Background: Political violence and war are push factors for migration and social determinants of health among migrants. Somali migration to Sweden has increased threefold since 2004, and now comprises refugees with more than 20 years of war experiences. Health is influenced by earlier life experiences with adverse sexual and reproductive health, violence, and mental distress being linked. Adverse pregnancy outcomes are reported among Somali born refugees in high-income countries. The aim of this study was to explore experiences and perceptions on war, violence, and reproductive health before migration among Somali born women in Sweden. Method: Qualitative semi-structured individual interviews were conducted with 17 Somali born refugee women of fertile age living in Sweden. Thematic analysis was applied. Results: Before migration, widespread war-related violence in the community had created fear, separation, and interruption in daily life in Somalia, and power based restrictions limited access to reproductive health services. The lack of justice and support for women exposed to non-partner sexual violence or intimate partner violence reinforced the risk of shame, stigmatization, and silence. Social networks, stoicism, and faith constituted survival strategies in the context of war. Conclusions: Several factors reinforced non-disclosure of violence exposure among the Somali born women before migration. Therefore, violence-related illness might be overlooked in the health care system. Survival strategies shaped by war contain resources for resilience and enhancement of well-being and sexual and reproductive health and rights in receiving countries after migration.

Bakgrund Cancer är en sjukdom som anses komma att öka med 70 % de kommande 20 åren. Cancer kan orsaka livsförändringar och den drabbade kan bli beroende av hjälp från närstående Syfte Att genom en vetenskaplig litteraturöversikt beskriva närståendes upplevelser när en anhörig är cancersjuk och i samband med mötet med sjukvården. Metod En litteraturöversikt som innefattar 13 vetenskapliga artiklar som har framtagits med hjälp av sökord relaterade till syftet från databaserna CINAHL och PubMed. Resultat Två teman uppstod: förändrad livssituation och delaktighet med sjukvården. Den närstående kan uppleva att livssituationen förändras när en anhörig är cancersjuk. En skyldighet upplevs till att vara en vårdare. Med vårdandet uppstår till största del negativa känslor. Okunskap och svårighet upplevs i vårdandet. De flesta närstående upplever att stöttning av sjukvården är nödvändigt men bristande. Slutsats Genom sjukvårdens hjälp kan närståendes erfarenheter i vårdandet av sin cancersjuka anhörig förbättras och därmed minska de negativa upplevelserna. Ytterligare forskning krävs inom området för att en större överblick över hur problemet ska identifieras.

In this qualitative user evaluation participants in the project refugee guide, Falun, was interviewed about their experience of the project and how the project contributed to their integration into society and establishment on the labor market. The collection of empirical data has been made by six interviews. The results of the evaluation indicate that participants feel that they through the project had the opportunity to meet new people, practice the language and learn about each other's culture. The results also show that participants are satisfied with the project. Some participants expressed a desire to meet with several people at once while others don´t have that need. Furthermore, the interviews shows that establishment on labor market occurred through job placement and not through participation in the project. Integration proved to be a difficult concept to define, but the interviews show that the participants have a perception that it is about learning the Swedish language and culture.

The present study's purpose was to investigate the home care staff's experiences of working as key workers, which factors they consider is relevant to key working and what obstacles and opportunities they experience in their work as a key worker. The study also focused on how the home care staff perceive that key working can enable user influence. A qualitative approach was used and the material for the study was collected through semi-structured interviews. The result shows that there are areas for improvement that may develop key working. The study shows that the staff generally feel that working as a key worker works well and that it can enable user influence in many areas. The study finally illustrates factors such as continuity, working relationship oriented, and taking responsibility, as significant for key working to work as successfully as possible.

Victims of domestic violence (DV) are growing, still the number of cases reported isn't. In Rättvik only half the amount of cases are reported compared to two years ago. Rättvik is working to reduce DV by starting the project "Våga Hjälpa!". The purpose of this study was to use a quantitative approach to examine if the project is known in Rättvik. The study's based on a survey with a 27% response rate. The analysis was made by SPSS and interpreted on the basis of conformity, the bystander effect and the Theory of Planned Behaviour. Results show that the majority haven't been in contact with DV and that the knowledge of how to act is evenly spread. The study concludes with an objective evalution of "Våga Hjälpa!" where our study results are related to the project objectives. The evaluation partly shows a fullfilled result but also some improvement opportunities.

Syfte: Syftet med studien är att beskriva distriktssköterskans erfarenhet av hälsosamtal avseende levnadsvanor med patienter som lever med övervikt eller fetma. Metod: En empirisk studie med kvalitativ ansats utfördes via enkätfrågor som var beskrivande med öppna svarsalternativ. I studien deltog fem distriktssköterskor som arbetade på vårdcentral i Dalarna eller på Gotland. Genom fenomenografisk design analyserades data. Resultat: Resultatet läggs fram genom två beskrivningskategorier; Evidensbaserade metoder som innehöll uppfattningsgrupperna; Utredning och behandling samt Bejaka patientens resurser. Beskrivningskategorin Omgivnings faktorer, hade uppfattningsgrupperna; Frekvensen av hälsosamtal samt Pedagogiskt förhållningssätt. Användning av evidensbaserade verktyg såsom midjemått och midja/höftkvot var låg bland distriktssköterskorna, men body mass index (BMI) samt motiverande samtal (MI) användes flitigt. Patienter söker ofta för andra besvär (högt blodtryck, ledbesvär) som kan återkopplas till kroppsvikten. Distriktssköterskorna erfarenhet var att det är svårt att komma tillrätta med viktproblem där motivation till viktnedgång var stark i början hos patienten men att den avtar. Dessa svårigheter kan bero på depression, skuld och skam hos patienterna. Slutsats: För att patienter med övervikt och fetma ska få den ultimata behandlingen krävs mer tid och resurser från hälso- och sjukvården då det kan vara en lång process att gå ner i vikt. Motivationen har en avgörande roll samt användning av evidensbaserade verktyg.

Background Young people are at an increased risk for illness in working life. The authorities stipulate certain goals for training in occupational health and safety (OHS) in vocational schools. A previous study concluded that pupils in vocational education had limited knowledge in the prevention of health risks at work. The aim of the current study, therefore, was to study how OHS training is organized in school and in workplace-based learning (WPL).   Method The study design featured a qualitative approach, which included interviews with 12 headmasters, 20 teachers, and 20 supervisors at companies in which the pupils had their WPL. The study was conducted at 10 upper secondary schools, located in Central Sweden, that were graduating pupils in four vocational programs.   Result The interviews with headmasters, teachers, and supervisors indicate a staggered picture of how pupils are prepared for safe work. The headmasters generally give teachers the responsibility for how goals should be reached. Teaching is very much based on risk factors that are present in the workshops and on teachers’ own experiences and knowledge. The teaching during WPL also lacks the systematic training in OHS as well as in the traditional classroom environment.   Conclusion Teachers and supervisors did not plan the training in OHS in accordance with the provisions of systematic work environment management. Instead, the teachers based the training on their own experiences. Most of the supervisors did not get information from the schools as to what should be included when introducing OHS issues in WPL.

Utvecklingen och framväxten av det sk informationssamhället har medfört att det svenska högskolesystemet har byggts ut. Ny grupper av studenter söker sig till högskolan. Från drygt 40.000 studenter i mitten av 60-talet finns idag över 400.000 studenter vid våra högskolor och universitet. Ett vidgat tillträde till högre utbildning ställer flera nya och utmanande frågor. Har så många människor motiv och förmåga att ta sig an högskolestudier? Är utbygnaden motiverad och löner den sig? Hur skall gymnasieskola och vuxenutbildning förbereda de kommande studenterna på ett bra sätt? Behöver så många utbilda sig, eller med andra ord, passar alla dessa nya studenter för högskolans krav? Är högskolan verkligen intresserad av att anpassa sig och ge dessa nya grupper tillträde till den högre utbildningen? Dessa och andra frågor, som utgår från den nya situation som högskolan befinner sig i, diskuterade och bearbetade en grupp lärare från Högskolan Dalarna och fem av Dalarnas folkhögskolor i en forskningscirkel. Syftet med detta paper är att sammanfatta erfarenheterna från forskningscirkeln.

Negative outcomes of a poor work environment are more frequent among young workers. The aim of the current study was to study former pupils’ conditions concerning occupational health and safety by investigating the workplaces’, safety climate, the degree of implementation of SWEM and the their introduction programs. Four branches were included in the study: Industrial, Restaurant, Transport and Handicraft, specialising in wood. Semi-structured dialogues were undertaken with 15 employers at companies in which former pupils were employed. They also answered a questionnaire about SWEM. Former pupils and experienced employees were upon the same occasion asked to fill in a questionnaire about safety climate at the workplace. Workplace introduction programs varied and were strongly linked to company size. Most of the former pupils and experienced employees rated the safety climate at their company as high, or good. Employers in three of the branches rated the SWEM implemented at their workplaces to be effective. The Industry companies, which had the largest workplaces, gave the most systematic and workplace introduction for new employees. There are no results from this study explaining the fact that young workers have a higher risk for workplace accidents.

Background Somali-born women constitute one of the largest groups of childbearing refugee women in Sweden after more than two decades of political violence in Somalia. In Sweden, these women encounter antenatal care that includes routine questions about violence being asked. The aim of the study was to explore how Somali-born women understand and relate to violence and wellbeing during their migration transition and their views on being approached with questions about violence in Swedish antenatal care. Method Qualitative interviews (22) with Somali-born women (17) living in Sweden were conducted and analysed using thematic analysis. Findings A balancing actbetween keeping private life private and the new welfare system was identified, where the midwife's questions about violence were met with hesitance. The midwife was, however, considered a resource for access to support services in the new society. A focus on pragmatic strategies to move on in life, rather than dwelling on potential experiences of violence and related traumas, was prominent. Social networks, spiritual faith and motherhood were crucial for regaining coherence in the aftermath of war. Dialogue and mutual adjustments were identified as strategies used to overcome power tensions in intimate relationships undergoing transition. Conclusions If confidentiality and links between violence and health are explained and clarified during the care encounter, screening for violence can be more beneficial in relation to Somali-born women. The focus on “moving on” and rationality indicates strength and access to alternative resources, but needs to be balanced against risks for hidden needs in care encounters. A care environment with continuity of care and trustful relationships enhances possibilities for the midwife to balance these dual perspectives and identify potential needs. Collaborations between Somali communities, maternity care and social service providers can contribute with support to families in transition and bridge gaps to formal social and care services.

BACKGROUND: Even though Swedish national guidelines for stroke care (SNGSC) have been accessible for nearly a decade access to stroke rehabilitation in out-patient health care vary considerably. In order to aid future interventions studies for implementation of SNGSC, this study assessed the feasibility and acceptability of study procedures including analysis of the context in out-patient health care settings. METHODS: The feasibility and acceptability of recruitment, observations and interviews with managers, staff and patients were assessed, as well as the feasibility of surveying health care records. RESULTS: To identify patients from the the hospitals was feasible but not from out-patient care where a need to relieve clinical staff of the recruitment process was identified. Assessing adherence to guidelines and standardized evaluations of patient outcomes through health care records was found to be feasible and suitable assessment tools to evaluate patient outcome were identified. Interviews were found to be a feasible and acceptable tool to survey the context of the health care setting. CONCLUSION: In this feasibility study a variety of qualitative and quantitative data collection procedures and measures were tested. The results indicate what can be used as a set of feasible and acceptable data collection procedures and suitable measures for studying implementation of stroke guidelines in an out-patient health care context.

Background: Violence against women is associated with serious health problems, including adverse maternal and child health. Antenatal care (ANC) midwives are increasingly expected to implement the routine of identifying exposure to violence. An increase of Somali born refugee women in Sweden, their reported adverse childbearing health and possible links to violence pose a challenge to the Swedish maternity health care system. Thus, the aim was to explore ways ANC midwives in Sweden work with Somali born women and the questions of exposure to violence. Methods: Qualitative individual interviews with 17 midwives working with Somali-born women in nine ANC clinics in Sweden were analyzed using thematic analysis. Results: The midwives strived to focus on the individual woman beyond ethnicity and cultural differences. In relation to the Somali born women, they navigated between different definitions of violence, ways of handling adversities in life and social contexts, guided by experience based knowledge and collegial support. Seldom was ongoing violence encountered. The Somali-born women’s’ strengths and contentment were highlighted, however, language skills were considered central for a Somali-born woman’s access to rights and support in the Swedish society. Shared language, trustful relationships, patience, and networking were important aspects in the work with violence among Somali-born women. Conclusion: Focus on the individual woman and skills in inter-cultural communication increases possibilities of overcoming social distances. This enhances midwives’ ability to identify Somali born woman’s resources and needs regarding violence disclosure and support. Although routine use of professional interpretation is implemented, it might not fully provide nuances and social safety needed for violence disclosure. Thus, patience and trusting relationships are fundamental in work with violence among Somali born women. In collaboration with social networks and other health care and social work professions, the midwife can be a bridge and contribute to increased awareness of rights and support for Somali-born women in a new society.

A tool for standardized calculation of solar collector performance has been developed in cooperation between SP Technical Research Institute of Sweden, DTU Denmark and SERC Dalarna University. The tool is designed to calculate the annual performance of solar collectors at representative locations in Europe. The collector parameters used as input in the tool are compiled from tests according to EN12975, without any intermediate conversions. The main target group for this tool is test institutes and certification bodies that are intended to use it for conversion of collector model parameters (derived from performance tests) into a more user friendly quantity: the annual energy output. The energy output presented in the tool is expressed as kWh per collector module. A simplified treatment of performance for PVT collectors is added based on the assumption that the thermal part of the PVT collector can be tested and modeled as a thermal collector, when the PV electric part is active with an MPP tracker in operation. The thermal collector parameters from this operation mode are used for the PVT calculations. © 2012 The Authors.

A new managerial task arises in today’s working life: to provide conditions for and influence interaction between actors and thus to enable the emergence of organizing structure in tune with a changing environment. We call this the enabling managerial task. The goal of this paper is to study whether training first line managers in the enabling managerial task could lead to changes in the work for the subordinates. This paper presents results from questionnaires answered by the subordinates of the managers before and after the training. The training was organized as a learning network and consisted of eight workshops carried out over a period of one year (September 2009–June 2010), where the managers met with each other and the researchers once a month. Each workshop consisted of three parts, during three and a half hours. The first hour was devoted to joint reflection on a task that had been undertaken since the last workshop; some results were presented from the employee pre-assessments, followed by relevant theory and illuminating practices, finally the managers created new tasks for themselves to undertake during the following month. The subordinates’ answers show positive change in all of the seventeen scales used to assess it. The improvements are significant in scales measuring the relationship between the manager and the employees, as well as in those measuring interaction between employees. It is concluded that the result was a success for all managers that had the possibility of using the training in their management work.
