Background Hypertension is a major contributor to the global non-communicable disease burden. Family history is an important non-modifiable risk factor for hypertension. The present study aims to describe the influence of family history (FH) on hypertension prevalence and associated metabolic risk factors in a large cohort of South Asian adults, from a nationally representative sample from Sri Lanka. Methods A cross-sectional survey among 5,000 Sri Lankan adults, evaluating FH at the levels of parents, grandparents, siblings and children. A binary logistic regression analysis was performed in all patients with ‘presence of hypertension’ as dichotomous dependent variable and using family history in parents, grandparents, siblings and children as binary independent variables. The adjusted odds ratio controlling for confounders (age, gender, body mass index, diabetes, hyperlipidemia and physical activity) are presented below. Results In all adults the prevalence of hypertension was significantly higher in patients with a FH (29.3 %, n = 572/1951) than those without (24.4 %, n = 616/2530) (p < 0.001). Presence of a FH significantly increased the risk of hypertension (OR:1.29; 95 % CI:1.13-1.47), obesity (OR:1.36; 95 % CI: 1.27–1.45), central obesity (OR:1.30; 95 % CI 1.22–1.40) and metabolic syndrome (OR:1.19; 95 % CI: 1.08–1.30). In all adults presence of family history in parents (OR:1.28; 95 % CI: 1.12–1.48), grandparents (OR:1.34; 95 % CI: 1.20–1.50) and siblings (OR:1.27; 95 % CI: 1.21–1.33) all were associated with significantly increased risk of developing hypertension. Conclusions Our results show that the prevalence of hypertension was significantly higher in those with a FH of hypertension. FH of hypertension was also associated with the prevalence of obesity, central obesity and metabolic syndrome. Individuals with a FH of hypertension form an easily identifiable group who may benefit from targeted interventions.

We studied the community prevalence, patterns and predictors of hypertension in a large sub-population of South Asian adults with a view of identifying differential risk factors. Data were collected between years 2005-2006 and 5000 adults were invited for the study. The sample size was 4485, and about 39.5% were males. Mean systolic and diastolic blood pressures were 127.1 ± 19.8 mmHg and 75.4 ± 11.3 mmHg, respectively. Age-adjusted prevalence in all adults, males and females was 23.7%, 23.4% and 23.8%, respectively. Urban adults had a significantly higher prevalence of hypertension than rural adults. In the binary logistic-regression analysis, male gender (OR: 1.2), increasing age, Sri Lankan Moor ethnicity (OR: 1.6), physical inactivity (OR: 1.7), presence of diabetes (OR: 2.2) and central obesity (OR: 2.3) all were significantly associated with hypertension. In conclusion, nearly one-third of the Sri Lankan adult population is hypertensive. Hence, public health initiatives should encourage healthier lifestyles with emphasis on preventing obesity and increasing physical activity.

O aumento da longevidade evidenciou nas mulheres os efeitos deletérios da deficiência ovariana como, a obesidade central e as doenças neurodegenerativas. Os hábitos alimentares modernos favorecem o consumo excessivo de gorduras, que contribuem para efeitos adversos sobre a saúde, incluindo alteração da composição corporal e da função nervosa. Assim, objetivamos estudar como a dieta hiperlipídica (DH), contendo óleo de soja, associada à perda da função gonadal influenciam o comportamento e o desenvolvimento corporal de ratas adultas. Ratas Wistar adultas foram ovariectomizadas (OVX) ou pseudo-operadas (C) e após 7d passaram a receber dieta contendo 4%(normo) ou 19%(hiperlípidica) de óleo de soja: C4 (n=29), OVX4 (n=30), C19 (n=30), OVX19 (n=30). Foram avaliados a massa e o comprimento corporais e a ingestão alimentar, e aspectos comportamentais de ansiedade, através de teste no Labirinto em Cruz Elevado (LCE); de busca pela novidade, por testes no Campo Vazado (CV); e aprendizagem/memória avaliada no labirinto aquático radial de 8 braços (LAROB). Ao final dos testes, os animais foram anestesiados, exsanguinados e o tecido adiposo intra-abdominal foi coletado e pesado. O soro foi utilizado para dosagens de triglicerídeo (TG), VLDL-c, colesterol (COL), HDL-c e estradiol. A ingestão alimentar média e o comprimento corporal não diferiram entre os grupos, mas o grupo OVX19 mostrou ganho significativo (13,6%) de massa corporal mais precocemente que os demais. O mesmo padrão foi observado quanto à massa de tecido adiposo, que foi 86% maior no OVX19. Os TG, VLDL e COL aumentaram cerca de 28% no OVX19, porém sem alteração de HDL-c (33,762,2 mg/dl). A DH e a ovariectomia se mostraram ansiogênicos e quando associados, a DH parece reverter o efeito ansiogênico da castração. Padrão similar de resposta foi observado na resposta ao teste de busca por novidade. No teste de aprendizagem/memória o OVX4 apresentou maior tempo de latência no primeiro dia, e todos responderam de forma semelhante nos demais dias. Conclui-se que a deficiência de estradiol acompanhada da ingestão de dieta hiperlipídica mantém o desempenho cognitivo, o comportamento emocional e a motivação, embora prejudique a adiposidade e o metabolismo lipídico.

O baixo peso ao nascer (BPN) possui grande impacto na mortalidade neonatal, assim como no desenvolvimento de complicações futuras, como obesidade, hipertensão arterial sistêmica e resistência insulínica, condições relacionadas à doença cardiovascular aterosclerótica, principal causa de morbimortalidade no mundo. O objetivo desta pesquisa foi estudar o perfil clínico, metabólico, hormonal e inflamatório relacionado à doença cardiovascular em crianças pré-púberes de BPN, bem como avaliar a influência do BPN, prematuridade e restrição do crescimento intrauterino nas variáveis de interesse. Realizou-se estudo transversal com 58 crianças de dois a sete anos de BPN, sendo 32 prematuros adequados para idade gestacional (AIG), 17 prematuros pequenos para idade gestacional (PIG), 9 a termo PIG e 38 crianças de peso ao nascer adequado, nascidas no Hospital Universitário Pedro Ernesto da Universidade do Estado do Rio de janeiro, oriundas do Ambulatório de Pediatria Geral deste mesmo hospital. Frequências de perfil lipídico alterado, assim como medianas das variações no Z escore de peso e estatura do nascimento até o momento do estudo, do Z escore de índice de massa corporal (ZIMC), da circunferência da cintura, da pressão arterial sistólica e diastólica, do colesterol total, da lipoproteína de baixa densidade, da lipoproteína de baixa densidade, do triglicerídeo, da glicose, insulina, do Homeostasis Assessment for Insulin Resistance (HOMA-IR), da leptina, da adiponectina, da interleucina 6 e da proteína C reativa foram comparadas entre os dois grupos. No grupo de BPN, avaliou-se a correlação entre estas mesmas variáveis e peso de nascimento, idade gestacional, Z escores de peso e comprimento de nascimento e variações no Z escore de peso e comprimento até o primeiro ano, e até o momento do estudo, com ajuste para idade e sexo. O grupo de BPN apresentou maiores variações nos Z escore de peso (p-valor 0,0002) e estatura (p-valor 0,003) até o momento do estudo e menores níveis de adiponectina (p-valor 0,027). Não houve correlação entre as variáveis associadas ao risco cardiovascular e o grau de baixo peso, prematuridade ou crescimento intrauterino retardado. Os níveis de ZIMC (p-valor 0,0001), circunferência da cintura (p-valor 0,0008), pressão arterial diastólica (p-valor 0,046), insulina (p-valor 0,02), HOMA-IR (p-valor 0,016) e leptina (p-valor= 0,0008) se correlacionaram com a variação no Z escore de peso no primeiro ano. O ZIMC (p-valor 0,042) também se correlacionou com a variação do Z escore de comprimento no primeiro ano. Houve ainda correlação entre o ZIMC (p-valor 0,0001), circunferência da cintura (p-valor 0,0001), pressão arterial sistólica (p-valor 0,022), pressão arterial diastólica (p-valor 0,003), insulina (p-valor 0,007), HOMA-IR (p-valor 0,005) e leptina (p-valor 0,0001) com a variação no Z escore de peso até o momento do estudo. Os achados mostram que este grupo de crianças pré-púberes com BPN ainda não diferem do grupo de crianças nascidas com peso adequado exceto pelos níveis de adiponectina, sabidamente um protetor cardiovascular. Em relação às análises de correlação, nem o peso ao nascer, tampouco a prematuridade ou CIUR, influenciaram as variáveis de interesse. No entanto, fatores pós-natais como o ganho pondero-estatural se correlacionaram com o ZIMC, circunferência da cintura, pressão arterial sistólica e diastólica, insulina, HOMA-IR e leptina. Mais estudos são necessários para avaliar se os achados configuram risco cardiovascular aumentado neste grupo de pacientes.

OBJECTIVES: Barrett’s esophagus (BE) is a common premalignant lesion for which surveillance is recommended. This strategy is limited by considerable variations in clinical practice. We conducted an international, multidisciplinary, systematic search and evidence-based review of BE and provided consensus recommendations for clinical use in patients with nondysplastic, indefinite, and low-grade dysplasia (LGD). METHODS: We defined the scope, proposed statements, and searched electronic databases, yielding 20,558 publications that were screened, selected online, and formed the evidence base. We used a Delphi consensus process, with an 80% agreement threshold, using GRADE (Grading of Recommendations Assessment, Development and Evaluation) to categorize the quality of evidence and strength of recommendations. RESULTS: In total, 80% of respondents agreed with 55 of 127 statements in the final voting rounds. Population endoscopic screening is not recommended and screening should target only very high-risk cases of males aged over 60 years with chronic uncontrolled reflux. A new international definition of BE was agreed upon. For any degree of dysplasia, at least two specialist gastrointestinal (GI) pathologists are required. Risk factors for cancer include male gender, length of BE, and central obesity. Endoscopic resection should be used for visible, nodular areas. Surveillance is not recommended for <5 years of life expectancy. Management strategies for indefinite dysplasia (IND) and LGD were identified, including a de-escalation strategy for lower-risk patients and escalation to intervention with follow-up for higher-risk patients. CONCLUSIONS: In this uniquely large consensus process in gastroenterology, we made key clinical recommendations for the escalation/de-escalation of BE in clinical practice. We made strong recommendations for the prioritization of future research.

Central obesity is the hallmark of a number of non-inheritable disorders. The advent of imaging techniques such asMRI has allowed for a fast and accurate assessment of body fat content and distribution. However, image analysis continues to be one of the major obstacles to the use of MRI in large-scale studies. In this study we assess the validity of the recently proposed fat–muscle quantitation system (AMRATM Profiler) for the quantification of intra-abdominal adipose tissue (IAAT) and abdominal subcutaneous adipose tissue (ASAT) from abdominal MR images. Abdominal MR images were acquired from 23 volunteers with a broad range of BMIs and analysed using sliceOmatic, the current gold-standard, and the AMRATM Profiler based on a non-rigid image registration of a library of segmented atlases. The results show that there was a highly significant correlation between the fat volumes generated by the two analysis methods, (Pearson correlation r = 0.97, p < 0.001), with the AMRATM Profiler analysis being significantly faster (~3 min) than the conventional sliceOmatic approach (~40 min). There was also excellent agreement between the methods for the quantification of IAAT (AMRA 4.73 ± 1.99 versus sliceOmatic 4.73 ± 1.75 l, p = 0.97). For the AMRATM Profiler analysis, the intra-observer coefficient of variation was 1.6% for IAAT and 1.1% for ASAT, the inter-observer coefficient of variationwas 1.4%for IAAT and 1.2%for ASAT, the intra-observer correlationwas 0.998 for IAAT and 0.999 for ASAT, and the inter-observer correlation was 0.999 for both IAAT and ASAT. These results indicate that precise and accurate measures of body fat content and distribution can be obtained in a fast and reliable form by the AMRATM Profiler, opening up the possibility of large-scale human phenotypic studies.

The prevalence of type 2 diabetes mellitus and of the metabolic syndrome is rising worldwide and reaching epidemic proportions. These pathologies are associated with significant morbidity and mortality, in particular with an excess of cardiovascular deaths. Type 2 diabetes mellitus and the cluster of pathologies including insulin resistance, central obesity, high blood pressure, and hypertriglyceridemia that constitute the metabolic syndrome are associated with low levels of HDL cholesterol and the presence of dysfunctional HDLs. We here review the epidemiological evidence and the potential underlying mechanisms of this association. We first discuss the well-established association of type 2 diabetes mellitus and insulin resistance with alterations of lipid metabolism and how these alterations may lead to low levels of HDL cholesterol and the occurrence of dysfunctional HDLs. We then present and discuss the evidence showing that HDL modulates insulin sensitivity, insulin-independent glucose uptake, insulin secretion, and beta cell survival. A dysfunction in these actions could play a direct role in the pathogenesis of type 2 diabetes mellitus.

Résumé Objectifs : Le syndrome métabolique (MetS) est un ensemble de composantes (obésité, résistance à l'insuline, intolérance au glucose, dyslipidémie, hypertension) qui sont associées à une augmentation du risque de diabète de type 2 et de maladies cardiovasculaires. Aux États-Unis, la fréquence du MetS atteint des proportions épidémiques avec une prévalence de 25% de la population. Les études nutritionnelles traditionnelles se sont concentrées sur l’effet d’un nutriment alors que les études plus récentes ont déterminé l’effet global de la qualité alimentaire sur les facteurs de risque. Cependant, peu d'études ont examiné la relation entre la qualité alimentaire et le MetS. Objectif: Déterminer l'association entre la qualité alimentaire et le MetS et ses composantes. Méthodes: La présence du MetS a été déterminée chez 88 femmes post-ménopausées en surpoids ou obèses, selon la définition du National Cholesterol Education Program Adult treatment Panel III alors que la qualité alimentaire a été évaluée selon le Healthy Eating Index (HEI). La sensibilité à l’insuline, la composition corporelle et le métabolisme énergétique ont été mesurés. Résultats: Le HEI corrélait négativement avec la plupart des mesures de masse grasse et du poids mais pas avec la sensibilité à l'insuline, l’hypertension et la plupart des marqueurs lipidiques. Cependant, l’HEI corrélait positivement avec LDL-C/ApoB et négativement avec le métabolisme énergétique. Conclusion: Les résultats démontrent que l’HEI est associé avec les mesures de gras corporel et la grosseur des LDL. Les patients atteints de diabète de type 1 (DbT1) présentent un risque accru de maladies cardiovasculaires. De plus, sous l’influence sociétale et iatrogénique, leur profil de risque cardiométabolique a défavorablement évolué au cours des dernières décennies. Cette population aurait aussi davantage de barrières à l’adoption d’un mode de vie sain. L’objectif de ce mémoire visait à explorer le mode de vie des adultes DbT1 et sa relation avec les facteurs de risque cardiométabolique. Une étude transversale observationnelle menée auprès de 124 adultes DbT1 a permis d’obtenir une caractérisation de leur profil cardiométabolique et de leurs habitudes de vie. Les résultats démontrent que la majorité des adultes atteints de DbT1 n'adoptent pas les saines habitudes de vie recommandées (bonne qualité alimentaire, pratique régulière d’activité physique, non fumeur) et que ceux-ci présentent un profil cardiométabolique altéré comparativement aux adultes DbT1 qui les adoptent. De plus, il y aurait une relation proportionnelle entre l’adoption d’un plus grand nombre de saines habitudes de vie et l’amélioration du profil cardiométabolique. Des études d’interventions ciblant les patients DbT1 et visant à améliorer ces trois grandes composantes des habitudes de vie sont nécessaires.

Objetivo: Determinar los valores del índice cintura/cadera (ICC) en una población escolar de Bogotá, Colombia, pertenecientes al estudio FUPRECOL. Métodos: Estudio descriptivo y transversal, realizado en 3.005 niños y 2.916 adolescentes de entre 9 y 17,9 años de edad, pertenecientes a 24 instituciones educativas oficiales de Bogotá, Colombia. Se tomaron medidas de peso, talla, circunferencia de cintura, circunferencia de cadera. El estado de maduración sexual se recogió por auto-reporte. Se calcularon los percentiles (P3, P10, P25, P50, P75, P90 y P97) según sexo y edad y se realizó una comparación entre los valores del ICC observados con estándares internacionales. Resultados: De la población general (n=5.921), el 57,0% eran mujeres (promedio de edad 12,7 ± 2,3 años). En todas las edades el ICC fue mayor en los varones que en las mujeres, observándose un descenso en la media de los valores obtenidos desde los 9 hasta los 17,9 años. En varones, los valores del ICC mayores del P90 (asociados a riesgo cardiovascular) estuvieron en el rango 0,87 y 0,93 y en las mujeres entre 0,85 y 0,89. Al comparar los resultados de este estudio, por grupos de edad y sexo, con trabajos internacionales de niños y adolescentes de Europa, Suramérica, Asia y África, se observa que los valores del ICC fueron menores en este estudio en ambos sexos, con excepción de los escolares originarios de Grecia y Venezuela. Conclusiones: Se presentan percentiles del ICC según edad y sexo que podrán ser usados de referencia en la evaluación del estado nutricional y en la predicción del riesgo cardiovascular desde edades tempranas en población de Bogotá, Colombia.

Indian Asians living in the UK have a 50% higher CHD mortality rate compared with the indigenous Caucasian population, which cannot be attributed to traditional risk factors. Instead, features of the metabolic syndrome, including raised plasma triacylglycerol, reduced HDL-cholesterol (HDL-C) and an increased proportion of small dense LDL particles, together with insulin resistance and central obesity, are prevalent among this population. The present review examines evidence to support the hypothesis that an imbalance in dietary PUFA intake, specifically a higher intake of n-6 PUFA in combination with a lower intake of the long-chain (LC) n-3 PUFA, plays an important role in the prevalence of the metabolic syndrome observed in Indian Asians. Data are presented to illustrate the impact of manipulation of the background n-6 PUFA intake (moderate or high n-6 PUFA) and the subsequent response to supplementation with LC n-3 PUFA on blood lipids and insulin action in a group of Indian Asian volunteers. The results demonstrate that supplementation with LC n-3 PUFA had no impact on insulin action in those subjects consuming either the moderate-or high-n-6 PUFA diet. In the postprandial phase reductions in plasma triacylglycerol concentrations were greater in those consuming the high-n-6 PUFA background diet subsequent to fish oil supplementation. The present study concludes that, contrary to the central hypothesis, the prevalence of metabolic abnormalities in Indian Asians compared with Caucasians may not be attributable to differences in intakes of n-6 and n-3 PUFA.

The role of low-density lipoprotein in the development of coronary heart disease (CHD) is well recognised. There is also growing evidence that high-density lipoprotein cholesterol (HDL-C) is a powerful inverse predictor for premature CHD and that maintaining a high HDL-C level may guard against atherosclerosis. Patients with low HDL-C levels often also have central obesity, insulin resistance and other features of the metabolic syndrome. This syndrome is both increasingly common and strongly implicated in the growing worldwide epidemic of type 2 diabetes. HDL-C may be increased by lifestyle changes, e.g. weight loss, physical activity and smoking cessation. Pharmacological agents such as fibrates, niacin and statins have also been shown significantly to elevate HDL-C. Although current guidelines are beginning to recognise the protective role of HDL-C level in preventing coronary events, HDL-C should be adopted soon as a target for intervention in its own right.

Background: Obesity may affect the respiratory system, causing changes in respiratory function and in the pulmonary volumes and flows. Objectives: To evaluate the influence of obesity in the movement of thoracoabdominal complex at rest and during maximal voluntary ventilation (MVV), and the contribution between the different compartments of this complex and the volume changes of chest wall between obese and non-obese patients. Materials and Methods: We studied 16 patients divided into two groups: the obese group (n = 8) and group non-obese (n = 8). The two groups were homogeneous in terms of spirometric characteristics (FVC mean: 4.97 ± 0.6 L - 92.91 ± 10.17% predicted, and 4.52 ± 0.6 L - 93.59 ± 8.05%), age 25.6 ± 5.0 and 26.8 ± 4.9 years, in non-obese and obese respectively. BMI was 24.93 ± 3.0 and 39.18 ± 4.3 kg/m2 in the groups investigated. All subjects performed breathing calm and slow and maneuver MVV, during registration for optoelectronic plethysmography. Statistical analysis: we used the unpaired t test and Mann-Whitney. Results: Obese individuals had a lower percentage contribution of the rib cage abdominal (RCa) during breathing at rest and VVM. The variation of end expiratory (EELV) and end inspiratory (EILV) lung volumes were lower in obese subjects. It has been found asynchrony and higher distortion between compartments of thoracoabdominal complex in obese subjects when compared to non-obese. Conclusions: Central obesity impairs the ventilation lung, reducing to adaptation efforts and increasing the ventilatory work
