INTRODUCTION: The use of drug-eluting stents in the context of mechanical reperfusion following ST-segment elevation myocardial infarction (MI) was initially viewed with concern. The main fear was that the drugs' action in unstable lesions could increase the risk of thrombotic stent occlusion. Furthermore, there was no evidence that the proven benefit of reduced instent restenosis could be extended to such patients, since they were excluded from the initial clinical trials. OBJECTIVES: To assess the safety and long-term clinical outcomes of the use of drug-eluting stents in primary angioplasty. METHODS: The first 100 consecutive and non-selected patients admitted for MI and treated by primary angioplasty with drug-eluting stent implantation in the target lesion were analyzed retrospectively. The efficacy and safety of the procedure, in-hospital clinical evolution and the occurrence of major adverse cardiac events in the first year were assessed. RESULTS: Patients' mean age was 58.2 +/- 11.5 years, and 78 were male. The success rate of primary angioplasty was 99%. Stents coated with sirolimus were used in 67 patients, paclitaxel in 19 and dexamethasone in 16. In-hospital mortality was 3%. The follow-up rate at 12 months was 98%. During this period, the rate of target vessel revascularization was 1% (with no patient requiring target lesion revascularization), MI 2%, and overall mortality 3.9%. Fourteen patients had clinical indication for repeat coronary angiography, which showed no significant in-stent restenosis. One event was considered to be due to acute stent thrombosis. The incidence of major adverse events was 5.9%. CONCLUSION: The use of drug-eluting stents in MI patients undergoing primary mechanical revascularization is safe and is associated with a reduced incidence of major adverse events, thrombosis and clinical restenosis at one year.

As a result of the advances in the control of pulmonary insufficiency in tetanus, the cardiovascular system has increasingly been shown to be a determining factor in morbidity and mortality but detailed knowledge of the cardiovascular complications in tetanus is scanty. The 24h-Holter was carried out in order to detect arrhythmias and sympathetic overactivity in 38 tetanus patients admitted to an ICU. The SDNN Index (standard deviation from the normal R-to-R intervals), was useful in detecting adrenergic tonus, and ranged from 64.1 ± 27 in the more severe forms of tetanus to 125 ± 69 in the milder ones. Sympathetic overactivity occurred in 86.2% of the more severe forms of the disease, but was also detected in 33% of the milder forms. Half the patients had their sympathetic overactivity detected only by the Holter. The most frequent arrhythmias were isolated supraventricular (55.2%) and ventricular (39.4%) extrasystoles. There was no association of the arrhythmias with the clinical form of tetanus or with the presence of sympathetic overactivity. The present study demonstrated that major cardiovascular dysfunction, particularly sympathetic overactivity, occurs in all forms of tetanus, even in the milder ones. This has not been effectively detected with traditional monitoring in ICU and may not be properly treated.

INTRODUCTION: Metabolic syndrome (MS) is an independent predictor of acute cardiovascular events. However, few studies have addressed the relationship between MS and stable angiographic coronary artery disease (CAD), which has a different pathophysiological mechanism. We aimed to study the independent predictors for significant CAD, and to analyze the impact of MS (by the AHA/NHLBI definition) on CAD. METHODS: We prospectively included 300 patients, mean age 64±9 years, 59% male, admitted for elective coronary angiography (suspected ischemic heart disease), excluding patients with known cardiac disease. All patients underwent assessment of demographic, anthropometric, and laboratory data and risk factors, and subsequently underwent coronary angiography. RESULTS: In the study population, 23.0% were diabetic, 40.5% had MS (and no diabetes) and 36.7% had neither diagnosis. Significant CAD was present in 51.3% of patients. CAD patients were older and more frequently male and diabetic, with increased triglycerides and glucose and lower HDL cholesterol. Abdominal obesity was also less prevalent. MS was not associated with the presence of CAD (OR 0.94, 95% CI 0.59-1.48, p=0.778). Of the MS components, the most important predictors of CAD were increased glucose and triglycerides. Abdominal obesity was associated with a lower risk of CAD. In a multivariate logistic regression model for CAD, independent predictors of CAD were age, male gender, glucose and triglycerides. Body mass index had a protective effect. CONCLUSIONS: Although MS is associated with cardiovascular events, the same was not found for stable angiographically proven CAD. Age, gender, diabetes and triglycerides are the most influential factors for CAD, with abdominal obesity as a protective factor.

Does carotid intima-media thickness (cIMT), a surrogate marker of cardiovascular events, have predictive incremental value over established risk factors for stable coronary artery disease (CAD)? Prospective study of 300 patients, with suspected stable CAD, admitted for an elective coronary angiography and carotid ultrasound. The CAD patients had a higher cIMT, which showed a modest predictive accuracy for CAD (area under the receiver-operating characteristic curve 0.638, 95% confidence interval 0.576-0.701, P < .001). The cIMT was an independent predictor of CAD, together with age, gender, and diabetes. C-statistic for CAD prediction by traditional risk factors was not significantly different from a model that included cIMT, carotid plaque presence, or both. However, in women, it was significantly increased by the addition of cIMT or carotid plaque presence. Although cIMT cannot be used as a sole indicator of CAD, it should be considered in the panel of investigations that is requested, particularly in women who are candidates for coronary angiography.

RESUMO:Introdução: Reviu-se o conhecimento epidemiológico, fisiopatológico e clínico atual sobre a doença coronária, da sua génese até ao evento agudo, o Enfarte Agudo do Miocárdio (EAM). Valorizou-se, em especial, a teoria inflamatória da aterosclerose, que foi objeto de grandes desenvolvimentos na última década. Marcadores de instabilidade da placa aterosclerótica coronária: Aprofundou-se o conhecimento da placa aterosclerótica coronária instável. Descreveram-se detalhadamente os biomarcadores clínicos e laboratoriais associados à instabilidade da placa, com particular ênfase nos mecanismos inflamatórios. Objetivos:Estão divididos em dois pontos fundamentais:(1) Estudar em doentes com EAM a relação existente entre as moléculas inflamatórias: Interleucina-6 (IL-6), Fator de Necrose Tumoral-α (TNF-α) e Metaloproteinase de Matriz-3 (MMP3), não usados em contexto clínico, com um marcador inflamatório já em uso clínico: a Proteína C-Reativa ultrassensível (hs-CRP). Avaliar a relação de todas as moléculas inflamatórias com um biomarcador de lesão miocárdica: a Troponina Cardíaca I (cTnI). (2) Avaliar, no mesmo contexto de EAM, a Resposta de Fase Aguda (RFA) . Pretende-se demonstrar o impacto deste fenómeno, com repercussão clínica generalizada, no perfil lipídico e nos biomarcadores inflamatórios dos doentes. Métodos:(1) Estudo observacional prospetivo de doentes admitidos consecutivamente por EAM (grupo EAM) numa única unidade coronária, após exclusão de trauma ou infeção. Doseamento no sangue periférico, na admissão, de IL-6, TNF-α, MMP3, hs-CRP e cTnI. Este último biomarcador foi valorizado também nos valores séricos obtidos 6-9 horas depois. Procedeu-se a correlação linear (coeficiente de Pearson, de Rho-Spearman e determinação do R2) entre os 3 marcadores estudados com os valores de hs-CRP e de cTnI (valores da admissão e 6 a 9 horas após). Efetuou-se o cálculo dos coeficientes de regressão linear múltipla entre cTnI da admissão e cTnI 6-9h após, com o conjunto dos fatores inflamatórios estudados. (2) Estudo caso-controlo entre o grupo EAM e uma população aleatória de doentes seguidos em consulta de cardiologia, após exclusão de eventos cardiovasculares de qualquer território (grupo controlo) e também sem infeção ou trauma. Foram doseados os mesmos marcadores inflamatórios no grupo controlo e no grupo EAM. Nos dois grupos dosearam-se, ainda, as lipoproteínas: Colesterol total (CT), Colesterol HDL (HDLc), com as suas subfrações 2 e 3 (HDL 2 e HDL3), Colesterol LDL oxidado (LDLox),Triglicéridos (TG), Lipoproteína (a) [Lp(a)], Apolipoproteína A1 (ApoA1), Apolipoproteína B (ApoB) e Apolipoproteína E (ApoE). Definiram-se, em cada grupo, os dados demográficos, fatores de risco clássicos, terapêutica cardiovascular e o uso de anti-inflamatórios. Procedeu-se a análise multivariada em relação aos dados demográficos, fatores de risco e à terapêutica basal. Compararam-se as distribuições destas mesmas caraterísticas entre os dois grupos, assim como os valores séricos respetivos para as lipoproteínas estudadas. Procedeu-se à correlação entre as moléculas inflamatórias e as lipoproteínas, para todos os doentes estudados. Encontraram-se os coeficientes de regressão linear múltipla entre cada marcador inflamatório e o conjunto das moléculas lipídicas, por grupo. Finalmente, efetuou-se a comparação estatística entre os marcadores inflamatórios do grupo controlo e os marcadores inflamatórios do grupo EAM. A regressão múltipla marcadores inflamatórios/perfil lipídico (grupo EAM) foi: hs-CRP (R=0,647, R2=0,418) p=0,004; IL-6 (R=0,544, R2=0,300), p=0,073; MMP3 (R=0,539, R2=0,290) p=0,089; TNF-α (R=0,595; R2=0,354) p=0,022. Da comparação entre os marcadores inflamatórios dos dois grupos resultou (mediana/intervalo interquartil, grupo controlo vs EAM): hs-CRP (0,19/0,27 vs 0,42/2,53 mg/dl) p=0,001, IL-6 (4,90/5,48 vs 13,07/26,41 pg/ml) p<0,001, MMP3 (19,70/13,70 vs 10,10/10,40 ng/ml) p<0,001;TNF-α (8,67/6,71 vs 8,26/7,80 pg/dl) p=0,805. Conclusões: (1) Nos doentes com EAM, existe correlação entre as moléculas inflamatórias IL-6, MMP3 e TNF-α, quer com o marcador inflamatório hs-CRP, quer com o marcador de lesão miocárdica cTnI. Esta correlação reforça-se para os valores de cTnI 6-9 horas após admissão, especialmente na correlação múltipla com o grupo dos quatro marcadores inflamatórios. (2) IL-6 está inversamente ligada às lipoproteínas de colesterol; hs-CRP e IL-6 têm excelentes correlações com o perfil lipídico valorizado no seu conjunto. No grupo EAM encontram-se níveis séricos mais reduzidos para as lipoproteínas de colesterol. Para TNF-α não foram encontradas diferenças significativas entre os grupos, as quais foram observadas para a IL-6 e hs-CRP (mais elevadas no grupo EAM). Os valores de MMP3 no grupo controlo estão mais elevados. ------------- ABSTRACT: Introduction: We reviewed the epidemiology, pathophysiology and current clinical knowledge about coronary heart disease, from its genesis to the acute myocardial infarction (AMI). The inflammatory theory for atherosclerosis, which has undergone considerable development in the last decade, was especially detailed. Markers of coronary atherosclerotic vulnerable plaque: The clinical and laboratory biomarkers associated with the unstable coronary atherosclerotic plaque vulnerable plaque are detailed. An emphasis was placed on the inflammatory mechanisms. Objectives: They are divided into two fundamental points: (1) To study in AMI patients, the relationship between the inflammatory molecules: Interleukin-6 (IL-6), Tumor Necrosis Factor-α (TNF-α) and Matrix metalloproteinase-3 (MMP3), unused in the clinical setting, with an inflammatory marker in clinical use: ultrasensitive C-reactive protein (hs-CRP), as well as a biomarker of myocardial injury: cardiac troponin I (cTnI). (2) To study, in the context of AMI, the Acute Phase Response (APR). We intend to demonstrate the impact of that clinical relevant phenomenon in the lipid profile and inflammatory biomarkers of our patients. Methods: (1) Prospective observational study of patients consecutively admitted for AMI (AMI group) in a single coronary care unit, after exclusion of trauma or infection. A peripheral assay at admission for IL-6, TNF-α, MMP3, hs-CRP and cTnI was performed. The latter was also valued in assays obtained 6-9 hours after admission. Linear correlation (Pearson's correlation coefficient, Spearman Rho's correlation coefficient and R2 regression) was performed between the three markers studied and the values of hs-CRP and cTnI (on admission and 6-9 hours after admission). Multiple linear regression was also obtained between cTnI on admission and 6-9h after, with all the inflammatory markers studied. (2) Case-control study between the AMI group and a random population of patients from an outpatient cardiology setting (control group). Cardiovascular events of any kind and infection or trauma were excluded in this group. The same inflammatory molecules were assayed in control and AMI groups. The following lipoproteins were also assayed: total cholesterol (TC), HDL cholesterol (HDLc) and subfractions 2 and 3 (HDL2 and HDL 3), oxidized LDL cholesterol (oxLDL), Triglycerides (TG), Lipoprotein (a) [Lp(a)], Apolipoprotein A1 (apoA1), Apolipoprotein B (ApoB) and Apolipoprotein E (ApoE). Demographics, classical risk factors, cardiovascular therapy and the use of anti-inflammatory drugs were appreciated in each group. The authors conducted a multivariate analysis with respect to demographics, risk factors and baseline therapy. The distribution of the same baseline characteristics was compared between the two groups, as well as the lipoprotein serum values. A correlation was performed between each inflammatory molecule and each of the lipoproteins, for all the patients studied. Multiple linear regression was determined between each inflammatory marker and all the lipid molecules per group. Finally, the statistical comparison between the inflammatory markers in the two groups was performed. In the linear correlations between inflammatory variables and lipid variables for all patients, we found a negative relationship between IL-6 and TC, HDLc, HDL3, ApoA1 and ApoB. The multiple linear regression inflammatory markers/lipid profile (control group) was: hs-CRP (R= 0.883, R2=0.780) p=0.022; IL6 (R=0.911, R2=0.830) p=0.007; MMP3 (R=0.498, R2=0.248) p=0.943; TNF-α (R=0.680, R2=0.462) p=0.524. For the linear regression inflammatory markers/lipid profile (AMI group) we found: hs-CRP (R=0.647, R2=0.418) p=0.004; IL-6 (R=0.544, R2=0.300) p=0.073; MMP3 (R=0.539, R2 =0.290) p=0.089; TNF-α (R=0.595, R2=0.354) p=0.022. The comparison between inflammatory markers in both groups (median/interquartile range, control group vs AMI) resulted as: hs-CRP (0.19/0.27 vs 0.42/2.53 mg/dl) p=0.001; IL-6 (4.90/5.48 vs 13.07/26.41 pg/ml) p<0.001; MMP3 (19.70/13.70 vs 10.10/10.40 ng/ml) p<0.001; TNF-α (8.67/6.71 vs 8.26/7.80 pg/dl) p=0.805. Conclusions: (1) In AMI patients there is a correlation between the inflammatory molecules IL-6, TNF-α and MMP3 with both the inflammatory marker hs-CRP and the ischemic marker cTnI. This correlation is strengthened for the cTnI at 6-9h post admission, particularly in the multiple linear regression to the four inflammatory markers studied. (2) IL-6 correlates negatively with the cholesterol lipoproteins. Hs-CRP and IL-6 are strongly correlated to the whole lipoprotein profile. AMI patients display reduced serum lipid levels. For the marker TNF-α no significant differences were found between groups, which were observed for IL-6 and hs-CRP (higher in the AMI group). MMP3 values are higher in the control group.

WHAT IS ALREADY KNOWN ABOUT THIS SUBJECT: * In previous work, we showed a long-term and concentration-dependent beneficial effect of the non-nucleoside reverse transcriptase inhibitor efavirenz (EFV) on high-density lipoproteins (HDL) in human immunodeficiency virus (HIV)-infected patients. * Furthermore, it has been suggested that instead of the current practice of only measuring HDL-chelesterol values, the evaluation of HDL function, namely its antioxidant properties, might be an improved tool for identifying subjects at increased risk for cardiovascular events. * Paraoxonase-1 (PON-1) is an enzyme associated with HDL that is responsible for HDL antioxidant function. WHAT THIS STUDY ADDS: * In the present work, we studied the effect of EFV on the activity of PON-1 and showed, for the first time, that EFV-based antiretroviral therapy is associated with a better antioxidant function, i.e. with a higher PON-1 activity. AIMS: A long-term and concentration-dependent beneficial effect of efavirenz (EFV) on cholesterol associated with high-density lipoprotein (HDL-c) in human immunodeficiency virus (HIV)-infected patients has been documented. Furthermore, it has been suggested that, instead of the current practice of only measuring HDL-c values, the evaluation of HDL quality might be an improved tool for identifying subjects at increased risk of cardiovascular events. Paraoxonase-1 (PON-1) is an enzyme associated with HDL that is involved in the onset of cardiovascular disease and responsible for HDL antioxidant function. The aim of the present study was to investigate the effect of EFV on the circulating activity of PON-1 in HIV-infected patients. METHODS: The patients included were adults with a documented HIV-1 infection, nontreated or treated with antiretroviral regimens including EFV 600 mg once daily as first therapeutic regimen for at least 3 months. The influence of treatment with EFV, HDL-c and CD4 cell count on PON-1 activity was analysed. RESULTS: HIV-infected White patients treated with EFV had higher PON-1 activity [77.35 U l(-1) (65.66, 89.04)] (P < 0.05) and higher PON-1 activity : HDL-c ratio [1.88 (1.49, 2.28)] (P < 0.01) than untreated patients. PON-1 activity was higher in Black patients (P < 0.001) and in patients with a CD4 cell count >500 cells ml(-1) (P= 0.0120). CONCLUSIONS: EFV-based antiretroviral regimens are associated with HDL particles with a better antioxidant function, i.e. with a higher PON-1 activity. The PON-1 activity of Black patients is higher than that found in Whites regardless of treatment. Ethnicity should be taken into consideration when studying drug effects on PON-1 activity.

BACKGROUND: Variations in emergency department admissions have been reported to happen as a result of major sports events. The work presented assessed changes in volume and urgency level of visits to a major Emergency Department in Lisbon during and after the city's football derby. MATERIAL AND METHODS: Volume of attendances and patient urgency level, according to the Manchester Triage System, were retrospectively analyzed for the 2008-2011 period. Data regarding 24-hour periods starting 45 minutes before kick-off was collected, along with data from similar periods on the corresponding weekdays in the previous years, to be used as controls. Data samples were organized according to time frame (during and after the match), urgency level, and paired accordingly. RESULTS: A total of 14 relevant periods (7 match and 7 non-match) were analyzed, corresponding to a total of 5861 admissions. During the match time frame, a 20.6% reduction (p = 0.06) in the total number of attendances was found when compared to non-match days. MTS urgency level sub-analysis only showed a statistically significant reduction (26.5%; p = 0.05) in less urgent admissions (triage levels green-blue). Compared to controls, post-match time frames showed a global increase in admissions (5.6%; p = 0.45), significant only when considering less urgent ones (18.9%; p = 0.05). DISCUSSION: A decrease in the total number of emergency department attendances occurred during the matches, followed by a subsequent increase in the following hours. These variations only reached significance among visits triaged green-blue. CONCLUSION: During major sports events an overall decrease in emergency department admissions seems to take place, especially due to a drop in visits associated with less severe conditions.

Severe chronic kidney disease may lead to disturbances, such as hyperphosphatemia, increased secretion of fibroblast growth factor -23 (FGF -23) and vitamin D deficiency. These may increase plasmatic levels of parathyroid hormone, and decrease plasmatic levels of calcium. Altogether, these may contribute to the development of secondary hyperparathyroidism, and to abnormalities in mineral metabolism. Kidney transplantation is the best option to improve longevity and quality of life in end -stage chronic kidney disease patients. Vitamin D deficiency has been associated with cardiovascular disease, which is the leading cause of death in chronic kidney disease. Therefore, diagnosing this deficiency may be pivotal for minimizing mortality in chronic kidney disease, because pharmacological treatments for this deficiency may be prescribed. Calcitriol is indicated for the treatment of vitamin D deficiency, both in chronic kidney disease and in kidney transplanted patients. However, calcitriol may increase the plasmatic levels of calcium and phosphorous, which can lead to vascular calcifications, that have been associated with cardiovascular mortality. Selective vitamin D receptor activators are indicated for the treatment of vitamin D deficiency in chronic kidney disease. These have the advantage of being associated with lower increases of plasmatic levels of calcium and phosphorous. These drugs also seem to have additional effects that may minimise patient morbidity and mortality, especially due to potentially reducing cardiovascular events. Unfortunately, there are few studies about the use of these drugs in kidney transplanted patients. Here we present a review about the physiology of vitamin D, the consequences of its deficiency in chronic kidney disease and in kidney transplanted patients, and about the diagnosis and treatment of this deficiency. Finally, we discuss the new line of research about the efficacy and safety of selective vitamin D receptor activators in kidney transplanted patients.

RESUMO - Introdução As quedas ocorridas em ambiente hospitalar são um dos principais eventos reportados e são responsáveis por consequências físicas, psicológicas e sociais nos doentes e seus familiares, mas também nos profissionais e nas organizações de saúde. Paralelamente, tais ocorrências representam significativo impacto clínico, económico e social. Objetivo, material e métodos O propósito desta investigação é caracterizar o impacte clínico e económico da ocorrência de quedas em ambiente hospitalar. De forma a alcançar este objetivo, optou-se por um estudo assente no paradigma quantitativo, observacional, transversal, com recolha de informação de forma retrospetiva. Resultados e discussão No ano de 2013, ocorreram 118 quedas, num total de 108 indivíduos hospitalizados no CHMT. A incidência de quedas foi 0,84 quedas por mil dias de internamento. Do total de doentes em que ocorreu a queda, verificou-se que 59% tiveram consequências físicas e estimou-se um aumento de dias de internamento num total de 1114 dias, com consequente aumento dos custos totais em cerca de 1.027.591,2€. Conclusões A incidência de quedas verificada neste estudo, quando comparada com outros estudos internacionais, é menor. No que se refere ao impacto clínico e económico, constatou-se que a maioria das quedas teve consequências físicas e que, os custos adicionais relacionados com o aumento da demora média de internamento são de uma magnitude importante. Tal circunstância, justifica assim, a necessidade de desenvolvimento de estudos nesta área, no sentido de implementar estratégias efetivas que atuem ao nível do sistema e do individuo, com vista à redução deste tipo de ocorrências.

Nowadays, safety and security regarding the tourism and events industries are a fundamental subject to society. Portugal’s tourism has significantly increased its number of visitors, whether due to the increasing number of cruisers docking in Lisbon, or due to visitors arriving by air, travelling the country from North to South and staying in the most varied accommodation units. Issues like human security and internal security of the different countries, even the security in the world, as development factors of a modern society, are discussed on a daily basis. On the contrary, few deal with tourism and major events security as being part of internal security, as well as the existing barriers tourism encounters to integrate the system for fighting terrorism. Although two distinct activities, they are complementary and may influence the country’s economy, provided that they can offer certainty to all actors involved. It is this substance that organised crime groups look for when planning terror acts. Therefore, as tourism can offer deception and shelter opportunities and events the theatre of a possible attack, those events assemble all the necessary conditions for an attack to achieve its goals.

In this thesis we address the paradigms and inconsistencies involving crisis kidnappings and hostage-taking. This work particularly focuses on foreign hostages seized by terrorists and guerrilla movements. It highlights major global events that have contributed to the collective awareness of the phenomenon and the evolution of international agreements and protocols. We focus on the episodes involving Portugal or Portuguese abroad, particularly on the kidnapping of Portuguese in the Cabinda enclave. It also deals with the problems associated with the payment of ransoms, the increasing involvement of private agents in addressing these crises, and incongruities in the motivations of the kidnappers.

RESUMO - A doença arterial periférica (DAP) apresenta uma carga de doença significativa, afetando cerca de 3 a 10% da população em geral e 15 a 20% dos indivíduos com idade superior a 70 anos. A sua prevalência em Portugal foi estimada em cerca de 5,9% no continente; 6,6% na Região Autónoma dos Açores (RAA) e 3,8% na Região Autónoma da Madeira (RAM). Para além da importante carga de doença, quer em termos epidemiológicos, quer económicos, a DAP confere aos seus portadores um risco cardiovascular agravado, sendo que os mesmos apresentam cerca do triplo do risco de mortalidade e de eventos cardiovasculares quando comparados com indivíduos sem DAP. Tratou-se de um estudo observacional, transversal e descritivo tendo como base duas populações de estudo. A primeira é referente aos hospitais do Serviço Nacional de Saúde (SNS) com a valência de cirurgia vascular e a segunda à população portuguesa com episódios de internamento por diagnóstico de DAP dos membros inferiores (MI) nos anos de 2013 e 2014 na totalidade dos hospitais do SNS. Através da análise dos resultados do questionário procedeu-se à descrição de algumas das características dos serviços e unidades de cirurgia vascular de sete hospitais do SNS; através da análise da base de dados dos GDH para os anos de 2013 e 2014 procedeu-se à caracterização do peso do internamento por DAP dos membros inferiores a nível nacional no mesmo período. A DAP tem uma carga significativa e atendendo aos seus fatores de risco e história natural da doença, apresenta uma tendência crescente durante os próximos anos, representando por isso um enorme desafio para os sistemas de saúde. São, no entanto, necessários estudos mais aprofundados sobre o tema que permitam conhecer melhor o peso desta patologia e, de forma global, melhorar o planeamento, tendo por base a caracterização quer do lado da procura (dados epidemiológicos e peso no internamento), quer da oferta (capacidade instalada).

PURPOSE: The authors analyzed the 30-day and 6-month outcomes of 1,126 consecutive patients who underwent coronary stent implantation in 1996 and 1997. METHODS: The 30-day results and 6-month angiographic follow-up were analyzed in patients treated with coronary stents in 1996 and 1997. All patients underwent coronary stenting with high-pressure implantation (>12 atm) and antiplatelet drug regimen (aspirin plus ticlopidine). RESULTS: During the study period, 1,390 coronary stents were implanted in 1,200 vessels of 1,126 patients; 477 patients were treated in the year 1996 and 649 in 1997. The number of percutaneous procedures performed using stents increased significantly in 1997 compared to 1996 (64 % vs 48%, p=0.0001). The 30-day results were similar in both years; the success and stent thrombosis rates were equal (97% and 0.8%, respectively). The occurrence of new Q wave MI (1.3% vs 1.1%, 1996 vs 1997, p=NS), emergency coronary bypass surgery (1% vs 0.6%, 1996 vs 1997, p=NS) and 30-day death rates (0.2% vs 0.5%, 1996 vs 1997, p=NS) were similar. The 6-month restenosis rate was 25% in 1996 and 27% in 1997 (p= NS); the target vessel revascularization rate was 15% in 1996 and 16% in 1997 (p = NS). CONCLUSIONS: Intracoronary stenting showed a high success rate and a low incidence of 30-day occurrence of new major coronary events in both periods, despite the greater angiographic complexity of the patients treated with in 1997. These adverse variables did not have a negative influence at the 6-month clinical and angiographic follow-up, with similar rates of restenosis and ischemia-driven target lesion revascularization rates.

OBJECTIVE: Analysis of the in-hospital results, in progressively elderly patients who undergo primary percutaneous coronary intervention (PCI) in the first 24 hours of AMI. METHODS: The patients were divided into three different age groups (60/69, 70/79, and > or = 80 years) and were treated from 7/95 until 12/99. The primary success rate and the occurrence of major clinical events were analyzed at the end of the in-hospital phase. Coronary stent implantation and abciximab use were employed at the intervencionist discretion. RESULTS: We analyzed 201 patients with age ranging from 60 to 93 years, who underwent primary PCI. Patients with ages above 70 were more often female (p=.015). Those with ages above 80 were treated later with PCI (p=.054), and all of them presented with total occlusion of the infarct-related artery. Coronary stents were implanted in 30% of the patients. Procedural success was lower in > or = 80 year old patients (p=.022), and the death rate was higher in > or = 70 years olds (p=.019). Reinfarction and coronary bypass surgery were uncommon events. A trend occurred toward a higher combined incidence of major in-hospital events according to increased age (p=.064). CONCLUSION: Elderly patients ( > or = 70 years) presented with adverse clinical and angiographic profiles and patients > or = 80 years of age obtained reduced TIMI 3 flow success rates after primary PTCA, and those > or = 70 years had a higher death rate.
