The purpose of this study was to determine the effectiveness of the “Champion” training of the Therapeutic Recreation Specialist – Certified (TRSC) at Baycrest Health Sciences (BHS). BHS recently implemented a new model for Therapeutic Recreation Services that employs the model of champion for implementation of both best and next practices within the organization. This mixed methods study used both case study and program evaluation in order to understand whether the training that comprised of five different topics allowed the six participants to develop the skills needed to be champions. The results supported that learning did occur during the training and that the experience was positive for the participants. The overall finding from this study is that while the training was useful, the participants did not feel confident about utilizing these skills without further training; hence, this training can only be considered an introduction to the concepts presented.

Le p-tert-octylphénol est un produit présent dans l’environnement et issu de la dégradation des alkylphénols éthoxylés. Ce composé a la capacité de se lier au récepteur œstrogénique et d’exercer ainsi un léger effet œstrogénique. Les objectifs de cette étude étaient de 1) développer une méthode d'identification de l'octylphénol dans le sang et les tissus à l'aide de la chromatographie en phase gazeuse jumelée à la spectrométrie de masse, 2) caractériser la toxicocinétique sanguine et tissulaire de l’octylphénol chez le rat Sprague-Dawley mâle et femelle et 3) développer un modèle toxicocinétique à base physiologique permettant de décrire la cinétique sanguine et tissulaire de l’octylphénol inchangé. Pour ce faire, des rats mâle et femelle Sprague-Dawley ont reçu des doses uniques d’octylphénol par les voies intraveineuse, orale et sous-cutanée. Deux autres groupes ont reçu des doses répétées d'octylphénol par voie orale pour une durée de 35 jours consécutifs pour les femelles ou 60 jours pour les mâles. Les concentrations sanguines et tissulaires d’octylphénol ont été mesurées à différents moments après administration à partir d’une méthode d’analyse développée dans nos laboratoires dans le cadre de ce projet. Les expériences impliquant des administrations uniques ont montré que les concentrations sanguines et tissulaires d'octylphénol étaient en général plus élevées chez les femelles que chez les mâles. Des expériences réalisées avec des microsomes hépatiques ont confirmé que ces différences étaient vraisemblablement reliées au métabolisme de l'octylphénol. Les expériences impliquant des administrations répétées ont montré qu'il n'y avait pas d'accumulation d'octylphénol dans l'organisme aux doses étudiées. Les résultats obtenus expérimentalement ont servi à développer et valider un modèle toxicocinétique à base physiologique. Ce modèle a permis de simuler adéquatement les concentrations sanguines et tissulaires d'octylphénol suite à des expositions intraveineuses, orales et sous-cutanées. En conclusion, cette étude a fourni des données essentielles sur la toxicocinétique de l'octylphénol. Ces données sont nécessaires pour établir la relation entre la dose externe et la dose interne et vont contribuer à une meilleure évaluation des risques liés à l'octylphénol.

De nombreux travailleurs utilisent le captan et le folpet comme fongicides en agriculture, mais leur exposition n’est pas toujours mesurée de manière spécifique et précise. La surveillance biologique est un excellent outil à cet effet puisqu’elle permet de quantifier l’exposition réelle. Toutefois, la majorité des connaissances toxicologiques pour ces fongicides proviennent d’études sur les animaux, et les données chez l’humain sont limitées. Le but du présent projet est donc de développer des outils de surveillance biologique pour évaluer l’exposition de travailleurs au captan et au folpet. Dans cette perspective, le projet a été subdivisé en trois parties complémentaires, soit i) de développer des méthodes analytiques spécifiques pour quantifier les biomarqueurs d’intérêt du captan, à savoir le tétrahydrophtalimide (THPI), et du folpet, à savoir le phtalimide (PI) et l’acide phtalique, dans le plasma et l’urine; ii) de déterminer la toxicocinétique des deux fongicides en exposant des volontaires de façon aigüe à de faibles doses de captan ou de folpet par voie orale et cutanée dans des conditions semi-contrôlées et en quantifiant les biomarqueurs dans chacune des deux matrices, excepté l’acide phtalique qui a été mesuré seulement dans l’urine; iii) de valider les biomarqueurs d’exposition sélectionnés et d’évaluer l’exposition réelle des travailleurs et les voies prédominantes d’exposition au captan et au folpet en collectant des données biologiques chez des travailleurs en arboriculture et en viticulture lors d’activités de traitement et d’effeuillage pendant sept jours consécutifs. Selon ces travaux, le THPI et le PI sont deux biomarqueurs valides et spécifiques pour quantifier l’exposition au captan et au folpet, respectivement, chez l’humain. En effet, les méthodes développées pour ces deux métabolites sont robustes avec des limites de détection plus sensibles que celles rapportées dans la littérature, un taux de recouvrement de 90% pour le THPI et de 75% pour le PI, une très bonne linéarité (R2>0,99) et une bonne stabilité avec des variations intra- et inter-journalières faibles (RSD<15%). Elles ont permis de déterminer les profils cinétiques des deux métabolites chez les volontaires et chez les travailleurs. Ces derniers indiquent d’ailleurs une élimination rapide, avec une demi-vie d’élimination dans l’urine de 11,7 h et 18,7 h pour le THPI et de 27,3 h et 28,8 h pour le PI, respectivement après une absorption par voie orale et cutanée, ainsi qu’une faible absorption cutanée lorsque les valeurs sont comparées pour les deux voies d’exposition. Des profils parallèles sont aussi observés entre le PI et l’acide phtalique pour les volontaires et les agriculteurs, mais le folpet se retrouve davantage métabolisé sous forme d’acide phtalique que de PI. Quant à l’étude des agriculteurs, elle montre que la voie principale d’exposition de ces travailleurs est la voie cutanée. Il est aussi souligné qu’il est important 1) de favoriser les collectes d’urines complètes sur 24 h au urines ponctuelles, 2) de mesurer plusieurs métabolites, et 3) d’associer les données de surveillance biologique à la toxicocinétique. Ainsi, les connaissances acquises par cette étude peuvent s’appliquer à d’autres fongicides, voire d’autres substances.

Objectives In April 2010, the Université de Montréal’s Health Sciences Library has implemented shared filters in its institutional PubMed account. Most of these filters are designed to highlight resources for evidence-based practice, such as Clinical Queries, Systematic Reviews and Evidence-based Synopsis. We now want to measure how those filters are perceived and used by our users. Methods For one month, data was gathered through an online questionnaire proposed to users of Université de Montréal’s PubMed account. A print version was also distributed to participants in information literacy workshops given by the health sciences librarians. Respondents were restricted to users affiliated to Université de Montréal’s faculties of Medicine, Dentistry, Veterinary Sciences, Nursing and Pharmacy. Basic user information such as year/program of study or department affiliation was also collected. The questionnaire allowed users to identify the filters they use, assess the relevance of filters, and also suggest new ones. Results Survey results showed that the shared filters of Université de Montreal’s PubMed account were found useful by the majority of respondents. Filters allowing rapid access to secondary resources ranked among the most relevant (Reviews, Systematic Reviews, Cochrane Database of Systematic Reviews, Practice Guidelines and Clinical Evidence). For Clinical Study Queries, Randomized Controlled Trial (Therapy/Narrow) was considered the most useful. Some new shared filters have been suggested by respondents. Finally, 18% of the respondents indicated that they did not quite understand the relevance of filters. Conclusion Based on the survey results, shared filters considered most useful will be kept, some will be enhanced and others removed so that suggested ones could be added. The fact that some respondents did not understand well the relevance of filters could potentially be addressed through our PubMed workshops, online library guides or by renaming some filters in a more meaningful way.

Objective: To portray an information literacy programme demonstrating a high level of integration in health sciences curricula and a teaching orientation aiming towards the development of lifelong learning skills. The setting is a French-speaking North American university. Methods: The offering includes standard workshops such as MEDLINE searching and specialised sessions such as pharmaceutical patents searching. A contribution to an international teaching collaboration in Haiti where workshops had to be thoroughly adapted to the clientele is also presented. Online guides addressing information literacy topics complement the programme. Results and evaluation: A small team of librarians and technicians taught 276 hours of library instruction during the 2011-2012 academic year. Methods used for evaluating information skills include scoring features of literature searches and user satisfaction surveys. Discussion: Privileged contacts between librarians and faculty resulting from embedded library instruction as well as from active participation in library committees result in a growing reputation of library services across academic departments and bring forth collaboration opportunities. Sustainability and evolution of the library instruction programme is warranted by frequent communication with partners in the clinical field, active involvement in academic networks and health library associations, and reflective professional strategies.

Educating health professionals implies the challenge of creating and developing an inquiring mind, ready to be in a state of permanent questioning. For this purpose, it is fundamental to generate a positive attitude toward the generation of knowledge and science. Objective: to determine the attitude toward science and the scientific method in undergraduate students of health sciences. Materials and methods: a cross-sectional study was made by applying a self-administered survey, excluding those who were transferred from other universities and repeated. The attitude toward science and the scientific method were valued using the scale validated and published by Hren, which contains three domains: value of scientific knowledge, value of scientific methodology, and value of science for health professions. Results: 362 students were included, 86,6% of them graded the attitude toward scientific knowledge above 135 points, neutral scale value. Similar scores were registered in the domains value of scientific knowlede for the human dimension of the students and value of science for health professions. 91,4% of the students graded the value of scientific methodology below 48 points. Conclusions: the favorable attitude of the students can be explained by the contact that they have with the scientific method since the beginning of their studies and its concordance with the evolution of science. The domain value of scientific methodology obtained the lowest grade on the part of the students, which could be related to the lack of knowledge about scientific methodology.

Objective: To identify and address particular challenges in the teaching of epidemiological concepts to undergraduate students in non-clinical health disciplines. Methods and Results: Relevant pedagogical literature was reviewed to identify a range of evidence-based teaching approaches. The authors also drew on their experience in curriculum development and teaching in this field to provide guidelines for teaching epidemiology in a way that is engaging to students and likely to promote deep, rather than surface, learning. Discussion of a range of practical strategies is included along with applied examples of teaching epidemiological content. Conclusions and Implications: Increasingly, there is a greater emphasis on improved learning outcomes in higher education. Graduates from non-clinical health courses are required to have a core understanding of epidemiology and teachers of epidemiology need to be able to access resources that are relevant and useful for these students. A theoretically grounded framework for effective teaching of epidemiological principles to non-clinical undergraduates is provided, together with a range of useful teaching resources (both paper and web-based). Implementation of the strategies discussed will help ensure graduates are able to appropriately apply epidemiological skills in their professional practice.

Environmental context Soils contaminated with metals can pose both environmental and human health risks. This study showed that a common crop vegetable grown in the presence of cadmium and zinc readily accumulated these metals, and thus could be a source of toxicity when eaten. The work highlights potential health risks from consuming crops grown on contaminated soils. Abstract Ingestion of plants grown in heavy metal contaminated soils can cause toxicity because of metal accumulation. We compared Cd and Zn levels in Brassica rapa, a widely grown crop vegetable, with that of the hyperaccumulator Solanum nigrum L. Solanum nigrum contained 4 times more Zn and 12 times more Cd than B. rapa, relative to dry mass. In S. nigrum Cd and Zn preferentially accumulated in the roots whereas in B. rapa Cd and Zn were concentrated more in the shoots than in the roots. The different distribution of Cd and Zn in B. rapa and S. nigrum suggests the presence of distinct metal uptake mechanisms. We correlated plant metal content with the expression of a conserved putative natural resistance-associated macrophage protein (NRAMP) metal transporter in both plants. Treatment of both plants with either Cd or Zn increased expression of the NRAMP, with expression levels being higher in the roots than in the shoots. These findings provide insights into the molecular mechanisms of heavy metal processing by S. nigrum L. and the crop vegetable B. rapa that could assist in application of these plants for phytoremediation. These investigations also highlight potential health risks associated with the consumption of crops grown on contaminated soils.
