A esofagectomia trans-hiatal oferece a vantagem de não necessitar da toracotomia ou toracoscopia. Apresenta a desvantagem de ter que ser realizada, pelo menos em parte, com dissecação romba, às cegas, ocorrendo com frequência lesão pleural, maior sangramento, entre outras complicações. A associação da transecção mediana do diafragma com a esternotomia parcial permite o isolamento do esôfago totalmente sob visão direta. Os autores apresentam a técnica da esofagectomia trans-hiatal com esternotomia parcial.

ABSTRACTObjective:to investigate the effect of standardized interventions in the management of tube thoracostomy patients and to assess the independent effect of each intervention.Methods:A chest tube management protocol was assessed in a retrospective cohort study. The tube thoracostomy protocol (TTP) was implemented in August 2012, and consisted of: antimicrobial prophylaxis, chest tube insertion in the operating room (OR), admission post chest tube thoracostomy (CTT) in a hospital floor separate from the emergency department (ED), and daily respiratory therapy (RT) sessions post-CTT. The inclusion criteria were, hemodynamic stability, patients between the ages of 15 and 59 years, and injury severity score (ISS) < 17. All patients had isolated injuries to the chest wall, lung, and pleura. During the study period 92 patients were managed according to the standardized protocol. The outcomes of those patients were compared to 99 patients treated before the TTP. Multivariate logistic regression analysis was performed to assess the independent effect of each variable of the protocol on selected outcomes.Results:Demographics, injury severity, and trauma mechanisms were similar among the groups. As expected, protocol compliance increased after the implementation of the TTP. There was a significant reduction (p<0.05) in the incidence of retained hemothoraces, empyemas, pneumonias, surgical site infections, post-procedural complications, hospital length of stay, and number of chest tube days. Respiratory therapy was independently linked to significant reduction (p<0.05) in the incidence of seven out of eight undesired outcomes after CTT. Antimicrobial prophylaxis was linked to a significant decrease (p<0.05) in retained hemothoraces, despite no significant (p<0.10) reductions in empyema and surgical site infections. Conversely, OR chest tube insertion was associated with significant (p<0.05) reduction of both complications, and also significantly decreased the incidence of pneumonias.Conclusion:Implementation of a TTP effectively reduced complications after CTT in trauma patients.

Objective: To evaluate patients with chest trauma submitted to videothoracoscopy during hospitalization. In 2007, the Trauma Surgery Group was created in the General Surgery Department of the Hospital Municipal Lourenço Jorge of Rio de Janeiro-RJ, and started following all trauma victims who were admitted to the Hospital. Methods : We conducted a retrospective analysis of patients submitted to thoracoscopy from July 2007 to May 2015, based on a database started at the beginning of this period and on data collection from patients who underwent thoracoscopy. We evaluated the following parameters: procedure effectiveness, indication of the procedure, conversion rate, complications and mortality. We included patients who presented post-traumatic pleural collections, such as retained hemothorax and pleural empyema, and penetrating injury in the thoracoabdominal transition. All patients were hemodynamic stable and signed an informed consent. Results: In the analyzed period 53 patients were submitted to videothoracoscopy; 24 had penetrating trauma (45.3%) and 29, blunt (54.7%), with a predominance of males (75.5%). The procedure was performed in 26 cases of retained hemothorax (49%), 14 cases of empyema (26.5%) and in 13 patients for evaluation of injury in the thoracoabdominal transition (24.5%). The thoracoscopy was effective in resolution of 36 cases (80%), without need for further procedure. There was a conversion rate of 15.5% and 3 procedure complications related (6.6%). Mortality was nil. Conclusion: In this series, videothoracoscopy proved that this diagnostic and therapeutic procedure is safe and effective, if performed by a surgeon with appropriate training, especially when it is indicated in cases of retained hemothorax and evaluation of penetrating thoracoabdominal trauma.

Objective: to assess the prevalence of abnormalities found by computed tomography (CT) of the chest in patients with squamous cell carcinoma of the head and neck. Methods: we retrospectively analyzed chest CT exams of 209 patients with squamous cell carcinoma of the head and neck. The CT findings were stratified as inflammatory / infectious, parenchymal, nodular uncharacteristic and nodular metastatic / tumoral Results: alterations were diagnosed in 66.6% of patients. Of these, 25.3% represented emphysema; 18.8%, uncharacteristic micronodules; 12.9%, metastases; 11.9%, thoracic lymph node enlargements; and in 6.6% we detected active pulmonary tuberculosis or its sequelae, pneumonia or inflammatory / infectious signs and pleural thickening or effusion. Conclusion: the prevalence of exams with alterations and the considerable rate of detected metastases indicate that chest CT should be required for diagnostic and / or staging in cases of head and neck cancer.

OBJETIVO: Estudar se a artrite reumatoide (AR) sofre influências de idade de menarca, número de gestações e tempo de vida reprodutiva. MÉTODOS: Estudo transversal e retrospectivo dos prontuários de 247 pacientes com AR, cujos dados sobre menarca, menopausa, número de gestações, autoanticorpos, serosites, nódulos reumatoides e índice funcional de Steinbrocker foram coletados. Os estudos de associação foram feitos pelos testes t de Student e Mann Whitney; os de correlação, pelos testes de Pearson e de Spearman. A significância adotada foi de 5%. RESULTADOS: A idade média de diagnóstico da AR foi de 43,2±14,1 anos, a idade mediana da menarca de 13 anos e o número mediano de gestações foi de 3. Em 63,9% existia fator reumatoide; o fator antinuclear em 20%; os nódulos reumatoides em 8,8%; derrame pleural em 2,8% e pericardite em 2,4%. O índice funcional de Steinbrocker mostrou que 45,6% tinham escore 1; 40,8% o escore 2; 9,1% o escore 3 e 4,3% o escore 4. Encontrou-se correlação inversa entre a quantidade de gestações e idade de surgimento da AR (p CONCLUSÃO: A precocidade da menarca e brevidade da vida reprodutiva indicam pior prognóstico, já que se relacionam com pleurites. A maior quantidade de gestações e menopausa tardia mostram efeito protetor, retardando o aparecimento da doença.

A distrofia muscular de Duchenne (DMD) é uma doença de origem genética, cuja principal manifestação clínica é enfraquecimento e atrofia progressiva dos músculos. Os cães da raça Golden Retriever podem apresentar distrofia muscular, com características genotípicas e fenotípicas muito próximas à distrofia muscular humana, sendo considerado o modelo animal mais apropriado para o estudo da DMD. Foram realizadas radiografias torácicas látero-laterais e dorsoventrais de 10 cães Golden Retriever afetados pela distrofia muscular, com o objetivo de relatar as alterações radiográficas associadas a essa patologia. O exame radiográfico da cavidade torácica evidenciou: (a) padrão pulmonar intersticial e alveolar predominante, (b) um quadro de pneumonia e edema pulmonar em fase inicial, (c) a cardiomegalia como o principal achado de comprometimento circulatório na cavidade torácica, (d) O megaesôfago torácico foi observado deslocando a traquéia e silhueta cardíaca ventralmente e, (e) a cúpula diafragmática apresentou modificação morfológica, mostrando protrusão para o interior da cavidade torácica e hérnia hiatal, com deslocamento do estômago para o espaço mediastino caudal. Os achados de necropsia evidenciaram efusão pleural e enfisema pulmonar e lesões compatíveis com processos degenerativos e metaplásicos da musculatura diafragmática e intercostal. A avaliação radiográfica constituiu-se como um meio diagnóstico auxiliar essencial na identificação de doença cardíaca e respiratória em cães Golden Retriever acometidos pela Distrofia Muscular, capaz de identificar processos pneumônicos primários, permitindo o estabelecimento de terapêutica adequada de tratamento, com prognóstico reservado nos estágios mais avançados desta alteração.

The present study was carried out in order to determine the effect of lung resection on the frequency of infections in alloxan-diabetic rats. Adult female Wistar rats were injected with alloxan (40 mg/kg, iv) to induce diabetes mellitus (group D; N = 45) or with vehicle (1.0 ml/kg, iv) to be used as controls (group C; N = 45). Thirty-six days after receiving alloxan both groups were randomly divided into three subgroups: no operation (NO; N = 15), sham operation (SO; N = 15), and left pneumonectomy (PE; N = 15). The rats were sacrificed 36 days after surgery and their lungs were examined microscopically and macroscopically. The occurrence of thoracic wall infection, thoracic wall abscess, lung abscess and pleural empyema was similar in groups D and C. In contrast, the overall infection rate was higher (P<0.05) in the diabetic rats (SO-D and PE-D subgroups, but not in the NO-D subgroup). Considering that the overall infection rate was similar in the SO-D and PE-D subgroups, we suggest that surgery but not pneumonectomy was related to the higher prevalence of infection in diabetic rats.

Myrtaceae is a plant family widely used in folk medicine and Syzygium and Eugenia are among the most important genera. We investigated the anti-allergic properties of an aqueous leaf extract of Syzygium cumini (L.) Skeels (SC). HPLC analysis revealed that hydrolyzable tannins and flavonoids are the major components of the extract. Oral administration of SC (25-100 mg/kg) in Swiss mice (20-25 g; N = 7/group) inhibited paw edema induced by compound 48/80 (50% inhibition, 100 mg/kg; P <= 0.05) and, to a lesser extent, the allergic paw edema (23% inhibition, 100 mg/kg; P <= 0.05). SC treatment also inhibited the edema induced by histamine (58% inhibition; P <= 0.05) and 5-HT (52% inhibition; P <= 0.05) but had no effect on platelet-aggregating factor-induced paw edema. SC prevented mast cell degranulation and the consequent histamine release in Wistar rat (180-200 g; N = 7/group) peritoneal mast cells (50% inhibition, 1 µg/mL; P <= 0.05) induced by compound 48/80. Pre-treatment of BALB/c mice (18-20 g; N = 7/group) with 100 mg/kg of the extract significantly inhibited eosinophil accumulation in allergic pleurisy (from 7.662 ± 1.524 to 1.89 ± 0.336 x 10(6)/cavity; P <= 0.001). This effect was related to the inhibition of IL-5 (from 70.9 ± 25.2 to 12.05 ± 7.165 pg/mL) and CCL11/eotaxin levels (from 60.4 ± 8.54 to 32.8 ± 8.4 ng/mL) in pleural lavage fluid, using ELISA. These findings demonstrate an anti-allergic effect of SC, and indicate that its anti-edematogenic effect is due to the inhibition of mast cell degranulation and of histamine and serotonin effects, whereas the inhibition of eosinophil accumulation in the allergic pleurisy model is probably due to an impairment of CCL11/eotaxin and IL-5 production.

Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) is an emergent pathogen in Brazil. However, there are no data on the prevalence of CA-MRSA. We report here the first well-characterized case of severe life-threatening CA-MRSA infection in a child living in Rio de Janeiro city. The patient had many complications including hematogenous osteomyelitis and involvement of multiple sites requiring drainage of soft-tissue abscess, and pleural and pericardial empyema. The MRSA isolates recovered were genotyped using PFGE, SCCmec typing and multilocus sequence typing. Disk diffusion tests were performed following Clinical and Laboratory Standards Institute recommendations. In addition, the presence of Panton-Valentine leukocidin (PVL) was assessed by PCR amplification, using specific primers for lukF-pv (encoding for the F subunit of the PVL). The bacterial isolates were related to the ST30-SCCmecIV lineage (Oceania Southwest Pacific clone), a PVL producer CA-MRSA previously detected in Porto Alegre, RS, Brazil. Also, the isolates analyzed were susceptible to all non-β-lactam antibiotics tested. The present report demonstrates that disseminated CA-MRSA disease is also occurring in Rio de Janeiro. Thus, the empirical treatment of moderate or severe infections suspected of being associated with CA-MRSA needs to be reviewed in order to allow prompt initiation of an effective therapy that also covers these microorganisms.

En démontrant sa capacité d’identifier les pneumothorax, de différencier les différentes causes d’insuffisance respiratoire chez les patients dyspnéiques et de confirmer la position d’un tube endotrachéal lors d’une intubation endotrachéale, l’échographie pulmonaire a pris une place prépondérante dans la prise en charge des patients de soins critiques. La majorité des études, notamment celles sur l’intubation endotrachéale, ont évalué la performance de cliniciens possédant une expérience considérable en échographie pulmonaire et souvent dans un cadre idéal permettant des examens d’une durée prolongée. Considérant la disponibilité grandissante de l’échographie ciblée lors des situations de stabilisation et de réanimation des patients de soins critiques, nous voulions évaluer la capacité d’un groupe de clinicien hétérogène en termes de formation échographique à identifier la présence ou l’absence de glissement pleural sur de courtes séquences (comparable à la durée probable d’un examen lors de condition de réanimation) d’échographie pulmonaire enregistrées chez des patients intubés. Un total de 280 courtes séquences (entre 4 et 7 secondes) d’échographie pulmonaire démontrant la présence ou l’absence de glissement pleural chez des patients intubés en salle d’opération ont été enregistrées puis présentées de façon aléatoire à deux groupes de cliniciens en médecine d’urgence. Le deuxième groupe avait la possibilité de s’abstenir advenant une incertitude de leur réponse. Nous avons comparé la performance selon le niveau de formation académique et échographique. Le taux moyen d’identification adéquate de la présence ou l’absence du glissement pleural par participant était de 67,5% (IC 95% : 65,7-69,4) dans le premier groupe et 73,1% (IC 95% : 70,7-75,5) dans le second (p<0,001). Le taux médian de réponse adéquate pour chacune des 280 séquences était de 74,0% (EIQ : 48,0-90,0) dans le premier groupe et 83,7% (EIQ : 53,3-96,2) dans le deuxième (p=0,006). Le taux d’identification adéquate de la présence ou absence d’un glissement pleural par les participants des deux groupes était nettement supérieur pour les séquences de l’hémithorax droit par rapport à celles de l’hémithorax gauche (p=0,001). Lorsque des médecins de formation académique et échographique variable utilisent de courtes séquences d’échographie pulmonaire (plus représentatives de l’utilisation réelle en clinique), le taux d’identification adéquate de la présence ou l’absence de glissement pleural est plus élevé lorsque les participants ont la possibilité de s’abstenir en cas de doute quant à leur réponse. Le taux de bonnes réponses est également plus élevé pour les séquences de l’hémithorax droit, probablement dû à la présence sous-jacente du cœur à gauche, la plus petite taille du poumon gauche et l’effet accru du pouls pulmonaire dans l’hémithorax gauche. Considérant ces trouvailles, la prudence est de mise lors de l’utilisation de l’identification du glissement pleural sur de courtes séquences échographique comme méthode de vérification de la position d’un tube endotrachéal lors d’une intubation endotrachéale, et ce, particulièrement pour l’hémithorax gauche. Aussi, une attention particulière devrait être mise sur la reconnaissance du pouls pulmonaire lors de l’enseignement de l’échographie pulmonaire.

Objetivos : evaluar las características operativas del examen físico en el diagnóstico de neumonía y evaluar su acuerdo inter-observador. Marco de referencia : los estudios que avaluaron al examen físico como prueba diagnóstica en neumonía son metodológicamente deficientes. Diseño : estudio ciego de corte transversal para evaluación de prueba diagnóstica. Pacientes : adultos quienes consultan al servicio de urgencias y hospitalización de la FCI por síntomas respiratorios agudos o exacerbación de los mismos. Mediciones : examen físico por dos observadores independientes, toma de radiografía de tórax y lectura por radiólogo experto. Se tomaron los datos que permitieron calcular el índice de severidad de neumonía (PSI). Resultados : de 198 pacientes, 85(42%) tenían neumonía radiográficamente. Las características operativas del examinador1 fueron: Sensibilidad:63.2%, Especificidad:54,1%, LR(+)=1,36, LR(-)=0,68; para el examinador2: Sensibilidad:34,3%, Especificidad:71,7%, LR(+)=1,17, LR(-)=0,92. La correlación entre diagnóstico clínico para derrame pleural fue k=-0,052, no significativa (p=0,445); y para neumonía k=0.25 significativa (p=0.022). Al medirse la severidad de neumonía por PSI, la sensibilidad aumento estratificada a severidad (II:Sensibilidad:40%; III:Sensibilidad: 57%; IV:Sensibilidad;75%; V:Sensibilidad:80%). Conclusiones : el examen físico no es sensible ni especifico en el diagnóstico de neumonía. Existe un índice de acuerdo débil en el examen físico de tórax para el diagnóstico de derrame pleural y neumonía Es más probable el diagnóstico clínico de neumonía al aumentar la severidad por PSI.

Objective. This study was performed to determine the prevalence of and associated risk factors for cardiovascular disease (CVD) in Latin American (LA) patients with systemic lupus erythematosus (SLE). Methods. First, a cross-sectional analytical study was conducted in 310 Colombian patients with SLE in whom CVD was assessed. Associated factors were examined by multivariate regression analyses. Second, a systematic review of the literature on CVD in SLE in LA was performed. Results. There were 133 (36.5%) Colombian SLE patients with CVD. Dyslipidemia, smoking, coffee consumption, and pleural effusion were positively associated with CVD. An independent effect of coffee consumption and cigarette on CVD was found regardless of gender and duration of disease. In the systematic review, 60 articles fulfilling the eligibility criteria were included. A wide range of CVD prevalence was found (4%–79.5%). Several studies reported ancestry, genetic factors, and polyautoimmunity as novel risk factors for such a condition.Conclusions. A high rate of CVD is observed in LA patients with SLE. Awareness of the observed risk factors should encourage preventive population strategies for CVD in patients with SLE aimed at facilitating the suppression of cigarette smoking and coffee consumption as well as at the tight control of dyslipidemia and other modifiable risk factors.

Em 2007-2008, o Rio de Janeiro viveu uma epidemia de dengue com 80.404 notificações, com 109 óbitos, 42% em < 15 anos. Objetivos: 1. Descrever a distribuição espacial e o perfil clínico-epidemiológico da epidemia de dengue por faixa etária no município do Rio de Janeiro. 2. Avaliar os fatores clínicos e laboratoriais preditivos de dengue grave em uma coorte de crianças internadas. Métodos: Esta tese foi formatada com dois artigos: 1) Distribuição espacial e caracterização clínico-epidemiológico do dengue no município do Rio de Janeiro; 2) Sinais clínicos e laboratoriais associados ao dengue grave em crianças hospitalizadas. Artigo 1: estudo ecológico com base no Sistema de Informação de Agravos de Notificação do município do Rio de Janeiro (2007-2008). Foram incluídos todos os casos de dengue confirmados pelo critério clínico-laboratorial dos residentes do município do Rio de Janeiro. Foram descritas as variáveis sócio demográficas e clínicas (classificação do caso, hospitalização e evolução) segundo a idade. Foi utilizado o critério de classificação de caso do Ministério da Saúde de 2005 (febre do dengue, dengue com complicações e febre hemorrágica do dengue - FHD). Autocorrelação espacial das taxas de incidência, mortalidade e letalidade foi verificada pelos índices de Moran e de Geary Artigo 2: coorte retrospectiva das hospitalizações por dengue (até 18 anos) em uma unidade de referência (2007-2008). Dengue grave foi definido pelo uso de terapia de suporte avançado de vida ou óbito. Diferenças de medianas e de proporções foram avaliadas, respectivamente, pelo teste de Mann-Whitney e pelo exato de Fisher (p<0,05). Foram calculados os parâmetros de acurácia dos sinais e sintomas. Resultados: Artigo 1: De 159887 notificações, 151527 eram residentes do município do Rio de Janeiro, 38.808 preencheram os critérios de inclusão. Destes, 8897 (22,9%) tiveram complicações e 1051 (2,7%) FHD; 16.436 (42,4%) eram < 18 anos e 15.288 (39,4%) foram confirmados laboratorialmente. A letalidade foi maior em lactentes (1,4%) e nos casos de FHD (7,7%). Extravasamento plasmático foi mais comum de 2 a 11 anos. Houve autocorrelação espacial global quanto à mortalidade e letalidade e autocorrelação local para os três índices (p<0,05). Artigo 2: De 145 pacientes, 53,1% eram do sexo feminino, com mediana de idade de 8,7 anos (IIQ=7,2-11,5); 15,9% evoluíram para gravidade A duração da febre em dias foi menor no grupo grave (G: Mediana Md=3,0, IIQ= 2,0\20134,5; NG: Md=4,0, IIQ=3,0\20135,0; p=0.062). Letargia, irritabilidade, sangramento, dispneia, ausculta pulmonar alterada, derrame pleural, distensão abdominal e edema foram associados à evolução para gravidade (p<0,05). Níveis de hematócrito (p=008) e de albumina sérica (p<0,001) foram menores nos graves. Letargia apresentou Valor Preditivo Negativo (VPN) > 90%, com Valor Preditivo Positivo (VPP) de 80%. As demais variáveis com p-valor <0,05 apresentaram VPN próximo ou > 90%, com baixo VPP. Conclusão: Apesar da incidência de FHD ter sido maior entre escolares, os lactentes tiveram maior letalidade. Extravasamento plasmático foi mais frequente em escolares e pré-escolares. Sangramento, sonolência e irritabilidade na admissão hospitalar associaram-se à gravidade e o derrame cavitário obteve maior acurácia do que a hemoconcentração como sinal de extravasamento plasmático

Background: Community-acquired pneumonia (CAP) is a leading cause of childhood death. There are few published reports of radiographic findings among children with severe CAP. Objective: To describe chest X-ray (CXR) findings and assess association between these radiographic findings and pneumococcal isolation in children with severe CAP. Methods: A prospective, multicenter, observational study was conducted in 12 centers in Argentina, Brazil, and the Dominican Republic. Children aged 3-59 months, hospitalized with severe pneumonia, were included. On admission, blood and pleural effusion cultures were performed. Streptococcus pneumoniae was identified according to standard procedures in the respective national reference laboratory. Chest X-rays were taken on admission and read before the culture results were reported. Results: Out of 2,536 enrolled patients, 283 (11.2%) had S. pneumoniae isolated, in 181 cases (7.1%) from blood. The follow radiographic patterns were observed: alveolar infiltrate (75.2%), pleural effusion (15.6%), and interstitial infiltrate (9.2%). Overall, pleural effusion was associated with pneumococcal isolation and pneumococcal bacteremia (P < 0.001). Infiltrates were unilateral (78.7%) or bilateral (21.3%), right-sided (76%) or left-sided (24%), in the lower lobe (53.6%) or the upper lobe (46.4%). Multivariate analysis including patients with affection of only one lobe showed that upper lobe affection and pleural effusion were associated with pneumococcal isolation (OR 1.8, 95% CI, 1.3-2.7; OR 11.0, 95% CI, 4.6-26.8, respectively) and with pneumococcal bacteremia (OR 1.7, 95% CI, 1.2-2.6; OR 3.1, 95% CI, 1.2-8.0, respectively). Conclusions: Three-quarters of the patients studied had alveolar infiltrates. Upper lobe compromising and pleural effusion were associated with pneumococcal invasive disease. BACKGROUND AND PURPOSE The serine and cysteine peptidase inhibitor, BbCI, isolated from Bauhinia bauhinioides seeds, is similar to the classical plant Kunitz inhibitor, STI, but lacks disulphide bridges and methionine residues. BbCI blocks activity of the serine peptidases, elastase (K(iapp) 5.3 nM) and cathepsin G (K(iapp) 160.0 nM), and the cysteine peptidase cathepsin L (K(iapp) 0.2 nM). These three peptidases play important roles in the inflammatory process. EXPERIMENTAL APPROACH We measured the effects of BbCI on paw oedema and on leucocyte accumulation in pleurisy, both induced by carrageenan. Leucocyte-endothelial cell interactions in scrotal microvasculature in Wistar rats were investigated using intravital microscopy. Cytokine levels in pleural exudate and serum were measured by ELISA. KEY RESULTS Pretreatment of the animals with BbCI (2.5 mg.kg(-1)), 30 min before carrageenan-induced inflammation, effectively reduced paw oedema and bradykinin release, neutrophil migration into the pleural cavity. The number of rolling, adhered and migrated leucocytes at the spermatic fascia microcirculation following carrageenan injection into the scrotum were reduced by BbCI pretreatment. Furthermore, levels of the rat chemokine cytokine-induced neutrophil chemo-attractant-1 were significantly reduced in both pleural exudates and serum from animals pretreated with BbCI. Levels of interleukin-1 beta or tumour necrosis factor-alpha, however, did not change. CONCLUSIONS AND IMPLICATIONS Taken together, our data suggest that the anti-inflammatory properties of BbCI may be useful in investigations of other pathological processes in which human neutrophil elastase, cathepsin G and cathepsin L play important roles.
