Drug-resistant tuberculosis (TB) threatens global TB control and is a major public health concern in several countries. We therefore developed a multiplex assay (LINE-TB/MDR) that is able to identify the most frequent mutations related to rifampicin (RMP) and isoniazid (INH) resistance. The assay is based on multiplex polymerase chain reaction, membrane hybridisation and colorimetric detection targeting of rpoB and katG genes, as well as the inhA promoter, which are all known to carry specific mutations associated with multidrug-resistant TB (MDR-TB). The assay was validated on a reference panel of 108 M. tuberculosis isolates that were characterised by the proportion method and by DNA sequencing of the targets. When comparing the performance of LINE-TB/MDR with DNA sequencing, the sensitivity, specificity and agreement were 100%, 100% and 100%, respectively, for RMP and 77.6%, 90.6% and 88.9%, respectively, for INH. Using drug sensibility testing as a reference standard, the performance of LINE-TB/MDR regarding sensitivity, specificity and agreement was 100%, 100% and 100% (95%), respectively, for RMP and 77%, 100% and 88.7% (82.2-95.1), respectively, for INH. LINE-TB/MDR was compared with GenoType MTBDRplus for 65 isolates, resulting in an agreement of 93.6% (86.7-97.5) for RIF and 87.4% (84.3-96.2) for INH. LINE-TB/MDR warrants further clinical validation and may be an affordable alternative for MDR-TB diagnosis.

We examined the contribution of each alpha(1)-adrenoceptor (AR) subtype in noradrenaline (NAd)-evoked contraction in the thoracic aortas and mesenteric arteries of mice. Compared with the concentration-response curves (CRCs) for NAd in the thoracic aortas of wild-type (WT) mice, the CRCs of mutant mice showed a significantly lower sensitivity. The pD(2) value in rank order is as follows: WT mice (8.21) > alpha(1B)-adrenoceptor knockout (alpha(1B)-KO) (7.77) > alpha(1D)-AR knockout (alpha(1D)-KO) (6.44) > alpha(1B)- and alpha(1D)-AR double knockout (alpha(1BD)-KO) (5.15). In the mesenteric artery, CRCs for NAd did not differ significantly between either WT (6.52) and alpha(1B)-KO mice (7.12) or alpha(1D)-KO (6.19) and alpha(1BD)-KO (6.29) mice. However, the CRC maximum responses to NAd in alpha(1D)- and alpha(1BD)-KO mice were significantly lower than those in WT and alpha(1B)-KO mice. Except in the thoracic aortas of alpha(1BD)-KO mice, the competitive antagonist prazosin inhibited the contraction response to NAd with high affinity. However, prazosin produced shallow Schild slopes in the vessels of mice lacking the alpha(1D)-AR gene. In the thoracic aorta, pA(2) values in WT mice for KMD-3213 and BMY7378 were 8.25 and 8.46, respectively, and in alpha(1B)-KO mice they were 8.49 and 9.13, respectively. In the mesenteric artery, pA(2) values in WT mice for KMD-3213 and BMY7378 were 8.34 and 7.47, respectively, and in alpha(1B)-KO mice they were 8.11 and 7.82, respectively. These pharmacological findings were in fairly good agreement with findings from comparison of CRCs, with the exception of the mesenteric arteries of WT and alpha(1B)-KO mice, which showed low affinities to BMY7378. We performed a quantitative analysis of the mRNA expression of each alpha(1)-AR subtype in these vessels in order to examine the correlation between mRNA expression level and the predominance of each alpha(1)-AR subtype in mediating vascular contraction. The rank order of each alpha(1)-AR subtype in terms of its vasoconstrictor role was in fairly good agreement with the level of expression of mRNA of each subtype, that is, alpha(1D)-AR > alpha(1B)-AR > alpha(1A)-AR in the thoracic aorta and alpha(1D)-AR > alpha(1A)-AR > alpha(1B)-AR in the mesenteric artery. No dramatic compensatory change of alpha(1)-AR subtype in mutant mice was observed in pharmacological or quantitative mRNA expression analysis.

BACKGROUND & AIMS: Regulation of gene expression in the follicle-associated epithelium (FAE) over Peyer's patches is largely unknown. CCL20, a chemokine that recruits immature dendritic cells, is one of the few FAE-specific markers described so far. Lymphotoxin beta (LTalpha1beta2) expressed on the membrane of immune cells triggers CCL20 expression in enterocytes. In this study, we measured expression profiles of LTalpha1beta2-treated intestinal epithelial cells and selected CCL20 -coregulated genes to identify new FAE markers. METHODS: Genomic profiles of T84 and Caco-2 cell lines treated with either LTalpha1beta2, flagellin, or tumor necrosis factor alpha were measured using the Affymetrix GeneChip U133A. Clustering analysis was used to select CCL20 -coregulated genes, and laser dissection microscopy and real-time polymerase chain reaction on human biopsy specimens was used to assess the expression of the selected markers. RESULTS: Applying a 2-way analysis of variance, we identified regulated genes upon the different treatments. A subset of genes involved in inflammation and related to the nuclear factor kappaB pathway was coregulated with CCL20 . Among these genes, the antiapoptotic factor TNFAIP3 was highly expressed in the FAE. CCL23 , which was not coregulated in vitro with CCL20 , was also specifically expressed in the FAE. CONCLUSIONS: We have identified 2 novel human FAE specifically expressed genes. Most of the CCL20 -coregulated genes did not show FAE-specific expression, suggesting that other signaling pathways are critical to modulate FAE-specific gene expression.

ABSTRACT Diffuse reflectance spectroscopy (DRS) is a fast and cheap alternative for soil clay, but needs further investigation to assess the scope of application. The purpose of the study was to develop a linear regression model to predict clay content from DRS data, to classify the soils into three textural classes, similar to those defined by a regulation of the Brazilian Ministry of Agriculture, Livestock and Food Supply. The DRS data of 412 soil samples, from the 0.0-0.5 m layer, from different locations in the state of Rio Grande do Sul, Brazil, were measured at wavelengths of 350 to 2,500 nm in the laboratory. The fitting of the linear regression model developed to predict soil clay content from the DRS data was based on a R2 value of 0.74 and 0.75, with a RMSE of 7.82 and 8.51 % for the calibration and validation sets, respectively. Soil texture classification had an overall accuracy of 79.0 % (calibration) and 80.9 % (validation). The heterogeneity of soil samples affected the performance of the prediction models. Future studies should consider a previous classification of soil samples in different groups by soil type, parent material and/or sampling region.

STUDY OBJECTIVES: There is limited information regarding sleep duration and determinants in Switzerland. We aimed to assess the trends and determinants of time in bed as a proxy for sleep duration in the Swiss canton of Geneva. METHODS: Data from repeated, independent cross-sectional representative samples of adults (≥ 18 years) of the Geneva population were collected between 2005 and 2011. Self-reported time in bed, education, monthly income, and nationality were assessed by questionnaire. RESULTS: Data from 3,853 participants (50% women, 51.7 ± 10.9 years) were analyzed. No significant trend was observed between 2005 and 2011 regarding time in bed or the prevalence of short (≤ 6 h/day) and long (> 9 h/day) time in bed. Elderly participants reported a longer time in bed (year-adjusted mean ± standard error: 7.67 ± 0.02, 7.82 ± 0.03, and 8.41 ± 0.04 h/day for 35-50, 50-65, and 65+ years, respectively, p < 0.001), while shorter time in bed was reported by non-Swiss participants (7.77 ± 0.03 vs. 7.92 ± 0.03 h/day for Swiss nationals, p < 0.001), participants with higher education (7.92 ± 0.02 for non-university vs. 7.74 ± 0.03 h/day for university, p < 0.001) or higher income (8.10 ± 0.04, 7.84 ± 0.03, and 7.70 ± 0.03 h/day for < 5,000 SFr; 5,000-9,500 SFr, and > 9,500 SFr, respectively, p < 0.001). Multivariable-adjusted polytomous logistic regression showed short and long time in bed to be positively associated with obesity and negatively associated with income. CONCLUSION: In a Swiss adult population, sleep duration as assessed by time in bed did not change significantly between 2005 and 2011. Both clinical and socioeconomic factors influence time in bed.

CONTEXT: The current standard for diagnosing prostate cancer in men at risk relies on a transrectal ultrasound-guided biopsy test that is blind to the location of the cancer. To increase the accuracy of this diagnostic pathway, a software-based magnetic resonance imaging-ultrasound (MRI-US) fusion targeted biopsy approach has been proposed. OBJECTIVE: Our main objective was to compare the detection rate of clinically significant prostate cancer with software-based MRI-US fusion targeted biopsy against standard biopsy. The two strategies were also compared in terms of detection of all cancers, sampling utility and efficiency, and rate of serious adverse events. The outcomes of different targeted approaches were also compared. EVIDENCE ACQUISITION: We performed a systematic review of PubMed/Medline, Embase (via Ovid), and Cochrane Review databases in December 2013 following the Preferred Reported Items for Systematic reviews and Meta-analysis statement. The risk of bias was evaluated using the Quality Assessment of Diagnostic Accuracy Studies-2 tool. EVIDENCE SYNTHESIS: Fourteen papers reporting the outcomes of 15 studies (n=2293; range: 13-582) were included. We found that MRI-US fusion targeted biopsies detect more clinically significant cancers (median: 33.3% vs 23.6%; range: 13.2-50% vs 4.8-52%) using fewer cores (median: 9.2 vs 37.1) compared with standard biopsy techniques, respectively. Some studies showed a lower detection rate of all cancer (median: 50.5% vs 43.4%; range: 23.7-82.1% vs 14.3-59%). MRI-US fusion targeted biopsy was able to detect some clinically significant cancers that would have been missed by using only standard biopsy (median: 9.1%; range: 5-16.2%). It was not possible to determine which of the two biopsy approaches led most to serious adverse events because standard and targeted biopsies were performed in the same session. Software-based MRI-US fusion targeted biopsy detected more clinically significant disease than visual targeted biopsy in the only study reporting on this outcome (20.3% vs 15.1%). CONCLUSIONS: Software-based MRI-US fusion targeted biopsy seems to detect more clinically significant cancers deploying fewer cores than standard biopsy. Because there was significant study heterogeneity in patient inclusion, definition of significant cancer, and the protocol used to conduct the standard biopsy, these findings need to be confirmed by further large multicentre validating studies. PATIENT SUMMARY: We compared the ability of standard biopsy to diagnose prostate cancer against a novel approach using software to overlay the images from magnetic resonance imaging and ultrasound to guide biopsies towards the suspicious areas of the prostate. We found consistent findings showing the superiority of this novel targeted approach, although further high-quality evidence is needed to change current practice.

Objective Undertaking of a complete audit of the service of mammography, as recommended by BI-RADS®, in a private reference institution for breast cancer diagnosis in the city of São Paulo, SP, Brazil, and comparison of results with those recommended by the literature. Materials and Methods Retrospective, analytical and cross-sectional study including 8,000 patients submitted to mammography in the period between April 2010 and March 2011, whose results were subjected to an internal audit. The patients were followed-up until December 2012. Results The radiological classification of 7,249 screening mammograms, according to BI-RADS, was the following: category 0 (1.43%), 1 (7.82%), 2 (80.76%), 3 (8.35%), 4 (1.46%), 5 (0.15%) and 6 (0.03%). The breast cancer detection ratio was 4.8 cases per 1,000 mammograms. Ductal carcinoma in situ was found in 22.8% of cases. Positive predictive values for categories 3, 4 and 5 were 1.3%, 41.3% and 100%, respectively. In the present study, the sensitivity of the method was 97.1% and specificity, 97.4%. Conclusion The complete internal audit of a service of mammography is essential to evaluate the quality of such service, which reflects on an early breast cancer detection and reduction of mortality rates.

Objetivo: determinar o desempenho da ultra-sonografia para detecção de gestações que apresentem risco de anomalia cromossômica fetal. Método: estudamos 436 pacientes submetidas a cariótipo fetal entre março de 1993 e março de 1998. Destas, 277 gestantes foram submetidas à coleta de material fetal para estudo citogenético por ultra-sonografia alterada e 159 por ansiedade parental, apresentando ultra-sonografia morfológica normal. Foram avaliadas a sensibilidade e a especificidade da ultra-sonografia utilizando o cariótipo fetal como padrão-ouro. Foi calculado o risco relativo de anomalia cromossômica apresentado segundo o sistema acometido à ultra-sonografia e os riscos de acordo com a presença de uma ou mais malformações, utilizando-se o pacote de análise estatística Epi-Info 6.0. Resultados: observamos que o risco relativo para cromossomopatia fetal foi de 89 para as malformações de face, 53 para malformações de parede abdominal e aparelho cardiovascular, 49,6 para malformações de pescoço, 44,6 para malformações de membros, 42,4 para malformações de pulmão, 32,7 para malformações de trato gastrointestinal, 27,4 para malformações de sistema nervoso central e 23,0 para malformações de trato urinário. Não foi possível o cálculo de risco para malformações de genitais, tórax, coluna e músculo-esqueléticas, por terem ocorrido em um pequeno número de casos. A presença de uma alteração ultra-sonográfica isolada se associa a risco relativo 7,82 vezes maior de cromossomopatia, ao passo que a associação de alterações morfológicas tem risco 33,8 vezes maior de cromossomopatia. Conclusões: conclui-se que a ultra-sonografia apresentou bom desempenho na detecção de gestações de risco para cromossomopatia.

Este estudo teve como objetivos monitorar a distribuição de populações de Brachiaria subquadripara e caracterizar seu ambiente de ocorrência em duas épocas (seca e águas). As coletas foram realizadas em julho de 2004 e janeiro de 2005 em pontos previamente selecionados e georreferenciados, os quais se constituíram de 13 pontos no reservatório Barra Bonita (sete pontos no braço do rio Piracicaba e seis no braço do rio Tietê). Foram realizadas coletas de solo nas margens dos rios, sedimento, água e plantas. A distribuição em relação à área de infestação das populações de B. subquadripara observada na estação chuvosa foi influenciada pela variação sazonal, a qual ocorreu de forma heterogênea em relação à densidade populacional. O rio Tietê foi considerado um ambiente mais eutrófico do que o rio Piracicaba tanto em relação ao solo quanto em relação à coluna d'água. As amostras de água coletadas no rio Tietê apresentaram valores médios de pH na estação seca de 7,02 a 7,82, e na estação das águas registrou-se maior amplitude de variação (6,80 a 8,0). A maioria dos pontos amostrados apresentou altos teores de matéria orgânica no sedimento, sendo observados teores menores que 1,3% em dois pontos de coleta na época da seca e em cinco pontos na época das águas.

During adolescence, the sleep phase delay associated with early school times increases daytime sleepiness and reduces psychomotor performance. Some studies have shown an effect of gender on psychomotor performance in adults and children. Males present faster reaction times (RT) compared with females. The aim of the present study was to evaluate the influence of gender on Palm psychomotor vigilance task (PVT) performance in adolescents. Thirty-four adolescents (19 girls, 13 to 16 years old) attending morning school classes of a public school in Curitiba, PR, Brazil, participated in the study. Sleep patterns were measured using actigraphy and sleepiness data were accessed with the Karolinska Sleepiness Scale (KSS). KSS and PVT measurements were collected at two times in the morning (8:00 and 11:00 h). The data were compared using one-way ANOVA, considering gender as a factor. ANOVA indicated that gender did not affect sleep patterns and subjective somnolence; however, a statistically significant effect of gender was detected for PVT performance. Boys presented faster RT (RT-PVT1: 345.51 ms, F = 6.08, P < 0.05; RT-PVT2: 343.30 ms, F = 6.35, P < 0.05) and fewer lapses (lapses-PVT1: 8.71, F = 4.45, P < 0.05; lapses-PVT2: 7.82, F = 7.06, P < 0.05) compared with girls (RT-PVT1: 402.96; RT-PVT2: 415.70; lapses-PVT1: 16.33; lapses-PVT2: 17.80). These results showed that this effect of gender, already reported in adults and children, is also observed in adolescents. The influence of gender should be taken into account in studies that use Palm PVT to evaluate psychomotor performance in this age range.

Le cancer du sein est une maladie multifactorielle, plusieurs facteurs socio-économiques, alimentaires ainsi que le style de vie ayant été incriminés dans son développement. Une mutation germinale sur un des gènes BRCA1 ou BRCA2 serait responsable d’une augmentation du risque de développer un cancer du sein de 50 à 80% chez les femmes porteuses d’une mutation sur BRCA1 ou BRCA2 par comparaison aux non- porteuses. Plusieurs études rapportant l’existence d’une association entre la fréquence des cancers du sein sporadiques, les habitudes alimentaires et le style de vie des femmes atteintes, ceci, nous a amené à nous interroger sur le rôle que pourrait jouer ces mêmes facteurs chez les femmes porteuses d’une mutation sur BRCA1 ou BRCA2. Pour répondre à cette question, nous avons effectué une étude cas-témoin. Nos quarante-quatre cas sont porteuses d’une mutation germinale parmi 6 des 14 mutations fondatrices sur BRCA1 et BRCA2 les plus fréquentes dans la population Canadienne Française. Elles sont issues d’une cohorte de plus de 1000 femmes Canadiennes Françaises atteintes de cancers du sein recrutées depuis 1994 et testées pour ces 6 mutations. Les témoins sont recrutés parmi des femmes de la population Canadienne Française, également porteuses d’une de ces six mutations fondatrices de BRCA1 ou BRCA2, mais indemnes de cancers. Quinze d’entre elles ont été recrutées dans les familles des cas de l’étude initiale. Vingt-neuf ont été recrutées à la clinique des cancers familiaux du CHUM, nous permettant ainsi de totaliser 44 témoins. Deux questionnaires ont été administrés aux cas et aux témoins. Le premier, dit Questionnaire de base, a servi à recenser les informations sociodémographiques et le style de vie, couvrant ainsi les deux années précédant la découverte du cancer du sein pour les cas et les deux années précédant la découverte de la mutation pour les témoins. Le deuxième questionnaire, Questionnaire de nutrition, a permis de colliger les informations sur les habitudes alimentaires durant la même période de recueil de données. Une association positive et significative entre le risque de cancer du sein et le niveau d’éducation a été observé parmi les sujets de niveau universitaire (>14 années d’étude) comparés aux sujets n’ayant pas dépassé le niveau d’études secondaires (<11 années d’études) [OR= 7,82; IC95% : (1,99-30,69); p=0,003]. Nous avons mis en évidence que le risque de cancer du sein augmentait lorsque les sujets atteignaient leur poids maximum à un âge avancé > 48 ans [OR = 4,27 ; IC 95% : (0,82-22,25)]. Nous avons montré que le risque du cancer du sein diminuait pour une durée d’allaitement supérieure à 7 mois par comparaison aux femmes n’ayant jamais allaité [OR= 0,35; IC 95% : (0,12-1,06)] mais cette association est non significative. Les porteuses qui pratiquent plus de 22,45 Met-h-sem d’activité physique modérée, comparativement à celles qui pratiquent moins de 11,45 Met-h-sem voient leur risque de cancer du sein diminué de 72% [OR=0,28- IC 95% : (0,08-0,95); p=0,04]. Celles qui pratiquent plus de 31,95 Met-h-sem d’activité physique totale comparativement à celles qui pratiquent moins de 16,40 Met-h-sem voient leur risque de cancer du sein réduit de 79 % [OR=0,21; IC 95% : (0,06-0,75); p= 0,02]. L’analyse des macro et micronutriments et des groupes alimentaires a démontré qu’une consommation de plus de 23,20 g/j d’acide gras monoinsaturés est responsable d’une augmentation du risque de cancer du sein de 6 fois par comparaison à une consommation inférieure à 17,08 g/j [OR=6,00; IC 95% : (0,97-37,02); p=0,05]. Une consommation de plus de 221,79 µg/j de vitamine K réduit le risque du cancer du sein de 83 % par comparaison à une consommation inférieure à 143,57 µg/j [OR= 0,17; IC95% : (0,05-0,61) ; p=0,007]. La consommation de fruits est associée à une réduction du risque de cancer du sein de 73% chez les porteuses de mutations qui en consomment plus de 563,31 g/j comparée à celles qui en consomment moins de 356,18 g/j [OR= 0,27; IC 95% : (0,07-1,01) ; p=0,05]. Nos résultats confortent l’hypothèse selon laquelle le style de vie et les habitudes alimentaires jouent un rôle dans le développement du cancer du sein chez les Canadiennes Françaises porteuses de mutations d’une des 6 mutations fondatrices de BRCA1 ou 2 étudiées. En effet, un niveau d’éducation élevé, un gain de poids sont associés à un risque élevé de développer un cancer du sein. De plus la pratique de l’allaitement et d’une activité physique modérée sont associées à une réduction de ce risque. Nous montrons aussi que la consommation d’acides gras monoinsaturés est responsable d’une augmentation du risque de ce cancer et que la consommation de vitamine K et de fruits permet de réduire ce risque. Nos résultats ouvrent une nouvelle voie de recherche par rapport au rôle de certains nutriments dans le développement du cancer du sein chez les porteuses de mutation d’un des gènes BRCA. Cette voie pourrait également être explorée chez les non porteuses.

La osteoporosis es una de las condiciones patológicas en mayor crecimiento a medida que la población de tercera edad aumenta, esto se traduce en fracturas por fragilidad como lo son las fracturas de radio distal y las fracturas de cadera, actualmente no se cuentas con datos de la población a estudio que correlacione este tipo de fracturas. Es un estudio retrospectivo de casos y controles donde se obtuvo un grupo de pacientes con fractura de cadera que consultaron a un hospital universitario de alta complejidad en la ciudad de Bogotá, se evaluó la presencia de antecedente de fractura de radio distal y se comparó con un grupo control de trauma en cadera. Se obtuvo un total de 325 casos (72,5%) y 123 (25%) controles. El promedio de edad fue de 81 años, el 70% de los pacientes en ambos grupos correspondió a mujeres. No hubo diferencia en cuanto a la prevalencia de tabaquismo, hipertensión arterial o diabetes en los grupos. No se encontraron diferencias significativas en cuanto a niveles de glicemia, calcio, vitamina D. La presencia de antecedente de fractura de radio distal en grupo con fractura de cadera fue del 7,1% encontrando un OR de 3,91 IC 95%(1,17– 13,10). La presencia de fractura de radio distal como antecedente es un predictor para la fractura de cadera en pacientes mayores. Se necesitan más estudios que correlacionen otras variables que pueden influir en la asociación para fractura de cadera y radio, para así identificar una población específica que se beneficie de un tratamiento temprano.

Programa de asesoramiento vocacional para los profesionales de la orientación que pretende dar respuesta desde una perspectiva globalizadora a la situación de incertidumbre de los estudiantes de secundaria, teniendo en cuenta todos los miembros de la comunidad educativa. Se propone un plan de intervención que desarrolla actividades programadas para orientar al alumnado, fundamentado en un marco teórico sobre la conducta vocacional en sus diferentes facetas, asesoramiento vocacional, proceso de madurez vocacional e información vocacional, y basado en varios años de experiencia con alumnos de enseñanzas medias y secundaria. El proceso que sigue se centra en cuatro bloques: dinámicas individuales, Sistema de Asesoramiento Vocacional Renovado (SAV-R), dinámicas de grupo, información a las familias. Incluye un anexo referente al papel de los padres en el ámbito de la evolución académica y profesional de sus hijos, y otro que relaciona los materiales de apoyo que formarán parte del dossier personal de cada alumno.

O ácido azelaico é um fármaco com actividade bacteriostática para muitos microorganismos sendo por isso frequentemente aplicado no tratamento do acne. Porém, às formulações tópicas deste fármaco estão geralmente associados alguns efeitos adversos e fracas adesões à terapêutica. Assim, a nanotecnologia pode ser aqui considerada como uma estratégia inovadora para ultrapassar os obstáculos anteriores. O objectivo deste estudo centrou-se no desenvolvimento e caracterização de nanopartículas de PLGA contendo o ácido azelaico. As nanopartículas foram produzidas através do método modificado de emulsificação/difusão do solvente e posteriormente incluídas num gel de Carbopol 940. Foram caracterizados vários parâmetros da formulação tais como potencial zeta, tamanho da partícula e eficiência de encapsulação. O tamanho médio das partículas foi de 378,63 nm (com I.P. cerca de 0,09) e o potencial zeta foi de -7,82 mV. Aeficiência de encapsulação do ácido azelaico foi de 76 ± 3,81%. Consequentemente, estas nanopartículas poderão ser consideradas ferramentas úteis para a veiculação do ácido azelaico.
