A anemia por deficiência de ferro configura um problema epidemiológico da maior relevância atuando nos gastos públicos de saúde, nas consequências sociais do aumento de riscos no período gestacional, na redução da produtividade e, ainda, nas consequências, em longo prazo, do desenvolvimento mental. Algumas datas marcam o envolvimento do governo brasileiro em busca de alternativas de controle da deficiência marcial: 1977 - 1ª Reunião do Ministério da Saúde (INAN), com especialistas nacionais e internacionais, para discutir perspectivas e intervenções para o controle da anemia; 1982/83 - distribuição de suplemento de ferro para a clientela do Programa de Atenção à Gestante e dosagem de hemoglobina na 1ª consulta; 1992 - assinatura de compromisso brasileiro de reduzir em 1/3 a prevalência de anemia em gestantes; 1994 - implantação do Programa de Leite Vivaleite, no estado de São Paulo, fornecendo leite fortificado com ferro a famílias com crianças até 6 anos e renda inferior a dois salários mínimos; 2002/junho 2004 - fortificação das farinhas de trigo e de milho com ferro; 2005 - programa de suplementação de ferro a lactentes; 2009/março - divulgação do resultado do levantamento de prevalência de anemia em mulheres (15-49 anos) e crianças (6 - 59 meses) no Brasil; 2009/agosto - foi reeditada a Portaria no 1793/GM/agosto/2009 do Ministério da Saúde, instituindo a Comissão Interinstitucional para implementação, acompanhamento e monitorização das ações de fortificação das farinhas de trigo e milho e seus subprodutos.

Examinou-se a prevalência e fatores associados à anemia em estudo transversal com 429 crianças de 6 a 59 meses do Município de Jordão, Estado do Acre, Brasil. Modelos múltiplos de regressão de Poisson foram utilizados com seleção hierárquica das variáveis independentes. A anemia foi altamente prevalente (57,3%; IC95%: 52,5%-62,1%). Ter idade entre 6 e 23,9 meses [razão de prevalência - RP (IC95%): 1,40 (1,09-1,74)], morar na área rural [RP: 1,23 (1,04-1,44)], morar em domicílio com 5 a 14 crianças [RP: 1,23 (1,04-1,44)], ter mãe que fumou na gravidez [RP: 1,29 (1,09-1,53)], mãe anêmica [RP: 1,18 (1,00-1,39)] e apresentar déficit de altura para idade [RP: 1,19 (1,01-1,39)] foram fatores associados ao risco de anemia, e ter mãe que trabalha fora [RP: 0,78 (0,64-0,94)] foi fator de proteção. A anemia é um grave problema de saúde pública nesse município. Estratégias multissetoriais de combate à pobreza, aumento da cobertura e qualidade de serviços de assistência à saúde materno-infantil devem ser implementados.

Estudo transversal de base populacional que investigou prevalênciasde anemia e fatores associados à anemia, anemia ferropriva e deficiência de ferro entre crianças de 6 a 60 meses da área urbana de dois municípios do Acre, Brasil (N = 624). Dosagens de hemoglobina sanguínea, ferritina e receptor solúvel de transferrina plasmáticas foram realizadas mediante sangue venoso. Condições sócio-econômicas, demográficas e de morbidade foram obtidas por questionário. Razões de prevalências foram calculadas por regressão de Poisson em modelo hierárquico. As prevalências de anemia, anemia ferropriva e deficiência de ferro foram de 30,6%, 20,9% e 43,5%, respectivamente. Menores de 24 meses apresentaram maior risco para anemia, anemia ferropriva e deficiência de ferro. Pertencer ao maior tercil do índice de riqueza conferiu proteção contra anemia ferropriva (RP = 0,62; IC95%: 0,40-0,98). Pertencer ao maior quartil do índice estatura/idade foi protetor contra anemia (0,62; 0,44-0,86) e anemia ferropriva (0,51; 0,33-0,79), e ocorrência recente de diarréia representou risco (anemia: 1,47; 1,12-1,92 e anemia ferropriva: 1,44; 1,03-2,01). A infestação por geohelmintos conferiu risco para anemia, anemia ferropriva e deficiência de ferro.

Identification of animals that are decomposing or have been run over or burnt and cannot be visually identified is a problem in the surveillance and control of infectious diseases. Many of these animals are wild and represent a valuable source of information for epidemiologic research as they may be carriers of an infectious agent. This article discusses the results obtained using a method for identifying mammals genetically by sequencing their mitochondrial DNA control region. Fourteen species were analyzed and identified. These included the main reservoirs and transmitters of rabies virus, namely, canids, chiroptera and primates. The results prove that this method of genetic identification is both efficient and simple and that it can be used in the surveillance of infectious diseases which includes mammals in their epidemiologic cycle, such as rabies.

Background: To estimate the prevalence of and identify factors associated with physical activity in leisure, transportation, occupational, and household settings. Methods: This was a cross-sectional study aimed at investigating living and health conditions among the population of São Paulo, Brazil. Data on 1318 adults aged 18 to 65 years were used. To assess physical activity, the long version of the International Physical Activity Questionnaire was applied. Multivariate analysis was conducted using a hierarchical model. Results: The greatest prevalence of insufficient activity related to transportation (91.7%), followed by leisure (77.5%), occupational (68.9%), and household settings (56.7%). The variables associated with insufficient levels of physical activity in leisure were female sex, older age, low education level, nonwhite skin color, smoking, and self-reported poor health; in occupational settings were female sex, white skin color, high education level, self-reported poor health, nonsmoking, and obesity; in transportation settings were female sex; and in household settings, with male sex, separated, or widowed status and high education level. Conclusion: Physical activity in transportation and leisure settings should be encouraged. This study will serve as a reference point in monitoring different types of physical activities and implementing public physical activity policies in developing countries

OBJETIVO: Avaliar a efetividade da suplementação universal profilática com sulfato ferroso, em administração diária ou semanal, na prevenção da anemia em lactentes. MÉTODOS: Ensaio de campo randomizado com crianças de seis a 12 meses de idade, atendidas em unidades básicas de saúde do município do Rio de Janeiro, em 2004-2005. Foram formadas três coortes concorrentes com suplementação universal com sulfato ferroso com grupos: diário (n=150; 12,5mgFe/dia), semanal (n=147; 25mgFe/semana) e controle (n=94). A intervenção durou 24 semanas e foi acompanhada por ações educativas promotoras de adesão. A concentração de hemoglobina sérica foi analisada segundo sua distribuição, média e prevalência de anemia (Hb<110,0g/L) aos 12 meses de idade. A avaliação da efetividade foi realizada segundo intenção de tratar e adesão ao protocolo, utilizando-se análises de regressão múltipla (linear e de Poisson). RESULTADOS: Os grupos mostraram-se homogêneos quanto às variáveis de caracterização. A intervenção foi operacionalizada com sucesso, com elevada adesão ao protocolo em ambos os grupos expostos a ela, sem diferença estatística entre eles. Após ajuste, somente o esquema diário apresentou efeito protetor. Na análise por adesão, o esquema diário apresentou evidente efeito dose-resposta para média de hemoglobina sérica e prevalência de anemia, não sendo observado nenhum efeito protetor do esquema semanal. CONCLUSÕES: Apenas o esquema diário de suplementação universal com sulfato ferroso dos seis aos 12 meses de idade foi efetivo em aumentar a concentração de hemoglobina sérica e em reduzir o risco de anemia

O trabalho objetivou comparar a prevalência de anemia de crianças, antes e após 6 meses de consumo de leite fortificado com ferro, do Projeto Vivaleite, acompanhados de orientação nutricional. O estudo foi realizado em Itapeva, Piracicaba, São José dos Campos, Taubaté e São Paulo, locais selecionados devido ao aumento representativo do número de crianças beneficiadas pelo Projeto. A amostra foi constituída de 399 crianças, de 6 a 36 meses de idade. Os dados da criança e da família foram obtidos mediante aplicação de formulário. O diagnóstico da anemia foi efetuado por meio da dosagem de hemoglobina, utilizando-se o aparelho HemoCue® e o valor crítico proposto pela OMS, de 11,0g/dL. Os níveis médios de concentração de hemoglobina foram comparados pelo Teste T Pareado e Qui-quadrado de Pearson, com nível de significância de 5 por cento. Foi encontrada diferença significativa (p<0,05) entre as concentrações de hemoglobina antes (10,18g/dL) e após o consumo do leite do Vivaleite (10,99 g/dL), verificando-se um incremento de 0,81g/dL nos índices de hemoglobina. Ainda, houve uma diminuição significativa (p<0,05) na prevalência de crianças com anemia, ou seja, 38,8 por cento das crianças que apresentavam anemia no início do estudo não estavam anêmicas no final. Está bem documentada a efetividade da intervenção proposta pelo Vivaleite no controle da deficiência marcial, sendo que a proposta deve ser acompanhada por atividades de orientação nutricional constante, como as aplicadas neste trabalho o que, provavelmente, motivou o sucesso do programa

A anemia por deficiência de ferro caracteriza-se como o mais prevalente problema nutricional em todo o mundo. Nesta revisão reuniu-se informações a respeito do metabolismo da hepcidina, avaliando-se seu valor como parâmetro bioquímico na anemia por deficiência de ferro. Realizou-se um levantamento bibliográfico nas bases de dados PUBMED e LILACS, período 2006-2010, referentes à hepcidina como um biomarcador para a regulação do metabolismo do ferro. Foram localizados 35 estudos publicados em revistas internacionais e um estudo sobre o assunto em revista nacional. A produção de hepcidina é regulada homeostaticamente pela anemia e hipóxia. Quando a oferta de oxigênio está inadequada ocorre diminuição do nível de hepcidina. Consequentemente, maior quantidade de ferro proveniente da dieta e dos estoques dos macrófagos e hepatócitos se tornam disponíveis. A hepcidina possui a função de se ligar à ferroportina, regulando a liberação do ferro para o plasma. Quando as concentrações de hepcidina estão baixas, as moléculas de ferroportina são expostas na membrana plasmática e liberam o ferro. Quando os níveis de hepcidina aumentam, a hepcidina liga-se às moléculas de ferroportina induzindo sua internalização e degradação, e o ferro liberado diminui progressivamente. Aparentemente o desenvolvimento do diagnóstico e terapia da anemia baseados no bioindicador hepcidina pode oferecer uma abordagem mais efetiva. Estudos epidemiológicos são necessários para comprovar o valor da hepcidina no diagnóstico diferencial das anemias, incluindo protocolos de amostragem para análise, com padronização similar às utilizadas em outras avaliações bioquímicas, e estabelecimento de pontos de corte para a expressão urinária e plasmática desse peptídeo

Background: Population antimicrobial use may influence resistance emergence. Resistance is an ecological phenomenon due to potential transmissibility. We investigated spatial and temporal patterns of ciprofloxacin (CIP) population consumption related to E. coli resistance emergence and dissemination in a major Brazilian city. A total of 4,372 urinary tract infection E. coli cases, with 723 CIP resistant, were identified in 2002 from two outpatient centres. Cases were address geocoded in a digital map. Raw CIP consumption data was transformed into usage density in DDDs by CIP selling points influence zones determination. A stochastic model coupled with a Geographical Information System was applied for relating resistance and usage density and for detecting city areas of high/low resistance risk. Results: E. coli CIP resistant cluster emergence was detected and significantly related to usage density at a level of 5 to 9 CIP DDDs. There were clustered hot-spots and a significant global spatial variation in the residual resistance risk after allowing for usage density. Conclusions: There were clustered hot-spots and a significant global spatial variation in the residual resistance risk after allowing for usage density. The usage density of 5-9 CIP DDDs per 1,000 inhabitants within the same influence zone was the resistance triggering level. This level led to E. coli resistance clustering, proving that individual resistance emergence and dissemination was affected by antimicrobial population consumption.

Background: Major depression is one of the leading causes of disability worldwide, yet epidemiologic data are not available for many countries, particularly low- to middle-income countries. In this paper, we present data on the prevalence, impairment and demographic correlates of depression from 18 high and low-to middle-income countries in the World Mental Health Survey Initiative. Methods: Major depressive episodes (MDE) as defined by the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DMS-IV) were evaluated in face-to-face interviews using the World Health Organization Composite International Diagnostic Interview (CIDI). Data from 18 countries were analyzed in this report (n = 89,037). All countries surveyed representative, population-based samples of adults. Results: The average lifetime and 12-month prevalence estimates of DSM-IV MDE were 14.6% and 5.5% in the ten high-income and 11.1% and 5.9% in the eight low- to middle-income countries. The average age of onset ascertained retrospectively was 25.7 in the high-income and 24.0 in low- to middle-income countries. Functional impairment was associated with recency of MDE. The female: male ratio was about 2: 1. In high-income countries, younger age was associated with higher 12-month prevalence; by contrast, in several low-to middle-income countries, older age was associated with greater likelihood of MDE. The strongest demographic correlate in high-income countries was being separated from a partner, and in low- to middle-income countries, was being divorced or widowed. Conclusions: MDE is a significant public-health concern across all regions of the world and is strongly linked to social conditions. Future research is needed to investigate the combination of demographic risk factors that are most strongly associated with MDE in the specific countries included in the WMH.

Background: Myelodysplastic syndromes (MDS) are a group of clonal hematological disorders characterized by ineffective hematopoiesis with morphological evidence of marrow cell dysplasia resulting in peripheral blood cytopenia. Microarray technology has permitted a refined high-throughput mapping of the transcriptional activity in the human genome. Non-coding RNAs (ncRNAs) transcribed from intronic regions of genes are involved in a number of processes related to post-transcriptional control of gene expression, and in the regulation of exon-skipping and intron retention. Characterization of ncRNAs in progenitor cells and stromal cells of MDS patients could be strategic for understanding gene expression regulation in this disease. Methods: In this study, gene expression profiles of CD34(+) cells of 4 patients with MDS of refractory anemia with ringed sideroblasts (RARS) subgroup and stromal cells of 3 patients with MDS-RARS were compared with healthy individuals using 44 k combined intron-exon oligoarrays, which included probes for exons of protein-coding genes, and for non-coding RNAs transcribed from intronic regions in either the sense or antisense strands. Real-time RT-PCR was performed to confirm the expression levels of selected transcripts. Results: In CD34(+) cells of MDS-RARS patients, 216 genes were significantly differentially expressed (q-value <= 0.01) in comparison to healthy individuals, of which 65 (30%) were non-coding transcripts. In stromal cells of MDS-RARS, 12 genes were significantly differentially expressed (q-value <= 0.05) in comparison to healthy individuals, of which 3 (25%) were non-coding transcripts. Conclusions: These results demonstrated, for the first time, the differential ncRNA expression profile between MDS-RARS and healthy individuals, in CD34(+) cells and stromal cells, suggesting that ncRNAs may play an important role during the development of myelodysplastic syndromes.

The seroprevalence of Toxocara canis and risk factors for infection with this parasite were explored in a rural settlement in Sao Paulo state, Brazil. Total IgA and IgE levels in 79 subjects were determined by turbidimetry and chemiluminescence, respectively. Total counts of leucocytes and erythrocytes and differential counts of leucocytes were made by flow cytometry. ELISA for the detection of anti-Toxocara IgG, IgA and IgE were standardized using Toxocara excretory-secretory antigens (TES) obtained from the cultured second-stage larvae of T. canis. Seventeen (21.5%) of the subjects were found positive for anti-Toxocara IgG, with no significant differences in such seropositivity with age or gender. Thirty (38%) of the subjects showed eosinophilia and 70 (89%) had elevated levels of total IgE. Among the 17 subjects found seropositive for anti-Toxocara IgG, the percentage of leucocytes represented by eosinophils (P=0.0069) and total levels of IgE (P=0.0452) were positively correlated with the levels of anti-TES IgE. Although anti-TES IgA was detected in 10 (59%) of the subjects, there was no significant correlation between the levels of total IgA and those of Toxocara-specific IgA. Only one of the 17 subjects found positive for anti-Toxocara IgG had attended a secondary school and all but two belonged to households with monthly incomes of < U.S.$100. In the study community at least, seropositivity may be related to poor living standards and lack of basic sanitary conditions. The presence of anti-Toxocara IgE and IgA may facilitate the diagnosis of toxocariasis and may well be useful for monitoring the success of treatment.

Iron and oxidative stress have a regulatory interplay. During the oxidative burst, phagocytic cells produce free radicals such as hypochlorous acid (HOCl). Nevertheless, scarce studies evaluated the effect of either iron deficiency anemia (IDA) or anemia of chronic disease (ACD) on phagocyte function in the elderly. The aim of the present study was to determine the oxidative burst, phagocytosis, and nitric oxide ((aEuro cent)NO) and HOCl, reactive species produced by monocytes and neutrophils in elderly with ACD or IDA. Soluble transferrin receptor, serum ferritin, and soluble transferrin receptor/log ferritin (TfR-F) index determined the iron status. The study was constituted of 39 patients aged over 60 (28 women and 11 men) recruited from the Brazilian Public Health System. Oxidative burst fluorescence intensity per neutrophil in IDA group and HOCl generation in both ACD and IDA groups were found to be lower (p < 0.05). The percentages of neutrophils and monocytes expressing phagocytosis in ACD group were found to be higher (p < 0.05). There was an overproduction of (aEuro cent)NO from monocytes, whereas the fundamental generation of HOCl appeared to be lower. Phagocytosis, oxidative burst, and (aEuro cent)NO and HOCl production are involved in iron metabolism regulation in elderly patients with ACD and IDA.

A prevalence study of Parkinson's disease (PD) was conducted in the rural town of Nambour, Australia. There were 5 cases of PD in a study population of 1207, yielding a crude prevalence ratio of 414 per 100,000 (95% confidence interval; 53-775). We performed a separate case-control study involving 224 patients with FD and 310 controls from South East Queensland and Central West New South Wales, to determine which factors increase the risk for PD in Australia. A positive family history of PD was the strongest risk factor for the development of the disease (odds ratio = 3.4; p < 0.001). In addition, rural residency was a significant risk factor for PD (odds ratio = 1.8, p < 0.001). Hypertension, stroke and well water ingestion were inversely correlated with the development of PD. There was no significant difference between patients and controls for exposure to herbicides and pesticides, head injury, smoking or depression. The high prevalence of PD in Nambour may be explained by rural residency. However, the most significant risk factor for PD was a positive family history. This demonstrates the need for improved understanding of the genetic nature of the disease.
