As Doenças inflamatórias intestinais (DII) são multifatoriais e sua etiologia envolve susceptibilidade genética, fatores ambientais, disbiose e ativação exacerbada do sistema imunológico no intestino. Essas doenças também tem sido relacionadas a baixos níveis de dehidroepiandrosterona (DHEA), um hormônio precursor de diversos esteroides e relacionado à modulação das respostas imunes. Porém, os mecanismos precisos que relacionam as ações deste hormônio com a proteção ou susceptibilidade à doença de Crohn ou colite ulcerativa ainda não são totalmente conhecidos. Sendo assim, este projeto buscou entender o papel imunomodulador do DHEA exógeno in vitro e in vivo durante a inflamação intestinal experimental induzida por dextran sulfato de sódio (DSS) em camundongos C57BL/6. Inicialmente, in vitro, DHEA inibiu a proliferação de células do baço de forma dose dependente nas concentrações de 5?M, 50?M ou 100?M, com diminuição da produção de IFN-?. Este hormônio não foi tóxico para células de linhagem mieloide, embora tenha causado necrose em leucócitos nas doses mais elevada (50 ?M e 100?M), o que pode ter influenciado a diminuição das citocinas in vitro. Nos ensaios in vivo, os camundongos tratados com DHEA (40 mg/Kg) foram avaliados na fase de indução da doença (dia 6) e durante o reparo tecidual, quando os animais expostos ao DSS e ao DHEA por 9 dias foram mantidos na ausência destas drogas até o dia 15. Houve diminuição do escore pós-morte, melhora no peso e nos sinais clínicos da inflamação intestinal, com redução de monócitos no sangue periférico com 6 dias e aumento de neutrófilos circulantes na fase de reparo tecidual (15 dias). Ainda, a suplementação com DHEA levou à redução da celularidade da lâmina própria (LP) e ao restabelecimento do comprimento normal do intestino. O uso deste hormônio também diminuiu a expressão do RNAm de IL-6 e TGF-?, enquanto aumentou a expressão de IL-13 no colón dos animais durante a fase de indução da doença, o que provavelmente ajudou na atenuação da inflamação intestinal. Além disso, houve acúmulo de linfócitos CD4+ e CD8+ no baço e diminuição apenas de linfócitos CD4+ nos linfonodos mesentéricos (LNM), indicando retenção das células CD4+ no baço após uso do DHEA. O tratamento foi também capaz de aumentar a frequência de células CD4 produtoras de IL-4 e diminuir CD4+IFN-?+ no baço, além de reduzir a frequência de CD4+IL-17+ nos LNM, sugerindo efeito do DHEA no balanço das respostas Th1/Th2/Th17 relacionadas à colite. Em adição, as células de baço dos animais tratados com DHEA e expostos ao DSS se tornaram hiporresponsivas, como visto pela diminuição da proliferação após re-estímulos in vitro. Finalmente, DHEA foi capaz de atuar no metabolismo dos camundongos tratados, levando à diminuição de colesterol total e da fração LDL no soro durante a fase de indução da doença, sem gerar quaisquer disfunções hepáticas. Com isso, podemos concluir que o DHEA atua por meio do balanço das respostas imunes exacerbadas, minimizando os danos locais e sistêmicos causados pela inflamação intestinal induzida por DSS.

Salivary cortisol (C) and DHEA concentrations were measured in 9 elite swimmers (4 female and 5 male) over a 37-week period, 5 to 12 times per swimmer, before 68 competitions. For female and male swimmers, no significant relationship was found between C, DHEA and performance. For the whole group, C was negatively correlated with week number of training (r = -0.31, p < 0.01). The incorporation of the cumulated distance swum as a second variable in the regression increased r to 0.56 (p < 0.01). The higher the cumulated distance swum, the higher C. No significant relationship was found between DHEA and distance swum. For individual swimmers, 3 of 4 females showed a significant negative relationship between C and cumulated dry-land training. No equivalent relationship was found for DHEA. The 2 males practicing dry-land training showed a significant and negative relationship between DHEA and cumulated dry-land training. No equivalent relationship was found for C. Thus, C and DHEA were not good predictors of swimming performance. C for individual females, and DHEA for individual males were considered useful markers for dry-land training stress.

The ability of gonadal hormones to influence and induce diverse immunological functions during the course of a number of parasitic infections has been extensively studied in the latest decades. Dehydroepiandrosterone and its sulfate are the most abundant steroid hormones secreted by the human adrenal cortex and are considered potent immune-activators. The effects of orchiectomy on the course of Trypanosoma cruzi infection in rats, treated and untreated with DHEA were examined, by comparing blood and cardiac parasitism, macrophage numbers, nitric oxide and IFN-gamma levels. Orchiectomy enhanced resistance against infection with elevated numbers of macrophages, enhanced concentrations of NO and IFN-gamma and reduced amastigote burdens in heart when compared to control animals. DHEA replacement exerted a synergistic effect, up-modulating the immune response. Male sex steroids appear to play fundamental role in determining the outcome of disease, through the regulation and modulation of the activity of the immune response. (C) 2008 Elsevier Inc. Dehydroepiandrosterone (DHEA) enhances immune responses against a wide range of viral, bacterial, and parasitic pathogens. In a previous study, we reported that administration of DHEA significantly decreased the numbers of blood parasites in Trypanosoma cruzi experimental infection. The present study was undertaken to determine the effectiveness of DHEA in reducing the severity of acute phase T cruzi infection of male and female Wistar rats. Animals were treated subcutaneously with 40 mg/kg body weight/day of DHEA. The concentration of nitric oxide (NO) was determined in spleen peritoneal cavity. Interleukin-2 (IL-2) and interferon-gamma (IFN-gamma) were determined in the sera of uninfected and infected animals. DHEA treatment augments NO production for both sexes after in vitro LPS treatment for uninfected animals. Infection triggered enhanced NO levels although not significant. IL-2 and IFN-gamma were detectable in higher concentrations in treated and infected rats of both genders when compared to untreated controls. These data suggest that DHEA may have a potent immunoregulatory function that can affect the course of T cruzi infection. (c) 2008 Elsevier B.V. All rights reserved.

DHEA, a steroid hormone synthesized from cholesterol by cells of the adrenal cortex, plays an essential role in enhancing the host`s resistance to different experimental infections. Receptors for this hormone can be found in distinct immune cells (especially macrophages) that are known to be the first line defense against Trypanosoma cruzi infection. These cells operate through an indirect pathway releasing nitric oxide (NO) and cytokines such TNF-alpha and IL-12 which in turn trigger an enhancement of natural killer cells and lymphocytes which finally secrete pro and anti-inflammatory cytokines. The effects of pre- and post-infection DHEA treatment on production of IL-12, TNF alpha and NO were evaluated. T. cruzi infected macrophages post treated with DHEA displayed enhanced concentrations of TNF-alpha, IL-12 and NO. Probably, the mechanisms that induced the production of cytokines by infected cells are more efficient when the immune system has been stimulated first by parasite invasion, suggesting that the protective role of DHEA is greater when administered post infection. Understanding the mechanisms responsible for mediating the effects of stress on Trypanosoma cruzi infection is crucial for determining the full impact of stress on Chagas` disease and for devising effective interventions. Dehydroepiandrosterone (DHEA), a steroid hormone synthesized from pregnenolone, is secreted by the adrenal cortex in response to stress. Although its physiologic role has not been fully defined, DHEA has been shown to modulate immune function. In the present study, we evaluated the levels of corticosterone and the ability of T. cruzi infection to modulate the expression of Th2 cytokines in Wistar rats with chronic Chagas` disease submitted to repetitive stress. The animals submitted to stress displayed enhanced levels of corticosterone as compared to control counterparts. Stress and infection triggered the most elevated concentrations of corticosterone. DHEA significantly reduced corticosterone levels for infected and stressed animals with DHEA. The infected animals displayed enhanced levels of IL-10 and IL-4 as compared to control ones. Stress combined with infection triggered the higher levels of IL-10 and IL-4. DHEA alone and combined with infection and stress significantly increased IL-10 and IL-4 levels. Then, this study might provide additional clues about factors that regulate some of the immunoregulatory aspects of T. cruzi infection and might offer new opportunities for therapeutic interventions. Previous studies showed that melatonin or dehydroepiandrosterone (DHEA) enhances the immune response against parasitic pathogens. The present study investigated the in vitro activity of melatonin combined with DHEA in a period of 24 hr during the course of in vivo T. cruzi infection. The in vitro activity of melatonin or DHEA alone, as well as together, were tested for the trypomastigote forms (doses ranging from 0.5 to 128 mu m). In vitro, neither melatonin nor DHEA alone had any activity against trypomastigote forms, although when the highest concentration of combined melatonin and DHEA was used, it was active against the trypomastigote forms of the parasite. However, for this concentration, a quite toxicity on peritoneal macrophages was observed. For in vivo evaluation, male Wistar rats were infected with the Y strain of T. cruzi. They were orally treated with 10 mg/kg body weight/day of melatonin and subcutaneously with 40 mg/kg body weight/day of DHEA. Treatment with melatonin, DHEA and the association showed a significant reduction in the number of blood trypomastigotes during the acute phase of infection as compared to untreated animals (P < 0.05). A significant increase in the number of macrophages and nitric oxide (NO) concentrations were observed during the peak of parasitaemia with melatonin alone or combined with DHEA. However, with DHEA alone the highest concentration of NO was observed (P < 0.05). Moreover, DHEA treatment increased TNF-alpha levels during the infection (P < 0.05). These results show that melatonin, DHEA or the combination of both reduces parasitemia during the acute phase of infection. The combined action of both molecules did not exert a synergic action on the host`s ability to fight infection, and it seems that among all treatments DHEA induces a more efficient immune response.

Previous studies have shown a significant effect of insulin administration on serum dehydroepiandrosterone sulfate (DHEA-S) concentration and its metabolic rate, with evidence for the effect in men, but not in women. This could lead to differences in the sources of variation in serum DHEA-S between men and women and in its covariation with insulin concentration. This study aimed to test whether these hypotheses were supported in a sample of healthy adult twins. Serum DHEA-S (n=2287) and plasma insulin (n=2436) were measured in samples from adult male and female twins recruited through the Australian Twin Registry. Models of genetic and environmental sources of variation and covariation were tested against the data. DHEA-S showed substantial genetic effects in both men and women after adjustment for covariates, including sex, age, body mass index, and time since the last meal. There was no significant phenotypic or genetic correlation between DHEA-S and insulin in either men or women. Despite the experimental evidence for insulin infusion producing a reduction in serum DHEA-S and some effect of meals on the observed DHEA-S concentration, there were no associations between insulin and DHEA-S at the population level. Variations in DHEA-S are due to age, sex, obesity, and substantial polygenic genetic influences.

Objetivo: Conduzir uma revisão sistemática sobre resiliência psicológica e/ou hardiness em militares, explorando seus aspectos psicossociais, neurobiológicos, preditores e promotores. Métodos: Utilizaram-se as bases de dados PubMed/MedLine, ISI/Web of Science e PsycINFO, incluindo artigos empíricos publicados nas línguas inglesa, portuguesa e espanhola até maio de 2012. Os seguintes termos foram utilizados: “militar*”, “Army”, “war”, “veteran*”, “resilien*” e “hardiness”. Resultados: Foram incluídos 32 estudos selecionados a partir de 1.205 artigos. O foco da maioria das pesquisas recai sobre a correlação resiliência/hardiness e aspectos psicossociais. Confirmou-se o papel protetivo da resiliência/hardiness quanto ao transtorno de estresse pós-traumático (TEPT), assim como a associação direta entre resiliência e saúde. Neuropeptídeo Y (NPY) e deidroepiandrosterona (DHEA) foram os biomarcadores mais estudados. Os níveis de NPY no plasma podem representar um correlato biológico de resiliência ou recuperação dos efeitos adversos do estresse. Somente dois estudos abordaram fatores preditores de resiliência em amostras militares, sugerindo ser a exposição a situações adversas, o apoio social e o gênero fatores considerados preditores desse construto. Apenas um estudo avaliou a eficiência de intervenção para fortalecer a resiliência. Conclusão: Apesar da crucial relevância da resiliência, há poucos estudos em amostras militares. Estudos neurobiológicos como os do NPY são promissores. A ausência de ensaio randomizado controlado avaliando eficácia de intervenções promotoras da resiliência demonstra como esse construto vem sendo negligenciado nessa profissão de risco, constituindo área prioritária para foco de estudos futuros.

We present a method for the analysis of urinary 16(5alpha)-androsten-3alpha-ol together with 5beta-pregnane-3alpha,20alpha-diol and four testosterone metabolites: androsterone (Andro), etiocholanolone (Etio), 5alpha-androstane-3alpha,17beta-diol (5alphaA), 5beta-androstane-3alpha,17beta-diol (5betaA) by means of gas chromatography/combustion/isotopic ratio mass spectrometry (GC/C/IRMS). The within-assay and between-assay precision S.D.s of the investigated steroids were lower than 0.3 and 0.6 per thousand, respectively. A comparative study on a population composed of 20 subjects has shown that the differences of the intra-individual delta(13)C-values for 16(5alpha)-androsten-3alpha-ol and 5beta-pregnane-3alpha,20alpha-diol are less than 0.9 per thousand. Thereafter, the method has been applied in the frame of an excretion study following oral ingestion of 50 mg DHEA initially and oral ingestion of 50mg pregnenolone 48 h later. Our findings show that administration of DHEA does not affect the isotopic ratio values of 16(5alpha)-androsten-3alpha-ol and 5beta-pregnane-3alpha,20alpha-diol, whereas the isotopic ratio values of 5beta-pregnane-3alpha,20alpha-diol vary by more 5 per thousand upon ingestion of pregnenolone. We have observed delta(13)C-value changes lower than 1 per thousand for 16(5alpha)-androsten-3alpha-ol, though pregnenolone is a precursor of the 16-ene steroids. In contrast to 5beta-pregnane-3alpha,20alpha-diol, the 16-ene steroid may be used as an endogenous reference compound when pregnenolone is administered.

RAPPORT DE SYNTHESE : L'hirsutisme, l'acné et l'alopécie chez la femme sont souvent associés à des troubles menstruels et à une production excessive d'androgènes, raison pour laquelle ces symptômes cutanés font l'objet d'évaluation endocrinienne. L'hyperandrogénie affecte 5 à 10 % des femmes en âge de reproduction et constitue un motif fréquent de consultation. Récemment, les sociétés d'endocrinologie ont émis des recommandations sur l'investigation et le traitement de l'hyperandrogénie. Longtemps confrontés à la demande de patientes souffrant d'hirsutisme, d'acné ou d'alopécie, nous avons décidé d'effectuer une approche diagnostique et thérapeutique comportant des dosages hormonaux et un traitement antiandrogénique. Un grand nombre de patientes a été ainsi étudié au fil des années. Les paramètres mesurés incluaient la testostérone plasmatique totale, l'androstènedione, le sulfate de déhydroépiandrostérone (DHEAS), la sex hormone-binding globulin (SHBG) et la testostérone salivaire. Cette dernière est considérée comme un bon reflet de la testostérone libre plasmatique, indépendamment des protéines de liaison. L'analyse rétrospective des dossiers nous a permis de comparer nos données avec celles de la littérature. Des 318 dossiers de patientes ayant consulté notre Service pour hirsutisme, acné ou alopécie pendant 6 ans, 228 ont pu être retenus pour une évaluation adéquate. Chez les patientes présentant ces symptômes de façon isolée, les taux d'androgènes et la prévalence de l'oligo-aménorhée étaient plus élevés en cas d'hirsutisme qu'en cas d'alopécie, avec des valeurs intermédiaires en cas d'acné. Aucun des androgènes mesurés ne permettait, à lui seul, d'identifier tous les cas d'hyperandrogénie, mais la testostérone salivaire a montré la meilleure corrélation positive avec l'hirsutisme, alors que la testostérone plasmatique totale montrait la moins bonne corrélation, et l'androstènedione, le DHEAS et la SHBG des corrélations intermédiaires (corrélation négative pour la SHBG). De plus, au cours du traitement antiandrogénique, la testostérone salivaire a montré l'abaissement proportionnel le plus marqué de tous les androgénes mesurés. Comparées aux patientes originaires d'Europe centrale, les patientes originaires d'Europe du sud consultaient avec des degrés d'hirsutisme supérieurs, mais aucune différence n'a été observée dans les corrélations entre l'hirsutisme et les taux hormonaux de ces deux groupes. En l'absence d'un nombre suffisant d'échographies ovariennes, .la prévalence du syndrome des ovaires polykystiques a été probablement sous-estimée (63 patientes, 27.6 % des cas), au bénéfice du diagnostic d'hyperandrogénie avec euménorrhée (101, 44.3 %); les autres diagnostics étaient: androgénes normaux (51, 22.4%), SHBG basse isolée (7, 3.1%), hyperplasie surrénalienne congénitale non-classique (4, 1.8%), et tumeur ovarienne (2, 0.9%). Nous avons comparé les divers traitements médicaux de l'hirsutisme publiés au cours des 25 dernières années, quant à leur efficacité et leur coût. La sensibilisation à l'insuline avec metformin est moins efficace, mais aussi moins chère. L'anti-androgène flutamide et l'inhibiteur de la 5-α reductase finastéride figurent parmi les traitements les plus performants, mais ils sont aussi les plus chers. Le traitement anti-androgénique et de suppression hormonale avec acétate de cyprotérone et éthinyloestradiol, utilisé dans cette étude, est également parmi les plus efficaces, tout en étant nettement moins cher. Cette étude est la première comparant directement les taux d'androgènes et la prévalence de l'oligoaménorrhée dans les 3 symptômes cutanés d'hyperandrogénie, hirsutisme, acné et alopécie, et elle démontre leur différente dépendance aux androgènes. La salive apparait comme un milieu de choix pour identifier ces patientes et la recommandation actuelle de doser la testostérone plasmatique totale en premier, pour distinguer l'hyperandrogénie de l'hirsutisme idiopathique, nous paraît inadéquate. Nous proposons, au contraire, d'abandonner ce dosage au profit de celui de la testostérone salivaire. Par ailleurs, notre étude infirme l'hypothèse d'une sensibilité cutanée accrue aux androgènes chez les femmes originaires du sud de l'Europe. Finalement, elle est la seule à comparer les effets cliniques, les changements biologiques et le coût annuel des traitements publiés de l'hirsutisme.

OBJECTIVES: We compared androgen and gonadotropin values in HIV-infected men who did and did not develop lipoatrophy on combination antiretroviral therapy (cART). METHODS: From a population of 136 treatment-naïve male Caucasians under successful zidovudine/lamivudine-based cART, the 10 patients developing lipoatrophy (cases) were compared with 87 randomly chosen controls. Plasma levels of free testosterone (fT), dehydroepiandrosterone (DHEA), follicle-stimulating hormone and luteinizing hormone (LH) were measured at baseline and after 2 years of cART. RESULTS: At baseline, 60% of the cases and 71% of the controls showed abnormally low fT values. LH levels were normal or low in 67 and 94% of the patients, respectively, indicating a disturbance of the hypothalamic-pituitary-gonadal axis. fT levels did not significantly change after 2 years of cART. Cases showed a significant increase in LH levels, while controls showed a significant increase in DHEA levels. In a multivariate logistic regression model, lipoatrophy was associated with higher baseline DHEA levels (P=0.04), an increase in LH levels during cART (P=0.001), a lower body mass index and greater age. CONCLUSIONS: Hypogonadism is present in the majority of HIV-infected patients. The development of cART-related lipoatrophy is associated with an increase in LH and a lack of increase in DHEA levels.

Tribulus terrestris is a nutritional supplement highly debated regarding its physiological and actual effects on the organism. The main claimed effect is an increase of testosterone anabolic and androgenic action through the activation of endogenous testosterone production. Even if this biological pathway is not entirely proven, T. terrestris is regularly used by athletes. Recently, the analysis of two female urine samples by GC/C/IRMS (gas chromatography/combustion/isotope-ratio-mass-spectrometry) conclusively revealed the administration of exogenous testosterone or its precursors, even if the testosterone glucuronide/epitestosterone glucuronide (T/E) ratio and steroid marker concentrations were below the cut-off values defined by World Anti-Doping Agency (WADA). To argue against this adverse analytical finding, the athletes recognized having used T. terrestris in their diet. In order to test this hypothesis, two female volunteers ingested 500 mg of T. terrestris, three times a day and for two consecutive days. All spot urines were collected during 48 h after the first intake. The (13)C/(12)C ratio of ketosteroids was determined by GC/C/IRMS, the T/E ratio and DHEA concentrations were measured by GC/MS and LH concentrations by radioimmunoassay. None of these parameters revealed a significant variation or increased above the WADA cut-off limits. Hence, the short-term treatment with T. terrestris showed no impact on the endogenous testosterone metabolism of the two subjects.

Carbon isotope ratio (CIR) analysis has been routinely and successfully applied to doping control analysis for many years to uncover the misuse of endogenous steroids such as testosterone. Over the years, several challenges and limitations of this approach became apparent, e.g., the influence of inadequate chromatographic separation on CIR values or the emergence of steroid preparations comprising identical CIRs as endogenous steroids. While the latter has been addressed recently by the implementation of hydrogen isotope ratios (HIR), an improved sample preparation for CIR avoiding co-eluting compounds is presented herein together with newly established reference values of those endogenous steroids being relevant for doping controls. From the fraction of glucuronidated steroids 5β-pregnane-3α,20α-diol, 5α-androst-16-en-3α-ol, 3α-Hydroxy-5β-androstane-11,17-dione, 3α-hydroxy-5α-androstan-17-one (ANDRO), 3α-hydroxy-5β-androstan-17-one (ETIO), 3β-hydroxy-androst-5-en-17-one (DHEA), 5α- and 5β-androstane-3α,17β-diol (5aDIOL and 5bDIOL), 17β-hydroxy-androst-4-en-3-one and 17α-hydroxy-androst-4-en-3-one were included. In addition, sulfate conjugates of ANDRO, ETIO, DHEA, 3β-hydroxy-5α-androstan-17-one plus 17α- and androst-5-ene-3β,17β-diol were considered and analyzed after acidic solvolysis. The results obtained for the reference population encompassing n = 67 males and females confirmed earlier findings regarding factors influencing endogenous CIR. Variations in sample preparation influenced CIR measurements especially for 5aDIOL and 5bDIOL, the most valuable steroidal analytes for the detection of testosterone misuse. Earlier investigations on the HIR of the same reference population enabled the evaluation of combined measurements of CIR and HIR and its usefulness regarding both steroid metabolism studies and doping control analysis. The combination of both stable isotopes would allow for lower reference limits providing the same statistical power and certainty to distinguish between the endo- or exogenous origin of a urinary steroid.

This review discuss several studies published in 2007 that will modify current therapeutic attitudes. New evidence confirm that antipsychotics increase mortality in demented elderly persons with behavioral symptoms. Unfortunately, donepezil was no more effective than placebo to treat agitation in these patients. Aspirine, nonsteroidal anti-inflammatories, as well as supplementation with folate, vitamines B6 and B12 did not prove beneficial to prevent cognitive decline. Hormonal substitution with DHEA, testosterone, and growth hormone is ineffective as anti-aging medicine, while calcium and vitamin d'effectiveness was confirmed in fracture prevention. Finally, several studies report prognostic information that will prove important to improve preparation plan for future heat wave.
