Objective: To evaluate contractile reserve (CR) determined by exercise echocardiography in predicting clinical outcome and left ventricular (LV) function in asymptomatic severe mitral regurgitation (MR). Design: Cohort study. Setting: Regional cardiac centre. Patients and outcome measures: LV volumes and ejection fraction (EF) were measured at rest and after stress in 71 patients with isolated MR. During follow up (mean (SD) 3 (1) years), EF and functional capacity were serially assessed and cardiac events ( cardiac death, heart failure, and new atrial fibrillation) were documented. Results: CR was present in 45 patients (CR+) and absent in 26 patients (CR-). Age, resting LV dimensions, EF, and MR severity were similar in both groups. Mitral surgery was performed in 19 of 45 (42%) CR+ patients and 22 of 26 (85%) CR2 patients. In patients undergoing surgery, CR was an independent predictor of follow up EF (p = 0.006) and postoperative LV dysfunction (EF < 50%) persisted in five patients, all in the CR2 group. Event-free survival was lower in surgically treated patients without CR (p = 0.03). In medically treated patients, follow up EF was preserved in those with intact CR but progressively deteriorated in patients without CR, in whom functional capacity also deteriorated. Conclusions: Evaluation of CR by exercise echocardiography may be useful for risk stratification and may help to optimise the timing of surgery in asymptomatic severe MR.

The socio-economic system underpinning apartheid in South Africa was based on the exploitation of black workers in the mines, the factories, the fields and the shops. It is widely recognized that the struggles of the South African black working class contributed decisively to the overthrow of the racist regime. In recognition of the power of organised labour, the democratic government elected in 1994 granted South Africa's unions unprecedented legal and constitutional rights. However, despite these gains, the country's labour movement has been facing a fresh set of challenges, from macroeconomic policy to the factory floor, many of them emanating from labour’s political allies in Government. The purpose of this book is to examine how the South African labour movement is responding to these challenges in the new millennium. A variety of experts on South African labour, both within the country and outside deal with crucial issues: How has South Africa's labour movement reacted to the ANC Government's neoliberal economic agenda? How do the unions relate to an increasingly diversifying, “flexible” and vulnerable workforce? What are labour’s prospects of contributing to a left project in democratic South Africa? What are the challenges facing the unions in relation to new forms of militancy and social movements?

Sarah Poulton Kalley é conhecida, em quase todos os segmentos do protestantismo do Brasil, devido à organização e compilação d e Salmos e Hinos, o mais antigo hinário protestante editado no vernáculo em nosso país. Seus hinos, ainda em uso em muitas igrejas, marcaram por mais de um século a teologia do protestantismo no Brasil. Apesar desta notoriedade, sua influência na gênese do protestantismo brasileiro nunca foi objeto de estudo. Assim, o objetivo desta pesquisa é resgatar e visibilizar áreas e estratégias de atuação que conferem a esta mulher um perfil de atuação relativamente autônomo. Contudo, centrada no estudo da trajetória intelectual e biográfica de um sujeito histórico, a investigação se defronta com um universo de personagens anônimos, envoltos numa complexa teia de relações, através das quais o protestantismo se insere no Brasil em um contexto especifico: huguenotes, puritanos, luddistas, famílias não-conformistas inglesas, líderes políticos e eclesiásticos, exilados madeirenses, brasileiros, portugueses, imigrantes alemães e, principalmente, a mulher protestante brasileira. A busca por informações sobre este universo relegado ao anonimato pela historiografia do protestantismo no Brasil, reve lou alguns documentos inéditos, inclusive um livro escrito por Sarah Poulton Kalley, em 1866: o A Alegria da Casa. Muito além do papel de esposa de um missionário e médico, Sarah Poulton Kalley emerge de uma rede de relações e práticas como professora, missionária e poetisa. Nestes três campos de atuação e através do desenvolvimento de múltiplos contatos e relacionamentos, procurava transformar e influenciar atitudes e crenças de seus interlocutores. Junto com a nova fé divulgava uma cosmovisão própria da cultura anglo-saxã, protestante e puritana, adaptando-a seletivamente ao universo cultural e social de seus interlocutores.(AU)

Este estudo tem por objetivo avaliar o nível de sociabilização no comportamento da criança/adolescente com deficiência intelectual em terapia assistida por animais, TAA, e por objetivos específicos levantar o perfil sócio-demográfico-clínico dessa população, observar o comportamento da criança em atendimento mediado pela TAA e verificar a opinião dos pais/responsáveis e profissionais sobre essa terapia. A TAA é uma técnica na qual o animal é parte integrante do processo terapêutico. Enquadra-se em uma abordagem multidisciplinar, que requer a intervenção de especialistas, na qual o cão ocupa uma posição mediadora entre o paciente e os objetivos terapêuticos. A pesquisa abrange 46 sujeitos, sendo 20 pacientes, 20 pais e/ou responsáveis e seis terapeutas, e desenvolve-se em uma clínica de Reabilitação Clínica de ONG em cidade de grande porte. Foram observadas 12 intervenções em TAA, aplicada a escala com foco em sociabilização baseada em Achenbach (ASEBA)e realizada entrevista junto aos pais/responsáveis e entrevistados os terapeutas em TAA. Os resultados dos instrumentos utilizados convergem no sentido de apontara validade da TAA como facilitadora da sociabilização das crianças/adolescentes com deficiência intelectual, com aumento da motivação e engajamento às intervenções, assim como, com repercussões positivas em sua autonomia, em seu humor e em sua organização cognitiva temporal e narrativa linguística. Os terapeutas ressaltam o componente lúdico presente nas intervenções, o qual facilita atingir seus objetivos terapêuticos. Já para os pais dos atendidos por essa abordagem, de forma unanime referem que filhos demonstram motivação e maior autonomia frente aos atendimentos. O estudo sugere novas investigações que possam dar suporte à divulgação dessa modalidade de terapia.

Contrast sensitivity is better with two eyes than one. The standard view is that thresholds are about 1.4 (v2) times better with two eyes, and that this arises from monocular responses that, near threshold, are proportional to the square of contrast, followed by binocular summation of the two monocular signals. However, estimates of the threshold ratio in the literature vary from about 1.2 to 1.9, and many early studies had methodological weaknesses. We collected extensive new data, and applied a general model of binocular summation to interpret the threshold ratio. We used horizontal gratings (0.25 - 4 cycles deg-1) flickering sinusoidally (1 - 16 Hz), presented to one or both eyes through frame-alternating ferroelectric goggles with negligible cross-talk, and used a 2AFC staircase method to estimate contrast thresholds and psychometric slopes. Four naive observers completed 20 000 trials each, and their mean threshold ratios were 1.63, 1.69, 1.71, 1.81 - grand mean 1.71 - well above the classical v2. Mean ratios tended to be slightly lower (~1.60) at low spatial or high temporal frequencies. We modelled contrast detection very simply by assuming a single binocular mechanism whose response is proportional to (Lm + Rm) p, followed by fixed additive noise, where L,R are contrasts in the left and right eyes, and m, p are constants. Contrast-gain-control effects were assumed to be negligible near threshold. On this model the threshold ratio is 2(?1/m), implying that m=1.3 on average, while the Weibull psychometric slope (median 3.28) equals 1.247mp, yielding p=2.0. Together, the model and data suggest that, at low contrasts across a wide spatiotemporal frequency range, monocular pathways are nearly linear in their contrast response (m close to 1), while a strongly accelerating nonlinearity (p=2, a 'soft threshold') occurs after binocular summation. The election on 6 May 2007 of Nicolas Sarkozy as President of the Republic ushered in the promise of a new era in France. Sarkozy’s presidency follows those of the Socialist François Mitterrand (1981-95) and the neo-Gaullist Jacques Chirac (1995-2007), who together occupied France’s highest political office for more than a quarter-century. From the outset, Sarkozy’s presidential campaign was predicated on the need for change in France, for a “rupture” with the past; and his emphatic victory against the Socialist Ségolène Royal gave him a mandate to effect this. The legislative elections of June 2007, by assuring a strong majority in the National Assembly for Sarkozy’s centre-right Union pour un Mouvement Populaire (UMP), left the way clear for implementing the new President’s reform agenda over the next five years. This article examines the political context within which Sarkozy was elected to power, the main proposals of his presidential program, the challenges he faces, and his prospects for bringing real change to France.

Since El Salvador’s civil war formally ended in 1992 the small Central American nation has undergone profound social changes and significant reforms. However, few changes have been as important or as devastating as the nation’s emergence as a central hub in the transnational criminal “pipeline” or series of recombinant, overlapping chains of routes and actors that illicit organizations use to traffic in drugs, money weapons, human being, endangered animals and other products. The erasing of the once-clear ideological lines that drove the civil war and the ability of erstwhile enemies to join forces in criminal enterprises in the post-war period is an enduring and dangerous characteristic of El Salvador’s transnational criminal evolution. Trained, elite cadres from both sides, with few legitimate job opportunities, found their skills were marketable in the growing criminal structures. The groups moved from kidnapping and extortion to providing protection services to transnational criminal organizations to becoming integral parts of the organizations themselves. The demand for specialized military and transportation services in El Salvador have exploded as the Mexican DTOs consolidate their hold on the cocaine market and their relationships with the transportista networks, which is still in flux. The value of their services has risen dramatically also because of the fact that multiple Mexican DTOs, at war with each other in Mexico and seeking to physically control the geographic space of the lucrative pipeline routes in from Guatemala to Panama, are eager to increase their military capabilities and intelligence gathering capacities. The emergence of multiple non-state armed groups, often with significant ties to the formal political structure (state) through webs of judicial, legislative and administrative corruption, has some striking parallels to Colombia in the 1980s, where multiple types of violence ultimately challenged the sovereignty of state and left a lasting legacy of embedded corruption within the nation’s political structure. Organized crime in El Salvador is now transnational in nature and more integrated into stronger, more versatile global networks such as the Mexican DTOs. It is a hybrid of both local crime – with gangs vying for control off specific geographic space so they can extract payment for the safe passage of illicit products – and transnational groups that need to use that space to successfully move their products. These symbiotic relationships are both complex and generally transient in nature but growing more consolidated and dangerous.
