Las oportunidades conocidas de intervenciones tempranas en los accidentes cardiocerebrovasculares (ACV) desde el punto de vista médico y de rehabilitación hacen necesario avanzar en la formulación de indicadores del desempeño clínico en el manejo fisioterapéutico hospitalario de una persona con ACV. Objetivo: identificar los indicadores de desempeño clínico fisioterapéutico en el manejo hospitalario temprano de personas sobrevivientes de ACV. Materiales y métodos: se trata de un estudio exploratorio descriptivo que indaga sobre las pruebas y prácticas fisioterapéuticas existentes, la evidencia científica sobre indicadores clínicos en ACV, los indicadores estándar y los posibles indicadores clínicos en el escenario. Se llevó a cabo una revisión sistemática de estudios descriptivos, guías de práctica clínica, revisiones sistemáticas, estudio de casos clínicos basados en la evidencia en bases de datos como Pubmed, Proquest, Pedro y en revistas electrónicas, además del análisis de datos epidemiológicos de la prevalencia del ACV en Colombia y en Chile, en páginas web de la Organización Mundial de la Salud, Ministerio de Salud y Departamento AdministrativoNacional de cada país. Resultados: la evidencia señala que la rehabilitación temprana del ACV debe iniciarse durante la hospitalización, tan pronto como el diagnóstico se establezca y los problemas que ponen en riesgo la vida del paciente estén controlados. Las prioridades en la intervención fisioterapéutica en ACV son prevenir las complicaciones (trombosis venosa, infecciones y dolor) y facilitar la movilización temprana. Actualizaciones recientes de estas directrices incluyen la rehabilitación temprana, en particular la movilidad luego de las veinticuatro horas posteriores al ACV.Las pruebas que sustentan los indicadores de desempeño en rehabilitación para la atención en la etapa subaguda del ACV son escasas. Conclusiones: se hace notoria la importancia de la atención temprana fisioterapéutica en el proceso agudo del paciente con ACV, puesto que la evidencia disponible resalta un mejor pronóstico para pacientes que son intervenidos por el área dentro de las primeras veinticuatro horas posteriores al evento y la importancia de los indicadores de atención del paciente, como la calidad y el tiempo de atención, la periodicidad y el aprovechamiento de la intervención dentro de la ventana temporal aguda de recuperación del paciente.

The curriculum of the Bucknell University Chemical Engineering Department includes a required senior year capstone course titled Process Engineering, with an emphasis on process design. For the past ten years library research has been a significant component of the coursework, and students working in teams meet with the librarian throughout the semester to explore a wide variety of information resources required for their project. The assignment has been the same from 1989 to 1999. Teams of students are responsible for designing a safe, efficient, and profitable process for the dehydrogenation of ethylbenzene to styrene monomer. A series of written reports on their chosen process design is a significant course outcome. While the assignment and the specific chemical technology have not changed radically in the past decade, the process of research and discovery has evolved considerably. This paper describes the solutions offered in 1989 to meet the information needs of the chemical engineering students at Bucknell University, and the evolution in research brought about by online databases, electronic journals, and the Internet, making the process of discovery a completely different experience in 1999.

OBJECTIVE: To describe the electronic medical databases used in antiretroviral therapy (ART) programmes in lower-income countries and assess the measures such programmes employ to maintain and improve data quality and reduce the loss of patients to follow-up. METHODS: In 15 countries of Africa, South America and Asia, a survey was conducted from December 2006 to February 2007 on the use of electronic medical record systems in ART programmes. Patients enrolled in the sites at the time of the survey but not seen during the previous 12 months were considered lost to follow-up. The quality of the data was assessed by computing the percentage of missing key variables (age, sex, clinical stage of HIV infection, CD4+ lymphocyte count and year of ART initiation). Associations between site characteristics (such as number of staff members dedicated to data management), measures to reduce loss to follow-up (such as the presence of staff dedicated to tracing patients) and data quality and loss to follow-up were analysed using multivariate logit models. FINDINGS: Twenty-one sites that together provided ART to 50 060 patients were included (median number of patients per site: 1000; interquartile range, IQR: 72-19 320). Eighteen sites (86%) used an electronic database for medical record-keeping; 15 (83%) such sites relied on software intended for personal or small business use. The median percentage of missing data for key variables per site was 10.9% (IQR: 2.0-18.9%) and declined with training in data management (odds ratio, OR: 0.58; 95% confidence interval, CI: 0.37-0.90) and weekly hours spent by a clerk on the database per 100 patients on ART (OR: 0.95; 95% CI: 0.90-0.99). About 10 weekly hours per 100 patients on ART were required to reduce missing data for key variables to below 10%. The median percentage of patients lost to follow-up 1 year after starting ART was 8.5% (IQR: 4.2-19.7%). Strategies to reduce loss to follow-up included outreach teams, community-based organizations and checking death registry data. Implementation of all three strategies substantially reduced losses to follow-up (OR: 0.17; 95% CI: 0.15-0.20). CONCLUSION: The quality of the data collected and the retention of patients in ART treatment programmes are unsatisfactory for many sites involved in the scale-up of ART in resource-limited settings, mainly because of insufficient staff trained to manage data and trace patients lost to follow-up.

Adobe's Acrobat software, released in June 1993, is based around a new Portable Document Format (PDF) which offers the possibility of being able to view and exchange electronic documents, independent of the originating software, across a wide variety of supported hardware platforms (PC, Macintosh, Sun UNIX etc.). The principal features of Acrobat are reviewed and its importance for libraries discussed in the context of experience already gained from the CAJUN project (CD-ROM Acrobat Journals Using Networks). This two-year project, funded by two well-known journal publishers, is investigating the use of Acrobat software for the electronic dissemination of journals, on CD-ROM and over networks.

Introducción El modelo desequilibrio esfuerzo – recompensa es un importante predictor de estrés laboral y por consiguiente de alteraciones en la salud cardiovascular, por ello en los últimos 10 años se ha encontrado importante evidencia y estudios respecto al tema; las publicaciones científicas se han enfocado en establecer relación entre factores psicosociales en el trabajo y consecuencias sobre la salud dándole relevancia a los factores biológicos principalmente los cardiovasculares. Objetivo Se evaluó la evidencia entre el desequilibrio esfuerzo recompensa y las alteraciones cardiovasculares en los trabajadores. Metodología Se realizó una revisión de la literatura en principales bases de datos como son pubmed, ovid y revistas electrónicas, en idioma inglés – español durante los últimos 10 años, usando palabras de búsqueda como desequilibrio esfuerzo recompensa, alteraciones cardiovasculares, trabajadores. Resultados Se encontró fuerte evidencia entre el desequilibrio esfuerzo recompensa con enfermedades cardiovasculares en los trabajadores, principalmente con las alteraciones en la tensión arterial en los trabajadores más expuestos, al igual que una relación mayor en mujeres que hombres que aumentaba con la edad. Se evidenció que después de haber sufrido un evento coronario agudo los trabajadores que continuaban expuestos a altas cargas de tensión laboral eran más propensos a desarrollar enfermedad coronaria recurrente. El sobrecompromiso (PR 1,91, IC 95% 1,35-2,69) y el desequilibrio esfuerzo recompensa (PR 2,47, IC 95% 1,62-3,75) se asociaron con riesgo de hipertensión arterial después de ajustar las variables de confusión. Se encontró asociación entre el desequilibrio esfuerzo recompensa e hipertensión arterial (OR 1,53-3,71 IC 95%). La ERI alta y la baja recompensa se asociaron con enfermedad coronaria recurrente (RR = 1,75, intervalo de confianza del 95% [IC] = 0,99 - 3,08 y HR = 1,77, IC del 95% = 1,16 - 2,71). Hubo una interacción de género que mostró efectos más fuertes entre las mujeres (HR respectivamente ajustadas para ERI alta y baja recompensa: HR = 3,95, IC del 95% = 0,93- 16,79, y HR = 9,53, IC del 95% = 1,15- 78,68). Conclusiones Se encontró una relación entre el desequilibrio esfuerzo recompensa con enfermedades cardiovasculares, principalmente con las alteraciones en la tensión arterial y eventos coronarios

The research landscape is changing rapidly, and as a consequence the roles of libraries and librarians in supporting and working with researchers is also changing. Some of the drivers behind the changes in research practices and culture include: new technologies, government funding and measurement of research impact, and the importance of open access to data. In Australia, librarians work with researchers to help them identify high quality resources, increase their publication rate and manage and promote access to their research. QUT Library has established a number of initiatives to support researchers, including: establishment of the QUT digital repository ‘ePrints’; purchase of electronic books and electronic journals; programmes of workshops for researchers ; redesign of Library space and, and the creation of new staffing positions. The creation of the QUT ePrints repository was a major new initiative for the QUT Library. ePrints is a web-accessible repository of research outputs created for QUT staff and postgraduate students. The ePrints information is harvested by Google, and anyone searching for a QUT staff member on Google can find their publications listed in ePrints. This keynote address will discuss the context for the role of libraries in building research endeavours, highlight some examples of strategies and resources to supporting researchers. It will conclude with an outline of some key online resources for researchers in education. This presentation should be relevant for both individual researchers interested in conducting and promoting their own research, and for staff and organisations focused on building their support for research.

The Queensland Centre for Social Science Innovation was formed in 2012 to develop collaborations among the Queensland Government and five Queensland universities—The University of Queensland, Griffith University, Queensland University of Technology, James Cook University and Central Queensland University. Three priorities for initial projects were established by the Queensland Government with response by the participating universities. This project addressed the identified priority area: factors affecting educational achievement and investigation of the link between school design, refurbishment and educational outcomes. The proposal for this project indicated that a Review of research literature would be undertaken that linked school and classroom design with educational outcomes for learners in the 21st century. Further, research would be examined for impact of technology on staff and students, as well as learning spaces that addressed the diversity of student learners. Specific investigation of research on effective design to enhance learning outcomes for Aboriginal and Torres Strait Islander students was to be undertaken. The project therefore consists of a Review of research literature to provide an evidence base on the impact of school and classroom on educational outcomes. The original proposal indicated that indicators of successful school and classroom design would be student learning outcomes on a range of variables, with input, the specific architectural design elements. The review was undertaken during the period July 2012 to June 2013. A search was undertaken of journals, databases, and websources to identify relevant material. These were examined for evidence-based statements and design of learning spaces to enhance learning. The Review is comprehensive, and representative of issues raised in research, and conceptualisations and debates informing modern educational design. Initial findings indicated two key findings central to reading this Review. The first key finding is that the predominant focus of modern design of learning space is on process and the engagement of stakeholders. Schools are social institutions and development of a school as a learning space to suit 21st century learning needs necessarily involves the staff, students and other members of the community as key participants. The concept of social aspects of design is threaded throughout the Review. The second key finding is that little research explicitly examined the relationship between the design of learning spaces and educational outcomes. While some research does exist, the most explicitly-focused research uses narrow test-based achievement as the learning outcomes. These are not sympathetic to the overall framings of the research on 21st century learning, future schooling and the needs of the new generation of learners and society.

The current mainstream scientific-publication process has so far been only marginally affected by the possibilities offered by the Internet, despite some pioneering attempts with free electronic-only journals and electronic preprint archives. Additional electronic versions of traditional paper journals for which one needs a subscription are not a solution. A clear trend, for young researchers in particular, is to go around subscription barriers (both for paper and electronic material) and rely almost exclusively on what they can find free on the Internet, which often includes working versions posted on the home pages of the authors. A survey of how scientists retrieve publications was conducted in February 2000, aimed at measuring to what extent the opportunities offered by the Internet are already changing the scientific information exchange and how researchers feel about this. This paper presents the results based on 236 replies to an extensive Web-based questionnaire, which was announced to around 3,000 researchers in the domains of construction information technology and construction management. The questions dealt with how researchers find, access, and read different sources; how many and what publications they read; how often and to which conferences they travel; how much they publish, and criteria for where they eventually decide to publish. Some of the questions confronted traditional and electronic publishing, with one final section dedicated to opinions about electronic publishing. According to the survey, researchers already download half of the material that they read digitally from the Web. The most popular method for retrieving an interesting publication is downloading it for free from the author's or publisher's Web site. Researchers are not particularly willing to pay for electronic scientific publications. There is much support for a scenario of electronic journals available freely in their entirety on the Web, where the costs could be covered by, for instance, professional societies or the publishing university.

A survey in the closely related domains of construction information technology and construction management was conducted in February 2000, aimed at measuring to what extent these opportunities are already changing the scientific information exchange and how researchers feel about the changes. The paper presents the results based on 236 replies to an extensive Web based questionnaire. 65% of the respondents stated their primary research interest as IT in A/E/C and 20% as construction management and economics. The questions dealt with how researchers find, access and read different sources; how much and what publications they read; how often and to which conferences they travel; how much they publish, and what are the criteria for where they eventually decide to publish. Some of the questions confronted traditional and electronic publishing with one final section dedicated to opinions about electronic publishing. According to the survey researchers already download half of the material that they read digitally from the Web. The most popular method for retrieving an interesting publication is downloading it for free from the author’s or publisher’s website. Researchers are not particularly willing to pay for electronic scientific publications. There is much support for a scenario of electronic journals available totally freely on the Web, where the costs could be covered by for instance professional societies or the publishing university. The shift that the Web is causing seems to be towards the "just in time" reading of literature. Also, frequent users of the Web rely less on scientific publications and tend to read fewer articles. If available with little effort, papers published in traditional journals are preferred; if not, the papers should be on the Web. In these circumstances, the role of paper-based journals published by established publishers is shifting from the core "information exchange" to the building of authors' prestige. The respondents feel they should build up their reputations by publishing in journals and relevant conferences, but then make their work freely available on the Web.

A commuting triple of operators (A, B, P) on a Hilbert space H is called a tetrablock contraction if the closure of the set E = {(a(11),a(22),detA) : A = GRAPHICS] with parallel to A parallel to <1} is a spectral set. In this paper, we construct a functional model and produce a set of complete unitary invariants for a pure tetrablock contraction. In this construction, the fundamental operators, which are the unique solutions of the operator equations A - B* P = DPX1DP and B - A* P = DPX2DP where X-1, X-2 is an element of B(D-P) play a pivotal role. As a result of the functional model, we show that every pure tetrablock isometry (A, B, P) on an abstract Hilbert space H is unitarily equivalent to the tetrablock contraction (MG1*+G2z, MG2*+G1z, M-z) on H-DP*(2). (D), where G(1) and G(2) are the fundamental operators of (A*, B*, P*). We prove a Beurling Lax Halmos type theorem for a triple of operators (MF1*+F2z, MF2*+F1z, M-z), where epsilon is a Hilbert space and F-1, F-2 is an element of B(epsilon). We also deal with a natural example of tetrablock contraction on a functions space to find out its fundamental operators.

A presente pesquisa Design de jornal impresso: a relação entre formato e usabilidade tem como principal objetivo reforçar a importância do envolvimento do usuário no design de produtos, através da compreensão da influência que o formato definido pelo tamanho das páginas e organização dos cadernos exerce sobre a usabilidade do jornal impresso. São apresentados os aspectos tecnológicos e econômicos relacionados a esse produto e verificado a influência exercida por eles sobre o formato. É feito um mapeamento sobre os principais aspectos do design de jornais e sobre os diferentes formatos de jornais, suas origens e sua relevância para o design de jornais. Busca-se mapear o formato dos jornais em diferentes mercados e os possíveis impactos das mudanças de formato na circulação, no conteúdo e na publicidade dos jornais impressos. Faz-se uma revisão do panorama atual dos jornais eletrônicos e suas possíveis interferências nos hábitos de leitura. Revisa-se a bibliografia referente à usabilidade, concentrando-se na metodologia de design de documentos e sua aplicabilidade para testes de usabilidade em jornais impressos. Por fim, descreve-se a pesquisa de campo através de entrevistas com especialistas em design de jornais, de observações sistemáticas em gráficas de jornal e sobre o mercado de jornais impressos da cidade do Rio de Janeiro e de testes de usabilidade aplicados a jornais. Conclui-se que, considerando o público participante dos testes profissionais que trabalham com impressão de jornais , apenas o aspecto do formato relacionado à organização dos cadernos parece exercer influência sobre a usabilidade dos jornais.

Purpose – To consider the economic and physical impact of electronic journals on remotely stored print stock. Design/methodology/approach – A collection of print journals was used as an object for consideration. Physical and heritage aspects of the collection are examined and questions are posed regarding the wisdom of future retention in response to increased demand for electronic alternatives. Findings – Emerging trends predict a predominance of periodical literature in electronic form. The future of local remote storage for low demand printed journal collections needs to be evaluated in economic as well as cultural terms. Research limitations/implications – Based on a collection at the Boole Library, University College Cork, Ireland. Practical implications – Similar consideration should be given to collections in other regional libraries. Originality/value – Contributes to discussions on the long-term value of retaining print journal holdings.

Objective: To assess the impact of treatment foster care (TFC) on psychosocial and behavioral outcomes, delinquency, placement stability, and discharge status for children and adolescents who, for reasons of severe medical, social, psychological and behavioural problems, were placed in out-of-home care in restrictive settings or at risk of placement in such settings. Method: Electronic bibliographic databases, web searches, and article reference lists were used to identify randomized controlled trials (RCTs) investigating the effectiveness of TFC with children and young people. The Cochrane Collaboration’s criteria were used to assess the methodological quality of studies that met the inclusion criteria. Wherever possible, extracted outcome data from similar studies were synthesized with random effects meta-analyses. Results: A total of 5 studies including 390 participants were included in this review. Data suggest that TFC may be a useful intervention for children and young people with complex emotional, psychological, and behavioural need, who are at risk of placements in nonfamily settings that restrict their liberty and opportunities for social inclusion. Conclusion: Although the inclusion criteria for this systematic review set a study design threshold higher than that of previous reviews, the findings mirror those of earlier reviews. While the results of individual studies generally indicate that TFC is a promising intervention for children and youth experiencing mental health problems, behavioral problems, or problems of delinquency, the evidence base is not robust and more research is needed due to the limited number of studies in this area.

The full extent to which inherited disorders occur in different breeds of domestic horse (Equus caballus) has not been previously been investigated. A systematic search was carried out to review scientific literature on inherited disorders in domestic horse breeds and examine patterns in potentially inherited disorders. A two-part search was conducted: (i) electronic bibliographic databases for published studies; and (ii) existing online databases of inherited disorders in animals. A total of 230 papers were identified, discussing 102 inherited disorders in the horse. Few cases (17) were found in which disorders were reported to have a direct link to a conformational or phenotypic trait. Forty-nine breeds of domestic horse were described as being predisposed to one or more inherited disorders, but such predispositions did not distinguish between genetic or environmental causes. There were few patterns in the number of disorders to which breeds were reportedly predisposed or in the extent to which disorders were researched. The structure and grouping of disorders presented here could assist with standardisation in the terminology used for describing inherited disorders. © 2012 Universities Federation for Animal Welfare The Old School.
