With the recent technical improvement in echocardiography imaging (second harmonics) the number of interatrial septum aneurysms (ASA) increased and are easily recognized. We assist to an overdiagnosing number of cases and diagnostic criteria emerged to face this problem. In the great majority of the cases ASA are small and inoffensive, but as ASA is considered a risk factor for cardioembolism when associated with persistence of foramen oval (PFO), an examination by transesophageal echocardiography (TEE) for exclusion of PFO makes the sense and is a common testing in patients with cryptogenic stroke. Besides these frequent ASA, other forms exist; the authors describe two cases of uncommon and huge ASA, one mimicking a right atrial tumor and the other a quistic, hipoechoic mass. The first case was associated with mitral stenosis and was submitted to surgery and the second was closed with an Amplatzer occluder device usually used in atrial septal defect (ASD).

OBJECTIVES: Atrio-ventricular septal (AVSD) defects include a variable spectrum of congenital malformations with different forms of clinical presentation. We report the surgical results, from a single institution, with this type of congenital cardiac malformation. Patients with hypoplasia of one of the ventricles were excluded from this analysis. POPULATION: Between November of 1998 and June of 2005, 49 patients with AVSD were operated on by the same team and in the same department. The average age was 37.3 months (medium 6 months) and 31 patients were female. In 38 patients (78%) an inter-ventricular communication was present (AVSD-complete) and of these, 26 were of the type A of Rastelli, being 13 of type B or C. The age for defect correction of the complete form was of 5.5 months, palliative surgery was not carried out on any of the patients. Associated lesions included: Down's syndrome in 22 patients (45%), patent arterial duct in 17 patients (35%), severe AV regurgitation in 4 patients (8%), tetralogy of Fallot in two (4%) and sub-aortic stenosis in one patient (2%). Pre-operatively 10 patients presented severe congestive heart failure and two were mechanically ventilated. RESULTS: Complete biventricular correction was carried out in all patients. The average time on bypass (ECC) was 74.1+/-17.5 min. and time of aortic clamping was 52.0+/-12.9 min. The complete defects were corrected by the double patch technique, and in all patients the mitral cleft was closed, except in two with single papillary muscle. There was no intra-operative mortality, but hospital mortality was 8%(4 patients), due to pulmonary hypertension crises, in the first 15 post-operative days. The mean ventilation time was of 36.5+/-93 hours (medium 7 h) and the average ICU stay was of 4.3+/-4.8 days (medium 3 days). The minimum follow-up period is 1 month and the maximum is 84 months (medium 29.5 months), during which time 4 re-operations (8%) took place: two for residual VSD's and two for mitral regurgitation. There was no mortality at re-do surgery. At follow up there was residual mitral regurgitation, mild in 17 patients and moderate in two. Four other patients presented with minor residual defects. CONCLUSIONS: The complete correction of AVSD can be carried out with acceptable results, in a varied spectrum of anatomic forms and of clinical severity. Despite the age of correction, for the complete forms, predominantly below 12 months, pulmonary hypertension was the constant cause for post operative mortality. Earlier timing of surgery and stricter peri-operative control might still improve results.

Introdução: A terapêutica de ressincronização cardíaca (TRC) tem benefícios significativos em doentes seleccionados. O impacto desta modalidade na incidência de taquidisritmias ventriculares permanece controverso. Analisámos a ocorrência de terapêuticas apropriadas em doentes submetidos a TRC combinada com cardioversor-desfibrilhador (CDI). Métodos: Estudo de 123 doentes com fracção de ejecção ventricular esquerda (FEVE) < 35%, submetidos a implantação com sucesso de TRC-CDI ou CDI isoladamente (prevenção primária). Resultados: Idade média foi 63±12 anos, FEVE de 25±6%, seguimento mediano de 372 dias. Implantou-se TRC-CDI em 63 doentes (grupo A) e CDI isoladamente em 60 doentes (grupo B). No grupo A tivemos 86% de respondedores clínicos, menor prevalência de miocardiopatia isquémica(30% versus 72%), e mais doentes em classe III da NYHA antes da implantação do dispositivo(90% versus 7%) comparativamente com o grupo com CDI isoladamente. Não se identificaram diferenças relativamente à incidência de terapêuticas apropriadas (19% versus 12%) ou no tempo para a primeira terapêutica (305 dias versus 293 dias). A mortalidade total foi de 11% no grupo A e de 12% no grupo B. As curvas de Kaplan-Meier para eventos arrítmicos em doentes com TRC, não mostraram diferenças significativas (HR 3,02, IC 95% 0,82-11,09, p = NS) comparativamente com doentes sem TRC. Conclusões: Em doentes submetidos a TRC-CDI por prevenção primária, apesar da elevada taxa de respondedores, a incidência de terapêuticas apropriadas não foi diferente do obtido em doentes com CDI isoladamente.

Apresentamos o caso de uma mulher de 46 anos com episódios frequentes de dor precordial e síncope associados a elevação do segmento ST e períodos de bloqueio aurículoventricular de alto grau transitórios. A coronariogafia excluiu lesões significativas e a doente foi tratada com nitratos e antagonistas do cálcio. Verificou-se persistência da sintomatologia associada a períodos de bloqueio aurículo-ventricular sintomáticos, refratários a otimização terapêutica. Implantação de pacemaker DDD-R. Follow-up de 4 meses sem intercorrências.

A presente dissertação insere-se no âmbito da unidade curricular “ Dissertação” do 2º ano do mestrado em Engenharia Eletrotécnica – Sistemas Elétricos de Energia. Com o aumento crescente do número de consumidores de energia, é cada vez mais imperioso a adoção de medidas de racionalização e gestão dos consumos da energia elétrica. Existem diferentes tipos de dificuldades no planeamento e implementação de novas centrais produtoras de energia renovável, pelo que também por este motivo é cada vez mais importante adoção de medidas de gestão de consumos, quer ao nível dos clientes alimentados em média tensão como de baixa tensão. Desta forma será mais acessível a criação de padrões de eficiência energética elevados em toda a rede de distribuição de energia elétrica. Também a economia é afetada por uma fraca gestão dos consumos por parte dos clientes. Elevados desperdícios energéticos levam a que mais energia tenha que ser produzida, energia essa que contribui ainda mais para a elevada taxa de dependência energética em Portugal, e para o degradar da economia nacional. Coloca-se assim a necessidade de implementar planos e métodos que promovam a eficiência energética e a gestão racional de consumos de energia elétrica. Apresenta-se nesta dissertação várias propostas, algumas na forma de projetos já em execução, que visam sensibilizar o consumidor para a importância da utilização eficiente de energia e, ao mesmo tempo, disponibilizam as ferramentas tecnológicas adequadas para auxiliar a implementação dos métodos propostos. Embora os planos apresentados, sobejamente conhecidos, tenham imensa importância, a implementação nos vários consumidores de sistemas capazes de efetivamente reduzir consumos tem um papel fundamental. Equipamentos de gestão de consumos, que são apresentados nesta dissertação, permitem ao consumidor aceder diretamente ao seu consumo. Podem aceder não apenas ao consumo global da instalação mas também ao consumo específico por equipamento, permitindo perceber onde se verifica a situação mais desfavorável. Funcionalidades de programação de perfis tipo, com limitações de potência em vários períodos horários, bem como possibilidades de controlo remoto com recurso a aplicações para Smartphones permitem a redução de consumos ao nível da rede de distribuição e, desta forma, contribuir para a redução dos desperdícios e da dependência energética em Portugal. No âmbito do trabalho de dissertação é desenvolvida uma metodologia de comercialização de potência, que é apresentada nesta tese. Esta metodologia propõem que o consumidor, em função dos seus consumos, pague apenas a quantidade de potência que efetivamente necessita num certo período de tempo. Assim, o consumidor deixa de pagar uma tarifa mensal fixa associada á sua potência contratada, e passará a pagar um valor correspondente apenas à potência que efetivamente solicitou em todas as horas durante o mês. Nesta metodologia que é apresentada, o consumidor poderá também fazer uma análise do seu diagrama de cargas e simular uma alteração da sua tarifa, tarifa esta que varia entre tarifa simples, bi-horária semanal, bi-horária diária, tri-horária semanal ou tri-horária diária, de forma a perceber em qual destas pagará um menor valor pela mesma energia. De forma a que o consumidor possa perceber se haverá vantagem de uma alteração para uma potência contratada flexível, ou para uma outra tarifa associada á energia, tem ao seu dispor uma ferramenta, que em função dos seus consumos, permite retirar conclusões sobre o preço final a pagar na fatura, após cada tipo de alteração. Esta ferramenta foi validada com recurso a várias simulações, para diferentes perfis de consumidores. Desta forma, o utilizador fica a perceber que realmente pode poupar com uma potência contratada flexível, ao mesmo tempo que pode identificar-se com um perfil de simulação e, mais facilmente, perceber para que alteração tarifária pode usufruir de uma maior poupança.

The identification of predictors for the progression of chronic Chagas cardiomyopathy (CCC) is essential to ensure adequate patient management. This study looked into a non-concurrent cohort of 165 CCC patients between 1985 and 2010 for independent predictors for CCC progression. The outcomes were worsening of the CCC scores and the onset of left ventricular dysfunction assessed by means of echo-Doppler cardiography. Patients were analyzed for social, demographic, epidemiologic, clinical and workup-related variables. A descriptive analysis was conducted, followed by survival curves based on univariate (Kaplan-Meier and Cox’s univariate model) and multivariate (Cox regression model) analysis. Patients were followed from two to 20 years (mean: 8.2). Their mean age was 44.8 years (20-77). Comparing both iterations of the study, in the second there was a statistically significant increase in the PR interval and in the QRS duration, despite a reduction in heart rates (Wilcoxon < 0.01). The predictors for CCC progression in the final regression model were male gender (HR = 2.81), Holter monitoring showing pauses equal to or greater than two seconds (HR = 3.02) increased cardiothoracic ratio (HR = 7.87) and time of use of digitalis (HR = 1.41). Patients with multiple predictive factors require stricter follow-up and treatment.

We report the case of a 52-year-old man who presented to our emergency department (ED) after three episodes of syncope in the seven hours before admission. During his stay in the ED he had recurrent ventricular tachycardia (VT) requiring external electrical cardioversion. A 12-lead electrocardiogram (ECG) showed a short QT (SQT) interval (270 ms, QTc 327 ms), with frequent R-on-T extrasystoles triggering sustained polymorphic VT. After exclusion of other precipitating causes, the patient was diagnosed as having SQT syndrome (SQTS) according to the Gollob criteria. To our knowledge, this is the first known documentation of an SQT-caused arrhythmic episode on a 12-lead ECG, as well as the first reported case of SQTS in Portugal. The patient received an implantable cardioverter-defibrillator and was discharged. At a follow-up assessment 14 months later he was symptom-free, interrogation of the device showed no arrhythmic events, and the ECG showed a QT interval of 320 ms (QTc 347 ms).

Objective:We aimed to identify if there was any difference in Levonorgestrel-releasing intrauterine system (LNG-IUS) efficacy or weight gain when used in heavy menstrual bleeding (HMB) treatment, between obese and non-obese women. Population and methods: This was a case-controlled retrospective study undertaken between 2002-2007. 194 women with HMB were treated with LNG-IUS and stratified into two groups accordingly with body mass index (BMI): Obese Group – BMI ≥ 30 (n=53) and Non-obese Group – BMI < 30 (n=141). Age, weight, days of spotting and days of menses were analyzed at 1, 3 and 6 months after insertion and then annually until 2 years. Analytic parameters of anemia (hemoglobin, serum ferritin, mean corpuscular volume) were reviewed at pre-insertion, at 6 months and then annually until 2 years. Results: During the 2-year follow-up there was a similar improvement in two groups regarding duration of menses, spotting and in analytic parameters of anemia. A statistically significant improvement was observed in obese group after 2 years of treatment regarding analytic parameters of anemia and menstrual characteristics, without weight gain. Conclusion: In obese women, the LNG-IUS is an effective treatment for heavy menstrual bleeding, without being associated to weight gain.

INTRODUCTION: Mitral regurgitation (MR) is the most common valvular disease and has recently become the target of a number of percutaneous approaches. The MitraClip is virtually the only device for which there is considerable experience, with more than 20,000 procedures performed worldwide. OBJECTIVE: To describe our initial experience of the percutaneous treatment of MR with the MitraClip device. METHODS: We describe the first six MitraClip cases performed in this institution (mean age 58.5 ± 13.1 years), with functional MR grade 4+ and New York Heart Association (NYHA) heart failure class III or IV (n=3), with a mean follow-up of 290 ± 145 days. RESULTS: Procedural success (MR ≤ 2+) was 100%. Total procedure time was 115.8 ± 23.7 min, with no in-hospital adverse events and discharge between the fourth and eighth day, and consistent improvement in the six-minute walk test (329.8 ± 98.42 vs. 385.33 ± 106.95 m) and in NYHA class (three patients improved by two NYHA classes). During follow-up there were two deaths, in two of the four patients who had been initially considered for heart transplantation. CONCLUSION: In patients with functional MR the MitraClip procedure is safe, with both a high implantation and immediate in-hospital success rate. A longer follow-up suggests that the clinical benefit decreases or disappears completely in patients with more advanced heart disease, namely those denied transplantation or on the heart transplant waiting list.

INTRODUCTION: Left ventricular reverse remodeling (LVRR), defined as reduction of end-diastolic and end-systolic dimensions and improvement of ejection fraction, is associated with the prognostic implications of cardiac resynchronization therapy (CRT). The time course of LVRR remains poorly characterized. Nevertheless, it has been suggested that it occurs ≤6 months after CRT.
OBJECTIVE: To characterize the long-term echocardiographic and clinical evolution of patients with LVRR occurring >6 months after CRT and to identify predictors of a delayed LVRR response.
METHODS: A total of 127 consecutive patients after successful CRT implantation were divided into three groups according to LVRR response: Group A, 19 patients (15%) with LVRR after >6 months (late LVRR); Group B, 58 patients (46%) with LVRR before 6 months (early LVRR); and Group C, 50 patients (39%) without LVRR during follow-up (no LVRR).
RESULTS: The late LVRR group was older, more often had ischemic etiology and fewer patients were in NYHA class ≤II. Overall, group A presented LVRR between group B and C. This was also the case with the percentage of clinical response (68.4% vs. 94.8% vs. 38.3%, respectively, p<0.001), and hospital readmissions due to decompensated heart failure (31.6% vs. 12.1% vs. 57.1%, respectively, p<0.001). Com o objetivo de avaliar as características clínicas e epidemiológicas do aneurisma ventricular esquerdo na doença de Chagas crônica, 388 indivíduos não selecionados: 298 chagásicos e 90 não-chagásicos, foram submetidos ao exame ecocardiográfico. A função ventricular foi avaliada ao modo M através do cálculo da fração de ejeção e ao bidimensional através da análise subjetiva da função sistólica global e a contratilidade regional foi avaliada pelo modelo da Sociedade Americana de Ecocardiografia. Foram diagnosticados 56 (18,8%) aneurismas do ventrículo esquerdo, todos entre os chagásicos, sendo 38 (12,7%) no segmento apical, 10 (3,4%) no septo interventricular, 2 (0,7%) ápico-septal, 2 (0,7%) na parede posterior, 2 (0,7%) na parede inferior e 2 (0,7%) no segmento ínfero-posterior. Não houve diferença significativa nas freqüências dos aneurismas em relação à faixa etária, ao sexo e à etnia. Não houve associação entre aneurismas e hipertensão arterial. Dos 56 indivíduos com aneurismas, 55 (98,2%) eram sintomáticos com predominância de palpitações, 53 (94,6,%) apresentaram ECG anormais, com predominância de extra-sístoles ventriculares, seguidas de alterações da condução e 34 (60,7%) apresentaram comprometimento da função ventricular, sem diferença quanto ao segmento acometido. Diante destes resultados podemos considerar o aneurisma ventricular esquerdo, principalmente apical, como um marcador de doença de Chagas e um indicador da alta morbidade da infecção humana pelo T. cruzi em Virgem da Lapa.
