OBJECTIVE To study the factors associated with choice of therapy and prognosis in octogenarians with severe symptomatic aortic stenosis (AS). STUDY DESIGN Prospective, observational, multicenter registry. Centralized follow-up included survival status and, if possible, mode of death and Katz index. SETTING Transnational registry in Spain. SUBJECTS We included 928 patients aged ≥80 years with severe symptomatic AS. INTERVENTIONS Aortic-valve replacement (AVR), transcatheter aortic-valve implantation (TAVI) or conservative therapy. MAIN OUTCOME MEASURES All-cause death. RESULTS Mean age was 84.2 ± 3.5 years, and only 49.0% were independent (Katz index A). The most frequent planned management was conservative therapy in 423 (46%) patients, followed by TAVI in 261 (28%) and AVR in 244 (26%). The main reason against recommending AVR in 684 patients was high surgical risk [322 (47.1%)], other medical motives [193 (28.2%)], patient refusal [134 (19.6%)] and family refusal in the case of incompetent patients [35 (5.1%)]. The mean time from treatment decision to AVR was 4.8 ± 4.6 months and to TAVI 2.1 ± 3.2 months, P < 0.001. During follow-up (11.2-38.9 months), 357 patients (38.5%) died. Survival rates at 6, 12, 18 and 24 months were 81.8%, 72.6%, 64.1% and 57.3%, respectively. Planned intervention, adjusted for multiple propensity score, was associated with lower mortality when compared with planned conservative treatment: TAVI Hazard ratio (HR) 0.68 (95% confidence interval [CI] 0.49-0.93; P = 0.016) and AVR HR 0.56 (95% CI 0.39-0.8; P = 0.002). CONCLUSION Octogenarians with symptomatic severe AS are frequently managed conservatively. Planned conservative management is associated with a poor prognosis.

This study evaluated parasitological and molecular techniques for the diagnosis and assessment of cure of schistosomiasis mansoni. A population-based study was performed in 201 inhabitants from a low transmission locality named Pedra Preta, municipality of Montes Claros, state of Minas Gerais, Brazil. Four stool samples were analysed using two techniques, the Kato-Katz® (KK) technique (18 slides) and the TF-Test®, to establish the infection rate. The positivity rate of 18 KK slides of four stool samples was 28.9% (58/201) and the combined parasitological techniques (KK+TF-Test®) produced a 35.8% positivity rate (72/201). Furthermore, a polymerase chain reaction (PCR)-ELISA assay produced a positivity rate of 23.4% (47/201) using the first sample. All 72 patients with positive parasitological exams were treated with a single dose of Praziquantel® and these patients were followed-up 30, 90 and 180 days after treatment to establish the cure rate. Cure rates obtained by the analysis of 12 KK slides were 100%, 100% and 98.4% at 30, 90 and 180 days after treatment, respectively. PCR-ELISA revealed cure rates of 98.5%, 95.5% and 96.5%, respectively. The diagnostic and assessment of cure for schistosomiasis may require an increased number of KK slides or a test with higher sensitivity, such as PCR-ELISA, in situations of very low parasite load, such as after therapeutic interventions.

Oxidative metabolism of isolated toad skin epithelium (Bufo viridis) was investigated in vitro under open-circuit conditions using the spectrophotometric oxyhemoglobin micromethod. This highly sensitive technique has been adapted for studying several epithelia in parallel and for detecting possible regional variations of oxygen uptake in individual epithelium. Changes in the proportion of mitochondria-rich cells (MRC) by ionic acclimation affected oxidative metabolism under nontransporting condition. After acclimation of animals to either NaNO3 or NaCl solutions (100 mmol/l, for greater than 2 wk), the number of MRC per square millimeter in epithelia from nonacclimated and NaNO3- and NaCl-acclimated animals was 350 +/- 113, 460 +/- 196, and 107 +/- 52, respectively. O2 uptake of nonacclimated and NaNO3-acclimated epithelia was significantly higher than that of NaCl-acclimated epithelia (i.e., 0.89 and 0.90 vs. 0.57 nmol O2.h-1.mm-2, respectively). The correlation established between O2 uptake and number of MRC allowed evaluation of the respiration rate of one single MRC, i.e., approximately 1 pmol O2/h. The lowest mitochondrial oxidative activity was found in the epithelia from NaCl-acclimated toads where the uncoupler 2,4-dinitrophenol (50 mumols/l) had the highest relative stimulatory effect (+114%). Acetazolamide (50 mumols/l), a potent inhibitor of carbonic anhydrase mainly present in the MRC, reduced selectively by 31% O2 uptake of the MRC-rich epithelia (NaNO3 acclimated). O2 uptake increased significantly by approximately 80% when basolateral pH increased from 5.8 to 7.8, but did not depend on apical pH. These findings indicate that under nontransporting (open-circuit) conditions, aerobic metabolism of the isolated toad skin epithelium is related to the density and/or characteristics of the MRC.(ABSTRACT TRUNCATED AT 250 WORDS)

As instituições de longa permanência abrigam um grande número de idosos dependentes, embora essa não seja condição sine qua non para que ocorra a institucionalização do idoso. O objetivo deste estudo foi identificar o grau de independência para a realização de atividades da vida diária (AVDs) dos idosos residentes nas instituições da cidade de Taubaté-SP, segundo avaliação baseada no Índice de Independência nas AVDs de Katz, realizada em dezembro de 2001 e repetida em maio de 2002. A primeira avaliação, feita com todos os residentes com idade igual ou superior a 60 anos (139 mulheres e 48 homens), mostrou que 70 idosos eram considerados independentes para o desempenho das AVDs, sendo que, dentre eles, 52 idosos tinham entre 70 e 89 anos. A segunda avaliação mostrou que 53 idosos mantiveram-se independentes. Os dados corroboram estudos anteriores que apontam para o declínio da capacidade funcional de idosos institucionalizados.

Este estudo teve como objetivo avaliar a qualidade do sono de idosos residentes em quatro instituições de longa permanência para idosos (ILPI) de uma cidade do interior do estado de São Paulo e identificar problemas relacionados ao sono. Participaram 38 idosos, cujas funções cognitivas estavam preservadas e que residiam nas ILPI há pelo menos um ano. Foram empregues os instrumentos Ficha de Identificação, Índice de Katz e Índice de Qualidade do Sono de Pittsburgh (PSQI), todos preenchidos pela pesquisadora. Os resultados mostraram que 81,6% dos idosos referiam qualidade do sono boa ou muito boa; entretanto, os seguintes problemas relacionados ao sono destacaram-se por sua elevada frequência: levantar-se para ir ao banheiro (63,2%); acordar no meio da noite ou muito cedo pela manhã (50%); sentir muito calor (23,7%); sentir dores (21,1%). Evidencia-se uma contradição entre a percepção da qualidade do sono e o elevado número de problemas identificados.

O objetivo deste trabalho foi analisar a estrutura e função das redes de apoio social de idosos com alterações cognitivas, residentes em contexto de alta e muito alta vulnerabilidade social, além de identificar associações entre as características das redes e a capacidade funcional. Os sujeitos estudados foram 38 idosos, residentes em contexto de alta e muito alta vulnerabilidade social, que apresentaram resultado no Mini Exame do Estado Mental abaixo da nota de corte. Todos os cuidados éticos foram observados. Aplicaram-se o Mini Exame do Estado Mental, o Diagrama de Escolta, o Índice de Katz e o Questionário de Pfeffer. Os resultados demonstraram que os idosos avaliados possuem rede social grande, com predomínio de integrantes no círculo interno, porém poucos integrantes desempenham papéis funcionais. Observou-se correlação entre a variável sexo e o número de integrantes das redes sociais. Não foi observada correlação significativa entre as características das redes e a capacidade funcional dos idosos.

A identificação de variáveis associadas ao tipo de atenção domiciliária (AD) dos usuários do Sistema Único de Saúde (SUS) contribui para a gestão do cuidado na Rede de Atenção à Saúde (RAS). Objetiva-se identificar variáveis associadas ao tipo de AD dos usuários em Unidades Básicas de Saúde (UBS) selecionadas de Belo Horizonte. Estudo transversal em duas UBS com todos os usuários (n=114) em AD da área de abrangência. Utilizou-se a análise de regressão logística múltipla para seleção (stepwise) de variáveis significativas. Obteve-se maior comprometimento clínico dos usuários (OR=27,47), estado emocional triste (OR=24,36), risco para úlcera por pressão pela escala de Braden (OR=7,6) e a semidependência para as AVD pelo índice de Katz (OR=63,8) como fortemente associadas ao tipo de AD (p < 0,05). As variáveis fundadas no contexto social, familiar e clínico dos sujeitos subsidiam a abordagem integral e a tomada de decisão da equipe de saúde.

This exploratory, descriptive, cross-sectional, and quantitative study aimed to develop and validate an index of family vulnerability to disability and dependence (FVI-DD). This study was adapted from the Family Development Index, with the addition of social and health indicators of disability and dependence. The instrument was applied to 248 families in the city of Sao Paulo, followed by exploratory factor analysis. Factor validation was performed using the concurrent and discriminant validity of the Lawton scale and Katz Index. The descriptive level adopted for the study was p < 0.05. The final vulnerability index comprised 50 questions classified into seven factors contemplating social and health dimensions, and this index exhibited good internal consistency (Cronbach’s alpha = 0.82). FVI-DD was validated using both the Lawton scale and Katz Index. We conclude that FVI-DD can accurately and reliably assess family vulnerability to disability and dependence.

The role of interferon-gamma in autoimmune diabetes was assessed by breeding a null mutation of the interferon-gamma receptor alpha chain into the nonobese diabetic mouse strain, as well as into a simplified T cell receptor transgenic model of diabetes. In contrast to a previous report on abrogation of the interferon-gamma gene, mutation of the gene encoding its receptor led to drastic effects on disease in both mouse lines. Nonobese diabetic mice showed a marked inhibition of insulitis-both the kinetics and penetrance-and no signs of diabetes; the transgenic model exhibited near-normal insulitis, but this never evolved into diabetes, either spontaneously or after experimental provocation. This failure could not be explained by perturbations in the ratio of T helper cell phenotypes; rather, it reflected a defect in antigen-presenting cells or in the islet beta cell targets.

AbstractOBJECTIVEIdentify whether Heart Failure (HF) is a predictor of functional dependence for Basic Activities of Daily Living (BADL) in hospitalized elderly.METHODSWe investigated medical records and assessed dependence to BADL (by the Katz Index) of 100 elderly admitted to a geriatric ward of a university hospital. In order to verify if HF is a predictor of functional dependence, linear regression analyzes were performed.RESULTSThe prevalence of HF was 21%; 95% of them were dependent for BADLs. Bathing was the most committed ADL. HF is a predictor of dependence in hospitalized elderlies, increasing the chance of functional decline by 5 times (95% CI, 0.94-94.48), the chance of functional deterioration by 3.5 times (95% CI, 1.28-11.66; p <0.02) and reducing 0.79 points in the Katz Index score (p <0.05).CONCLUSIONHF is a dependency predictor of ADL in hospitalized elderly, who tend to be more dependent, especially for bathing.

Estudos sobre os partidos políticos nos países da terceira onda de democratização constituem um grande desafio aos estudiosos das instituições políticas. A presente pesquisa tem por objectivo analisar a natureza e os determinantes dos partidos políticos cabo-verdianos com representação política. A relevância do estudo compreende-se com o pressuposto de que a forma como estas organizações se estruturam internamente, enquanto organizações de representação política por excelência, poderá determinar o processo democrático. Pretendemos analisar o surgimento e as estratégias de estruturação adoptadas, a base eleitoral, formas de financiamento e a consequente aproximação ao Estado e a adesão aos princípios democráticos dos partidos políticos em análise. A abordagem recorre tanto à teoria clássica dos partidos políticos, nomeadamente a de Michels (1914), Kirchheimer (1966), Duverger (1970), Sartori (1982), como à teoria mais recente, Katz e Mair (1995) e Panebianco (2005), para compreendermos os partidos políticos cabo-verdianos em democracia.

Estimates of the e¤ect of education on GDP (the social return to education)have been hard to reconcile with micro evidence on the private return. We present a simple explanation that combines two ideas: imperfect substitution between worker types and endogenous skill biased technological progress. When types of workers are imperfect substitutes, the supply of human capital is negatively related to its return, and a higher education level compresses wage di¤erentials. We use cross-country panel data on income inequality to estimate the private return and GDP data to estimate the social return. The results show that the private return falls by 2 percentage points when the average education level increases by a year, which is consistent with Katz and Murphy's [1992] estimate of the elasticity of substitution between worker types. We find no evidence for dynamics in the private return, and certainly not for a reversal of the negative e¤ect as described in Acemoglu [2002]. The short run social return equals the private return.

Wage inequality in the United States has grown substantially in thepast two decades. Standard supply-demand analysis in the empiricsof inequality (e.g.Katz and Murphy (1992)) indicates that we mayattribute some of this trend to an outward shift in the demand forhigh skilled labor. In this paper we examine a simple static channelin which the wage premium for skill may grow -increased firm entry.We consider a model of wage dispersion where there are two types ofworkers and homogeneous firms must set wages and preferences forwhat type of worker they would like to hire. We find that both thewage differential and the demand for high skill workers can increasewith the proportion of high skill workers -these high skill workerstherefore 'create' their own demand without exogenous factors. Inaddition, within group wage inequality can increase in step with thebetween group wage inequality. Simulations of the model are providedin order to compare the findings with empirical results.
