The effect of dietary vitamin E on immunoglobulin A (IgA) antibody production, which acts as the first line of defence at the intestinal mucosa, has not been evaluated in chickens. In the present study the impact of the inclusion of supplementary levels of vitamin E to the diet, on total and antigen-specific IgA antibody titres, T-cell subsets and Ia+ cells, was assessed. From hatching, chickens received a maize-based diet which was supplemented with either 25, 250, 2500 or 5000 mg dl-alpha-tocopherol acetate/kg. Primary immunisation with tetanus toxoid (T. toxoid) emulsified in a vegetable oil-in-water adjuvant was administered by the intraperitoneal route at 21 d of age. At 35 d of age all birds received an oral booster vaccination of T. toxoid. Significantly higher total IgA antibody titres were present in the day 42 intestinal scrapings of birds receiving the 5000 mg/kg vitamin E-supplemented diet (VESD) (P=0.05) and a notable increase was observed in birds receiving the 250 mg/kg VESD (P=0.06). Clinical trials showing the benefits of reducing the effects of TNF-alpha in rheumatoid arthritis have highlighted the key role of the cytokine TNF-alpha in this inflammatory condition. A new approach to reducing the effects of TNF-alpha is to decrease its synthesis by inhibiting TNF-alpha converting enzyme with GW3333. In rat models of arthritis, GW3333 has some beneficial effects. Further longer-term studies of GW3333 in animal models are required to determine whether its benefit is maintained. TACE inhibition may represent a new approach to treating inflammation.

Mast cells are mobile granule-containing secretory cells that are distributed preferentially about the microvascular endothelium in oral mucosa and dental pulp. The enzyme profile of mast cells in oral tissues resembles that of skin, with most mast cells expressing the serine proteases tryptase and chymase. Mast cells in oral tissues contain the pro-inflammatory cytokine tumour necrosis factor-alpha in their granules, and release of this promotes leukocyte infiltration during evolving inflammation in several conditions, including lichen planus, gingivitis, pulpitis, and periapical inflammation, through induction of endothelial-leukocyte adhesion molecules. Mast cell synthesis and release of other mediators exerts potent immunoregulatory effects on other cell types, while several T-lymphocyte-derived cytokines influence mast cell migration and mediator release. Mast cell proteases may contribute to alterations in basement membranes in inflammation in the oral cavity, such as the disruptions that allow cytotoxic lymphocytes to enter the epithelium in oral lichen planus. A close relationship exists among mast cells, neural elements, and laminin, and this explains the preferential distribution of mast cells in tissues. Mast cells are responsive to neuropeptides and, through their interaction with neural elements, form a neural immune network with Langerhans cells in mucosal tissues. This facilitates mast cell degranulation in response to a range of immunological and non-immunological stimuli. Because mast cells play a pivotal role in inflammation, therapies that target mast cell functions could have value in the treatment of chronic inflammatory disorders in the oral cavity.

Background: The heavy usage of coxibs in Australia far outstrips the predicted usage that was based on the treatment of patients with risk factors for upper gastro-intestinal adverse events from conventional anti--inflammatory agents. This raises questions regarding the appropriateness of prescribing. Aims: To determine: (i) the relationship between prescriptions for cyclooxygenase 2 (COX-2) inhibitors and objective evidence of inflammatory arthritis, (ii) prior experience with paracetamol and/or conventional non-steroidal anti-inflammatory drugs (NSAIDs), and (iii) contraindications to the use of NSAIDs. Methods: Drug utilization evaluation and rheumato-logical assessment was conducted on 70 consecutive patients admitted on COX-2 inhibitors to a 480-bed metropolitan hospital. The main outcome measures were: the indication for COX-2 inhibitor; objective -evidence of inflammatory arthritis; previous trial of -paracetamol or conventional NSAIDs; and patient -satisfaction. Results: Only 11 patients (16%) had symptoms or signs of an inflammatory arthropathy, and met Pharmaceut-ical Benefits Schedule criteria for prescribing a COX-2 inhibitor. Fifty-nine patients (84%) had chronic osteo-arthritis, degenerative spinal disease, injury or malignancy, without overt active inflammation. Fourteen patients (20%) had trialled regular paracetamol prior to using any NSAID treatment. Conventional NSAIDs had been previously used by 51 patients (73%). Eleven patients (16%) reported previous adverse gastrointestinal effects from conventional NSAIDs. On the basis of significant renal impairment (creatinine clearance 5/10). Conclusions: Drug utilization data indicate that COX-2 inhibitors are frequently used first line for degenerative osteoarthritis in the absence of overt inflammation, without prior adequate trial of paracetamol and with disregard for the cautions and contraindications of these agents. These findings may explain the unprecedented Pharmaceutical Benefits Schedule expenditure on COX-2 inhibitors in Australia.

Os neuropeptídeos são neurotransmissores relevantes na fisiologia nasal e o conhecimento crescente acerca de seu papel na fisiopatologia de doenças nasais abre novas perspectivas. A sua investigação na mucosa nasal humana baseia-se em grande parte em marcação imunológica, método complexo e sujeito a inúmeros fatores de erro. Com o intuito de viabilizar este tipo de pesquisa em nosso meio, um método de imunofluorescência para peptídeo intestinal vasoativo (VIP) na mucosa nasal humana é proposto e avaliado. FORMA DE ESTUDO: Coorte transversal. MATERIAL E MÉTODO: Oito pacientes submetidos a cirurgia funcional do nariz têm um fragmento de mucosa coletado da concha inferior. O tecido foi fixado em solução de Zamboni (paraformaldeído 4% tamponado e ácido pícrico 0,4%), congelado em nitrogênio líquido e armazenado. Cortes de 14 µm foram realizados e submetidos à reação de imunofluorescência para VIP (Península Laboratories). As imagens microscópicas foram documentadas em fotografia convencional. A especificidade, sensibilidade e reprodutibilidade de execução foram avaliadas. A reprodutibilidade de interpretação de resultados foi avaliada através da comparação de graus de marcação (0 a 4) atribuídos às fotos por seis observadores. RESULTADOS: Os resultados mostraram ser o método suficientemente específico, sensível, além de reprodutível em sua execução. A interpretação de resultados mostrou depender do perfeito esclarecimento do observador no julgamento das imagens de imunofluorescência, mas mostrou uniformidade. CONCLUSÃO: O método proposto foi considerado útil na pesquisa de neuropeptídeos na mucosa nasal humana.

Foram estudados 10 doentes, portadores de anemia ancilostomótica e de grande parasitose. Foram determinados alguns parâmetros hematológicos como dosagem de hemoglobina, contagem de hemácias, hematócrito, volume corpuscular médio, hemoglobina corpuscular média, ferro sérico e siderofilina. Foram estudados o volume da perda de sangue intestinal, o ferro perdido nesta hemorragia e eliminado nas fezes, e o ferro reaborvido da hemorragia intestinal. Conclui-se que a reabsorção do ferro da hemoglobina por dia no tubo digestivo é de suma importância no retardamento da instalação da anemia ferropriva, causada pela parasitose ancilostomótica.

During the second half of 1986 the impact of the improvement of water supply and excreta disposal facilities on diarrheal diseases and intestinal parasitosis was studied in 254 children up to six years of age from two favelas (shanty towns) of Belo Horizonte, Brazil. The estimated incidence of diarrhea was 6.2 episodes/child year and the estimated period prevalence reached 31.0 episode days/ child/ year. The point prevalence of parasitosis was 70.7% (Ascaris lumbricoides: 55.4%, Trichuris trichiura: 19.6%, Giardia lamblia: 17.9%). The estimated prevalence of diarrhea decreased with improvement of water supply and sanitation facilities to 45% and 44% respectively, but no statistically significant impact was observed in the case of parasitosis. School education and weaning practice were found to be other important determinants of diarrhea.

The natural distribution of thermotolerant Campylobacter sp. in dogs (150 stray animals and 64 pets) was studied. Campylobacters were more frequently isolated (p<0.01) from stray dogs (51.3%) rather than from pet dogs (21.9%). All the biotypes described by Lior for C. jejuni and C. coli were found among stray animals, whereas only C. jejuni biotypes I and II and C. coli biotype II were found among pet dogs. The need for more studies related to the role of environmental sanitary conditions in the spreading of Campylobacter species is noted.

A acetilcolina (ACh) é o neurotransmissor mais importante no controlo da motilidade gastrointestinal. A libertação de ACh dos neurónios entéricos é regulada por receptores neuronais específicos (De Man et al., 2003). Estudos prévios demonstraram que a adenosina exerce um papel duplo na libertação de ACh dos neurónios entéricos através da activação dos receptores inibitórios A1 e facilitatórios A2A (Duarte-Araújo et al., 2004). O potencial terapêutico dos compostos relacionados com a adenosina no controlo da motilidade e da inflamação intestinal, levou-nos a investigar o papel dos receptores com baixa afinidade para a adenosina, A2B e A3, na libertação de acetilcolina induzida por estimulação eléctrica nos neurónios mioentéricos. Estudos de imunolocalização mostraram que os receptores A2B exibem um padrão de distribuição semelhante ao do marcador de células gliais (GFAP). No que respeita aos receptores A1 e A3, estes encontram-se distribuídos principalmente nos corpos celulares dos neurónios ganglionares mioentéricos, enquanto os receptores A2A estão localizados predominantemente nos terminais nervosos colinérgicos. Neste trabalho mostrou-se que a modulação da libertação de ACh-[3H] (usando os antagonistas selectivos DPCPX, ZM241385 e MRS1191) é balanceada através da activação tónica dos receptores inibitórios (A1) e facilitatórios (A2A e A3) pela adenosina endógena. O antagonista selectivo dos receptores A2B, PSB603, não foi capaz de modificar o efeito inibitório da NECA (análogo da adenosina com afinidade para receptores A2). O efeito facilitatório do agonista dos receptores A3, 2-Cl-IB MECA (1-10 nM), foi atenuado pelo MRS1191 e pelo ZM241385, os quais bloqueiam respectivamente os receptores A3 e A2A. Contrariamente à 2-Cl-IB MECA, a activação dos receptores A2A pelo CGS21680C, atenuou a facilitação da libertação de ACh induzida pela activação dos receptores nicotínicos numa situação em que a geração do potencial de acção neuronal foi bloqueada pela tetrodotoxina. A localização diferencial dos receptores excitatórios A3 e A2A ao longo dos neurónios mioentéricos explica porque razão a estimulação dos receptores A3 (com 2-Cl-IB MECA) localizados nos corpos celulares dos neurónios mioentéricos exerce um efeito sinérgico com os receptores facilitatórios A2A dos terminais nervosos no sentido de aumentarem a libertação de ACh. Os resultados apresentados consolidam e expandem a compreensão actual da distribuição e função dos receptores da adenosina no plexo mioentérico do íleo de rato, e devem ser tidos em consideração para a interpretação de dados relativos às implicações fisiopatológicas da adenosina nos transtornos da motilidade intestinal.

Introduction: In the XXI Century ’s Society the scientific investigation process has been growing steadily , and the field of the pharmaceutical research is one of the most enthusiastic and relevant . Here, it is very important to correlate observed functional alterations with possibly modified drug bio distribution patterns . Cancer, inflammation and inf ection are processes that induce many molecular intermediates like cytokines, chemokines and other chemical complexes that can alter the pharmacokinetics of many drugs. One cause of such changes is thought to be the modulator action of these complexes in t he P - Glyco p rotein activity, because they can act like inducers/inhibitors of MDR - 1 expression. This protein results from the expression of MDR - 1 gene, and acts as an ATP energy - dependent efflux pump, with their substrates including many drugs , like antiretrovirals, anticancers, anti - infectives, immunosuppressants, steroids or opioids . Objectives: Because of the lack of methods to provide helpful information in the investigation of in vivo molecular changes in Pgp activity during infection/infl ammation processes, and its value in the explanation of the altered drug pharmacokinetic, this paper want to evaluate the potential utility of 99m Tc - Sestamibi scintigraphy during this kind of health sciences investigation. Although the a im is indeed to create a technique to the in vivo study of Pgp activity, this preliminary Project only reaches the in vitro study phase, assumed as the first step in a n evaluation period for a new tool development. Materials and Methods: For that reason , we are performing in vitro studies of influx and efflux of 99m Tc - Sestamibi ( that is a substrate of Pgp) in hepatocytes cell line (HepG2). We are interested in clarify the cellular behavior of this radiopharmaceutical in Lipopolysaccharide(LPS) stimulated cells ( well known in vitro model of inflammation) to possibly approve this methodology. To validate the results, the Pgp expression will be finally evaluated using Western Blot technique. Results: Up to this moment , we still don’t have the final results, but we have already enough data to let us believe that LPS stimulation induce a downregulation of MDR - 1, and consequently Pgp, which could conduce to a prolonged retention of 99m Tc - Sestamibi in the inflamed cells . Conclusions: If and when this methodology demonstrate the promising results we expect, one will be able to con clude that Nuclear Medicine is an important tool to help evidence based research also on this specific field .

O objetivo do estudo foi estimar as freqüências de tuberculose e parasitoses intestinais na em comunidades indígenas da localidade de Iauareté (AM), em 2001. Estudo transversal (n=333) visando à obtenção de dados demográficos e amostras biológicas para exames de escarro e fezes. Dentre os 43 sintomáticos respiratórios, seis foram positivos na pesquisa de bacilos álcool-ácido resistentes no escarro. As parasitoses intestinais apresentaram freqüência significativamente maior entre a população Hüpda do que entre os índios que habitam os demais bairros (37,5% vs. 19,3% para Ascaris lumbricoides, 32,4% vs. 16,3% para Trichuris trichiura, 75% vs. 19,3% para ancilostomídeos, 75% vs. 35,4% para Entamoeba histolyticaD dispar e 33,3% vs. 10,7% para Giardia lamblia). Conclui-se que a tuberculose e o parasitismo intestinal são freqüentes nessas comunidades, exigindo medidas de controle e melhorias na assistência à saúde.

IBD is a gastro-intestinal disorder marked with chronic inflammation of intestinal epithelium, damaging mucosal tissue and manifests into several intestinal and extra-intestinal symptoms. Currently used medical therapy is able to induce and maintain the patient in remission, however no modifies or reverses the underlying pathogenic mechanism. The research of other medical approaches is crucial to the treatment of IBD and, for this, it´s important to use animal models to mimic the characteristics of disease in real life. The aim of the study is to develop an animal model of TNBS-induced colitis to test new pharmacological approaches. TNBS was instilled intracolonic single dose as described by Morris et al. It was administered 2,5% TNBS in 50% ethanol through a catheter carefully inserted into the colon. Mice were kept in a Tredelenburg position to avoid reflux. On day 4 and 7, the animals were sacrificed by cervical dislocation. The induction was confirmed based on clinical symptoms/signs, ALP determination and histopathological analysis. At day 4, TNBS group presented a decreased body weight and an alteration of intestinal motility characterized by diarrhea, severe edema of the anus and moderate morbidity, while in the two control groups weren’t identified any alteration on the clinical symptoms/signs with an increase of the body weight. TNBS group presented the highest concentrations of ALP comparing with control groups. The histopathology analysis revealed severe necrosis of the mucosa with widespread necrosis of the intestinal glands. Severe hemorrhagic and purulent exsudates were observed in the submucosa, muscular and serosa. TNBS group presented clinical symptoms/signs and histopathological features compatible with a correct induction of UC. The peak of manifestations became maximal at day 4 after induction. This study allows concluding that it’s possible to develop a TNBS induced colitis 4 days after instillation.

A esquistossomose mansônica compromete vários órgãos, sendo o intestino e o fígado os mais agredidos. Com a intenção de verificar o comprometimento do intestino delgado, dependente da intensidade e do tempo de infecção pelo Schistosoma mansoni, analisou-se a atividade das dissacaridases — lactase, sacarase e maltase — em 112 camundongos, distribuídos em 3 grupos: grupo I — controle, grupo II — infestado com 30 cercárias, grupo III — infestado com 60 cercárias. Observamos uma diminuição da atividade lactásica, sacarásica e maltásica do intestino delgado, decorrente da infestação esquistossomdtica, do tempo de infestação e da alteração entre ambos. O íleo é o segmento que demonstrou maior sensibilidade a esquistossomose, tendo uma diminuição das suas dissacaridases a partir da fase inicial de infestação. Opostamente, o jejuno só mais tardiamente mostra essas alterações, exceto em relação a lactase. Detectou-se um aumento da atividade dissacaridásica, inclusive para a lactase, em todos os grupos, com a evolução etária dos animais, quantitativamente menor nos infestados. Cargas de 30 e 60 cercárias devem ser consideradas de mesmo porte, pois produziram ledução semelhante na atividade dissacaridásica.

This is a case report of intestinal cryptosporidiosis diagnosed in histological specimen collected from autopsy. The patient was a child of 5 months admitted to the hospital with severe acute diarrhea associated with Pneumocystis carinii pneumonia, cytomegalic sialadenitis, oral and dermal candidiasis. The presence of multiple opportunistic infections in this case indicated immunodeficiency state. Cryptosporidium sp is a possible etiology of acute diarrhea in both immunodeficient and immunocompetent patients and has to be searched for at autopsy when diagnosis was not possible "in vivo".
