les concepteurs sont tous satisfaits du programme de formation Administration et enseignement de l’ENDSS. Les directions d'hôpitaux sont partagées quant à elles, à l’idée selon laquelle les étudiants sont préparés aux fonctions de chef de service des soins infirmiers. En plus, selon la grande majorité des directions le programme de formation ne répond pas à la stratégie de la réforme hospitalière. Elles affirment à l’unanimité qu’elles n’ont pas été associées au processus d’élaboration du programme et sont d’accord que, de manière générale, la formation reçue par le chef de services des soins infirmiers est de qualité. Unanimement, les directions d’hôpitaux jugent que des notions devraient être ajoutées ou améliorées au programme de formation Administration et enseignement de l’ENDSS. L’interprétation des données révèle la complexité qui entoure l’importance de l’équilibre du volume horaire à consacrer à la théorie et à la pratique dans un programme de formation. En plus, les résultats sur les connaissances à acquérir par les diplômés durant la formation montrent un besoin élevé de perfectionnement voire d’enseignement plus poussé du contenu des notions Système d’information sanitaire et Introduction à l’informatique, Introduction au management; Élaboration de programme et Administration hospitalière. Un choix approprié de terrain où l’étudiant pourrait réaliser les objectifs d’apprentissage, une sélection de la période adéquate pour le stage, de même qu’un encadrement signifiant, seraient le gage d’un stage profitable pour l’étudiant. Nous estimons que cette recherche a ouvert des pistes à des perspectives de recherches reliées au développement professionnel des futurs chefs de service des soins infirmiers. This study assessed if hospital-wide implementation of a needleless intravenous connection system reduces the number of reported percutaneous injuries, overall and those specifically due to intravenous connection activities.^ Incidence rates were compared before and after hospital-wide implementation of a needleless intravenous system at two hospitals, a full service general hospital and a pediatric hospital. The years 1989-1991 were designated as pre-implementation and 1993 was designated as post-implementation. Data from 1992 were not included in the effectiveness evaluation to allow employees to become familiar with use of the new device. The two hospitals showed rate ratios of 1.37 (95% CI = 1.22-1.54, p $\le$.0001) and 1.63 (95% CI = 1.34-1.97, p $\le$.0001), or a 27.1% and a 38.6% reduction in overall injury rate, respectively. Rate ratios for intravenous connection injuries were 2.67 (95% CI = 1.89-3.78, p $\le$.0001) and 3.35 (95% CI = 1.87-6.02, p $\le$.0001), or a 62.5% and a 69.9% reduction in injury rate, respectively. Rate ratios for all non-intravenous connection injuries were calculated to control for factors other than device implementation that may have been operating to reduce the injury rate. These rate ratios were lower, 1.21 and 1.44, demonstrating the magnitude of injury reduction due to factors other than device implementation. It was concluded that the device was effective in reduction of numbers of reported percutaneous injuries.^ Use-effectiveness of the system was also assessed by a survey of randomly selected device users to determine satisfaction with the device, frequency of use and barriers to use. Four hundred seventy-eight surveys were returned for a response rate of 50.9%. Approximately 94% of respondents at both hospitals expressed satisfaction with the needleless system and recommended continued use. The survey also revealed that even though over 50% of respondents report using the device "always" or "most of the time" for intravenous medication administration, flushing lines, and connecting secondary intravenous lines, needles were still being used for these same activities. Compatibility, accessibility and other technical problems were reported as reasons for using needles for these activities. These problems must be addressed, by both manufacturers and users, before the needleless system will be effective in prevention of all intravenous connection injuries. ^

Objective: To ascertain the extent to which neonatal analgesia was used in Australia for minor invasive procedures as an indicator of evidence-based practice in neonatology. Methods: A cross-sectional telephone survey of hospitals in all Australian states and territories with more than 200 deliveries per year was carried out. Questions were asked regarding awareness of the benefits and the use of analgesia for minor invasive procedures in term and near term neonates. Analysis was undertaken according to state and territory, annual birth numbers and the level of neonatal nursery care available. Results: Data were available from 212 of 214 eligible hospitals. Of the total respondents, 51% and 70% respectively were aware of the benefits of sucrose and breast-feeding for neonatal analgesia. Eleven per cent of units administered sucrose before venepuncture and 25% of units used breast-feeding. Ten per cent of units used sucrose before heel prick with 49% utilizing breast-feeding. Expressed breast milk was used in 10% of units. Analgesia was given less frequently before intravenous cannulation compared to venepuncture and heel prick. Awareness and implementation of neonatal analgesia varied widely in the states and territories. There was a trend for hospitals providing a higher level of neonatal care to have a greater awareness of sucrose as an analgesic (P < 0.0001) and the use of sucrose for venepuncture (P = 0.029), heel prick (P = 0.025) and intravenous catheter insertion (P = 0.013). Similar trends were found on analysis according to birth number of the maternity units. Smaller units had a greater usage of breast-feeding as an analgesic for heel prick (P = 0.017). Conclusion: Despite good evidence for the administration of sucrose and breast milk in providing effective analgesia for newborn infants, it is not widely used in Australia. It is imperative that the gap between research findings and clinical practice with regard to neonatal analgesia be addressed.

BACKGROUND: The neonatal and pediatric antimicrobial point prevalence survey (PPS) of the Antibiotic Resistance and Prescribing in European Children project (http://www.arpecproject.eu/) aims to standardize a method for surveillance of antimicrobial use in children and neonates admitted to the hospital within Europe. This article describes the audit criteria used and reports overall country-specific proportions of antimicrobial use. An analytical review presents methodologies on antimicrobial use.
METHODS: A 1-day PPS on antimicrobial use in hospitalized children was organized in September 2011, using a previously validated and standardized method. The survey included all inpatient pediatric and neonatal beds and identified all children receiving an antimicrobial treatment on the day of survey. Mandatory data were age, gender, (birth) weight, underlying diagnosis, antimicrobial agent, dose and indication for treatment. Data were entered through a web-based system for data-entry and reporting, based on the WebPPS program developed for the European Surveillance of Antimicrobial Consumption project.
RESULTS: There were 2760 and 1565 pediatric versus 1154 and 589 neonatal inpatients reported among 50 European (n = 14 countries) and 23 non-European hospitals (n = 9 countries), respectively. Overall, antibiotic pediatric and neonatal use was significantly higher in non-European (43.8%; 95% confidence interval [CI]: 41.3-46.3% and 39.4%; 95% CI: 35.5-43.4%) compared with that in European hospitals (35.4; 95% CI: 33.6-37.2% and 21.8%; 95% CI: 19.4-24.2%). Proportions of antibiotic use were highest in hematology/oncology wards (61.3%; 95% CI: 56.2-66.4%) and pediatric intensive care units (55.8%; 95% CI: 50.3-61.3%).
CONCLUSIONS: An Antibiotic Resistance and Prescribing in European Children standardized web-based method for a 1-day PPS was successfully developed and conducted in 73 hospitals worldwide. It offers a simple, feasible and sustainable way of data collection that can be used globally.

The traditional process of filling the medicine trays and dispensing the medicines to the patients in the hospitals is manually done by reading the printed paper medicine chart. This process can be very strenuous and error-prone, given the number of sub-tasks involved in the entire workflow and the dynamic nature of the work environment. Therefore, efforts are being made to digitalise the medication dispensation process by introducing a mobile application called Smart Dosing application. The introduction of the Smart Dosing application into hospital workflow raises security concerns and calls for security requirement analysis. This thesis is written as a part of the smart medication management project at Embedded Systems Laboratory, A° bo Akademi University. The project aims at digitising the medicine dispensation process by integrating information from various health systems, and making them available through the Smart Dosing application. This application is intended to be used on a tablet computer which will be incorporated on the medicine tray. The smart medication management system include the medicine tray, the tablet device, and the medicine cups with the cup holders. Introducing the Smart Dosing application should not interfere with the existing process carried out by the nurses, and it should result in minimum modifications to the tray design and the workflow. The re-designing of the tray would include integrating the device running the application into the tray in a manner that the users find it convenient and make less errors while using it. The main objective of this thesis is to enhance the security of the hospital medicine dispensation process by ensuring the security of the Smart Dosing application at various levels. The methods used for writing this thesis was to analyse how the tray design, and the application user interface design can help prevent errors and what secure technology choices have to be made before starting the development of the next prototype of the Smart Dosing application. The thesis first understands the context of the use of the application, the end-users and their needs, and the errors made in everyday medication dispensation workflow by continuous discussions with the nursing researchers. The thesis then gains insight to the vulnerabilities, threats and risks of using mobile application in hospital medication dispensation process. The resulting list of security requirements was made by analysing the previously built prototype of the Smart Dosing application, continuous interactive discussions with the nursing researchers, and an exhaustive stateof- the-art study on security risks of using mobile applications in hospital context. The thesis also uses Octave Allegro method to make the readers understand the likelihood and impact of threats, and what steps should be taken to prevent or fix them. The security requirements obtained, as a result, are a starting point for the developers of the next iteration of the prototype for the Smart Dosing application.
