Resveratrol (RESV) is a polyphenolic compound found in various plants, including grapes, berries and peanuts, and its processed foods as red wine. RESV possesses a variety of bioactivities, including antioxidant, anti-inflammatory, cardioprotective, antidiabetic, anticancer, chemopreventive, neuroprotective, renal lipotoxicity preventative, and renal protective effects. Numerous studies have demonstrated that polyphenols promote cardiovascular health. Furthermore, RESV can ameliorate several types of renal injury in animal models, including diabetic nephropathy, hyperuricemic, drug-induced injury, aldosterone-induced injury, ischemia-reperfusion injury, sepsis-related injury, and endothelial dysfunction. In addition, RESV can prevent the increase in vasoconstrictors, such as angiotensin II (AII) and endothelin-1 (ET-1), as well as intracellular calcium, in mesangial cells. Together, these findings suggest a potential role for RESV as a supplemental therapy for the prevention of renal injury.

La polykystose rénale autosomique dominante (PKRAD) est la maladie génétique rénale la plus commune touchant 1/500 personnes. Elle se caractérise principalement par la formation de kystes rénaux dans tous les segments du néphron, entraînant l’insuffisance rénale, et par des manifestations extrarénales kystiques (foie, pancréas, rate) et non-kystiques (anomalies cardiaques, vasculaires et cérébrales). Deux gènes, PKD1 et PKD2, sont responsables de 85 et 15% des cas respectivement. Ces gènes encodent les polycystine-1 (PC-1) et -2 (PC-2) qui forment un complexe à la membrane plasmique et ciliaire des cellules épithéliales rénales. PC-1 est une protéine transmembranaire de 4302 acides aminés possédant un court domaine intracellulaire incluant un motif coiled-coil impliqué dans l’interaction entre PC-1 et PC-2 in-vitro. L’importance du coiled-coil est démontrée par des mutations affectant spécifiquement ce motif chez des patients PKRAD. Le mécanisme pathogénétique responsable de la PKRAD est indéterminé. Chez la souris, la PKRAD se développe suite à l’ablation (Pkd1-/-) ou lors de la surexpression (SBPkd1TAG) de Pkd1, ce qui suggère un effet de dosage. Des anomalies ciliaires sont aussi souvent associées à PKRAD. Mon objectif était de déterminer in-vivo le mécanisme pathogénétique de la polycystine-1 dans le développement des symptômes PKRAD rénaux et extrarénaux et plus spécifiquement, le rôle du motif coiled-coil dans le mécanisme de kystogenèse. Pour ce faire, nous avons généré deux constructions, Pkd1 sauvage (Pkd1TAG) et Pkd1 tronquée de son motif coiled-coil (Pkd1ΔCoiled-coil), par recombinaison homologue à partir du BAC-Pkd1 sauvage comprenant la séquence murine entière de Pkd1. Trois lignées de souris Pkd1TAG générées par microinjection démontrent un niveau d’expression de Pkd1 qui corrèle avec le nombre de copie du transgène (2, 5 et 15 copies). Les souris Pkd1TAG reproduisent la PKRAD en développant des kystes rénaux dans toutes les parties du néphron et des cils primaires plus longs que les contrôles non transgéniques. Les analyses physiologiques supportent que les souris Pkd1TAG développent une insuffisance rénale et démontrent une augmentation du volume urinaire de même qu’une diminution de l’osmolalité, de la créatinine et des protéines urinaires. De plus, les souris Pkd1TAG développent des kystes hépatiques, des anomalies cardiaques associées à des dépôts de calcium et des anévrismes cérébraux. La sévérité du phénotype augmente avec l’expression de Pkd1 appuyant l’hypothèse d’un mécanisme de dosage. Nous avons aussi déterminé que l’expression du transgène Pkd1TAG complémente le phénotype létal-embryonnaire des souris Pkd1-/-. D’autre part, nous avons générés 4 lignées de souris Pkd1ΔCoiled-coil (2 et 15 copies du transgène) dont le nombre de copies corrèle avec le niveau d’expression du transgène. Ces souris Pkd1ΔCoiled-coil, contrairement aux Pkd1TAG de même âge, ne développent pas de kystes et possèdent des cils primaires de longueur normale. Afin d’évaluer le rôle du motif coiled-coil en absence de polycystine-1 endogène, nous avons croisé les souris Pkd1ΔCoiled-coil avec les souris Pkd1-/-. Contrairement aux souris Pkd1-/- qui meurent in-utéro, les souris Pkd1ΔCoiled-coil; Pkd1-/- survivent ~10 à 14 jours après la naissance. Elles démontrent des kystes rénaux et pancréatiques sévères, un retard de croissance et des anomalies pulmonaires. Tous les segments du néphron sont affectés. Mon projet démontre que la surexpression de Pkd1 est un mécanisme pathogénique de la PKRAD tant au niveau rénal qu’extrarénal. De plus, il démontre que le motif coiled-coil est un élément déterminant dans la kystogenèse/PKRAD in-vivo.

Introducción: El trasplante hepático es la terapia de elección para los pacientes con enfermedad hepática terminal, logrando mejorar su expectativa y calidad de vida, de acuerdo a estudios realizados en otros países. En la Fundación Cardioinfantil – Instituto de Cardiología (FCI-IC) se han realizado 332 trasplantes hepáticos hasta el 2014, pero no se conoce la supervivencia y los factores pronósticos propios de los pacientes intervenidos. Objetivo Principal: Estimar la supervivencia a 1, 3 y 5 años e identificar los principales factores pronósticos de los pacientes a quienes se les realizó trasplante hepático en el periodo 2005-2013 en la FCI-IC. Método: Estudio observacional y retrospectivo, basado en revisión de historias clínicas de los pacientes adultos a quienes se les realizó trasplante hepático en el periodo 2005-2013 en la FCI-IC. Resultados: La supervivencia al año fue de 90.91% (IC95% 86.40-93.98), a los 3 años 83.64% (IC95% 77.89-88.01) y a los 5 años de 79.18% (IC95% 72.54-84.39). Los principales factores pronósticos fueron el antecedente de ascitis (HR 2.449, IC 1.252 – 4.792), la edad del donante (HR 1.040, IC 1.009 – 1.071) y el receptor (HR 1.037, IC 1.014 – 1.060). Se encontró una mayor supervivencia en los pacientes con cirrosis alcohólica (HR 0.099, IC 0.021 – 0.467). Conclusiones: El estudio mostró una supervivencia mayor a la reportada en estudios realizados en Estados Unidos (67.4-73.0% a los 5 años) y España (73,3% a los 3 años) y similar a la de Chile (80.0% a los 5 años). Cabe resaltar que estos estudios incluyeron series más grandes de pacientes.

Focal and segmental glomerulosclerosis (FSGS) is one of the most important causes of end-stage renal failure. The bradykinin B1 receptor has been associated with tissue inflammation and renal fibrosis. To test for a role of the bradykinin B1 receptor in podocyte injury, we pharmacologically modulated its activity at different time points in an adriamycin-induced mouse model of FSGS. Estimated albuminuria and urinary protein to creatinine ratios correlated with podocytopathy. Adriamycin injection led to loss of body weight, proteinuria, and upregulation of B1 receptor mRNA. Early treatment with a B1 antagonist reduced albuminuria and glomerulosclerosis, and inhibited the adriamycin-induced downregulation of podocin, nephrin, and alpha-actinin-4 expression. Moreover, delayed treatment with antagonist also induced podocyte protection. Conversely, a B1 agonist aggravated renal dysfunction and even further suppressed the levels of podocyte-related molecules. Thus, we propose that kinin has a crucial role in the pathogenesis of FSGS operating through bradykinin B1 receptor signaling. Predictive models for chronic renal disease using decision trees, naïve bayes and case-based methods

Data mining can be used in healthcare industry to “mine” clinical data to discover hidden information for intelligent and affective decision making. Discovery of hidden patterns and relationships often goes intact, yet advanced data mining techniques can be helpful as remedy to this scenario. This thesis mainly deals with Intelligent Prediction of Chronic Renal Disease (IPCRD). Data covers blood, urine test, and external symptoms applied to predict chronic renal disease. Data from the database is initially transformed to Weka (3.6) and Chi-Square method is used for features section. After normalizing data, three classifiers were applied and efficiency of output is evaluated. Mainly, three classifiers are analyzed: Decision Tree, Naïve Bayes, K-Nearest Neighbour algorithm. Results show that each technique has its unique strength in realizing the objectives of the defined mining goals. Efficiency of Decision Tree and KNN was almost same but Naïve Bayes proved a comparative edge over others. Further sensitivity and specificity tests are used as statistical measures to examine the performance of a binary classification. Sensitivity (also called recall rate in some fields) measures the proportion of actual positives which are correctly identified while Specificity measures the proportion of negatives which are correctly identified. CRISP-DM methodology is applied to build the mining models. It consists of six major phases: business understanding, data understanding, data preparation, modeling, evaluation, and deployment.

O transplante hepático é o tratamento de escolha para uma série de doenças terminais agudas e crônicas do fígado. Contudo, sua oferta tem sido restringida pela falta de doadores, o que tem provocado o aumento do número de pacientes em lista de espera. A escassez de órgãos condiciona a aceitação para transplante de enxertos provindos de doadores sem as melhores condições para tal – os chamados doadores marginais. O dano de isquemia/reperfusão (IR) é resultado dos fatores perioperatórios inerentes ao procedimento, incluindo as condições do doador. Quanto pior o doador, pior o órgão transplantado, e maior a possibilidade de desenvolvimento de disfunção primária do enxerto (DPE). DPE comumente é definida pela elevação das enzimas hepáticas. As aminotransferases, entretanto, podem alterar-se por outras complicações que não a lesão de isquemia/reperfusão. A histologia hepática, por sua vez, pode fornecer informações acerca da IR. Com o objetivo de estimar a extensão histológica do dano de preservação (necrose hepatocelular e neutrofilia sinusoidal), correlacioná-la a variáveis bioquímicas (índice de reperfusão: AST + ALT + LDH / 3) e avaliar a sua influência no período pós-operatório imediato (até 7 dias), foi realizado um estudo transversal com análise sistemática de 55 pacientes adultos que receberam seu primeiro enxerto hepático entre Setembro de 1996 e Dezembro de 1999. Foram comparados os fatores de risco relacionados ao doador, ao receptor, ao procedimento cirúrgico e ao período pós-operatório e analisadas as biópsias feitas antes e imediatamente após o procedimento cirúrgico. Houve dano de preservação em todos os pacientes estudados tanto por critérios anatomopatológicos quanto por critérios bioquímicos. Houve associação significativa entre os achados bioquímicos e histológicos (p=0,04; coeficiente gamma=0,49). A extensão da necrose hepatocitária parece ser o dado anatomopatológico isolado que melhor se relaciona ao índice de reperfusão (p=0,05; coeficiente gamma=0,48). Houve associação entre DPE e a histologia hepática (p=0,02). O índice bioquímico associou-se à DPE (p=0,001) e à incidência de insuficiência renal aguda (IRA) (p<0,0001). A mortalidade inicial foi maior nos pacientes com índice de reperfusão grave (p=0,002). O índice de reperfusão foi um fator de risco independente para a função do enxerto (p=0,004) e IRA (0,04). A sobrevida atuarial em 1 ano foi significativamente menor nos pacientes com dano de preservação grave (p=0,003). A análise da biópsia de reperfusão é capaz de detectar o dano de preservação sofrido pelo enxerto e se correlaciona às variáveis bioquímicas em sua estimativa.

PURPOSE: Malnutrition is prevalent in patients with advanced liver disease (LD) related to multifactorial causes. Fluid retention can underestimate the nutritional status based on anthropometric measures. We evaluated nutritional indicators and body composition (BC) in patients with liver cirrhosis and correlated them with LD severity. METHODS: Forty three patients with LD enrolled for liver transplantation were evaluated by Anthropometric measures, subjective evaluation (Global Assessment of Nutritional Status - SGA) and biochemical indicators. Single-frequency electrical bioimpedance (SFE-BIA) was used to evaluate body composition (BC). It measured resistance (R), reactance (Xc) and the phase angle (PA). LD severity was estimated by Child-Pugh and Meld criteria (Model for End-Stage Liver Disease). RESULTS: Child-Pugh index between patients was 7.11±1.70 and Meld was 12.23±4.22. Arm Circumference, Arm Muscle Circumference and Arm Muscle Area, SGA, hemoglobin, hematocrit and albumin showed better correlation with disease severity. Xc and PA showed correlation both with Meld and Child-Pugh score when BC were evaluated. PA was depleted in 55.8% of the patients. CONCLUSIONS: Diagnosis of malnutrition varied according to the method. Global assessment of nutritional status showed better correlation with disease severity than with objective methods. Single-frequency electrical bioimpedance for body composition analysis in cirrhotic patients must be cautiously used; however, primary vectors seems to be valid and promising in clinical practice.

With the improvement in quality of life of animals, it is increasingly frequent clinical care of elderly patients, which present renal disorders, including chronic renal failure. Recent studies report the use of stem cells to treat renal failure, which would improve the levels of urea and creatinine, and in renal ultrasound evaluation. With the present work, the idea is to report a case of ultrasonographic evaluation in a patient with chronic renal failure, liver disease and splenic nodule, which underwent stem cell therapy, where there was an improvement in the sonographic evaluation of part of the liver.

Tamoxifen, a selective estrogen receptor modulator, has antifibrotic properties; however, whether it can attenuate renal fibrosis is unknown. In this study, we tested the effects of tamoxifen in a model of hypertensive nephrosclerosis (chronic inhibition of nitric oxide synthesis with L-NAME). After 30 days, treated rats had significantly lower levels of albuminuria as well as lower histologic scores for glomerulosclerosis and interstitial fibrosis than untreated controls. Tamoxifen was renoprotective despite having no effect on the sustained, severe hypertension induced by L-NAME. Tamoxifen prevented the accumulation of extracellular matrix by decreasing the expression of collagen I, collagen III, and fibronectin mRNA and protein. These renoprotective effects associated with inhibition of TGF-beta 1 and plasminogen activator inhibitor-1, and with a significant reduction in a-smooth muscle actin-positive cells in the renal interstitium. Furthermore, tamoxifen abrogated IL-1 beta- and angiotensin-II-induced proliferation of fibroblasts from both kidney explants and from the NRK-49F cell line. Tamoxifen also inhibited the expression of extracellular matrix components and the production and release of TGF-beta 1 into the supernatant of these cells. In summary, tamoxifen exhibits antifibrotic effects in the L-NAME model of hypertensive nephrosclerosis, likely through the inhibition of TGF-beta 1, suggesting that it may have therapeutic use in CKD treatment.

Background: Altered levels of matrix metalloproteinases (MMPs) and their inhibitors, the tissue inhibitors of metalloproteinases (TIMPs), are involved in cardiovascular alterations associated with end stage kidney disease (ESKD). Genetic polymorphisms in MMP-9 gene affect MMP-9 levels. We examined how MMP-9 polymorphisms and haplotypes affect the changes in plasma MMP-9 and TIMP-1 levels found in patients with ESKD undergoing hemodialysis. Methods: We studied 94 ESKD patients undergoing hemodialysis for at least 3 months. MMP-9 and TIMP-1 were measured by ELISA in plasma from blood samples collected before and after a session of hemodialysis. Genotypes for three MMP-9 polymorphisms (C-1562T, rs3918242; -90 (CA)(14-24), rs2234681; and Q279R, rs17576) were determined by Taqman (R) Allele Discrimination Assay and real-time polymerase chain reaction. Haplotype frequencies were determined with the software program PHASE 2.1. Results: Hemodialysis increased MMP-9 and TIMP-1 levels (P<0.05). Genotypes had no effects on baseline MMP-9 and TIMP-1 levels (P>0.05). Hemodialysis increased MMP-9 and TIMP-1 levels in subjects with the CC (but not CT or TT) genotype for the C-1562T polymorphism (P<0.05), and increased MMP-9 levels in subjects with the QQ (but not QR or RR) genotype for the Q279R polymorphism (P<0.05), whereas the CA(n)(14-24) polymorphism had no major effects. While MMP-9 haplotypes had no effects on baseline MMP-9 levels (P>0.05), hemodialysis increased MMP-9 levels and MMP-9/TIMP-1 ratios in subjects carrying the CLQ haplotype (P = 0.0012 and P = 0.0045, respectively). Conclusion: ESKD patients with the QQ genotype for the Q279R polymorphism or with the CLQ haplotype are exposed to more severe increases in MMP-9 levels after hemodialysis. Adipose tissue-derived stem cells (ASCs) are an attractive source of stem cells with regenerative properties that are similar to those of bone marrow stem cells. Here, we analyze the role of ASCs in reducing the progression of kidney fibrosis. Progressive renal fibrosis was achieved by unilateral clamping of the renal pedicle in mice for 1 h; after that, the kidney was reperfused immediately. Four hours after the surgery, 2 x 10(5) ASCs were intraperitoneally administered, and mice were followed for 24 h posttreatment and then at some other time interval for the next 6 weeks. Also, animals were treated with 2 x 10(5) ASCs at 6 weeks after reperfusion and sacrificed 4 weeks later to study their effect when interstitial fibrosis is already present. At 24 h after reperfusion, ASC-treated animals showed reduced renal dysfunction and enhanced regenerative tubular processes. Renal mRNA expression of IL-6 and TNF was decreased in ASC-treated animals, whereas IL-4. IL-10, and HO-1 expression increased despite a lack of ASCs in the kidneys as determined by SRY analysis. As expected, untreated kidneys shrank at 6 weeks, whereas the kidneys of ASC-treated animals remained normal in size, showed less collagen deposition, and decreased staining for FSP-1, type I collagen, and Hypoxyprobe. The renal protection seen in ASC-treated animals was followed by reduced serum levels of TNF-alpha, KC, RANTES, and IL-1 alpha. Surprisingly, treatment with ASCs at 6 weeks, when animals already showed installed fibrosis, demonstrated amelioration of functional parameters, with less tissue fibrosis observed and reduced mRNA expression of type I collagen and vimentin. ASC therapy can improve functional parameters and reduce progression of renal fibrosis at early and later times after injury, mostly due to early modulation of the inflammatory response and to less hypoxia, thereby reducing the epithelial-mesenchymal transition.

Background Oxidative stress is recognized as a major pathogenic factor of cellular damage caused by hyperglycemia. NOX/NADPH oxidases generate reactive oxygen species and NOX1, NOX2 and NOX4 isoforms are expressed in kidney and require association with subunit p22phox (encoded by the CYBA gene). Increased expression of p22phox was described in animal models of diabetic nephropathy. In the opposite direction, glutathione is one of the main endogenous antioxidants whose plasmatic concentrations were reported to be reduced in diabetes patients. The aim of the present investigation was to test whether functional single nucleotide polymorphisms (SNPs) in genes involved in the generation of NADPH-dependent O2•- (-675 T → A in CYBA, unregistered) and in glutathione metabolism (-129 C → T in GCLC [rs17883901] and -65 T → C in GPX3 [rs8177412]) confer susceptibility to renal disease in type 1 diabetes patients. Methods 401 patients were sorted into two groups according to the presence (n = 104) or absence (n = 196) of overt diabetic nephropathy or according to glomerular filtration rate (GFR) estimated by Modification of Diet in Renal Disease (MDRD) equation: ≥ 60 mL (n = 265) or < 60 mL/min/1.73 m2 (n = 136) and were genotyped. Results No differences were found in the frequency of genotypes between diabetic and non-diabetic subjects. The frequency of GFR < 60 mL/min was significantly lower in the group of patients carrying CYBA genotypes T/A+A/A (18.7%) than in the group carrying the T/T genotype (35.3%) (P = 0.0143) and the frequency of GFR < 60 mL/min was significantly higher in the group of patients carrying GCLC genotypes C/T+T/T (47.1%) than in the group carrying the C/C genotype (31.1%) (p = 0.0082). Logistic regression analysis identified the presence of at least one A allele of the CYBA SNP as an independent protection factor against decreased GFR (OR = 0.38, CI95% 0.14-0.88, p = 0.0354) and the presence of at least one T allele of the GCLC rs17883901 SNP as an independent risk factor for decreased GFR (OR = 2.40, CI95% 1.27-4.56, p = 0.0068). Conclusions The functional SNPs CYBA -675 T → A and GCLC rs17883901, probably associated with cellular redox imbalances, modulate the risk for renal disease in the studied population of type 1 diabetes patients and require validation in additional cohorts.

OBJECTIVE: To compare low and high MELD scores and investigate whether existing renal dysfunction has an effect on transplant outcome. METHODS: Data was prospectively collected among 237 liver transplants (216 patients) between March 2003 and March 2009. Patients with cirrhotic disease submitted to transplantation were divided into three groups: MELD > 30, MELD < 30, and hepatocellular carcinoma. Renal failure was defined as a ± 25% decline in estimated glomerular filtration rate as observed 1 week after the transplant. Median MELD scores were 35, 21, and 13 for groups MELD > 30, MELD < 30, and hepatocellular carcinoma, respectively. RESULTS: Recipients with MELD > 30 had more days in Intensive Care Unit, longer hospital stay, and received more blood product transfusions. Moreover, their renal function improved after liver transplant. All other groups presented with impairment of renal function. Mortality was similar in all groups, but renal function was the most important variable associated with morbidity and length of hospital stay. CONCLUSION: High MELD score recipients had an improvement in the glomerular filtration rate after 1 week of liver transplantation.

End-stage ankle arthritis should have an appropriate classification to assist surgeons in the management of end-stage ankle arthritis. Outcomes research also requires a classification system to stratify patients appropriately.
