O tracoma como principal causa de cegueira prevenível no mundo, é uma doença negligenciada relacionada a baixas condições socioeconômicas e locais sem saneamento básico. Presente principalmente nos países subdesenvolvidos traz grandes prejuízos aos cofres públicos com a perda de produtividade e a deficiência visual. Com a criação da Aliança para Eliminação Global de Tracoma em 1997 (GET2020), o Estado do Pará, com apoio do Ministério da Saúde do Brasil, realizaram em 2006 o inquérito epidemiológico do tracoma em escolares de 1ª a 4ª série da rede oficial de ensino, nos municípios com índice de desenvolvimento humano inferior a média nacional, para conhecer a prevalência da doença. Os dados obtidos no inquérito comprovaram que a doença não foi erradicada, revelando 35 municípios paraenses prioritários e prevalências acima de 5%. Uma sub-amostra da conjuntiva de escolares clinicamente positivos foi coletada para a confirmação diagnóstica por Imunofluorescência direta (IFD). O presente estudo utilizou 52 amostras crio conservadas obtidas durante o inquérito, para serem analisadas pelos métodos de IFD e de biologia molecular, na identificação laboratorial da Chlamydia trachomatis. Foram encontradas as frequências de 26,92% (14/52) e 49% (24/49) de resultados positivos pelas técnicas de IFD e reação em cadeia da polimerase (nested-PCR), respectivamente. Considerando as 49 amostras analisadas pelas duas metodologias, as sensibilidades para a detecção do agente etiológico, por IFD e PCR foram de 28,57% (14/49) e 48,98% (24/49), respectivamente (p = 0,0127). As duas técnicas juntas confirmaram a infecção em 57,14% (n=28) das amostras, onde 50% (n=14) foram positivas apenas pela PCR, 35,72% (n=10) para ambas as técnicas e 14,28% (n=4) somente pela IFD. A análise de sete sequências nucleotídicas demonstrou homologia para isolados de C. trachomatis genótipo L1. Este estudo é pioneiro no Brasil, pois além de confirmar a presença de C. trachomatis em amostras oculares de escolares clínicamente positivos para tracoma, validou protocolo de obtenção de DNA a partir de lâminas de IFD crioconservadas, demonstrou a maior sensibilidade do método molecular frente à IFD e identificou o genótipo L1 presente nas amostras.

AIMS: Juvenile idiopathic arthritis (JIA) is often associated with severe chronic anterior uveitis (CAU), and immunosuppressive therapy may be required. In this study, the value of cyclosporine A (CsA) as monotherapy or as combination therapy for treating uveitis was studied in a large cohort of JIA children. METHODS: Multicentre retrospective study including 82 JIA children (girls n=60) suffering from unilateral or bilateral (n=55) CAU. The indication for CsA was active uveitis, although patients were on topical or systemic corticosteroids, MTX, or other immunosuppressive drugs. RESULTS: Inactivity of uveitis during the entire treatment period (mean 3.9 years) was obtained with CsA monotherapy in 6 of 25 (24%) patients, but more often when CsA was combined with the immunosuppressives (35/72 patients; 48.6%, P=0.037), or MTX (18/37 patients, 48.6%, P=0.065), which had already been given. Purpose. This cross-sectional, observational study explored differences among groups staged for intent to decrease dietary fat intake in women with type 2 diabetes in relation to demographic, weight concern, physiological, and psychosocial variables. ^ Methods. A sample of 100 community-dwelling, English-speaking women, who were over age 30 and had type 2 diabetes for at least a year, was accessed through a culturally diverse endocrinology clinic. Subjects completed 7 self-report instruments: demographic sheet, with 11-point weight satisfaction scale; staging algorithm; fat intake (MEDFICTS); depression (CES-D); diabetes-specific dietary knowledge (ADKnowl), social support and self-efficacy scales (SE-Type 2). Physiological variables were abstracted from the medical record (HbA 1c, blood pressure, serum cholesterol and triglycerides). ^ Results. The women's average age was 57.69 years ( SD = 3.07); 50% were married. Subjects were well-educated ( M = 14 years; SD = 3.33), with average diabetes duration of 10.57 years (SD = 9.11), high body mass index (M = 35.72; SD = 8.36), low diabetes-specific dietary knowledge, low weight satisfaction, but in good diabetes control. Racial/ethnic composition was 44% non-Hispanic-White-American, 18% Hispanic-White-American, 15% non-Hispanic-African-American, 16% Hispanic-African-American and 5% other. Fat intake was low and differed by racial/ethnic demographics. The highest fat intake scores were for non-Hispanic-African-Americans (M = 53), followed by Hispanic-White-Americans (M = 51), non-Hispanic-White-Americans (M = 45), and Hispanic-African-Americans (M = 32), who had the lowest fat intake scores. ^ MANOVA analyses revealed no significant differences between stages of behavior change in relation to psychosocial or weight concern variables, age, education, HbA1c, or cholesterol levels. Single women were more likely to be in the three preaction stages (precontemplation, contemplation, and preparation); married women were equally distributed across stages (the preaction stages plus action and maintenance). African-American women (Hispanic and non-Hispanic) were more likely in contemplation and preparation. Triglycerides were higher in women in the action stage than contemplation or preparation. Systolic blood pressure was higher in action than preparation; diastolic blood pressure was higher in action than preaction. ^ Conclusions. Healthcare professionals should consider race, ethnicity, and marital status in client interactions. Dietary intake can vary according to both race and ethnicity; collapsing racial/ethnic groups can alter means and distributions, generating faulty conclusions. Further research is warranted to explore relationships between dietary self-care and marital status, race, ethnicity, and physiological variables. ^

Purpose. To evaluate the repeatability and reproducibility of subfoveal choroidal thickness (CT) calculations performed manually using optical coherence tomography (OCT). Methods. The CT was imaged in vivo at each of two visits on 11 healthy volunteers (mean age, 35.72 ± 13.19 years) using the spectral domain OCT. CT was manually measured after applying ImageJ processing filters on 15 radial subfoveal scans. Each radial scan was spaced 12° from each other and contained 2500 A-scans. The coefficient of variability, coefficient of repeatability (CoR), coefficient of reproducibility, and intraclass correlation coefficient determined the reproducibility and repeatability of the calculation. Axial length (AL) and mean spherical equivalent refractive error were measured with the IOLMaster and an open view autorefractor to study their potential relationship with CT. Results. The within-visit and between-visit coefficient of variability, CoR, coefficient of reproducibility, and intraclass correlation coefficient were 0.80, 2.97% 2.44%, and 99%, respectively. The subfoveal CT correlated significantly with AL (R = -0.60, p = 0.05). Conclusions. The subfoveal CT could be measured manually in vivo using OCT and the readings obtained from the healthy subjects evaluated were repeatable and reproducible. It is proposed that OCT could be a useful instrument to perform in vivo assessment and monitoring of CT changes in retinal disease. The preliminary results suggest a negative correlation between subfoveal CT and AL in such a way that it decreases with increasing AL but not with refractive error.

Analysis of bovine interphotoreceptor matrix and conditioned medium from human Y-79 retinoblastoma cells by gelatin SDS-PAGE zymography reveals abundant activity of a 72-kDa M(r) gelatinase. The 72-kDa gelatinase from either source is inhibited by EDTA but not aprotinin or NEM, indicating that it is a metalloproteinase (MMP). The 72-kDa MMP is converted to a 62-kDa species with APMA treatment after gelatin sepharose affinity purification typical of previously described gelatinase MMP-2. The latent 72-kDa gelatinase from either bovine IPM or Y-79 media autoactivates without APMA in the presence of calcium and zinc after 72 hr at 37°C, producing a fully active mixture of proteinase species, 50 (48 in Y-79 medium), 38 and 35 kDa in size. The presence of inhibitory activity was examined in both whole bovine IPM and IPM fractions separated by SDS-PAGE. Whole IPM inhibited gelatinolytic activity of autoactivated Y-79-derived MMP in a dose-dependent manner. Inhibitory activities are observed in two protein fractions of 27-42 and 20-25 kDa. Western blots using antibodies to human tissue inhibitor of metalloproteinase 1 and 2 (TIMP-1 and -2) reveal the presence of two TIMP-1-like proteins at 21 and 29 kDa in inhibitory fractions of the bovine IPM. TIMP-2 was not detected in the inhibitory IPM fractions, consistent with the observed autoactivation of bovine IPM 72-kDa gelatinase. Potential roles for this IPM MMP-TIMP system include physiologic remodelling of the neural retina-RPE cell interface and digestion of shed rod outer segment as well as pathological processes such as retinal detachment, PE cell migration, neovascularization and tumor progression. Cultured Y-79 cells appear to be a good model for studying the production and regulation of this proteinase system.

Purpose The purpose of this study is to examine the prevalence, sociodemographic and clinical predictors, and physical and psychosocial correlates of unmet needs among women 3–5 years following treatment for endometrial cancer. Methods Women with endometrial cancer completed a survey around the time of diagnosis and again 3–5 years later. The follow-up survey asked women about their physical and psychosocial functioning and supportive care needs (CaSUN). Multivariable-adjusted logistic regression identified the predictors and correlates of women’s unmet needs 3–5 years after diagnosis. Results Of the 629 women who completed the cancer survivors’ unmet needs measure (CaSUN), 24 % (n = 153) women reported one or more unmet supportive care needs in the last month. Unmet needs at 3–5 years post-diagnosis were predicted by younger age (OR = 4.47; 95 % CI: 2.09–9.56) and advanced disease stage at diagnosis (OR = 2.47; 95 % CI: 1.38–4.45) and correlated with greater cancer symptoms (OR = 1.78; 95 % CI: 1.05–3.02), lower limb swelling (OR = 2.50; 95 % CI: 1.51–4.15), symptoms of anxiety (OR = 2.21; 95 % CI: 1.31–3.72), and less availability of social support (OR = 3.42; 95 % CI: 1.92–6.11). Women with a history of comorbidities (OR = 0.47; 95 % CI: 0.27–0.82) and those living in a rural area at the time of diagnosis (OR = 0.56; 95 % CI: 0.34–0.92) were less likely to report unmet needs. Conclusions Sociodemographic, health, and psychosocial factors seem important for identifying women who will or will not have unmet needs several years following endometrial cancer. Longitudinal assessments of people’s needs over the course of their cancer trajectory may be an effective way to identify areas that should receive further attention by health providers.

O crescimento da expectativa de vida média da população mundial tem sido acompanhado do aumento na prevalência de doenças neurodegenerativas, como a doença de Parkinson, isquemia cerebral e, especialmente, a doença de Alzheimer. A doença de Alzheimer (DA) caracteriza-se por um crescente declínio na função mental e memória do paciente. Estes sintomas são explicados por uma profunda perda neuronal e pela presença de alterações estruturais no tecido cerebral: as placas senis, extracelulares e os emaranhados neurofibrilares, intracelulares. O passo que desencadeia a neurodegeneração na DA é ainda objeto de estudo, mas a hipótese mais aceita atualmente é a de que a secreção anormal do peptídeo β amilóide (Aβ), principal componente das placas senis, dê início ao processo. O presente trabalho teve como objetivos investigar a toxicidade induzida pelo peptídeo Aβ1-42 e seu fragmento, o peptídeo Aβ25-35 em culturas organotípicas de hipocampo de ratos. Na primeira parte do trabalho, investigamos a toxicidade induzida pela exposição de culturas organotípicas de hipocampo de ratos a 10 ou 20 μM do peptídeo Aβ1-42, por 24 ou 72 h. Além disso, investigamos o envolvimento das proteínas iNOS e GSK-3β no mecanismo de morte celular induzida pelo peptídeo Aβ1-42. Na segunda parte do trabalho, investigamos a toxicidade induzida pelo fragmento Aβ25-35 na concentração de 25 μM, nos tempos de 1, 3, 6, 12, 24 ou 48 h de exposição às culturas organotípicas, e seu efeito sobre as proteínas Akt, GSK-3β e PTEN. A morte celular foi quantificada pela incorporação do iodeto de propídeo, corante marcador excluído de células sadias, e as proteínas foram quantificadas por imunodetecção com o uso de anticorpos específicos. Nossos resultados mostraram que o peptídeo Aβ1-42 apresentou toxicidade nas concentrações de 10 e 20 μM, induzindo a uma considerável morte celular após 72 h de exposição. A análise do imunoconteúdo da proteína iNOS mostrou um aumento significativo em relação ao controle, após 72 h de exposição ao peptídeo e na concentração de 20 μM. Os resultados obtidos na segunda parte do trabalho mostraram uma morte celular significativa apenas após 48 h de exposição ao peptídeo Aβ25- 35 na concentração de 25 μM. O tratamento com peptídeo Aβ25-35 levou ao aumento no estado de fosforilação da proteína Akt após 6 h de exposição e também da proteína GSK-3β, um potencial substrato da Akt. Após 12 h de exposição ao peptídeo, observamos uma diminuição de ambas as proteínas (Akt e GSK-3β). Porém, após 24 h de exposição ao peptídeo, a proteína Akt continua menos fosforilada enquanto que a proteína GSK-3β apresenta um novo pico de fosforilação. O imunoconteúdo da proteína fosfatase PTEN apresentou um aumento significativo em 24 h e 48 h. Os resultados do presente estudo mostraram que o tratamento das culturas organotípicas de hipocampo de ratos com o peptídeo Aβ1-42 como com o fragmento Aβ25-35 apresentou toxicidade após um período de aproximadamente 48 h de tratamento, e mostrou ser um bom modelo para o estudo de sua toxicidade. Com relação ao mecanismo investigado, os dados sugerem que a proteína iNOS pode estar envolvida na toxicidade induzida pelo peptídeo Aβ1-42. Além disso, sugerem que o fragmento Aβ25-35 possa exercer sua toxicidade através da inibição da via de sobrevivência celular PI3-K, por diminuir a fosforilação/ativação da proteína Akt, parecendo envolver a proteína fosfatase PTEN, principal regulador negativo da via PI3-K/Akt.

BACKGROUND/AIMS: Alveolar echinococcosis (AE) is a serious liver disease. The aim of this study was to explore the long-term prognosis of AE patients, the burden of this disease in Switzerland and the cost-effectiveness of treatment. METHODS: Relative survival analysis was undertaken using a national database with 329 patient records. 155 representative cases had sufficient details regarding treatment costs and patient outcome to estimate the financial implications and treatment costs of AE. RESULTS: For an average 54-year-old patient diagnosed with AE in 1970 the life expectancy was estimated to be reduced by 18.2 and 21.3 years for men and women, respectively. By 2005 this was reduced to approximately 3.5 and 2.6 years, respectively. Patients undergoing radical surgery had a better outcome, whereas the older patients had a poorer prognosis than the younger patients. Costs amount to approximately Euro108,762 per patient. Assuming the improved life expectancy of AE patients is due to modern treatment the cost per disability-adjusted life years (DALY) saved is approximately Euro6,032. CONCLUSIONS: Current treatments have substantially improved the prognosis of AE patients compared to the 1970s. The cost per DALY saved is low compared to the average national annual income. Hence, AE treatment is highly cost-effective in Switzerland.
