OBJECTIVE: to review the evolution of acute respiratory distress syndrome (ARDS) definitions and present the current definition for the syndrome. DATA SOURCE: a literature review and selection of the most relevant articles on ARDS definitions was performed using the MEDLINE®/PubMed® Resource Guide database (last ten years), in addition to including the most important articles (classic articles) that described the disease evolution. DATA SYNTHESIS: the review included the following subjects: introduction; importance of definition; description of the first diagnostic criterion and subsequently used definitions, such as acute lung injury score; definition by the American-European Consensus Conference, and its limitations; description of the definition by Delphi, and its problems; accuracy of the aforementioned definitions; description of most recent definition (the Berlin definition), and its limitations; and practical importance of the new definition. CONCLUSIONS: ARDS is a serious disease that remains an ongoing diagnostic and therapeutic challenge. The evolution of definitions used to describe the disease shows that studies are needed to validate the current definition, especially in pediatrics, where the data are very scarce.

PURPOSE: Describe hearing aid use by the elderly population in the city of São Paulo and identify associated factors. METHODS: A cross-sectional, descriptive, quantitative study integrated with the SABE (health, well-being and aging) project developed in 2006. A total of 1.115 individuals aged 65 or over were interviewed. Sample selection occurred in two stages, with replacement and probabilities proportional to the population to complement those aged 75 or over. Structured questionnaires and validated instruments were used. The data were weighted, the Rao-Scott test was used for univariate analysis and backward stepwise logistic regression was used for multivariate analysis, performed on Stata 10® software. RESULTS: Three hundred and seventy-seven subjects (30.4%) were classified as hearing impaired and 10.1% of these reported using hearing aids. To acquire the devices, 78.8% used their own resources and 16.9% acquired them through the Brazilian public health system (SUS). Among non-users of hearing aids, 16.6% reported prior indication; however, 8.6% were unable to adapt to the device and 8.0% could not afford to acquire one. Hearing aid use was associated with lower prevalence of probable dementia. CONCLUSION: The low number of hearing aid users indicates the difficulties elderly people face in acquiring them and/or that the health services face in effectively helping them to adapt. These findings may influence the quality of life of elderly with hearing impairment, given the association with probable dementia revealed by this study.

This study compared the effect of the treatment protocol for correcting Class Il malocclusion using the Herbst appliance followed by full fixed Straigh-wire mechanics, in two populations, one Brazilian and one of North American origin As a untreated control sample the data from the University of Michigan Elementary and Secondary School Growth Study (UMGS) was used. Our sample was composed of 12 males and 12 females, with initial mean age of 12 years 7 months and final mean ages of 15 years and 3 months. The Michigan patients comprised 21 females and 7 males, with an initial mean age of 11 years and 9 months and final mean age of 14 years and 4 months. The control sample was paired in number, sex and age to the treated Michigan sample. ln both treated groups, lateral cephalometric radiographs were obtained before the Herbst appliance was cemented and at the end of the fixed appliance phase, The first comparison involved the Brazil group and the untreated controls, which demonstrated that the association of the Herbst appliance followed by fixed Straight-wire appliances provoked positive effects on the dentofacial complex, improving pre-existing maxillo-mandibular relationships, besides increasing dentoalveolar compensations which contributed to correct the malocclusion. The second comparison, involved the Brazl1ian and North-American patients treated with the same protocol. Although the pre-treatment comparison showed that the two groups were not similar in all aspects, they presented almost identical therapeutic modifications, which indicate that the effect of' treatment was very similar. These results point out that, in Class ll treatment, the combination of Herbst/Straight-wire mechanics produce consistent and systematic effects, correcting or minimizing possible skeletal imbalances

After the isolation of Helicobacter pylori from an injury at the stomach mucosa by Marshall and Wareen, work that was recognized with the Nobel Prize of Medicine or Physiology in 2005, many other works showed the relationship between the presence of H. pylori and diseases at the digestive system, such as gastritis, gastric, duodenal and peptic ulcer, and stomach cancer. The 13C-Urea Breath Test - 13C-UBT is a non-invasive diagnostic method that utilizes the breath of a patient to determine the presence of H. pylori through stable isotopes. This work aimed to find an ideal 13C-UBT Isotopic Ratio Mass Spectroscopy cut-off value (a threshold between positive and negative) to diagnose H. pylori infection at Brazilian population. Patients were selected at the UNESP-Botucatu Clinical Hospital Endoscopy Section. With these results it was possible to indicate that the best cut-off value is between 2.5 to 6 ‰ of Delta Over Baseline (DOB)

O núcleo parabraquial lateral (NPBL) e o núcleo Kölliker-Fuse (KF) são os mais importantes núcleos da ponte envolvidos com o controle da ventilação pulmonar (VE) e são conhecidos como grupo respiratório pontino ou centro pneumotáxico. Vários experimentos demonstraram que a estimulação ou a lesão do NPBL-KF produziram alterações nos padrões respiratórios. No entanto, ainda não estava claro qual seria a área exata (no complexo NPBL-KF) e o neurotransmissor envolvido nas alterações respiratórias. Estudos com imunohistoquímica demonstraram a presença de receptores purinérgicos (especialmente os P2X) em várias áreas envolvidas com o controle da ventilação, incluindo o NPBL. Estudos também demonstraram a presença de um denso plexo de varicosidades imunorreativas para o GABA ao longo do complexo NPB-KF, sendo que o processamento neural nessa região estaria sob forte inibição gabaérgica. No entanto, o papel dos receptores purinérgicos e gabaérgicos do NPBL na regulação da VE em ratos não anestesiados ainda não tinha sido investigado. Desta forma, no presente estudo investigamos as respostas ventilatórias após a injeção do α,β-metil-ATP (agonista purinérgico), do PPADS (antagonista purinérgico) e do muscimol (agonista GABA-A) no NPBL de ratos não anestesiados. Foram utilizados ratos com cânulas de aço inoxidável bilateralmente no NPBL, os animais foram submetidos a pletismografia de corpo inteiro para que fossem obtidas as medidas de VE. As injeções bilaterais do agonista purinérgico, α,β-metil-ATP (2 nmol/0,2 μl, n=8), no NPBL promoveu queda na freqüência respiratória (fR) (108  5 ciclos/min vs basal 137  6 ciclos/min, p = 0,005), não alterou o volume corrente (VT) (2  0,3 ml/kg vs basal 2  0,3 ml/kg, p = 0,967) e queda na VE (263  42 ml.kg-1.min-1 vs basal 325  43 ml.kg-1.min-1, p = 0,001)... The study and selection of an equipment of air conditioner for any type of environment, should be made without overestimate or undersizing the project. The undersizing does not provide an adequate comfort temperature if the environment is too hot because the air conditioner does not remove all the heat necessary in the environment. But if the project is oversized, energy consumption is higher and there is an unnecessary cost. To prevent these failures is necessary to make a calculation of the thermal load on the environment and choose the equipment that has a higher cooling capacity than the calculated heat load and closer to the calculated heat load. In this graduate work will be chosen an air-conditioned equipment for bus, showing the calculations made for the thermal load for various types of heat gain in this type of vehicle. The thermal load on vehicles is more complicated to calculate than in areas because there are several factors that vary with the movement of the vehicle. It will also explain the compression refrigeration cycle, which is the cooling system used in vehicles because it is weightless, compact and lower cost. From the calculated heat load, it will be chosen an air conditioner that best suits the project and, finally, a brief presentation of the selected equipment will be made

Segundo a Organização Mundial da Saúde, em países de baixa renda as infecções do trato respiratório ocupam o primeiro lugar dentre as causas de morte, sendo responsável por 11,2% das mesmas. Dentre estas, a causada por Streptococcus pneumoniae é uma das responsáveis pelo maior número de mortes em crianças ou adultos acometidos por doenças pulmonares crônicas. No pulmão os macrófagos alveolares (MAs) através de receptores expressos na superfície celular, como receptores “scavenger”, possuem um papel crítico na fagocitose e atividade microbicida de diferentes bactérias Gram-positivas. Em indivíduos acometidos por doenças pulmonares crônicas há um grande acúmulo de células apoptóticas (ACs) e estes podem ser mais susceptíveis a infecções pulmonares bacterianas como por S. pneumoniae. A presença destas ACs poderia contribuir para o aumento desta susceptibilidade através da supressão da resposta imune. Portanto, neste trabalho avaliamos se o efeito supressor mediado por ACs pode interferir na modulação das atividades microbicidas de MAs contra S. pneumoniae via receptores “scavenger”. A partir dos resultados encontrados, pode-se sugerir que 1) a eferocitose por MAs possui efeito imunossupressor na atividade microbicida contra S. pneumoniae; 2) a atividade supressora mediada pela fagocitose de ACs por MAs foi inibida na presença de bloqueadores de SR-A, sugerindo que os SR-B I/II são menos sensíveis a efeito supressor decorrente da eferocitose; 3) a PGE2, oriunda da eferocitose, liga-se ao receptor EP2 e suprime a atividade microbicida de MAs; 4) as produções de IL-1α, TNF-α e NO estão aumentadas na presença de ACs, sugerindo que a eferocitose por MAs promove um descontrole na ativação de MAs mediante a infecção por S. pneumoniae

Along with the advance of technology, in terms of the expansion of medical exams that uses the ionizing radiation for diagnosis, there is also the concern about quality control for maintaining quality in radiographic imaging and for delivering low dose to the patient. Based on the Federal Order 453 of the Secretariat of Health Surveillance, which takes account of the practical and justification of individual medical exposures, the optimization of radiological protection, limitation of individual dose, and the prevention of accidents, were done through this paper radiodiagnostic tests on medical equipment in order to accept it or not, according to SVS-453. Along with the help and support of P&R Consulting and Medical Physics Marilia, SP, were made Quality Control and Radiometric Control in equipment from various cities across the state of São Paulo. The equipment discussed in this work is classified as conventional X-ray. According to the Federal Order SVS-453, the quality control in the program of quality assurance should include the following minimum set of constancy tests, with following minimum frequency: biennial tests for representative values of dose given to the patients of radiography and CT performed in the service; annual tests for accuracy of the indicator tube voltage (kVp), accuracy of exposure time, half-value layer, aligning the central axis of the beam of x-ray tube, performance (mGy / mA.min.m²), linearity of the rate of kerma on air with the mAs, reproducibility of the kerma on air rates, reproducibility of the automatic exposure, focal spot size, integrity of accessories and clothing for individual protection; semiannually for collimation system accuracy; weekly for temperature processing system and sensitometry processing system.
