OBJECTIVE - To evaluate the Coronary Flow Reserve in the Coronary Sinus through transesophageal Doppler echocardiography in normal subjects. METHODS - We obtained technically adequate flow samples for analysis in 10 healthy volunteers (37±8 years, 5 men) with no history of heart or systemic disease and with mean left ventricular mass index by transthoracic echocardiography of 87±18 g/m². Coronary sinus flow velocity was recorded within the coronary sinus with the patient in a resting condition and during intravenous adenosine infusion at a dose of 140 µg/kg/min for 4 minutes. Recording of coronary sinus blood flow was possible in all cases with measurement of peak systolic, diastolic, and retrograde velocities (PSV, PDV, and PRV, cm/sec), mean systolic and diastolic velocities (MSV and MDV, cm/sec), and systolic and diastolic velocity time integral (VTI S and VTI D, cm/sec). RESULTS - The coronary flow reserve was calculated as the ratio between the blood flow in the basal state and the maximum measured hyperemic blood flow with adenosine infusion. Results are shown as mean and standard deviations. (CFR = PSV + PDV -- PRV/basal PSV): 1st min = 2.2±0.21; 2nd min = 3±0.3; 3rd min = 3.4±0.37; 4th min = 3.6 ± 0.33. CONCLUSION - Although coronary sinus flow had significantly increased in the first minute, higher velocities were seen at third and fourth minutes, indicating that these should be the best times to study coronary sinus flow with intravenous adenosine in continuous infusion.

My final project presents the directing process of my experimental production based on King Lear by William Shakespeare. I describe the process from the first visions of the beginning to the completed performance and finally the feedback from the audience. I concentrate on the special qualities of the production, such as interaction, small and moveable audience, cinematic qualities, polyphonic dramaturgy, and use of the video. The Project is divided into six parts. The first part introduces the project as a whole. The second part concentrates on the process before the beginning of the rehearsals. The third part focuses on the rehearsals and co-operation with the actors, whereas the fourth part deals with the performance and its special qualities. Following this, in the fifth part I study the interaction of the performance with the help of the audience feedback. In the final part conclusions are drawn. The focus of this work is the use of intuition even as a starting point of the theatre directing process. My conclusions drawn from the description of the process suggest that the starting point of the theatre production may be an intuitive vision as well as some conscious subject or message.

Opinnäytetyöni Kahden kulttuurin ääniä on monimuototyö, jonka teososa koostuu kuudesta suunnittelemastani ja äänittämästäni konkreettisen musiikin teoksesta. Materiaali on äänitetty 2006 kesällä ja syksyllä Suomessa ja Japanissa. Kirjallisessa osassa tarkastelin lajityypin historiaa sekä omaan työskentelyyni peilaten keinoja, joilla tein kenttä-äänistä musiikillisen teoksen jaettuna kolmeen eri teemaan kunkin maan äänimateriaalista: luontoäänet, kaupunkiäänet sekä uskonnollisten palvelusten äänet. Teoksille ei tehty ennakkokäsikirjoitusta, vaan niiden kokoaminen tapahtui konkreettisen musiikin perinteiden mukaisesti vasta äänityöasemalla. Työ koostettiin ja miksattiin stereona ja surroundina (5.0) Pro Toolsissa vuoden 2007 aikana. Osa äänimateriaalista säilyi käsittelemättömänä, osa käsiteltiin lähes tunnistamattomaksi. Kahden kulttuurin ääniä poikkeaa lajityypin klassikoista nopealla tempollaan ja runsaalla tunnelmien vaihtelevuudellaan. Toisinaan työ lähentelee jo nykyaikaisia elektronisen musiikin alalajeja. Teokset sisältävät paljon elementtejä, jotka aiheuttavat eri ihmisissä erilaisia assosiaatioita. Tämän vuoksi konkreettinen musiikki voisi hyvin toimia muun muassa musiikkiterapian välineenä.

OBJECTIVES: The purpose of this study was to determine whether thoracic endovascular aortic repair (TEVAR) reduces death and morbidity compared with open surgical repair for descending thoracic aortic disease. BACKGROUND: The role of TEVAR versus open surgery remains unclear. Metaregression can be used to maximally inform adoption of new technologies by utilizing evidence from existing trials. METHODS: Data from comparative studies of TEVAR versus open repair of the descending aorta were combined through meta-analysis. Metaregression was performed to account for baseline risk factor imbalances, study design, and thoracic pathology. Due to significant heterogeneity, registry data were analyzed separately from comparative studies. RESULTS: Forty-two nonrandomized studies involving 5,888 patients were included (38 comparative studies, 4 registries). Patient characteristics were balanced except for age, as TEVAR patients were usually older than open surgery patients (p = 0.001). Registry data suggested overall perioperative complications were reduced. In comparative studies, all-cause mortality at 30 days (odds ratio [OR]: 0.44, 95% confidence interval [CI]: 0.33 to 0.59) and paraplegia (OR: 0.42, 95% CI: 0.28 to 0.63) were reduced for TEVAR versus open surgery. In addition, cardiac complications, transfusions, reoperation for bleeding, renal dysfunction, pneumonia, and length of stay were reduced. There was no significant difference in stroke, myocardial infarction, aortic reintervention, and mortality beyond 1 year. Metaregression to adjust for age imbalance, study design, and pathology did not materially change the results. CONCLUSIONS: Current data from nonrandomized studies suggest that TEVAR may reduce early death, paraplegia, renal insufficiency, transfusions, reoperation for bleeding, cardiac complications, pneumonia, and length of stay compared with open surgery. Sustained benefits on survival have not been proven.
