És un estudi qualitatiu sobre les cançons-joc de picar que succeeixen a l'hora del pati de les escoles en les quals es construeix i es transmet oralment tot un joc de gènere de rols negociats entre els participants. Els resultats s’obtenen de grups de discussió i entrevista a adolescents i adults i treball de camp amb els propis participants. L’estudi conclou destacant la influencia del context sociocultural en la seva configuració, el gènere com a tema principal en aquestes cançons i la subversió dels cànons com a potenciador de les variacions significatives dels rols i les activitats permeses de cadascun

Diabète de type 2: n'oubliez pas les troubles du comportement alimentaire [Patient with type 2 diabetes: don't forget eating disorders!]

The coexistence of diabetes and eating disorder (ED) is more prevalent than we think; indeed it seems to occur in 10 to 20% of cases. Therefore ED deserve attention to be detected and treated in order to permit a decreased in morbidity and better outcomes in body weight loss. The association of type 2 diabetes and ED is frequently seen in younger, overweighed or obese patients suffering of at least one psychiatric co-morbidities and frequently in a difficult psychosocial setting. To succeed in ED treatment, a multidisciplinary and specialised team in eating disorder is requested, however these patients are highly fragile and complex and therapeutic failure has to be feared.

"Pied de Charcot": un diagnostic à ne pas manquer [Charcot osteoarthropathy: don't miss it!].

Charcot neuropathic osteoarthropathy (CNO) is a destructive process affecting the bone and joint structure of diabetic patients and resulting from peripheral neuropathy. It is a limb threatening condition resulting in dramatic deformities associated with severe morbi-mortality. The diagnosis is mostly made by the observation of inflammatory signs and higlight the importance of prompt foot evaluation. Imaging studies may help confirm the diagnosis and the severity of the condition but lack of specificity. The goal of the treatment is to maintain or achieve structural stability of the foot and ankle to prevent further deformity and plantar dislocation. The scientific evidences aren't strong enough to recommend bisphosphonates or acute surgical treatment. Surgery is unanimusly recommended to prevent secondary ulceration.

Fever of unknown origin in a Swiss-born child: don't miss tuberculosis!

Tuberculosis incidence is low in Switzer land. We report here on a Swiss-born toddler. Tuberculosis manifested with a fever of unknown origin, mimicking an inflammatory or autoimmune disorder triggering a high dose of corticosteroid treatment. The disease went unrecognized for several weeks until development of a miliary tuberculosis with advanced central nervous system involvement. This case highlights the difficulties encountered in diagnosing tuberculosis and in identifying the origin of this case. It reminds us that this disease must never be forgotten when facing a child with persistent fever who must be screened for, before starting immunosuppressive therapy.

Cette thèse examine la manière dont les tensions qui traversent la discipline de la médecine de transplantation se répercutent sur la décision de don d'organes en étudiant, d'une part, la décision de don d'organes considérée comme «un souhait possible pour le futur» et, d'autre part, les conditions concrètes qui ont conduit à une prise de décision de don d'organes réelle dans le passé.
