RESUMO - Face ao interesse crescente sobre os efeitos na saúde resultantes das alterações climáticas em curso, particularmente no que concerne aos fenómenos meteorológicos extremos, o presente trabalho visa sistematizar os conhecimentos científicos baseados na evidência já existentes, relativamente a esses efeitos na saúde, actuais e projectados, particularizando os desafios que tal situação apresenta para os serviços de saúde da Região de Lisboa e Vale do Tejo. São analisados os principais aspectos relativos aos potenciais efeitos das alterações climáticas na saúde dos indivíduos e das populações da Região, identificados os grupos populacionais mais em risco e realçados os aspectos que podem potenciar possíveis desigualdades em saúde. Termina-se com um enquadramento de orientações práticas sobre acções específicas que podem ser tomadas agora, a diferentes níveis, para preparar os serviços de saúde da Região em ordem à protecção dos cidadãos dos efeitos na saúde resultantes das alterações climáticas, sabendo-se que os efeitos adversos são largamente preveníveis. O presente trabalho deve suscitar uma reflexão sobre o impacto das alterações climáticas na saúde da população da Região, com as consequências que daí podem advir para os serviços de saúde, designadamente em termos do aumento da afluência dos cidadãos que, sendo mais vulneráveis, poderão adoecer e/ou ver agravadas situações de saúde pré- -existentes no decurso de fenómenos meteorológicos extremos súbitos.--------------------------ABSTRACT – Given the growing interest in the health effects from climate change, namely in what extreme weather events are concerned, this article aims to systematize the already existing evidencebased scientific knowledge concerning those actual and forecasted effects. This health services challenge is focused on the Region of Lisboa e Vale do Tejo. The main issues concerning the potential effects on individuals and populations of the Region are analyzed, the most at risk population groups are identified and the issues leading to health inequalities are highlighted. A framework about practical guidelines concerning specific actions to be taken at different levels is presented in order to prepare Region health services to protect citizens from health effects of climate changes, as it is known that adverse health effects are largely preventable. A terapêutica antidislipidémica redutora do colesterol é um paradigma da melhor aplicação da evidência científica na prática clínica. Face às opções disponíveis, é fundamental que as estatinas tenham um perfil claro de segurança e tolerabilidade e uma relação de benefícios e risco favorável. A terapêutica intensiva do colesterol não está associada a consequências deletérias, dependentes da maior eficácia, ou a efeitos laterais graves. As estatinas não são hepatotóxicas. A flutuação enzimática é um fenómeno corrente na dislipidemia. O risco de aumento das transamínases está directa e intimamente relacionada com a dose (e com o tipo de estatina usada). Alterações pouco importantes do perfil hepático basal não são uma contra-indicação ao seu uso (doentes com um risco cardiovascular justificável). A monitorização iterada do perfil hepático não está justificada. A causa da miotoxicidade com as estatinas não está devidamente esclarecida. O risco de miopatia ou rabdomiólise não tem relação com a redução percentual ou absoluta do LDL-C (nem com o valor de LDL-C alcançado). Os efeitos adversos das estatinas podem depender das características físico-químicas da molécula e das suas características farmacocinéticas. Numa estratégia de farmacovigilância, o doente tem também uma palavra. A partilha de propósitos obriga o doente a responsabilidades partilhadas com a sua Equipa de Saúde, motivadoras de tratamentos mais seguros e de melhor prevenção cardiovascular. Aperfeiçoar o ingresso em programas de Saúde de qualidade e aprimorar o tratamento e os objectivos alcançados são as razões que devem fundamentar a terapêutica e a redução efectiva intensiva das dislipidemias.

Acute generalized exanthematous pustulosis (AGEP) usually presents with the acute appearance of oedematous and erythematous lesions, on which multiple sterile pustules appear, associated with fever. Almost 90% of cases are associated with drugs, with antibiotics (penicillins and macrolides) being the most frequent causative agents. We describe a 36-year-old female patient, which started diltiazem 120mg/day for hypertension. After 6 days of therapy, multiple erythematous and oedematous lesions appeared, with associated multiple small non-follicular pustules. Oral corticosteroids were started, with progressive and complete improvement. Patch-tests were performed, which revealed positivity for diltiazem. Although a rare entity, AGEP must be considered in cases of acute eruptions with disseminated pustules and fever. The use of patch tests in this disease may be useful as positive reactions are frequent.
Anemia Megaloblástica Aguda por Inalação de Óxido Nitroso em Doente com Patologia Autoimune Múltipla

As anemias megaloblásticas são habitualmente vistas como doenças crónicas de aparecimento insidioso. No entanto, um estado megaloblástico pode aparecer em apenas alguns dias devido à deficiência aguda de ácido fólico ou vitamina B12. Uma das causas mais comuns, embora pouco relatada, é acção tissular do óxido nitroso (NO2), substancia volátil utilizada comummente em anestesia. Efectivamente, o N02 inactiva a metilcobalamina, levando ao rápido desenvolvimento de uma hematopoiese megaloblástica. Este fenómeno pode ocorrer mesmo em doentes sem défice prévio de vitamina B12, sendo no entanto frequente e mais grave quando há um estado carencial de base. Apresentamos o caso clínico de uma doente com fractura do colo do fémur, que desenvolveu anemia aguda no pós-operatório cuja investigação permitiu identificar uma anemia pemiciosa latente e patologia tiroideia autoimune em doente com vitiligo desde longa data.

Os autores descrevem um caso de síndrome de Sheehan surgido na sequência de complicação hemorrágica grave por interrupção voluntária tardia de gravidez sublinhando a infrequência de alterações lipídicas no contexto desta afecção. A pretexto deste exemplo particular de hipopituitarismo sublinham conceitos fundamentais sobre a fisiopatogenia, diagnóstico, terapêutica e complicações decorrentes de uma entidade nosológica cada vez mais rara nos tempos actuais.

RESUMO: Relevância e Objectivos: O objectivo deste estudo consistiu em verificar a eficácia e segurança de uma técnica de higiene brônquica – Active Cycle breathing technique (ACBT), na remoção de secreções e desinsuflação em crianças asmáticas. É uma técnica recente, que ainda não foi estudada nesta população. Metodologia: A amostra foi constituída por um total de 14 crianças, com idades compreendidas entre os 6 e 18 anos, com o diagnóstico de asma. Foi dividida em dois grupos: grupo de controlo, constituído por asmáticos estáveis e grupo experimental constituído por asmáticos pós-crise. Ambos receberam o mesmo tratamento, que consistiu numa única sessão de ACBT. Realizaram um exame da função respiratória (variáveis FEV1, PEF, MEF75%, RV e FRC) antes e após a aplicação da técnica. Mediu-se a Saturação de O2 antes, a meio e no fim da aplicação da técnica. Por último, pesaram-se as secreções recolhidas durante a sessão de tratamento. Resultados: Como resultados, quando omparamos o antes e após aplicação do ACBT, obtivemos diferenças significativas no RV (p <0,05) e FRC (p <0,05); muito significativa na Sa02 (p <0,01) e peso de secreções, nos asmáticos pós crise. Ocorreu uma melhoria significativa da saturação (p <0,05) em asmáticos estáveis, não havendo secreções. Não houve efeitos adversos nem broncoespasmo em ambos os grupos.Conclusão: O ACBT é uma técnica que promove a remoção de secreções e diminuição da insuflação na população de asmáticos.--------------------ABSTRACT:Relevance and Goals: the goal of this study was to verify the efficiency and security of a bronchial hygienic technique - Active Cycle Breathing Technique (ACBT) – in the obstruction and insufflation of asthmatic children. This is a new technique that has not yet been studied in this population. Procedures: The subjects were 14 children aged between 6 and 18 years old with asthma diagnosis. They were divided in two groups: the control group with stable asthmatics and the experimental group with post-crisis asthmatics. Both received the same treatment that was a single session of ACBT. They made a lung function test before and after the application of the technique (variables FEV1, PEF, MEF75%, RV and FRC). The level of O2 saturation was measured before, during and after the procedure. In the end, the secretions collected during the treatment were weighted. Results: We observed a significant difference on RV (p ˂0,05), FRC (p ˂0,05), Sa02 (p ˂0,01) and in the weight of the secretions in the post-crisis asthmatics after ACBT. There was a significant improvement of saturation (p ˂0,05) only in the stable asthmatics. There were no adverse effects or bronchospasm in both groups.

Spinal arachnoiditis, an inflammatory process involving all three meningeal layers as well as the nerve roots, is a cause of persistent symptoms in 6% to 16% of postoperative patients. Although spinal surgery is the most common antecedent associated with arachnoiditis, multiple causes have been reported, including infection, intrathecal steroids or anesthetic agents, trauma, subarachnoid hemorrhage and ionic myelographic contrast material--both oil soluble and water soluble. In the past, oil-based intrathecal contrast agents (Pantopaque) were associated with arachnoiditis especially when this material was introduced into the thecal sac and mixed with blood. Arachnoiditis is apparently rarely idiopathic. The pathogenesis of spinal arachnoiditis is similar to the repair process of serous membranes, such as the peritoneum, with a negligible inflammatory cellular exudate and a prominent fibrinous exudate. Chronic adhesive arachnoiditis of the lower spine is a myelographic diagnosis. The myelographic findings of arachnoiditis were divided into two types by Jorgensen et al. In type 1, "the empty thecal sac" appearance, there is homogeneous filling of the thecal sac with either absence of or defects involving nerve root sleeve filling. In type 2 arachnoiditis, there are localized or diffuse filling defects within the contrast column. MRI has demonstrated a sensitivity of 92% and a specificity of 100% in the diagnosis of arachnoiditis. The appearance of arachnoiditis on MRI can be assigned to three main groups. The MRI findings in group I are a conglomeration of adherent roots positioned centrally in the thecal sac. Patients in group II show roots peripherally adherent to the meninges--the so called empty sac. MRI findings in group III are a soft tissue mass within the subarachnoid space. It corresponds to the type 2 categorization defined by Jorgensen et al, where as the MRI imaging types I and II correspond to the myelographic type 1.

BACKGROUND: Surgery for congenital heart disease (CHD) has changed considerably during the last three decades. The results of primary repair have steadily improved, to allow treating almost all patients within the pediatric age; nonetheless an increasing population of adult patients requires surgical treatment. The objective of this study is to present the early surgical results of patients who require surgery for CHD in the adult population within a multicentered European study population. METHODS: Data relative to the hospital course of 2,012 adult patients (age > or = 18 years) who required surgical treatment for CHD from January 1, 1997 through December 31, 2004 were reviewed. Nineteen cardiothoracic centers from 13 European countries contributed to the data collection. RESULTS: Mean age at surgery was 34.4 +/- 14.53 years. Most of the operations were corrective procedures (1,509 patients, 75%), followed by reoperations (464 patients, 23.1%) and palliative procedures (39 patients, 1.9%). Six hundred forty-nine patients (32.2%) required surgical closure of an isolated ostium secundum atrial septal defect. Overall hospital mortality was 2%. Preoperative cyanosis, arrhythmias, and NYHA class III-IV, proved significant risk factors for hospital mortality. Follow-up data were available in 1,342 of 1,972 patients (68%) who were discharged home. Late deaths occurred in 6 patients (0.5%). Overall survival probability was 97% at 60 months, which is higher for corrective procedures (98.2%) if compared with reoperations (94.1%) and palliations (86.1%). CONCLUSIONS: Surgical treatment of CHD in adult patients, in specialized cardiac units, proved quite safe, beneficial, and low-risk.

INTRODUCTION: Invasive fungal infections (IFIs) are a life-threatening complication in patients with hematologic malignancies, mainly in acute leukemia patients, following chemotherapy. IFI incidence is increasing, and associated mortality remains high due to unreliable diagnosis. Antifungal drugs are often limited by inadequate antimicrobial spectrum and side effects. Thus, the detection of circulating fungal DNA has been advocated as a rapid, more sensitive diagnostic tool. PATIENTS AND METHODS: Between June 01 and January 03, weekly blood samples (1,311) were screened from 193 patients undergoing intensive myelosuppressive or immunosuppressive therapy. IFI cases were classified according to European Organization for Research and Treatment of Cancer/Mycoses Study Group criteria. Fungal DNA was extracted from whole blood and amplified using polymerase chain reaction (PCR) published primers that bind to the conserved regions of the fungal 18S rRNA gene sequence. In our study, two or more consecutive positive samples were always associated with fungal disease. RESULTS: PCR screening predicted the development of IFI to be 17 days (median). This test had a specificity of 91.1% and a sensitivity of 75%. IFI incidence was 7.8%. DISCUSSION: Therefore, our results confirm the potential usefulness of PCR serial screening and the clinical applicability in everyday routine. PCR screening offers a noninvasive repeatable aid to the diagnosis of IFI.

The autonomic nervous system (ANS) plays a role as a modulator in the pathogenesis of paroxysmal atrial fibrillation (PAF). The clinical pattern of vagally mediated PAF has been observed mainly in young patients. Neurocardiogenic responses during orthostatic stress are related to autonomic reflexes in which the vagal influence predominates. AIM: To evaluate the susceptibility of elderly patients with PAF to activation of vasovagal syncope mechanisms. METHODS: We performed passive head-up tilt testing (HUT) in 34 patients (62% women, aged 72 +/- 7 years), with > or = 1 year of clinical history of PAF--19 without structural heart disease, 11 with hypertensive heart disease and 4 with coronary artery disease (who had no previous myocardial infarction, had undergone myocardial revascularization, and had no documented ischemia) (PAF group), and compared the results with those obtained in a group of 34 age-matched patients (53% women, aged 74 +/- 6 years), who underwent HUT due to recurrent syncope (Sc group). In this group, 21 had no documented heart disease and none had a clinical history of AF. There was no diabetes, congestive heart failure or syncope in the PAF group. After a supine resting period, the subjects were tilted at 70 degrees for 20 minutes while in sinus rhythm. No provocative agents were used to complement the HUT. ECG and blood pressure were continuously monitored (Task Force Monitor, CNSystems). The test was considered positive when syncope or presyncope occurred with bradycardia and/or arterial hypotension. Abnormal responses were classified as cardioinhibitory, vasodepressor or mixed. RESULTS: HUT was positive in seven patients of the PAF group--vasodepressor response in five and mixed in two (20.5% of the total; 26.3% of those without heart disease)--and in eight patients (vasodepressor in six and mixed in two) of the Sc group (p=NS). During HUT, three patients of the PAF group had short periods of self-limited PAF (in one, after vasodepressor syncope). There were no differences in gender distribution, age or heart disease. No cardioinhibitory responses or orthostatic hypotension were observed. CONCLUSION: In elderly patients with PAF, a significant number of false positive results during passive HUT may be expected, suggesting increased vasovagal reactions despite aging. This suggests that ANS imbalances may be observed in this population.
