¿Se puede planificar cuantos dentistas necesitamos? Analisis de las tendencias de los recursos humanos y la utilizacion de servicios dentales (Cataluña 1991-2008)

Fundamento: Este estudio evidencia las dificultades de planificar recursos humanos en odontología analizando las variables que actúan en la demanda-oferta de servicios en un escenario de: baja utilización de servicios, baja patología y provisión de servicios principalmente privada. Material y Método: Los datos de utilización de servicios (visitas;año) se obtuvieron a partir de la Encuesta de Salud de Cataluña (1994-2006) y la Encuesta Nacional de Salud (1987-2006). La demografía profesional se obtuvo de censos colegiales. Resultados: Cataluña aumentaba 76 colegiados/año en los ochenta, 123 colegiados/año en los noventa y 287 colegiados/año en el período 2005-2008 asociándose a la aperturade facultades de odontología y la inmigración de profesionales con diplomas extranjeros (34,5% dedentistas en 2004). La demanda de ingreso a odontología es elevada y la inserción laboral excelente. La epidemiología de caries muestra una disminución continua (CAOD 12 años=0,73 en 2006) aunque los niveles de visita al dentista se encuentran entre los más bajos de la UE. Según la Encuesta de Salud de Cataluña un 31,5% de los hombres y un 38,1% de las mujeres había visitado un dentista el último año. Conclusiones: Es necesario aplicar nuevas políticas de planificación de recursos humanos y estrategiaspara minimizar las barreras de acceso a los servicios odontológicos en una situación de desequilibrio oferta-demanda.

Un modelo de producción en gruta: el Alfar de Cabrera D'Anoia, Barcelona

La utilización de grutas y cuevas para llevar a cabo actividades productivas fue un hecho habitual en época medieval. Este trabajo pretende analizar un modelo de producción cerámica que se adapta a un espacio físico considerablemente limitado por las características geológicas del entorno. El alfar de Cabrera d¿Anoia constituye un ejemplo paradigmático de dicho modelo e ilustra un proceso de evolución cronológica y de transformación del entorno para satisfacer las necesidades de la producción cerámica.

Adaptacion al medio y transformacion artesanal: balance actual de las investigaciones acerca del alfar medieval de Cabrera d'Anoia

Las investigaciones sobre Cabrera d"Anoia (Barcelona) han avanzado de forma considerable en los últimos años. La caracterización de los materiales y el estudio de la documentación arqueológica han logrado definir un particular sistema de producción de cerámica que se desarrolla en un marco rupestre de dimensiones limitadas. Una situación que obliga a la modificación continua del área de trabajo en beneficio de las actividades productivas. Este artículo analiza ese proceso de adaptación y transformación permanente del espacio artesanal a partir de los datos proporcionados por el yacimiento. Se trata, en consecuencia, de esbozar un primer balance de los resultados más recientes tras 20 años de investigación sobre este alfar.

Beta-lactam resistance mechanisms of methicillin-resistant Staphylococcus aureus.

In vitro and in vivo activity of amoxicillin and penicillin G alone or combined with a penicillinase inhibitor (clavulanate) were tested against five isogenic pairs of methicillin-resistant Staphylococcus aureus (MRSA) producing or not producing penicillinase. Loss of the penicillinase plasmid caused an eight times or greater reduction in the MICs of amoxicillin and penicillin G (from greater than or equal to 64 to 8 micrograms/ml), but not of the penicillinase-resistant drugs methicillin and cloxacillin (greater than or equal to 64 micrograms/ml). This difference in antibacterial effectiveness correlated with a more than 10 times greater penicillin-binding protein 2a affinity of amoxicillin and penicillin G than of methicillin and a greater than or equal to 90% successful amoxicillin treatment of experimental endocarditis due to penicillinase-negative MRSA compared with cloxacillin, which was totally ineffective (P less than .001). Amoxicillin was also effective against penicillinase-producing parent MRSA, provided it was combined with clavulanate. Penicillinase-sensitive beta-lactam antibiotics plus penicillinase inhibitors might offer a rational alternative treatment for MRSA infections.
