Ce travail examine les mécanismes de l’immunité innée impliqués dans l’hépatite aiguë virale du modèle murin d’infection par le virus de l’hépatite virale murine de type 3 (MHV-3). Afin de déterminer le rôle du TLR2 dans l’aggravation de l’hépatite, des infections avec le virus MHV-3 ont été réalisées in vivo chez des souris C57BL/6 et des souris déficientes pour le gène tlr2 et in vitro dans des macrophages et des hépatocytes infectés avec le virus MHV-3 et le virus moins virulent MHV-A59. Les niveaux de transcription et de traduction des senseurs microbiens, des interférons (IFN) de type I, des cytokines et/ou des chimiokines ont été évalués par qRT-PCR et ELISA. Les cellules ont été traitées avec des petits ARNs interférants (siRNAs) pour le TLR2 et le CEACAM1a ou mises en présence d’inhibiteurs des voies d’endocytose. Les résultats révèlent le rôle stimulateur du TLR2 pour la réplication virale, la production de cytokines pro-inflammatoires IL-6 et TNF-α et des chimiokines CXCL1, CXCL10 et CCL2. Un nouveau mécanisme d’échappement aux senseurs viraux dépendant du TLR2 a également été mis en évidence dans les macrophages et les hépatocytes lors de l’infection de ces cellules avec le virus MHV-3, et non pas avec le virus moins virulent MHV-A59. Ces différents travaux révèlent un nouveau rôle du TLR2 lors d’infections virales dans l'aggravation de la réponse inflammatoire tout en protégeant le virus des autres senseurs de la réponse immune innée.

Les cellules endothéliales (EC) constituent une première barrière physique à la dissémination de virus pléiotropiques circulant par voie hématogène mais leur contribution à la défense innée anti-virale est peu connue. Des dysfonctions des EC de la barrière hémato-encéphalique (BMEC) et des sinusoïdes hépatiques (LSEC) ont été rapportées dans des neuropathologies et des hépatites aiguës ou chroniques d’origine virale, suggérant que des atteintes à leur intégrité contribuent à la pathogenèse. Les sérotypes de coronavirus de l’hépatite murine (MHV), se différenciant par leur capacité à induire des hépatites et des maladies neurologiques de sévérité variable et/ou leur tropisme pour les EC, représentent des modèles viraux privilégiés pour déterminer les conséquences de l’infection des EC sur la pathogenèse virale. Lors d’infection par voie hématogène, le sérotype MHV3, le plus virulent des MHV, induit une hépatite fulminante, caractérisée par une réponse inflammatoire sévère, et des lésions neurologiques secondaires alors que le sérotype moins virulent, MHV-A59, induit une hépatite modérée sans atteintes secondaires du système nerveux central (SNC). Par ailleurs, le sérotype MHV3, à la différence du MHV-A59, démontre une capacité à stimuler la production de cytokines par la voie TLR2. Les variants atténués du MHV3, les virus 51.6-MHV3 et YAC-MHV3, sont caractérisés par un faible tropisme pour les LSEC et induisent respectivement une hépatite modérée et subclinique. Compte tenu de l’importance des LSEC dans le maintien de la tolérance hépatique et de l’élimination des pathogènes circulants, il a été postulé que la sévérité de l’hépatite et de la réponse inflammatoire lors d’infections par les MHV est associée à la réplication virale et à l’altération des propriétés tolérogéniques et vasculaires des LSEC. Les désordres inflammatoires hépatiques pourraient résulter d’une activation différentielle du TLR2, plutôt que des autres TLR et des hélicases, selon les sérotypes. D’autre part, compte tenu du rôle des BMEC dans la prévention des infections du SNC, il a été postulé que l’invasion cérébrale secondaire par les coronavirus est reliée à l’infection des BMEC et le bris subséquent de la barrière hémato-encéphalique (BHE). À l’aide d’infections in vivo et in vitro par les différents sérotypes MHV, chez des souris ou des cultures de BMEC et de LSEC, nous avons démontré, d’une part, que l’infection in vitro des LSEC par le sétotype MHV3, à la différence des variants 51.6- et YAC-MHV3, altérait la production du facteur vasodilatant NO et renversait leur phénotype tolérogénique en favorisant la production de cytokines et de chimiokines inflammatoires. Ces dysfonctions se traduisaient in vivo par une réponse inflammatoire incontrôlée et une dérégulation du recrutement intrahépatique de leucocytes, favorisant la réplication virale et les dommages hépatiques. Nous avons aussi démontré, à l’aide de souris TLR2 KO et de LSEC dont l’expression du TLR2 a été abrogée par des siRNA, que la sévérité de l’hépatite et de la réponse inflammatoire induite par le sérotype MHV3, dépendait en partie de l’induction et de l’activation préférentielle du TLR2 par le virus dans le foie. D’autre part, la sévérité de la réplication virale au foie et des désordres dans le recrutement leucocytaire intrahépatique induits par le MHV3, et non par le MHV-A59 et le 51.6-MHV3, corrélaient avec une invasion virale subséquente du SNC, au niveau de la BHE. Nous avons démontré que l’invasion cérébrale du MHV3 était associée à une infection productive des BMEC et l’altération subséquente des protéines de jonctions serrées occludine, VE-cadhérine et ZO-1 se traduisant par une augmentation de la perméabilité de la BHE et l’entrée consécutive du virus dans le cerveau. Dans l’ensemble, les résultats de cette étude mettent en lumière l’importance du maintien de l’intégrité structurale et fonctionnelle des LSEC et des BMEC lors d’infections virales aigües par des MHV afin de limiter les dommages hépatiques associés à l’induction d’une réponse inflammatoire exagérée et de prévenir le passage des virus au cerveau suite à une dissémination par voie hématogène. Ils révèlent en outre un nouveau rôle aggravant pour le TLR2 dans l’évolution de l’hépatite virale aigüe ouvrant la voie à de nouvelles avenues thérapeutiques visant à moduler l’activité inflammatoire du TLR2.

Background: Human capillariasis caused by Capillaria hepatica (syn. Calodium hepaticum) is a rare disease with no more than 40 cases registered around the world. Classically, the disease has severe symptoms that mimic acute hepatitis. Natural reservoirs of C. hepatica are urban rodents (Mus musculus and Rattus novergicus) that harbor their eggs in the liver. After examining the feces of 6 riverine inhabitants (Rio Preto area, 8 degrees 03`S and 62 degrees 53`W to 8 degrees 14`S and 62 degrees 52`W) of the State of Rondonia, Brazil, and identifying C. hepatica eggs in their feces, the authors decided to investigate the real dimension of these findings by looking for two positive signals. Methods: Between June 1(st) and 15(th), 2008, 246 out of 304 individuals were clinically examined. Blood samples were collected, kept under -20 degrees C, and test by the indirect immunofluorescence technique. Results: The first positive signal was the presence of specific antibodies at 1: 150 dilution, which indicates that the person is likely to have been exposed to eggs, most likely non-infective eggs, passing through the food chain or via contaminated food (total prevalence of 34.1%). A second more specific signal was the presence of antibodies at higher titers, thus indicating true infection. Conclusions: The authors concluded that only two subjects were really infected (prevalence of 0.81%); the rest was false-positives that were sensitized after consuming non-embryonated eggs. The present study is the first one carried out in a native Amazonian population and indicates the presence of antibodies against C. hepatica in this population. The results further suggest that the transmission of the parasite occurs by the ingestion of embryonated eggs from human feces and/or carcasses of wild animals. The authors propose a novel mode of transmission, describing the disease as a low pathogenic one, and showing low infectivity.

Objectives: To compare modes and sources of infection and clinical and biosafety aspects of accidental viral infections in hospital workers and research laboratory staff reported in scientific articles. Methods: PubMed, Google Scholar, ISI Web of Knowledge, Scirus, and Scielo were searched (to December 2008) for reports of accidental viral infections, written in English, Portuguese, Spanish, or German; the authors' personal file of scientific articles and references from the articles retrieved in the initial search were also used. Systematic review was carried out with inclusion criteria of presence of accidental viral infection's cases information, and exclusion criteria of absence of information about the viral etiology, and at least probable mode of infection.Results: One hundred and forty-one scientific articles were obtained, 66 of which were included in the analysis. For arboviruses, 84% of the laboratory infections had aerosol as the source; for alphaviruses alone, aerosol exposure accounted for 94% of accidental infections. of laboratory arboviral infections, 15.7% were acquired percutaneously, whereas 41.6% of hospital infections were percutaneous. For airborne viruses, 81% of the infections occurred in laboratories, with hantavirus the leading causative agent. Aerosol inhalation was implicated in 96% of lymphocytic choriomeningitis virus infections, 99% of hantavirus infections, and 50% of coxsackievirus infections, but infective droplet inhalation was the leading mode of infection for severe acute respiratory syndrome coronavirus and the mucocutaneous mode of infection was involved in the case of infection with influenza B. For blood-borne viruses, 92% of infections occurred in hospitals and 93% of these had percutaneous mode of infection, while among laboratory infections 77% were due to infective aerosol inhalation. Among blood-borne virus infections there were six cases of particular note: three cases of acute hepatitis following hepatitis C virus infection with a short period of incubation, one laboratory case of human immunodeficiency virus infection through aerosol inhalation, one case of hepatitis following hepatitis G virus infection, and one case of fulminant hepatitis with hepatitis B virus infection following exposure of the worker's conjunctiva to hepatitis B virus e antigen-negative patient saliva. of the 12 infections with viruses with preferential mucocutaneous transmission, seven occurred percutaneously, aerosol was implicated as a possible source of infection in two cases, and one atypical infection with Macacine herpesvirus 1 with fatal encephalitis as the outcome occurred through a louse bite. One outbreak of norovirus infection among hospital staff had as its probable mode of infection the ingestion of inocula spread in the environment by fomites.Conclusions: The currently accepted and practiced risk analysis of accidental viral infections based on the conventional dynamics of infection of the etiological agents is insufficient to cope with accidental viral infections in laboratories and to a lesser extent in hospitals, where unconventional modes of infection are less frequently present but still have relevant clinical and potential epidemiological consequences. Unconventional modes of infection, atypical clinical development, or extremely severe cases are frequently present together with high viral loads and high virulence of the agents manipulated in laboratories. In hospitals by contrast, the only possible association of atypical cases is with the individual resistance of the worker. Current standard precaution practices are insufficient to prevent most of the unconventional infections in hospitals analyzed in this study; it is recommended that special attention be given to flaviviruses in these settings. O vírus Morumbi é membro do sorogrupo Phlebotomus fever (família Bunyavírídae: gênero Phlebovírus) nativo da Região Amazônica. Seu vetor é desconhecido, mas supõem-se ser transmitido por flebotomíneos. Foi isolado em 1988 de ser humano apresentando quadro febril agudo. Este arbovírus, quando inoculado em camundongo por via cerebral, demonstrou viscerotropismo, induzindo inclusive lesões no fígado do animal inoculado. Com os objetivos de: i) estabelecer as características anátomo-patológicas e imuno-histoquímicas em fígado de camundongos albinos Swíss recém-nascidos experimentalmente infectados pelo vírus Morumbi; ii) verificar se o vírus apresenta hepatotropismo diferenciado na dependência de inoculação pelas vias cerebral, peritoneal ou subcutânea; iii) caracterizar detalhadamente os padrões anátomo-patológicos sequenciais no fígado; iv) demonstrar a localização do antígeno viral no tecido hepático ao longo da infecção experimental; v) estudar possíveis inter-relações entre os achados anátomo-patológicos e os imuno-histoquímicos. Foram estudados experimentalmente 71 camundongos Swíss recém-nascidos (dois e três dias), distribuídos ao final do experimento como segue: 21 animais inoculados por via intracerebral (IC), 21 por via intraperitoneal (IP) e 29 animais inoculados por via subcutânea (SC). Utilizou-se a dose infectante 5,0DL 50 /0,02ml de suspensão de vírus. Outros trinta, animais que não receberam inóculos, foram utilizados como grupo controle. Subgrupos de oito animais (seis inoculados e dois do grupo controle) foram sacrificados diariamente a intervalos de 24 em 24 horas, até 96 horas para os grupos IC e IP e até 120 horas para o grupo SC. Fragmentos de fígado de todos os animais foram fixados em solução de formalina neutra a 10%, incluídos em parafina, de onde foram obtidos cortes de 5 mm que foram corados pela técnica de hematoxilina-eosina para análise morfológica e, cortes adicionais, foram submetidos à técnica de imuno-histoquímica (Sistema Envision, DAKO, USA), utilizando a fosfatase alcalina e soro hiperimune do vírus Morumbi preparado em camundongos jovens, para detecção de antígeno viral. Foram estudados seis parâmetros de lesão em áreas portais e nove outros nos lóbulos, que foram semiquantificados numa escala que variou de zero (0) a três cruzes (+++), onde zero significou ausência de lesão e três cruzes lesão intensa. À microscopia óptica, ficou evidente que o vírus Morumbi inoculado em camundongos por três diferentes vias induz lesões em áreas portais e lobulares, caracterizando uma hepatite aguda com presença de corpúsculos acidófilos, semelhantes aos corpúsculos de Councilman -Rocha Lima, de distribuição irregular nos lóbulos, cujo aparecimento foi observado 24 horas pós-inoculação (p.i.) e atingiu o máximo de intensidade às 72 horas p.i. em animais inoculados por via IP. O exame imuno-histoquímico mostrou presença leve de antígeno viral a partir de 24 horas p.i. no grupo IC e a partir de 48 horas p.i. nos grupos IP e SC, havendo certo paralelismo em relação a intensidade de lesão morfológica, tendo- se observado o máximo de detecção de antígeno viral em animais inoculados por via IP e sacrificados às 72 horas p.i. A distribuição geral de antígeno foi observada especificamente nos lóbulos hepáticos, no citoplasma de hepatócitos íntegros e necrosados e no interior de células de Kupffer, não havendo preferência por nenhuma das três zonas do lóbulo. Concluiu-se que: i) o modelo de infecção experimental em camundongos foi excelente para o estudo das lesões causadas pelo vírus Morumbi, podendo ser selecionada a via IP como referencial; ii) em todas as vias utilizadas (IP, IC e SC) se confirmou a infecção pelo vírus Morumbi com marcante detecção de seu antígeno, no tecido hepático de camundongos Swiss; iii) a presença de antígeno do vírus Morumbi no fígado desses camundongos associou-se ao aparecimento de hepatite aguda, com necrose focal; iv)hepatite intensa pôde ser observada em fígado de camundongos sacrificados 72 h p.i. com o vírus Morumbi por via IP, o que não foi verificado com as outras duas vias; v) a hepatite aguda mostrou-se limitada, neste experimento, tendendo a desaparecer na maioria dos camundongos inoculados, com avançar das horas; vi) colestase não alteração freqüente na hepatite experimental pelo vírus Morumbi, quando inoculada por via IC, IP e SC; vii) o antígeno do vírus Morumbi teve predominância pela localização intracitoplasmática, padrão granular, nos hepatócitos e células de Kupffer; viii) antígeno viral foi detectado em fragmento hepático de animais experimentalmente inoculados com o vírus Morumbi, a partir das 24 horas via IC e a partir de 48 horas nas vias IP e SC.

O vírus da Hepatite E (HEV) é um RNA-vírus entericamente transmissível do gênero Hepevirus causador de hepatite aguda em humanos que apresenta ampla distribuição em diversas regiões do mundo. Suínos são relatados como a principal fonte de infecção para humanos relacionadas aos genótipos 3 e 4 em regiões consideradas não-endêmicas. Neste sentido, o presente estudo teve como objetivo demonstrar a infecção pelo HEV em suínos no Estado do Pará através de métodos sorológicos e moleculares aplicados a amostras de soro, fezes e fígado de 151 suínos abatidos na região Metropolitana de Belém. A investigação sorológica abrangeu a pesquisa de anticorpos anti-HEV das classes IgM e IgG e o diagnóstico molecular inclui a detecção do HEV-RNA, sequenciamento nucleotídico e análise filogenética das sequências obtidas. Como resultado, não foram detectados anticorpos anti- HEV IgM e a prevalência de animais sororeativos para IgG foi de 8,6% (13/151). A detecção molecular amplificou fragmentos do HEV genoma em 4,8% (22/453) das amostras testadas e a prevalência de animais positivos a pelo menos uma amostra foi de 9,9% (15/151). A análise filogenética concluiu que todas as sequências analisadas pertencem ao genótipo 3 do vírus, descrito como zoonótico. Foram identificados os subtipos 3c e 3f ocorrendo simultaneamente estre as amostras, de acordo com as duas regiões do genoma amplificadas. Estes resultados constam como as primeiras evidências sorológicas e moleculares da circulação do HEV entre suínos no Norte do Brasil e também como a primeira detecção e genotipagem do HEV na região.

HEV infection in swine from Eastern Brazilian Amazon: Evidence of co-infection by different subtypes

Hepatitis E virus (HEV) is a fecal-orally transmitted member of the genus Hepevirus that causes acute hepatitis in humans and is widely distributed throughout the world. Pigs have been reported as the main source of genotypes 3 and 4 infection to humans in non-endemic areas. To investigate HEV infection in pigs from different regions of Para state (Eastern Brazilian Amazon), we performed serological and molecular analyses of serum, fecal and liver samples from 151 adult pigs slaughtered between April and October 2010 in slaughterhouses in the metropolitan region of Belem, Para. Among the animals tested, 8.6% (13/151) were positive for anti-HEV IgG but not for anti-HEV IgM. HEV RNA was detected in 4.8% (22/453) of the samples analyzed and 9.9% (15/151) of the animals had at least one positive sample. Phylogenetic analysis showed that all sequences belonged to genotype 3 that were related to human isolates from other non-endemic regions, suggesting that the isolates had zoonotic potential. Subtypes 3c and 3f were simultaneously detected in some pigs, suggesting co-infection by more than one strain and/or the presence of a recombinant virus. These results constitute the first molecular and serologic evidence of swine HEV circulation in the Eastern Brazilian Amazon. (C) 2012 Elsevier Ltd. All rights reserved.

Hepatitis E is an infectious viral disease with clinical and morphological features of acute hepatitis. The aetiological agent is the Hepatitis E virus (HEV). The disease represents an important Public Health problem in developing countries where is frequently epidemic and primarily transmitted by fecal-oral route. In the last few years, a certain number of sporadic cases have been also described in industrialized countries, Italy included. A swine HEV was first identified in 1997 and is now considered an ubiquitous virus. Human and swine strains from the same geographical region have shown to have a high level of nucleotidic omology and in experimental infections, the possibility of interspecific transmission of swine strains to humans and of human strains to non-human primates has been demonstrated. Furthermore, some seroepidemiological studies have demonstrated that people working in contact with swine have a higher risk to get infected than normal blood donors. Recently, cases of HEV hepatitis have been directly associated to the ingestion of uncooked tissues from pigs, wild boar or deer and today the disease is considered an emerging zoonosis. The aims of this thesis were: evaluate HEV prevalence in Italian swine herds (both in fattening and in breeding animals); investigate the possibility of finding HEV in livers used for human consumption; investigate if there is any correlation between HEV infection and the presence of macroscopical lesions; investigate HEV prevalence in a demographic managed wild boar population; phylogenetically analyse viral strains identified. During an internship period at Veterinary Laboratories Agency (Weybridge, UK), furthermore, swine samples at different stages of production and slurry lagoons have been analysed. Six swine herds located in North Italy have been sampled at different stage of production. The overall prevalence resulted 42%, and both breeding and fattening animals were positive for HEV infection. A longitudinal study has been conducted in a herd across all stages of production until the slaughtering age. Livers have been collected from the animals at the abattoir and 11.8% of them were positive for HEV infection. No correlations have been identified between HEV infection and macroscopical lesions in pigs affected by different pathological conditions. Of 86 wild boars tested 22 (25%) were positive for HEV. Of the swine tested in UK 21,5 % and 2 of the 9 slurry lagoons (22,2%) were positive for HEV infection. All the strains identified belonged to genotype 3 and showed high percentages of nucleotidic identity with humans and swine strains identified in Europe. The high prevalence detected in these studies confirms the widespread diffusion of HEV in swine populations in Italy and in UK. Phylogenetical analysis of identified strains, similar to those identified in autochthonous human hepatitis E cases of the same geographical area, confirm the hypothesis that pigs can be a font of zoonotical infection. The finding that a fraction of the livers inserted in the food chain are positive for HEV infection it’s of some concern for Public Health. The finding of a high HEV prevalence in all examined farms, together with the observation that infection may be sub-clinical and affect animals at slaughtering age, raise concern because of the possible risk of transmission of HEV to humans by either direct contact with infected pigs, indirect contact with environment and working instruments contaminated with pig feces, or ingestion of contaminated undercooked meat.

Il virus dell’Epatite E (HEV) e i calicivirus (norovirus e sapovirus) causano rispettivamente epatite acuta e gastroenterite. Questi virus sono considerati agenti eziologici emergenti rappresentando un problema di sanità pubblica e di sicurezza alimentare. Per HEV, è ormai confermata la trasmissione zoonotica, e il suino è considerato il principale serbatoio asintomatico. Norovirus e sapovirus infettano sia i bambini che gli adulti. Sebbene questi virus siano stati identificati anche negli animali, la possibile trasmissione zoonotica non è stata dimostrata in modo conclusivo. Il lavoro sperimentale condotto durante il Dottorato di Ricerca è stato focalizzato sullo studio degli aspetti biologici ed epidemiologici dell’infezioni causate da HEV e da calicivirus. Per la prima volta in Italia, i risultati ottenuti hanno dimostrato la presenza del virus HEV nei fegati di suini in fase di macellazione ed hanno confermato, attraverso la ricerca di anticorpi, un’elevata esposizione degli animali al virus. Inoltre, mediante la produzione di antigeni e reattivi immunologici, sono stati messi a punto test diagnostici per la ricerca di anticorpi contro HEV nel suino e nei cinghiali. Il lavoro svolto per la ricerca di calicivirus nel suino e nel bovino ha dimostrato la circolazione dei sapovirus in popolazioni di suini asintomatici e la presenza di norovirus nei vitelli affetti da diarrea acuta.Sono stati inoltre sviluppati reattivi immunologici, utilizzando proteine del capside di norovirus umano e bovino espresse con il sistema ricombinante baculovirus. Questi hanno permesso di evidenziare la presenza di anticorpi contro norovirus umano e bovino, in sieri di veterinari professionalmente esposti. Inoltre, sono stati utilizzati per sviluppare metodi per la concentrazione dei virus da matrici a bassa concentrazione.Infine, le VLP sono state utilizzate per valutare l’attivazione del sistema immunitario umano ex vivo. I risultati hanno dimostrato che le VLP di NoV stimolano il sistema immunitario attivando risposte di tipo Th1 e Th2 .

Compte tenu de l’importance des LSEC dans le maintien de la tolérance hépatique et de l’élimination des pathogènes circulants, il a été postulé que la sévérité de l’hépatite et de la réponse inflammatoire lors d’infections par les MHV est associée à la réplication virale et à l’altération des propriétés tolérogéniques et vasculaires des LSEC. Les désordres inflammatoires hépatiques pourraient résulter d’une activation différentielle du TLR2, plutôt que des autres TLR et des hélicases, selon les sérotypes. D’autre part, compte tenu du rôle des BMEC dans la prévention des infections du SNC, il a été postulé que l’invasion cérébrale secondaire par les coronavirus est reliée à l’infection des BMEC et le bris subséquent de la barrière hémato-encéphalique (BHE). À l’aide d’infections in vivo et in vitro par les différents sérotypes MHV, chez des souris ou des cultures de BMEC et de LSEC, nous avons démontré, d’une part, que l’infection in vitro des LSEC par le sétotype MHV3, à la différence des variants 51.6- et YAC-MHV3, altérait la production du facteur vasodilatant NO et renversait leur phénotype tolérogénique en favorisant la production de cytokines et de chimiokines inflammatoires. Ces dysfonctions se traduisaient in vivo par une réponse inflammatoire incontrôlée et une dérégulation du recrutement intrahépatique de leucocytes, favorisant la réplication virale et les dommages hépatiques. Nous avons aussi démontré, à l’aide de souris TLR2 KO et de LSEC dont l’expression du TLR2 a été abrogée par des siRNA, que la sévérité de l’hépatite et de la réponse inflammatoire induite par le sérotype MHV3, dépendait en partie de l’induction et de l’activation préférentielle du TLR2 par le virus dans le foie. D’autre part, la sévérité de la réplication virale au foie et des désordres dans le recrutement leucocytaire intrahépatique induits par le MHV3, et non par le MHV-A59 et le 51.6-MHV3, corrélaient avec une invasion virale subséquente du SNC, au niveau de la BHE. Nous avons démontré que l’invasion cérébrale du MHV3 était associée à une infection productive des BMEC et l’altération subséquente des protéines de jonctions serrées occludine, VE-cadhérine et ZO-1 se traduisant par une augmentation de la perméabilité de la BHE et l’entrée consécutive du virus dans le cerveau. Dans l’ensemble, les résultats de cette étude mettent en lumière l’importance du maintien de l’intégrité structurale et fonctionnelle des LSEC et des BMEC lors d’infections virales aigües par des MHV afin de limiter les dommages hépatiques associés à l’induction d’une réponse inflammatoire exagérée et de prévenir le passage des virus au cerveau suite à une dissémination par voie hématogène. Ils révèlent en outre un nouveau rôle aggravant pour le TLR2 dans l’évolution de l’hépatite virale aigüe ouvrant la voie à de nouvelles avenues thérapeutiques visant à moduler l’activité inflammatoire du TLR2.

Disseminated adenoviral infection with hepatitis is rare in children undergoing standard chemotherapy. We report on a 3(1/2)-year-old male with fatal adenovirus hepatitis receiving maintenance chemotherapy for acute lymphoblastic leukemia (ALL). Adenoviral hepatitis was proven by histology, viral culture, and PCR in a liver biopsy. Quantitative real-time PCR in the peripheral blood showed adenoviral DNA copy number >10(9)/ml. Despite aggressive supportive care and antiviral treatment with cidofovir, the patient died rapidly due to fulminant liver failure. Diagnostic and treatment options for adenovirus infection remain unsatisfactory for these patients. We propose suggestions for diagnosis and therapy.

INTRODUCTION Herbal and dietary supplements are widely used as measures to improve and preserve health and well-being. Among the bestselling preparations are dietary supplement containing glucosamine and chondroitine sulfate taken to improve symptoms of osteoarthritis. METHODS AND RESULTS We here present a case of a male patient with biopsy-proven acute and severe autoimmune hepatitis subsequent to intake of a preparation containing glucosamine and chondroitine sulfate. Response to steroids was favorable and resulted in complete remission of the patient. Diagnostic work-up of the case revealed no other possible cause of liver injury, and causality assessment using the Roussel Uclaf Causality Assessment Method (RUCAM) resulted in a possible causal relationship between intake of glucosamine and chondroitine sulfate and the adverse hepatic reaction. CONCLUSION The present case recalls that products containing glucosamine and chondroitine sulfate can occasionally cause acute liver injury mimicking autoimmune hepatitis, and reminds of the potential dangers of compounds with poor efficacy and ill-defined safety records.
