O presente trabalho tem como objetivo observar a atuação da equipe de enfermagem na atenção básica, buscando caracterizar como se dão as interações com o usuário, e priorizando a escuta como requisito da integralidade. Para que os profissionais de saúde percebam, identifiquem o sofrimento do usuário, torna-se necessário uma interação maior entre eles, ou seja, um diálogo. Para o exercício de um bom diálogo, faz-se necessário uma escuta atenta, buscando-se detalhes que podem estar ocultos nas falas, além do acolhimento e o vínculo, que são atributos responsáveis pela prática da assistência integral. A redução do sujeito a objeto tem sido a realidade das unidades básicas de saúde, lembrando que os usuários que procuram os serviços de saúde possuem uma vida fora dali, com estilos variados. O modo de falar, os cuidados prestados variam conforme a rotina de cada um. A pesquisa teve como suporte teórico a integralidade como exercício de boas práticas e produção de cuidado, enfatizando-se a escuta. Trata-se de um estudo de caráter qualitativo com abordagem etnográfica, que permite ao pesquisador fazer uma leitura detalhada da realidade. Os dados foram coletados através da observação participante e acompanhamento das atividades realizadas pela equipe de enfermagem de duas unidades básicas do município de Paranavaí-PR, e os achados foram devidamente registrados em diário de campo. Após as observações, os profissionais foram entrevistados. Participaram como sujeitos da pesquisa sete técnicos de enfermagem e um profissional administrativo. O material foi submetido à técnica de análise de conteúdo. A análise compreendeu as seguintes etapas: pré-análise, que se refere à leitura inicial de todo o conteúdo e organização dos dados, e análise, que compreende categorização e tratamento do material. Foram identificados pontos críticos na assistência durante a observação, como a falta do acolhimento, desorganização do serviço, a falta do exercício da escuta e o trabalho mecanizado. Nas entrevistas, foi possível desenvolver categorias com os relatos coletados, destacando-se a sensibilidade ao paciente, procura do melhor, falta de tempo e sentido da escuta. Conclui-se que a integralidade ainda é uma utopia na realidade dos serviços de saúde, e que a escuta é raramente utilizada e seu sentido tão pouco conhecido. A desorganização do serviço de saúde nas unidades básicas e o aparente conformismo dos profissionais frente a suas atividades são fatores predisponentes ao não-exercício da escuta, dificultando ainda mais a prática da integralidade. Considera-se fundamental a reflexão, por parte dos profissionais, e solução dos problemas encontrados, através de conscientização e sensibilização, modificando a assistência através de reorganizações do trabalho.

O estudo objetivou compreender como se processa o respeito aos direitos da criança hospitalizada e sua família, pelo hospital e pelos profissionais de saúde. Procurou-se analisar as relações existentes entre os profissionais de saúde, a criança e sua família, no interior do hospital; como os profissionais percebem os direitos dessas crianças e de sua família, e como a própria família compreende os direitos dos seus filhos. Para isso, utilizou-se pesquisa qualitativa com enfoque etnográfico, tendo como técnicas a observação participante e entrevistas semiestruturadas Foram realizadas sete entrevistas com acompanhantes e onze com os profissionais de saúde, de um hospital pediátrico da cidade de Fortaleza CE, no ano de 2005 e 2006. A análise dos dados aconteceu após um processo de múltiplas leituras e estes foram interpretados com o referencial relativo aos temas elaborados. Surgiram quatro temáticas a partir da análise do material: a descoberta da doença na criança; a trajetória da criança hospitalizada; o conhecimento dos profissionais de saúde sobre os direitos da criança hospitalizada e as condições materiais de trabalho do profissional como um obstáculo na consecução desses direitos. Nas diversas conclusões, destacam-se: os múltiplos projetos de humanização existentes no hospital; a preocupação em manter um espaço destinado para a criança brincar; a criação de um espaço de higiene e alimentação para o acompanhante; o ambulatório da Cirurgia sem Medo, preparando a criança e família para a cirurgia, bem como a criação da fila única; a ampliação do horário de visita; a comunicação, muitas vezes deficiente, entre profissionais e acompanhantes; a pouca ou nenhuma orientação sobre normas e rotinas do hospital e sobre os direitos da criança e família no meio hospitalar; o limitado conhecimento sobre os direitos da criança hospitalizada, por alguns profissionais, desconhecendo os documentos importantes na área; e, finalmente, a falta de materiais de trabalho essenciais para a recuperação e bem-estar da criança hospitalizada como empecilhos a um cuidado pleno e de boa qualidade.

O trabalho tem por objeto a terceirização da gestão municipal de unidades ambulatoriais de saúde mental no segundo, terceiro e quarto distritos do município de Duque de Caxias, no Rio de Janeiro, na modalidade de organização social, entre 2009 a 2012. O marco inicial refere-se a assinatura do Termo de parceria entre a Secretaria Municipal de Saúde de Duque de Caxias (SMSDC) e uma Organização da Sociedade Civil de Interesse Público (OSCIP). O marco final refere-se a rescisão unilateral do Termo de Parceria com a OSCIP por parte da SMSDC. O objetivo geral: analisar a gestão terceirizada nos ambulatórios de saúde mental do município de Duque de Caxias no referido período na modalidade de OSCIP. Objetivos específicos: descrever as circunstâncias de implantação do processo de terceirização na gestão das unidades de saúde mental em Duque de Caxias na modalidade de OSCIP; analisar a participação da OSCIP nos ambulatórios de saúde mental no município de Duque de Caxias nos distritos de interesse do estudo; discutir as implicações do modelo de terceirização para a política de Saúde Mental em Duque de Caxias. Trata-se de abordagem quanti-qualitativa do tipo descritiva. As fontes primárias se constituíram de decretos, leis, portarias, resoluções, documentos, atas e relatórios de gestão da OSCIP e do programa de Saúde Mental de Duque de Caxias, atas do Conselho do Municipal de Caxias e atas das Conferencias Nacionais e Municipais de Saúde e de Saúde Mental. Para o processo de análise dos dados foi utilizada a analise documental e a analise estatística. Os dados quantitativos foram tabulados e analisados através de estatística simples e apresentados sob a forma de tabelas e gráficos a partir dos dados do TABNET, DATASUS, relatórios da OSCIP e do Programa de Saúde Mental de Duque de Caxias, em especial das unidades terceirizadas dos 2, 3 e 4 distritos do município. Os resultados mostraram que apesar da parceria entre a instituição terceirizada e a Secretaria Municipal de Saúde que previa aumento da produção de consultas em Saúde Mental, alcançando um mínimo 75% das consultas estimadas para psiquiatras e psicólogos, não resultou em diminuição significativa na taxa de internação psiquiátrica no período estudado, que passou de 1,19% do total de internações em 2009 para 0,77% deste total em 2012, apresentando até uma elevação para 1,26% no ano de 2010. Tal fato pode ser devido ao modelo de gestão terceirizado não estar adequado à complexidade da abordagem do paciente com transtorno mental, onde o envolvimento e a formação do profissional baseada no vínculo, na participação do paciente e a construção do seu plano terapêutico com a participação da família, são aspectos diferenciais na qualidade da assistência em saúde mental. O modelo de gestão adotado por Caxias para as Unidades estudadas não apresentou correspondência na redução efetiva da taxa de internação psiquiátrica dos pacientes residentes em Duque de Caxias.

In a hospital environment that demands a careful balance between commercial and clinical interests, the extent to which physicians are involved in hospital leadership varies greatly. This paper assesses the influence of the extent of this involvement on staff-to-patient ratios. Using data gathered from 604 hospitals across Germany, this study evidences the positive relationship between a full-time medical director (MD) or heavily involved part-time MD and a higher staff-to-patient ratio. The data allows us to control for a range of confounding variables, such as size, rural/urban location, ownership structure, and case-mix. The results contribute to the sparse body of empirical research on the effect of clinical leadership on organizational outcomes.

This research asks the question: “What are the relational dynamics in Masters (MA) supervision?” It does so by focusing upon the supervisory relationship itself. It does this through dialoguing with the voices of both MA supervisors and supervisees in the Humanities using a Cultural Historical Activity Theory (CHAT) framework. In so doing, this research argues for a re-evaluation of how MA supervision is conceptualised and proposes a new theoretical framework for conceptualising MA supervision as a relational phenomenon. The research design was derived from an Activity Theory-influenced methodology. Data collection procedures included the administration of Activity Theory Logs, individual semi-structured interviews with both supervisors and supervisees and the completion of reflective journals. Grounded Theory was used to analyse the data. The sample for the study consists of three supervisor-supervisee dyads from three disciplines in the Humanities. Data was collected over the course of one academic year, 2010-2011. This research found that both individual and shared relational dynamics play an important role in MA supervision. Individual dynamics, such as supervisors’ iterative negotiation of ambiguity/clarity and supervisees’ boundary work, revealed that both parties attempt to negotiate a separation between their professional-academic identities and personal identities. However, an inherent paradox emerged when the shared relational dynamics of MA supervision were investigated. It was found that the shared space created by the supervisory relationship did not only exist in a physical setting, but was also psychoactive in nature and held strong emotional resonances for both parties involved. This served to undermine the separation between professional-academic and personal identities. As a result, this research argues that the interaction between the individual and shared relational dynamics in MA supervision enables, for both supervisors and supervisees, a disciplined improvisation of academic identity.

© 2014 UICC.Cytokines such as Interleukin (IL)212p70 ("IL-12") and IL-23 can influence tumor progression. We tested the hypothesis that blood levels of IL-12p40, the common subunit of both cytokines, are associated with melanoma progression. Blood from 2,048 white melanoma patients were collected at a single institution between March 1998 and March 2011. Plasma levels of IL-12p40 were determined for 573 patients (discovery), 249 patients (Validation 1) and 244 patients (Validation 2). Per 10-unit change of IL-12p40 level was used to investigate associations with melanoma patient outcome among all patients or among patients with early or advanced stage. Among stage I/II melanoma patients in the pooled data set, after adjustment for sex, age, stage and blood draw time from diagnosis, elevated IL-12p40 was associated with melanoma recurrence [hazard ratio (HR)51.04 per 10-unit increase in IL-12p40, 95% CI 1.02-1.06, p58.48 × 10-5]; Elevated IL-12p40 was also associated with a poorer melanoma specific survival (HR51.06, 95% CI 1.03-1.09, p53.35 × 10-5) and overall survival (HR51.05, 95% CI 1.03-1.08, p58.78 × 10-7) in multivariate analysis. Among stage III/IV melanoma patients in the pooled data set, no significant association was detected between elevated IL-12p40 and overall survival, or with melanoma specific survival, with or without adjustment for the above covariates. Early stage melanoma patients with elevated IL-12p40 levels are more likely to develop disease recurrence and have a poorer survival. Further investigation with a larger sample size will be needed to determine the role of IL-12p40 in advanced stage melanoma patients.

Objectives: To evaluate the empirical evidence linking nursing resources to patient outcomes in intensive care settings as a framework for future research in this area. Background: Concerns about patient safely and the quality of care are driving research on the clinical and cost-effectiveness of health care interventions, including the deployment of human resources. This is particularly important in intensive care where a large proportion of the health care budget is consumed and where nursing staff is the main item of expenditure. Recommendations about staffing levels have been trade but may not be evidence based and may not always be achieved in practice. Methods: We searched systematically for studies of the impact of nursing resources (e.g. nurse-patient ratios, nurses' level of education, training and experience) on patient Outcomes, including mortality and adverse events, in adult intensive care. Abstracts of articles were reviewed and retrieved if they investigated the relationship between nursing resources and patient Outcomes. Characteristics of the studies were tabulated and the quality of the Studies assessed. Results: Of the 15 studies included in this review, two reported it statistical relationship between nursing resources and both mortality and adverse events, one reported ail association to mortality only, seven studies reported that they Could not reject the null hypothesis of no relationship to mortality and 10 studies (out of 10 that tested the hypothesis) reported a relationship to adverse events. The main explanatory mechanisms were the lack of time for nurses to perform preventative measures, or for patient surveillance. The nurses' role in pain control was noted by One author. Studies were mainly observational and retrospective and varied in scope from 1 to 52 units. Recommendations for future research include developing the mechanisms linking nursing resources to patient Outcomes, and designing large multi-centre prospective Studies that link patient's exposure to nursing care oil a shift-by-shift basis over time. Introduction: Evidence from studies conducted mainly in the US and mainland Europe suggests that characteristics of the workforce, such as nurse patient ratios and workload (measured in a number of different ways) may be linked to variations in patient outcomes across health care settings (Carmel and Rowan 2001). Few studies have tested this relationship in the UK thus questions remain about whether we are justified in extrapolating evidence from studies conducted in very different health care systems. Objectives: To investigate whether characteristics of the nursing workforce affect patient mortality UK Intensive Care Units. Data: Patient data came from the case mix programme, Intensive Care National Audit and Research Centre (ICNARC), while information about the units came from a survey of all ICUs in England (Audit Comission 1998). The merged data set contained information on 43,859 patients in 69 units across England. ICNARC also supplied a risk adjustment variable to control for patient characteristics that are often the most important determinants of survival. Methods: Multivariate multilevel logistic regression. Findings: Higher numbers of direct care nurses and lower scores on measures of workload(proportion of occupied beds at the time the patient was admitted and mean daily transfers into the unit) were associated with lower mortality rates. Furthermore, the effect of the number of direct care nurses was greatest on the life chances of the patients who were most at risk of dying. Implications: This study has wide implications for workforce policy and planning because it shows that the size of the nursing workforce is associated with mortality (West et al 2006). Few studies have demonstrated this relationship in the UK. This study has a number of strengths and weaknesses and further research is required to determine whether this relationship between the nursing workforce and patient outcomes is causal.

This article explores the experience of Continuing Professional Development (CPD) by supervisory-level clinical staff in the National Health Service. Four main themes are highlighted in the literature, namely the nature and experience of CPD, its relationship with human resource management practices and in particular in career development and planning. These themes are examined utilising sources of (triangulated) empirical data based on a 2500 sample survey conducted across five NHS Trusts. A key finding was that responsibility for learning and development was perceived as belonging to the individual rather than the organisation. Other findings concern a lack of resource-based commitment by the organisation to CPD for clinical staff undertaking supervisory-level roles and evidence of 'credentialism' with its emphasis on seeking certificated qualifications. The findings raise concerns about the potential for clinical staff to become disillusioned and to perceive a potential breach in their psychological contract because of problems in reconciling their own interests with those of their professional body, and that of their employer in relation to CPD.

The proportion of elderly in the population has dramatically increased and will continue to do so for at least the next 50 years. Medical resources throughout the world are feeling the added strain of the increasing proportion of elderly in the population. The effective care of elderly patients in hospitals may be enhanced by accurately modelling the length of stay of the patients in hospital and the associated costs involved. This paper examines previously developed models for patient length of stay in hospital and describes the recently developed conditional phase-type distribution (C-Ph) to model patient duration of stay in relation to explanatory patient variables. The Clinics data set was used to demonstrate the C-Ph methodology. The resulting model highlighted a strong relationship between Barthel grade, patient outcome and length of stay showing various groups of patient behaviour. The patients who stay in hospital for a very long time are usually those that consume the largest amount of hospital resources. These have been identified as the patients whose resulting outcome is transfer. Overall, the majority of transfer patients spend a considerably longer period of time in hospital compared to patients who die or are discharged home. The C-Ph model has the potential for considering costs where different costs are attached to the various phases or subgroups of patients and the anticipated cost of care estimated in advance. It is hoped that such a method will lead to the successful identification of the most cost effective case-mix management of the hospital ward.

Decreased survival in patients with cystic fibrosis has been related to FEV1, BMI, and infection with Burkholderia cepacia complex (BCC). We have assessed the relationship of blood, sputum, and urine inflammatory markers to lung function, BMI, colonization with B cenocepacia (Bc), and patient survival. Thirty-nine stable cystic fibrosis (CF) patients (10 with Bc) were enrolled in a study to determine the effect of alpha-1-antitrypsin on airways inflammation. Pre-treatment measurements were used in this study. Demographics, sputum microbiology, heart rate, oxygen saturation, lung function were recorded. Blood samples were obtained for white blood count (WBC), C-Reactive Protein (CRP), and plasma neutrophil elastase/AAT complexes (pNEC). Neutrophil elastase (NE), neutrophil elastase/AAT complexes (sNEC), interleukin-8 (IL-8), TNF-receptor 1 (sTNFr), and myeloperoxidase (MPO) were measured in sputum and urinary desmosine concentration determined. Patients with Bc had significantly higher levels of pNEC, 332?±?91.4 ng/ml (mean?±?SEM) versus 106?±?18.2 ng/ml (P?=?0.0005) and sNEC, 369?±?76.6 ng/ml versus 197?±?36.0 ng/ml compared to those who were not. Five deaths were reported at the end of 1 year, (four with Bc) (P?=?0.011). Patients who subsequently died had significantly lower lung function FEV1, 1.2?±?0.2 L versus 2.0?±?0.1 L (P?=?0.03) and FVC, 2?±?0.3 L versus 3.1?±?0.2 L (P?=?0.01), compared to those that survived. There was significantly higher NE activity, 3.6?±?1.6 U/ml versus 1.5?±?0.6 U/ml (P?=?0.03), pNEC, 274?±?99 ng/ml versus 142?±?30 ng/ml (P?=?0.05), MPO, 163?±?62 mcg/ml versus 54?±?6.9 mcg/ml (P?=?0.03), and urinary desmosines 108?±?19.9 pM/mg creatinine versus 51.1?±?3.3 pM/mg creatinine (P?=?0.001), in those patients who subsequently died compared to those that survived. These data suggest there is increased neutrophil degranulation in patients infected with Bc and these patients have a poor outcome.

The movement towards developing practice more firmly grounded on empirical research has, arguably, been one of the most significant international trends in social work during the past decade. However, in the UK the implications of this trend for pedagogical practices and the design of educational programmes have still to be fully explored. This paper reports on the findings of a repeated cross-sectional survey of MSW students in Queen's University Belfast which focused on their perceptions of the value of research training to professional practice. The study, conducted over a four year period, explored students' awareness of the relationship between research and practice and their readiness to engage with research training. The findings suggested that the majority of students perceived research training as a valuable component of professional development. However, the study also found a level of scepticism among students about its practical utility along with some resistance towards actively embracing a research agenda. The paper evaluates the significance of these findings for developing research and evidence-based practice as integral components of the new degrees in social work in the UK and for social work education programmes in other countries aiming to develop research-minded practice.

Introduction

Much has been written about the impact of conflict on the physical nature of cities; most obviously perhaps the damage, destruction, defensive construction and spatial reconfigurations that evolve in times of conflict. Set within the context of Belfast, Northern Ireland, this paper will focus on three areas. First, a closer reading of the long-term physical impact of conflict, in particular, the spatial forms and practices that persist conceptually and culturally, and/or resist re-conceptualisation. Secondly, the effect of conflict on the nature of architectural practice itself, considering whether issues such as appointment and procurement impacted on architectural expectation and the context of operation. Thirdly, the effect of conflict on people, in particular in relation to creativity and hence the psyche of practice itself. This section will also identify the conditions that undermine or support design quality and creativity not only within times of conflict but also as society evolves out of the shadow space. 1
Twelve years on from the Peace Agreement,2 it may seem remarkable from an external perspective that Northern Ireland still needs to be reflecting on its troubled past. But the immediate post-conflict phase offered the communities of Northern Ireland place and time to experience ‘normal life’, begin to reconcile themselves to the hurt they experienced and start to reconfigure their relationships to one another. Indeed, it has often been expressed that probing the issues too much, at too early a phase, might in fact ‘Open old wounds without resolving anything’ and/or ‘Destabilise the already fragile political system.’3 This tendency not to deliberate or be too probing is therefore understandable and might be the reason why, for example, Northern Ireland's first Architecture and Built Environment policy, published in June, 2006, contains only one routine reference to ‘the Troubles’.

Clearly, however, there is a time in the development of a healthy, functioning society, when in order effectively to plan its future, it must also carry out a closer reading and deeper understanding of its past. As Maya Angelou puts it, ‘History, despite its wrenching pain/ Cannot be unlived, and if faced/ With courage, need not be lived again.’4

Increasingly, those within the creative arts sector and the built environment professions are showing interest in carrying out that closer reading, teasing out issues around conflict. This was led in part by the recent publication of the Troubles Archive by the Arts Council of Northern Ireland.5 Those involved in the academic or professional development of future generations of architects are also concerned about the relevance of a post-conflict condition. As a profession, if architects purport to be concerned with context, then the almost tangible socio-political circumstances and legacy of Northern Ireland does inevitably require direct eye contact. This paper therefore aims to bring the relationship between conflict and architectural practice in Northern Ireland into sharp focus, not to constrain or dull creative practice but to heighten its potential.
