We evaluated patients with end-stage heart failure who have a high likelihood of response to cardiac resynchronization therapy (biventricular pacing). It appears that 20% of patients do not respond to this expensive therapy despite the use of selection criteria (dilated cardiomyopathy, heart failure, New York Heart Association class II or IV, left ventricular election fraction 120 ms). The presence of left ventricular dys-synchrony is needed to result in improvement after cardiac resynchronization therapy. (C)2003 by Excerpta Medica, Inc.

Background Latent left ventricular (LV) dysfunction in patients with valvular or myocardial disease may be identified by loss of contractile reserve (CR) at exercise echocardiography. Contraction in the LV longitudinal axis may be more sensitive than radial contraction to minor disturbances of LV function. We sought to determine whether tissue Doppler measurement of longitudinal function could be used to identify CR. Methods Exercise echocardiography was performed in 86 patients (20 women, age 53 +/- 18 years), 72 with asymptomatic or minimally symptomatic mitral regurgitation, and 14 normal controls. Pulsed-wave tissue Doppler imaging (DTI) was used to measure maximum annular systolic velocity at rest and stress. Inducible ischemia was excluded by analysis of wall motion by an experienced observer. CR was defined by greater than or equal to5% improvement of stress compared with rest ejection fraction (EF). Exercise capacity was assessed from expired gas analysis. B-type natriuretic peptide (BNP) levels increase in systolic heart failure (HF). However, the value of BNP in hypertensive patients with suspected diastolic HF (symptoms suggestive of HF but normal ejection fraction) and its relation to myocardial function in these patients is unclear. We prospectively studied 72 ambulatory hypertensive subjects (40 women, mean age 58 +/- 8 years) with exertional dyspnea and ejection fraction greater than or equal to50%. Diastolic function was evaluated with transmitral and pulmonary venous Doppler, mitral annular velocities (pulsed-wave tissue Doppler), and flow propagation velocity (color M-mode). Systolic function was assessed with strain and strain rate derived from color tissue Doppler imaging. BNP was related to myocardial function and the presence or absence of global diastolic dysfunction. By conventional Doppler criteria, 34 patients had normal left ventricular diastolic function and 38 had isolated diastolic dysfunction. BNP values were higher in patients with diastolic dysfunction (46 +/- 48 vs 20 +/- 20 pg/ml, p = 0.004) and were related independently to blood pressure, systolic strain rate, left atrial function (p < 0.01 for all), and age (p = 0.015). Patients with diastolic dysfunction and pseudonormal filling had higher BNP levels compared with impaired relaxation (89 +/- 47 vs 35 +/- 42 pg/ml, p = 0.001). However, 79% of patients with diastolic dysfunction had BNP levels within the normal range. We conclude that in ambulatory hypertensive patients with symptoms suggestive of mild HF and normal ejection fraction, BNP is related to atrial and ventricular systolic parameters, blood pressure, and age. Although elevated in the presence of diastolic dysfunction, the BNP level mostly is in the normal range and, therefore, has limited diagnostic value in stable patients with suspected diastolic HF. The left ventricular response to dobutamine may be quantified using tissue Doppler measurement of myocardial velocity or displacement or 3-dimensional echocardiography to measure ventricular volume and ejection fraction. This study sought to explore the accuracy of these methods for predicting segmental and global responses to therapy. Standard dobutamine and 3-dimensional echocardiography were performed in 92 consecutive patients with abnormal left ventricular function at rest. Recovery of function was defined by comparison with follow-up echocardiography at rest 5 months later. Segments that showed improved regional function at follow-up showed a higher increment in peak tissue Doppler velocity with dobutamine therapy than in nonviable segments (1.2 +/- 0.4 vs 0.3 +/- 0.2 cm/s, p = 0.001). Similarly, patients who showed a > 5% improvement of ejection fraction at follow-up showed a greater displacement response to dobutamine (6.9 +/- 3.2 vs 2.1 +/- 2.3 mm, p = 0.001), as well as a higher rate of ejection fraction, response to dobutamine (9 +/- 3% vs 2 +/- 2%, p = 0.001). The optimal cutoff values for predicting subsequent recovery of function at rest were an increment of peak velocity > 1 cm/s, >5 mm of displacement, and a >5% improvement of ejection fraction with low-dose dobutamine. The detection of preclinical heart disease is a new direction in diabetes care. This comment describes the study by Vinereanu and co-workers in this issue of Clinical Science in which tissue Doppler echocardiography has been employed to demonstrate subtle systolic and diastolic dysfunction in Type 11 diabetic patients who had normal global systolic function and were free of coronary artery disease. The aetiology of early ventricular dysfunction in diabetes relates to complex intramyocardial and extramyocardial mechanisms. The initiating event may be due to insulin resistance, and involves abnormal myocardial substrate utilization and uncoupling of mitochondrial oxidative phosphorylation. Dysglycaemia plays an important role via the effects of oxidative stress, protein kinase C activation and advanced glycosylation end-products on inflammatory signalling, collagen metabolism and fibrosis. Extramyocardial mechanisms involve peripheral endothelial dysfunction, arterial stiffening and autonomic neuropathy. The clinical significance of the ventricular abnormalities described is unknown. Confirmation of their prognostic importance for cardiac disease in diabetes would justify routine screening for presymptomatic ventricular dysfunction, as well as clinical trials of novel agents for correcting causal mechanisms. These considerations could also have implications for patients with obesity and the metabolic syndrome.

Myocardial infarction leads to compensatory ventricular remodeling. Disturbances in myocardial contractility depend on the active transport of Ca2+ and Na+, which are regulated by Na+-K+ ATPase. Inappropriate regulation of Na+-K+ ATPase activity leads to excessive loss of K+ and gain of Na+ by the cell. We determined the participation of Na+-K+ ATPase in ventricular performance early and late after myocardial infarction. Wistar rats (8-10 per group) underwent left coronary artery ligation (infarcted, Inf) or sham-operation (Sham). Ventricular performance was measured at 3 and 30 days after surgery using the Langendorff technique. Left ventricular systolic pressure was obtained under different ventricular diastolic pressures and increased extracellular Ca2+ concentrations (Ca2+e) and after low and high ouabain concentrations. The baseline coronary perfusion pressure increased 3 days after myocardial infarction and normalized by 30 days (Sham 3 = 88 ± 6; Inf 3 = 130 ± 9; Inf 30 = 92 ± 7 mmHg; P < 0.05). The inotropic response to Ca2+e and ouabain was reduced at 3 and 30 days after myocardial infarction (Ca2+ = 1.25 mM; Sham 3 = 70 ± 3; Inf 3 = 45 ± 2; Inf 30 = 29 ± 3 mmHg; P < 0.05), while the Frank-Starling mechanism was preserved. At 3 and 30 days after myocardial infarction, ventricular Na+-K+ ATPase activity and contractility were reduced. This Na+-K+ ATPase hypoactivity may modify the Na+, K+ and Ca2+ transport across the sarcolemma resulting in ventricular dysfunction.

Lead (Pb2+) poisoning causes hypertension, but little is known regarding its acute effects on cardiac contractility. To evaluate these effects, force was measured in right ventricular strips that were contracting isometrically in 45 male Wistar rats (250-300 g) before and after the addition of increasing concentrations of lead acetate (3, 7, 10, 30, 70, 100, and 300 µM) to the bath. Changes in rate of stimulation (0.1-1.5 Hz), relative potentiation after pauses of 15, 30, and 60 s, effect of Ca2+ concentration (0.62, 1.25, and 2.5 mM), and the effect of isoproterenol (20 ng/mL) were determined before and after the addition of 100 µM Pb2+. Effects on contractile proteins were evaluated after caffeine treatment using tetanic stimulation (10 Hz) and measuring the activity of the myosin ATPase. Pb2+ produced concentration-dependent force reduction, significant at concentrations greater than 30 µM. The force developed in response to increasing rates of stimulation became smaller at 0.5 and 0.8 Hz. Relative potentiation increased after 100 µM Pb2+ treatment. Extracellular Ca2+ increment and isoproterenol administration increased force development but after 100 µM Pb2+ treatment the force was significantly reduced suggesting an effect of the metal on the sarcolemmal Ca2+ influx. Concentration of 100 µM Pb2+ also reduced the peak and plateau force of tetanic contractions and reduced the activity of the myosin ATPase. Results showed that acute Pb2+ administration, although not affecting the sarcoplasmic reticulum activity, produces a concentration-dependent negative inotropic effect and reduces myosin ATPase activity. Results suggest that acute lead administration reduced myocardial contractility by reducing sarcolemmal calcium influx and the myosin ATPase activity. These results also suggest that lead exposure is hazardous and has toxicological consequences affecting cardiac muscle.

Myocardial contractility depends on several mechanisms such as coronary perfusion pressure (CPP) and flow as well as on a1-adrenoceptor stimulation. Both effects occur during the sympathetic stimulation mediated by norepinephrine. Norepinephrine increases force development in the heart and produces vasoconstriction increasing arterial pressure and, in turn, CPP. The contribution of each of these factors to the increase in myocardial performance needs to be clarified. Thus, in the present study we used two protocols: in the first we measured mean arterial pressure, left ventricular pressure and rate of rise of left ventricular pressure development in anesthetized rats (N = 10) submitted to phenylephrine (PE) stimulation before and after propranolol plus atropine treatment. These observations showed that in vivo a1-adrenergic stimulation increases left ventricular-developed pressure (P<0.05) together with arterial blood pressure (P<0.05). In the second protocol, we measured left ventricular isovolumic systolic pressure (ISP) and CPP in Langendorff constant flow-perfused hearts. The hearts (N = 7) were perfused with increasing flow rates under control conditions and PE or PE + nitroprusside (NP). Both CPP and ISP increased (P<0.01) as a function of flow. CPP changes were not affected by drug treatment but ISP increased (P<0.01). The largest ISP increase was obtained with PE + NP treatment (P<0.01). The results suggest that both mechanisms, i.e., direct stimulation of myocardial a1-adrenoceptors and increased flow, increased cardiac performance acting simultaneously and synergistically.

Objetivo: Determinar as alterações de atividade da enzima conversora de angiotensina (ECA) no coração com infarto do miocárdio (IM) e comparar os efeitos do captopril e losartan em parâmetros morfológicos e funcionais de ratos com IM. Métodos: O IM foi produzido em ratos Wistar por ligadura de ramos da artéria coronária esquerda. Os controles (Con) foram submetidos a uma cirurgia fictícia. Animais com IM e Con foram tratados com captopril (30mg/kg/dia) ou losartan (15mg/kg/dia) e estudados 30 dias após, determinando-se a atividade da ECA nos ventrículos direito (VD) e esquerdo (VE), as alterações hemodinâmicas e as concentrações de hidroxiprolina (OH-Pro) e proteína total no VD e VE. Resultados: A atividade da ECA aumentou no VD (+25%) e VE (+70%) após IM. A maior atividade foi observada na cicatriz fibrótica, onde atingiu cerca de 4,5 vezes a do músculo do VE que sobreviveu ao IM (420±68 vs 94±8nmoles/g/min; P<0,01). O IM determinou aumento da pressão diastólica final e hipertrofia do VD e VE. Captopril e losartan foram igualmente eficazes em atenuar a hipertrofia e o aumento da pré-carga. O captopril também atenuou o aumento de OH-Pro no VD e VE após IM. O IM reduziu a concentração de proteína principalmente no músculo de VE, efeito esse acentuado pelo captopril. Conclusão: A grande atividade da ECA na cicatriz deve produzir altas concentrações de angiotensina II (AII) no sangue que drena da cicatriz. Os efeitos dos inibidores da ECA seriam decorrentes, principalmente, da redução de geração local de AII, e não de aumento de cininas, uma vez que captopril e losartan exerceram efeitos similares no remodelamento pós-infarto.

Cardiac hypertrophy that accompanies hypertension seems to be a phenomenon of multifactorial origin whose development does not seem to depend on an increased pressure load alone, but also on local growth factors and cardioadrenergic activity. The aim of the present study was to determine if sympathetic renal denervation and its effects on arterial pressure level can prevent cardiac hypertrophy and if it can also delay the onset and attenuate the severity of deoxycorticosterone acetate (DOCA)-salt hypertension. DOCA-salt treatment was initiated in rats seven days after uninephrectomy and contralateral renal denervation or sham renal denervation. DOCA (15 mg/kg, sc) or vehicle (soybean oil, 0.25 ml per animal) was administered twice a week for two weeks. Rats treated with DOCA or vehicle (control) were provided drinking water containing 1% NaCl and 0.03% KCl. At the end of the treatment period, mean arterial pressure (MAP) and heart rate measurements were made in conscious animals. Under ether anesthesia, the heart was removed and the right and left ventricles (including the septum) were separated and weighed. DOCA-salt treatment produced a significant increase in left ventricular weight/body weight (LVW/BW) ratio (2.44 ± 0.09 mg/g) and right ventricular weight/body weight (RVW/BW) ratio (0.53 ± 0.01 mg/g) compared to control (1.92 ± 0.04 and 0.48 ± 0.01 mg/g, respectively) rats. MAP was significantly higher (39%) in DOCA-salt rats. Renal denervation prevented (P>0.05) the development of hypertension in DOCA-salt rats but did not prevent the increase in LVW/BW (2.27 ± 0.03 mg/g) and RVW/BW (0.52 ± 0.01 mg/g). We have shown that the increase in arterial pressure level is not responsible for cardiac hypertrophy, which may be more related to other events associated with DOCA-salt hypertension, such as an increase in cardiac sympathetic activity.

O equilíbrio corporal é fundamental no relacionamento espacial do organismo com o ambiente. Três sistemas são responsáveis pela manutenção do equilíbrio: a visão, o sistema proprioceptivo e o aparelho labiríntico ou vestibular. FORMA DE ESTUDO: retrospectivo clínico. Foi realizado um estudo retrospectivo de 3701 pacientes submetidos a exame clínico e exame cócleo-vestibular com registro vecto-eletronistagmográfico, em clínica particular de Otorrinolaringologia de Jundiaí, Estado de São Paulo, no período de 1979 a 2004. OBJETIVO: com a finalidade de se determinar a distribuição sindrômica dessa população e correlacioná-la com outros dados como sexo, faixa etária, sintomatologia, achados de exame clínico, audiológico, bem como quais são as especialidades médicas que solicitam essa avaliação com maior freqüência. RESULTADO: Na população estudada, foi encontrada uma maior prevalência do sexo feminino, numa proporção de 1,75:1. A faixa etária de indivíduos de 20 a 59 anos respondeu por 79% dos pacientes estudados, incluindo portanto pessoas em idade laborativa, que apresentaram maior prevalência de síndromes periféricas, contudo não houve predominância de sexo feminino ou idade entre as diferentes síndromes. O estudo também demonstrou a presença de sintomas otoneurológicos comuns aos diferentes tipos de síndromes otoneurológicas, por exemplo, sem o predomínio clássico apontado na literatura das tonturas rotatórias predominarem nos casos de acometimento periférico e as não-rotatórias nos centrais. Alterações da acuidade auditiva, presença de zumbidos e sintomas neurovegetativos ocorreram com maior prevalência nos casos periféricos. Os desvios harmônicos às provas segmentares foram mais prevalentes nos pacientes portadores de síndromes periféricas e os desarmônicos, nas centrais, em consonância com a literatura pesquisada. CONCLUSÃO: As conclusões desta análise retrospectiva apontam para as especialidades de Otorrinolaringologia e Neurologia como as que mais freqüentemente solicitam este tipo de avaliação e o diagnóstico de síndrome periférica foi encontrado em 36% dos pacientes, contudo ¼ da população estudada não apresentou qualquer alteração na avaliação clínica e armada.

OBJETIVO: Verificar se os parâmetros dos movimentos sacádicos fixos e randomizados, do rastreio pendular e do nistagmo optocinético na vectonistagmografia digital podem apresentar alterações em pacientes com hipótese diagnóstica de disfunção vestibular periférica. MÉTODO: Foram avaliados 60 pacientes, de ambos os sexos, com tontura de origem vestibular periférica e idade entre 12 e 82 anos. Os achados foram comparados com um padrão de normalidade para os parâmetros dos movimentos oculares estudados. RESULTADOS: Os movimentos sacádicos fixos estavam alterados em 100% dos casos quanto à latência e em 35% quanto à velocidade; os randomizados estavam alterados em 100% quanto à latência, em 78,3% quanto à precisão e 1,7% quanto à velocidade; o rastreio pendular apresentou alteração do ganho nas freqüências de 0,1Hz em 15%, 0,2Hz em 21,7% e 0,4Hz em 13,3%; o nistagmo optocinético apresentou alteração da velocidade angular da componente lenta em 1,7% e do ganho em 5%. CONCLUSÕES: A latência e a velocidade dos movimentos sacádicos fixos; a latência, a precisão e a velocidade dos movimentos randomizados; o ganho do rastreio pendular; a velocidade angular da componente lenta e o ganho do nistagmo optocinético na vectonistagmografia digital podem apresentar alterações em pacientes com hipótese diagnóstica de disfunção vestibular periférica.

A tontura de origem vestibular é comum entre idosos. OBJETIVO: Caracterizar idosos com disfunção vestibular crônica em relação aos dados sociodemográficos, clínico-funcionais e otoneurológicos. MATERIAL E MÉTODO: Estudo de casos que incluiu 120 idosos com disfunção vestibular crônica. Foram realizadas análises descritivas simples. RESULTADOS: A 5,77±amostra apresentou maioria feminina (68,3%), com média etária de 73,40 1,84±anos. O número médio de doenças associadas ao quadro vestibular foi de 3,83 e o número médio de medicamentos foi de 3,86±2,27. O exame vestibular evidenciou vestibulopatia periférica deficitária unilateral (29,8%) e as hipóteses diagnósticas prevalentes foram labirintopatia metabólica (40,0%) e vertigem posicional paroxística benigna (36,7%). Para 52 (43,3%) pacientes, a tontura começou há mais de 5 anos. Em relação à queda, 64 idosos (53,3%) apresentaram pelo menos uma queda no último ano e 35 (29,2%) referiram quedas recorrentes. CONCLUSÕES: A amostra foi representada por maioria feminina e média etária elevada, com doenças associadas ao quadro vestibular e polifarmacoterapia. As vestibulopatias e a topografia mais freqüentes foram, respectivamente, labirintopatia metabólica e vascular e síndrome periférica deficitária unilateral. A tontura é uma condição crônica e a associação de vestibulopatias é comum. A ocorrência de quedas é prevalente na população de idosos com disfunção vestibular crônica.
