A child with microdeletion at 22q11.21 was referred to a craniofacial center due to hypernasality, unintelligible speech, and bifid uvula. Velopharyngeal dysfunction remained after surgical repair of submucous cleft palate and speech therapy. A prosthetic-behavioral treatment approach involving total obturation of the velopharynx was successfully implemented for management of velopharyngeal hypodynamism.

Aims: This review provides evidence of which interventions need to be part of effective outpatient integrated treatment for patients with comorbid schizophrenia and substance use disorders. Methods: A total of 14 randomized controlled trials were included. Effect sizes are provided to assess the magnitude of the treatments’ efficacy. Results: Despite the studies’ heterogeneity, we can conclude that certain programs (e.g., Behavioral Treatment for Substance Abuse in Schizophrenia) and specific interventions (e.g., motivational interviewing, family interventions) seem to be effective. Moreover, programs integrating multiple interventions are more likely to be positively related to better outcomes than single interventions. Finally, the lack of difference between effect sizes of assertive community treatment compared to case management suggests that a lower caseload is not necessary for positive treatment outcomes. Conclusion: Integrated treatment seems advantageous, although effect sizes are mostly modest. More homogeneous and qualitative sound studies are needed.

BACKGROUND: Despite long-standing calls to disseminate evidence-based treatments for generalized anxiety (GAD), modest progress has been made in the study of how such treatments should be implemented. The primary objective of this study was to test three competing strategies on how to implement a cognitive behavioral treatment (CBT) for out-patients with GAD (i.e., comparison of one compensation vs. two capitalization models). METHODS: For our three-arm, single-blinded, randomized controlled trial (implementation of CBT for GAD [IMPLEMENT]), we recruited adults with GAD using advertisements in high-circulation newspapers to participate in a 14-session cognitive behavioral treatment (Mastery of your Anxiety and Worry, MAW-packet). We randomly assigned eligible patients using a full randomization procedure (1:1:1) to three different conditions of implementation: adherence priming (compensation model), which had a systematized focus on patients' individual GAD symptoms and how to compensate for these symptoms within the MAW-packet, and resource priming and supportive resource priming (capitalization model), which had systematized focuses on patients' strengths and abilities and how these strengths can be capitalized within the same packet. In the intention-to-treat population an outcome composite of primary and secondary symptoms-related self-report questionnaires was analyzed based on a hierarchical linear growth model from intake to 6-month follow-up assessment. This trial is registered at ClinicalTrials.gov (identifier: NCT02039193) and is closed to new participants. FINDINGS: From June 2012 to Nov. 2014, from 411 participants that were screened, 57 eligible participants were recruited and randomly assigned to three conditions. Forty-nine patients (86%) provided outcome data at post-assessment (14% dropout rate). All three conditions showed a highly significant reduction of symptoms over time. However, compared with the adherence priming condition, both resource priming conditions indicated faster symptom reduction. The observer ratings of a sub-sample of recorded videos (n = 100) showed that the therapists in the resource priming conditions conducted more strength-oriented interventions in comparison with the adherence priming condition. No patients died or attempted suicide. INTERPRETATION: To our knowledge, this is the first trial that focuses on capitalization and compensation models during the implementation of one prescriptive treatment packet for GAD. We have shown that GAD related symptoms were significantly faster reduced by the resource priming conditions, although the limitations of our study included a well-educated population. If replicated, our results suggest that therapists who implement a mental health treatment for GAD might profit from a systematized focus on capitalization models. O objetivo do estudo foi investigar o efeito do tratamento para enurese sobre os escores de outros problemas de comportamento. Foram coletadas as informações de 97 prontuários de crianças e adolescentes atendidos no período de 2002 a 2006 em uma clínica-escola de psicologia, em programa específico para enurese com uso do alarme de urina. Os dados sobre problemas de comportamento foram avaliados por meio do Child Behavior Checklist, respondido pelas mães antes e depois do tratamento. Foi encontrada uma redução significativa nos escores de problemas de comportamento, independentemente do sucesso ou não no tratamento para enurese.

Because of the increasing prevalence of obesity, prevention and treatment of overweight has become a major public health concern. In addition to diet and exercise, drugs are needed for patients who failed to lose weight with behavioral treatment. The current article aimed to summarize recent concerns on the safety and efficacy of appetite suppressants. Several appetite suppressants have been banned for safety reasons. In 2010, sibutramine was withdrawn from the market because a long-term study showed it increased the risks of cardiovascular events. So far no study with a sufficiently large sample size has demonstrated that appetite suppressants can reduce morbidity and mortality associated with overweight. The withdrawal of sibutramine highlights that guidelines for the evaluation of weight control drugs must be more stringent, and studies on their long-term health benefits are needed prior to their marketing.

Resumo: últimas décadas, sendo já considerada uma doença crónica. Ao longo dos anos, ainvestigação desenvolvida nessa área permitiu definir com maior rigor a sua forma de avaliação. Relativamente à avaliação do processo de ingestão alimentar no tratamento da obesidade, e com o objectivo de se obter uma perspectiva completa e contínua, não será suficiente avaliar apenas variáveis nutricionais, mas devem também constar variáveis relacionadas com o comportamento alimentar. O presente trabalho irá enquadrar as áreas fundamentais a avaliar nas componentes da ingestão nutricional e dos comportamentos alimentares associados, e, mais especificamente, apreciar a metodologia de avaliação usada nesse domínio, num programa de tratamento comportamental da obesidade, randomizado e controlado ‐ o programa PESO (Promoção do Exercício e Saúde na Obesidade). Embora este Programa não tenha sido especificamente concebido para avaliar as componentes acima referidas como variáveis‐alvo principais, os dados são relevantes para avaliar o impacte do Programa. Os principais resultados sugerem melhorias evidentes nas variáveis críticas na gestão do peso, tanto na componente da ingestão nutricional como no comportamento alimentar.-----------ABSTRACT: The prevalence of obesity has increased significantly in the last decades and is now considered a chronic disease. Over the years, the research undertaken in this area allowed to define more rigorous ways to assess it. To evaluate dietary intake process in the treatment of obesity, and to achieve a more complete and continuous measurement, it’s not enough to assess the nutritional variables, it’s also necessary to include eating behavior related variables. This work will focus in key areas to evaluate nutritional intake and eating behavior variables, and also in the methodology assessment used in this domain, in a behavioral treatment program of obesity, a randomized controlled trial, named PESO (Promotion of Exercise and Health in Obesity). Although this program was not specifically designed to assess the above components as variables main target, the data are relevant to assess the Program impact. The main results suggest an obvious improvement in weight management critical variables, both in nutritional intake component and eating behavior.

In therapy for chronic posttraumatic stress disorder (PTSD), prolonged exposure (PE) to stimuli associated with an original trauma experience is considered a state-of-the-art treatment method. The present case report outlines the use of Foa and Rothbaum's (1998) manual for this type of treatment in the year-long, 40-session treatment of Caroline, an adult female victim of child sexual abuse. The manual was supplemented by Caspar's (1995, 2007) Plan Analysis technique for individualized case formulation and treatment planning, along with Caspar's concept of the Motive-Oriented Therapeutic Relationship (MOTR). As indicated by standardized, quantitative measures, by changes in the client's behavior patterns, and by the client's subjective report, the treatment was very effective. An analysis of the therapy process illustrates the importance of a combination of manual-based procedures with individualized case formulations and interventions. The case is discussed in the context of enhancing the cognitive-behavioral treatment of PTSD.

La généralisation des acquis dans le domaine de l’agression sexuelle peut se subdiviser en deux volets, soit la généralisation qui se produit lors du traitement et celle suivant le retour dans la collectivité. Le modèle de traitement cognitivo-comportemental, basé sur les principes du risque, des besoins et de la réceptivité, permet une réduction significative des taux de récidive. Plus spécifiquement, les besoins criminogènes ciblés chez chacun des délinquants et le type de stratégies apprises en traitement peuvent influer sur le processus de généralisation des acquis. De la même façon, les caractéristiques propres à l’agresseur sexuel ont également un rôle à jouer. Lors de la libération, la considération et la mise en place de certaines mesures, telles que le plan de réinsertion sociale, les besoins sociaux et individuels, l’employabilité, le logement et la continuité thérapeutique importent afin de faciliter le maintien des acquis. Ainsi, le présent projet de maîtrise vise à mettre de l’avant une meilleure compréhension du phénomène de la généralisation des acquis chez quatre délinquants sexuels suivis dans la collectivité (Centre de psychiatrie légale de Montréal), à la suite d’un traitement d’un an complété à l’Institut Philippe-Pinel de Montréal. Dans le but de comprendre les facteurs pouvant favoriser ce processus, nous avons étudié la manière dont ces différents facteurs se sont présentés chez les délinquants sexuels à l’étude et l’impact lié à la présence ou à l’absence de ces variables. L’analyse clinique du matériel obtenu a démontré, d’une part, que la généralisation des acquis est facilitée lorsque l’ensemble des besoins criminogènes sont des cibles de traitement et que, d’autre part, le délinquant est en mesure d’appliquer des stratégies cognitivo-comportementales plutôt que des techniques purement cognitives. Par ailleurs, la présence d’impulsivité et de problèmes individuels non stabilisés peut nuire au processus. Finalement, il est ressorti que la généralisation des acquis est plus facilement atteignable lorsque les variables identifiées comme étant propices à une réinsertion sociale réussie sont présentes dans le quotidien des délinquants.

Au Québec, le trouble du déficit de l’attention/hyperactivité (TDA/H) est celui qui requiert le plus grand nombre de consultations en pédopsychiatrie (50 % à 75 %). À ce jour, l’intervention multimodale (traitement pharmacologique, programme d’entraînement aux habiletés parentales (PEHP) et programme d’intervention cognitive comportementale (PICC) auprès des enfants ayant un TDA/H) a obtenu de bons résultats à long terme. Dans cette étude, nous avons évalué les changements dans le fonctionnement familial suite à un PEHP. La conception de ce PEHP repose sur les deux approches : l’approche systémique familiale de Calgary (Wright & Leahey, 2013) et l’approche de solution collaborative et proactive (Greene, 2014). Le Family Assessment Device (FAD; Epstein, Baldwin, et Bishop, 1983), version courte, a été utilisé pour mesurer le fonctionnement général (FG) de la famille. La collecte de données a été réalisée auprès de deux groupes (groupe participant et groupe témoin) et à deux temps de mesure (avant et après le PEHP). L’échantillon contient 28 familles participantes et 18 familles témoins. L’analyse de variance à mesures répétées (ANOVA) a été utilisée pour tester l’effet des variables indépendantes (Temps et Intervention) sur la variable dépendante (FG). Les résultats indiquent que les parents qui participent à un PEHP perçoivent un fonctionnement familial général amélioré par rapport au groupe témoin. L’interprétation des changements à la suite du PEHP donne des pistes d’intervention infirmières à ces familles afin d’éviter les impacts de ce trouble sur le fonctionnement familial à long terme.

Low self-esteem is a common, disabling, and distressing problem that has been shown to be involved in the etiology and maintenance of range of Axis I disorders. Hence, it is a priority to develop effective treatments for low self-esteem. A cognitive-behavioral conceptualization of low self-esteem has been proposed and a cognitive-behavioral treatment (CBT) program described (Fennell, 1997, 1999). As yet there has been no systematic evaluation of this treatment with routine clinical populations. The current case report describes the assessment, formulation, and treatment of a patient with low self-esteem, depression, and anxiety symptoms. At the end of treatment (12 sessions over 6 months), and at 1-year follow-up, the treatment showed large effect sizes on measures of depression, anxiety, and self-esteem. The patient no longer met diagnostic criteria for any psychiatric disorder, and showed reliable and clinically significant change on all measures. As far as we are aware, there are no other published case studies of CBT for low self-esteem that report pre- and posttreatment evaluations, or follow-up data. Hence, this case provides an initial contribution to the evidence base for the efficacy of CBT for low self-esteem. However, further research is needed to confirm the efficacy of CBT for low self-esteem and to compare its efficacy and effectiveness to alternative treatments, including diagnosis-specific CBT protocols.

Eating attitudes are defined as beliefs. thoughts, feelings and behaviors towards food. Bulimia nervosa (BN) is ail eating disorder, in which the eating, attitudes are Seriously disturbed. Studies that evaluated nutritional aspects of BN focus mainly oil food intake, dietary restriction and binge eating. while the follow-up Studies evaluate mainly clinical symptoms. The objective of this study was to evaluate eating attitudes of patients with BN. during and after cognitive-behavioral intervention. Thirty nine (39) BN female patients received cognitive behavioral treatment with a Multidisciplinary team and had eating attitudes assessed by a questionnaire developed for this research. Frequencies of the attitudes assessed were compared at baseline. after 12 weeks and 24 weeks of treatment. After treatment, patients had less distorted beliefs about food, less guilty after eating ""forbidden"" foods and they felt more tranquil while caring outside home. Other negative behaviors, as dietary restriction, the desire of not cat, being angry when feeling hungry and using the food to relive stress. persisted. Eating attitudes of patients with BN are hard to be changed in a short-term. More attention to this disease`s component and new approaches to treatment are needed in order to have a better recovery.

Many experts now believe that pervasive problems in affect regulation constitute the central area of dysfunction in borderline personality disorder (BPD). However, data is sparse and inconclusive. We hypothesized that patients with BPD, in contrast to healthy gender and nationality-matched controls, show a higher frequency and intensity of self-reported emotions, altered physiological indices of emotions, more complex emotions and greater problems in identifying specific emotions. We took a 24-hour psychophysiological ambulatory monitoring approach to investigate affect regulation during everyday life in 50 patients with BPD and in 50 healthy controls. To provide a typical and unmanipulated sample, we included only patients who were currently in treatment and did not alter their medication schedule. BPD patients reported more negative emotions, fewer positive emotions, and a greater intensity of negative emotions. A subgroup of non-medicated BPD patients manifested higher values of additional heart rate. Additional heart rate is that part of a heart rate increase that does not directly result from metabolic activity, and is used as an indicator of emotional reactivity. Borderline participants were more likely to report the concurrent presence of more than one emotion, and those patients who just started treatment in particular had greater problems in identifying specific emotions. Our findings during naturalistic ambulatory assessment support emotional dysregulation in BPD as defined by the biosocial theory of [Linehan, M.M., 1993. Over the past decade, mindfulness practices have been used with increasing frequency as therapeutic components within cognitive behavioral treatment regimens. As is standard practice, prescriptive uses of mindfulness intervention are incorporated to improve end-state functioning by ameliorating problematic symptoms and conditions. Common change-targets include the control of cognitive and emotional content for purposes of enhancing psychological self-regulation and physical well-being. The term mindfulness applies to a heterogeneous range of practices, methods, and techniques. While there is no singular agreed upon definition for mindfulness, as a process concept, the term connotes an immediate, non-thetic access to events, wherein each occasioning event is experienced in toto within the broader contextual event-field, and distinct from intervening conceptual themes being noticed. Training in mindfulness practices may be conducted using individual, group, or small class formats. The current paper provides a meta-analytic review of 44 treatment outcome studies (extracted 1982 through 2006), which examines the clinical utility of mindfulness as the primary therapeutic approach. Results indicated that average effect sizes for mindfulness based interventions fell within the medium range for construct category variables examined (d = .56). These findings suggest that mindfulness training is a cost-effective treatment for a wide array of contemporary psychological problems and diagnoses, in addition to fostering positive psychology attributes such as quality and satisfaction with life. A critique of the research and recommendations for future research, including a need to examine the role of mindfulness as a tool for cultivating increased psychological acceptance and life satisfaction, is presented.
