O objetivo deste estudo foi avaliar o perfil clínico-epidemiológico de saúde bucal em pacientes portadores de hanseníase em PQT através da avaliação clínica da mucosa bucal, dos indicadores epidemiológicos IPV (Índice de Placa Visível), ISG (ìndice de Sangramento Gengival), ICPOD (Índice de Dentes Cariados, Perdidos e Obturados) e mensuração do fluxo salivar pelo ISS (Índice de Secreção Salivar), além da capacidade tampão pelo pH salivar. A amostra foi de 80 pacientes na faixa etária de 8 a 73 anos submetidos ao exame clínico bucal e coleta estimulada de saliva, divididos em Grupo de Estudo (GE), constituído de 40 pacientes portadores de hanseníase em PQT, atendidos no CRTDS Dr. Marcello Candia Marituba-Pa; e Grupo Controle (GC), de pacientes não portadores de hanseníase. Os resultados apontaram, 62,5% de alterações inespecíficas da mucosa bucal, dentre as mais frequentes pigmentação racial na gengiva, linha alba mordiscada na mucosa jugal, inflamação gengival, varicosidade no ventre lingual e trauma por uso de prótese no GE, não havendo diferença estatística significante (p=0,14) com o GC. A média do IPV no GE foi de 50,4% das superfícies dentárias com acúmulo de placa bacteriana, não havendo também diferença estatisticamente significante (p=0,40) com o GC. O IPV foi maior nos indivíduos sem acesso ao serviço odontológico e à medida que aumentou o grau de incapacidade dos pacientes menor foi o acesso ao serviço. A média do ISG foi de 29,7% das superfícies dentárias com sangramento gengival, não havendo diferença estatística significante (p=0,35) com o GC. O ICPOD médio foi de 11,6 variando de 4,0 a 24, o que aumentou com o avanço da idade. O maior acúmulo de placa, o sangramento gengival e o consumo de alimentos carigênicos fora do horário da refeição também contribuiram para o aumento do ICPOD. Outro fator relacionado ao aumento de ICPOD foi o não acesso ao serviço odontológico por 70% dos pacientes hansenianos, havendo diferença estatisticamente significante (p=0,0005) com o GC e a falta de orientação de higiene bucal em 60%, havendo diferença estatisticamente significante (p=0,01) com o GC. O ISS médio foi de 0,9ml/min e não apresentou associação com as doses do PQT e nem com o uso de prednisona. Dos pacientes do GE, 25% apresentaram hipossalivação, mas não houve aumento de ICPOD e nenhum dos grupos revelou alteração de pH salivar, variando de 5,85 a 7,34, com capacidade tampão dentro do padrão de normalidade. Concui-se que perfil clínico-epidemiológico dos pacientes portadores de hanseníase assemelha-se a do grupo controle, não tendo sido diagnosticado nenhuma alteração da mucosa bucal específica para hanseníase, o que não anula, entretanto a póssibilidade da cavidade bucal ser fonte de infecção para hanseníase necessitando de confirmação histopatológica e/ou PCR para detecção de M. leprae viável. Além disso, os dados mostraram que a maioria dos pacientes hansenianos avaliados não tem acesso ao serviço odontológico, nem orientação de higiene bucal, resultante da falta de políticas públicas de saúde bucal para hansenianos.

A infecção pela Helicobacter pylori é uma das mais comuns em humanos e apesar de possuir tropismo pelo estômago, pode ser encontrada na cavidade oral, mantendo uma relação comensal com o hospedeiro, enquanto a cárie dental também é uma doença infecciosa e resulta do metabolismo da placa bacteriana. Ambas as infecções apresentam alta prevalência em países em desenvolvimento, pois estas populações estão mais expostas a fatores ambientais de risco, e normalmente são adquiridas durante a infância. A prevalência destas infecções foi investigada na cavidade oral de escolares assintomáticos para doenças gástricas, provenientes de uma população de Belém-Pa, relacionando-se a alguns parâmetros de higiene e saúde bucal, condição socioeconômica e fatores de susceptibilidade genética como os grupos sanguíneos ABO e Lewis. Foram investigados 104 indivíduos, com idade média de 17 anos. De todos os participantes foram coletadas amostras de saliva e placa dental. A saliva foi coletada para identificação do estado secretor ABO e Lewis e estimação dos parâmetros salivares, e ambas, saliva e placa dental, foram coletadas para analise molecular dos genes 16S RNAr da H. pylori e FUT2. A H. pylori foi detectada em 79,8% dos escolares, com freqüência de 66,35% na placa dental e 58,65% na saliva. A prevalência de cárie foi de 82,8% na população estudada. A avaliação clínica da saúde bucal mostrou que o CPO-D médio encontrado foi de 3,53. Observou-se que a experiência de cárie tende a aumentar à medida que acresce a idade e que a infecção por H. pylori foi maior na primeira infância. O grau de instrução e o número de visitas ao dentista mostraram diferenças significantes em relação a presença de H. pylori. A distribuição fenotípica dos grupos sanguíneos ABO e Lewis não mostrou diferenças significantes entre indivíduos infectados e não-infectados, que expliquem haver maior susceptibilidade genética para infecção por H. pylori e cárie dental. No conjunto desta analise as elevadas freqüências encontradas denotam a necessidade de cuidados e tratamento das doenças dentais, como a cárie e sugere-se que a H. pylori na cavidade oral pode contribuir para a infecção e re-infecção do estômago após tratamento.

To compare the abrasion wear resistance and superficial roughness of different glass ionomer cements used as restorative materials, focusing on a new nanoparticulate material. Material and Method: Three glass ionomer cements were evaluated: Ketac Molar, Ketac N100 and Vitremer (3M ESPE, St. Paul, MN, USA), as well as the Filtek Z350 (3M ESPE, St. Paul, MN, USA). For each material were fabricated circular specimens (n=12), respecting the handling mode specified by the manufacturer, which were polished with sandpaper disks of decreasing grit. The wear was determined by the amount of mass (M) lost after brushing (10,000 cycles) and the roughness (Ra) using a surface roughness tester. Results: Except for the composite, significant loss of mass was observed for all glass ionomer cements and the ΔM was comparable for all of them. Significant increase in roughness was observed only for Vitremer and Ketac N100. At the end of the brushing cycle, just Vitremer presented surface roughness greater than the composite resin. Conclusion: All glass ionomer cements showed significant weight loss after 10,000 cycles of brushing. However, only Vitremer showed an increase of roughness greater than the Z350 resin, while the nanoparticulate cement Ketac N100 showed a smooth surface comparable to the composite.

Aim: observe the knowledge of dental caries and periodontal disease of primary schoolteachers, according to type of institution (public and private) from Araraquara city. methodology: for that participants were 113 teachers from public schools and 57 from private schools. The instrument of analysis was based on a questionnaire with closed and open questions concerning the etiology, prevention and development of dental caries and periodontal disease. Results: 97,3% of public ́s schoolteachers and 91,2% of private ́s schoolteachers answered that they knows what is dental caries, but they didn ́t answer correctly about the caries etiology as being multifactorial and about methods of prevention. Both of schoolteachers (public and private) had more sucessful when answered about bacterial plaque than periodontal disease and yours manifestation. Conclusion: the dental knowledge of schoolteachers in public and private school was evaluated and limited educational programs should be directed to both public and private schoolteachers, in order to increase the level of dental knowledge of them, so that they can act as educational agents with the children.

Objective: The aim of this study was to develop an educational oral health digital game for 5-to-7-year-old children. Method: The game, called “Dr. Trata Dente”, was based on the approach to three different oral health-related topics: a) bacterial plaque and the main diseases caused by it; b) oral hygiene methods for bacterial plaque control; c) dental caries and preventive measures for this disease. These topics were discussed in sequential order in three different stages of the game. Dr. Trata Dente is represented by the figure of a little super-hero dentist, who talks to the children about oral health during the game. The game is sub-divided according to its propositions to the children, into an association game, a memory game and a coloring game. After its development, the game was evaluated by three professionals of each of the following areas: Dentistry, Pedagogy and Psychology (n=9), who verified the suitability of the concepts presented in the game as regards oral health, linguistic abilities worked with the children, fulfillment of the intended didactic criteria, and the playful aspect of the game. Results: According to the dentists, the dental concepts presented in the game are adequate. According to the pedagogues, the choice of a super-hero was correct and the given explanations are well elaborated, organized and have accessible language, although long. For the psychologists, the game has a potential positive effect on the children’s learning, but there should be more interaction of the character with the children. After this evaluation, changes were made in the game according to the professionals’ suggestions. Conclusion: It was concluded that in spite of the suggested alterations, the game is suitable for teaching oral health by means of children’s play.

Halitosis is the unpleasant odor of exhaust air through the mouth and lungs. There are over 50 causes, and approximately 90% of cases, is of oral origin. May have physiological origin (stress, breath of morning, fasting, and inappropriate diets), local reasons (bad oral hygiene, bacterial plaque retained on the tongue and/or tonsils, lower production of saliva, gum disease) or systemic reasons (diabetes, kidney or liver problems, constipation, etc.). In the Third Age, this problem tends to increase significantly. Thus, the aim of this study was to detect the presence of halitosis in the geriatric group attended in the dental clinic of UNIRP, identifying their incidence and principal causes. The sample consisted of 48 volunteer patients aged 60 years or older and included more than 90% of patients treated in this age group. It was performed a anamnesis and physical examination. The intra-oral halitosis was detected using a breath meter (halimeter). In the group studied, approximately 54.16% had halitosis, 84.61% of these were users of the prosthesis. The tongue coating was present in 80.76% of individuals with halitosis. After brushing the tongue, 88.46% of patients with halitosis have had the initial index oral malodor decreased. Based on the results of this study it was concluded that factors as dental prostheses and tongue coating are critical factors in causing halitosis.

The increase of the retentive areas of bacterial plaque can be observed in patients who use dental braces in the mouth. The difficulty of making hygienic is one of the problems that this particular group of patients faces day by day, and consequently, the establishment of gingival inflammation becomes more frequent. The objective of this case report is to show the importance of the periodontist in preventing and promoting health to the users of dental braces, aimed at education and motivation as one of the priorities of this treatment. The ESA patient, 29 years old, male, attended the dental office of a periodontist, sent by the orthodontist, due to the extensive area of gingival hyperplasia and gingivitis, in both arches. The possibility of taking out the dental braces was considered by the orthodontist, but after the periodontist evaluation, this step was procrastinated. Thus, the periodontist started the adequacy of the oral environment together with the work of education and promotion of health, which lasted until the complete recovery of the healthy gingival condition of the patient. In this way, it is possible to observe the relevance of the work of the periodontist in the application of preventive methods in oral health for orthodontic patients. The motivation of these patients in relation to the orientations of buccal hygienic, maintenance of oral health and diet should not be considered as secondary and should be prioritized, because only in this way it would be possible to reach a good occlusion, without esthetic and functional prejudice.

Changes in the levels of female sex hormones during the menstrual cycle may cause cyclic differences in subgingival bacterial colonization patterns. The purpose of the present study was to test the hypothesis that hormonal changes in the menstrual cycle cause changes in the oral microbiota. METHODS: Bacterial plaque samples were collected in 20 systemically and periodontally healthy women using no hormonal contraceptives (test group) over a period of 6 weeks. Twenty age-matched systemically and periodontally healthy men were assigned to the control group. Samples were processed by checkerboard DNA-DNA hybridization assay, and 74 species were analyzed. RESULTS: No cyclic pattern of bacterial colonization was identified for any of the 74 species studied in women not using hormonal contraceptives. Aggregatibacter actinomycetemcomitans (previously Actinobacillus actinomycetemcomitans) (Y4) was common at the beginning of menstruation (mean: 32%) and increased during the following 2 weeks (36%) in women (P <0.05). No cyclic differences in bacterial presence were found among the men (P values varied between 0.14 and 0.98). Men presented with significantly higher bacterial counts for 40 of 74 species (P <0.001), including Staphylococcus aureus and Pseudomonas aeruginosa but not Porphyromonas gingivalis (P = 0.15) or Tannerella forsythia (previously T. forsythensis) (P = 0.42). CONCLUSIONS: During a menstruation period, cyclic variation in the subgingival microbiota of periodontally healthy women of child-bearing age who were not using oral hormonal contraceptives could not be confirmed. Male control subjects presented with higher levels of many species but also without a cyclic pattern.

Grande parte do tempo clínico do Médico Dentista é, atualmente, gasto a substituir restaurações dentárias realizadas no passado devido a fraturas ou fenómenos de cárie secundária. Pesquisou-se, assim, para a realização desta monografia em diferentes bases de pesquisa acerca dos fenómenos de cárie e cárie secundária. Um dos objetivos desta revisão bibliográfica estudar a microbiologia associada á cárie secundária, a histopatologia da lesão, o diagnóstico e os problemas associados ao diagnóstico. pretende-se, também, nesta monografia devido á sua pertinência investigar o potencial bacteriostático e bactericida dos diversos materiais disponíveis e em que situações esse efeito é potenciado, de modo a auxiliar o clinico na sua decisão no momento de restauração dentária, procurando prevenir ou adiar a necessidade de efetuar uma nova restauração. Para a realização desta monografia foram realizadas pesquisas nos motores de busca PubMed, B-on, Science Direct e Sci-elo. Depois de selecionados os artigos com interesse e com a utilização de seguintes palavras-chave e combinações de palavras sendo que no final foram selecionados 58 Artigos, dentro dos últimos 10 anos na língua portuguesa e na língua inglesa. Apesar da microbiologia e dos fenómenos histopatológicos de desenvolvimento da lesão estarem bem documentados, não existe consenso no que toca ao diagnóstico e tratamento das lesões. Existem diversos métodos de diagnóstico ao alcance da prescrição do Médico Dentista, sendo que, no entanto, nem todos estão indicados para o diagnóstico de lesões de cárie. Face á grande variedade de materiais restauradores e sistemas adesivos existentes no mercado torna-se imperativo que o Médico Dentista tenha os conhecimentos necessários sobre o potencial antibacteriano desses mesmos materiais. Sendo, assim, capaz de concluir mediante a situação apresentada: as vantagens, desvantagens e potencialidades dos diversos materiais á sua disposição na prevenção de lesões de cárie inicial e cárie secundária, aumentando a sobrevivência das peças dentárias. Conclui-se que a cárie secundária não é um fenómeno exclusivo a nenhum tipo de material forrador, ou restaurador não existindo também nenhum material capaz de efetuar com elevadas taxas de sucesso a sua prevenção. Contudo, atualmente, vem-se procurando a utilização de materiais que proporcionem um maior selamento da estrutura dentária, diminuindo a microinfiltração e consequentemente diminuindo os fenómenos de cárie secundária.

Background: Newly formed biofilm after implant debridement may challenge the long-term stability of peri-implant therapy. This in vitro study aimed to assess the roughness and adherence of Streptococcus sanguinis after treatment of smooth and rough titanium surfaces with an erbium-doped: yttrium, aluminum, and garnet (Er:YAG) laser, metal and plastic curets, and an air-powder abrasive system. Methods: Forty titanium disks with smooth-machined surfaces and 40 with sand-blasted and acid-etched surfaces were divided into the following treatment groups: Er:YAG laser; plastic curet; metal curet, and air-powder abrasive system. The surface roughness (roughness average [Raj) before and after treatments was determined using a profilometer. S. sanguinis (American Type Culture Collection 10556) was grown on treated and untreated specimens, and the amounts of retained bacteria on the surfaces were measured by the culture method. Rough and smooth surfaces with and without a suspension of S. sanguinis were also analyzed using scanning electron microscopy (SEM). Results: For smooth surfaces, the roughest surfaces were produced by metal curets (repeated - measures analysis of variance [ANOVA] and Tukey test; P<0.05). The rough-surface profile was not altered by any of the treatments (repeated-measures ANOVA; P>0.05). Rough surfaces treated with metal curets and air-powder abrasion showed the lowest level of bacteria] adhesion (two-way ANOVA and Tukey test; P<0.05). SEM analysis revealed distinct surface profiles produced by all devices. Conclusions: Metal curets are not recommended for smooth titanium surface debridement due to severe texture alteration. Rough surfaces treated with a metal curet and the air-powder abrasive system were less susceptible to bacterial adhesion, probably due to texture modification and the presence of abrasive deposits. Background: Herpesviruses may be related to the etiology of aggressive periodontitis (AgP) and chronic periodontitis (CP) by triggering periodontal destruction or by increasing the risk for bacterial infection. This case-control study evaluated the presence of herpes simplex virus type 1 (HSV-1), Epstein-Barr virus type 1 (EBV-1), human cytomegalovirus (HCMV), Aggregatibacter actinomycetemcomitans (previously Actinobacillus actinomycetemcomitans), Porphyromonas gingivalis, Prevotella intermedia, and Tannerella forsythia (previously T. forsythensis) in patients with generalized AgP (AgP group), CP (CP group), or gingivitis (G group) and in healthy individuals (C group). Methods: Subgingival plaque samples were collected with paper points from 30 patients in each group. The nested polymerase chain reaction (PCR) method was used to detect HSV-1, EBV-1, and HCMV. Bacteria were identified by 16S rRNA-based PCR. Results: HSV-1, HCMV, and EBV-1 were detected in 86.7%, 46.7%, and 33.3% of the AgP group, respectively; in 40.0%, 50.0%, and 46.7% of the CP group, respectively; in 53.3%, 40.0%, and 20.0% of the G group, respectively; and in 20.0%, 56.7%, and 0.0% of the C group, respectively. A. actinomycetemcomitans was detected significantly more often in the AgP group compared to the other groups (P<0.005). P. gingivalis and T. forsythia were identified more frequently in AgP and CP groups, and AgP, CP, and G groups had higher frequencies of P. intermedia compared to the C group. Conclusion: In Brazilian patients, HSV-1 and EBV-1, rather than HCMV, were more frequently associated with CP and AgP. J Periodontol 2008;79:2313-2321.

Amoebae are unicellular protozoan present worldwide in several environments mainly feeding on bacteria. Some of them, the amoebae-resistant bacteria (ARBs), have evolved mechanisms to survive and replicate inside amoebal species. These mainly include legionella, mycobacteria and Chlamydia-related bacteria. Amoebae can provide a replicative niche, can act as reservoir for bacteria whereas the cystic form can protect the internalized bacteria. Moreover, the amoebae represent a Trojan horse for ARBs to infect animals. The long interaction between amoebae and bacteria has likely selected for bacterial virulence traits leading to the adaptation towards an intracellular lifestyle, and some ARBs have acquired the ability to infect mammals. This review intends to highlight the important uses of amoebae in several fields in microbiology by describing the main tools developed using amoebal cells. First, amoebae such as Acanthamoeba are used to isolate and discover new intracellular bacterial species by two main techniques: the amoebal co-culture and the amoebal enrichment. In the second part, taking Waddlia chondrophila as example, we summarize some important recent applications of amoebae to discover new bacterial virulence factors, in particular thanks to the amoebal plaque assay. Finally, the genetically tractable Dictyostelium discoideum is used as a model organism to study host-pathogen interactions, in particular with the development of several approaches to manipulate its genome that allowed the creation of a wide range of mutated strains largely shared within the Dictyostelium community.
