FUNDAMENTO: A sedação com midazolam e meperidina é amplamente utilizada em ecocardiografia transesofágica, entretanto, não existe dose média estabelecida para cada caso. OBJETIVO: Correlacionar as doses médias de midazolam e meperidina para sedação adequada em ecocardiografia transesofágica com faixa etária, área de superfície corpórea e fração de ejeção do ventrículo esquerdo. MÉTODOS: Estudo retrospectivo envolvendo 1.841 pacientes submetidos à sedação baseada na escala de Ramsay, com solução contendo midazolam 1,5 mg (1,5 ml), meperidina 1 mg (1 ml) e água destilada (7,5 ml). Analisamos quatro grupos etários: G1: < 24 anos; G2: 25 a 44 anos; G3: 45 a 64 anos; e G4: > 65 anos. Obtivemos a área de superfície corpórea pela fórmula: {[(altura x 100)0,725] x (peso0,425) x 0,0071}. Com relação à fração de ejeção do ventrículo esquerdo, estudamos dois grupos: GA: < 55%; e GB: > 55%. Na análise estatística utilizamos o teste de Kruskal-Wallis para correlação com idade e fração de ejeção do ventrículo esquerdo, e correlação linear simples para área de superfície corpórea. RESULTADOS: No estudo da idade, as doses médias de sedação necessárias foram significativamente menores no G3 e G4 (p < 0,01). Na análise da fração de ejeção do ventrículo esquerdo, esta foi significativamente menor no GA (p < 0,01). O coeficiente de correlação linear entre dose de sedação e área de superfície corpórea foi 0,09 (nulo). CONCLUSÃO: Houve menor dose média necessária de sedativos nos indivíduos com maior idade e em portadores de disfunção sistólica do ventrículo esquerdo, e não houve correlação com área de superfície corpórea.

FUNDAMENTO: A arginina-vasopressina (AVP) tem sido amplamente utilizada no tratamento do choque vasodilatador. Entretanto, há muitas questões relativas ao seu uso clínico, especialmente em altas doses, pois sua utilização pode estar associada a efeitos cardíacos adversos. OBJETIVO: Investigar os efeitos cardiovasculares da AVP em infusão IV contínua nos parâmetros hemodinâmicos em cães. MÉTODOS: Dezesseis cães saudáveis sem raça definida, anestesiados com pentobarbital, receberam um cateter intravascular e foram aleatoriamente designados para dois grupos: controle (solução salina - placebo; n=8) e AVP (n=8). O grupo do estudo recebeu infusão de AVP por três períodos consecutivos de 10 minutos a doses logaritmicamente progressivas (0,01; 0,1 e 1,0 U/kg/min), a intervalos de 20 minutos. A frequência cardíaca (HR) e as pressões intravasculares foram continuamente registradas. O debito cardíaco foi medido através do método de termodiluição. RESULTADOS: Nenhum efeito hemodinâmico significante foi observado durante a infusão de 0,01 U/kg/min de AVP, mas com as doses mais altas, de 0,1 e 1,0U/kg/min, houve um aumento progressivo na pressão arterial média (PAM) e índice de resistência vascular sistêmica (IRVS), com significante diminuição na frequência cardíaca (FC) e índice cardíaco (IC). Com a dose de 1,0 U/kg/min, também foi observado um aumento significante no índice de resistência vascular pulmonar (IRVP), principalmente devido à diminuição no IC. CONCLUSÃO: A AVP em doses entre 0,1 e 1,0 U/kg/min resultou em significantes aumentos na PAM e no IRVS, com efeitos inotrópicos e cronotrópicos negativos em animais saudáveis. Embora essas doses sejam de 10 a 1.000 vezes maiores do que as rotineiramente utilizadas no tratamento do choque vasodilatador, nossos dados confirmam que a AVP deveria ser usada cuidadosamente e sob rígida monitoração hemodinâmica na prática clínica, especialmente se doses maiores do que 0,01 U/kg/min forem necessárias.

Although the ASP model has been around for over a decade, it has not achieved the expected high level of market uptake. This research project examines the past and present state of ASP adoption and identifies security as a primary factor influencing the uptake of the model. The early chapters of this document examine the ASP model and ASP security in particular. Specifically, the literature and technology review chapter analyses ASP literature, security technologies and best practices with respect to system security in general. Based on this investigation, a prototype to illustrate the range and types of technologies that encompass a security framework was developed and is described in detail. The latter chapters of this document evaluate the practical implementation of system security in an ASP environment. Finally, this document outlines the research outputs, including the conclusions drawn and recommendations with respect to system security in an ASP environment. The primary research output is the recommendation that by following best practices with respect to security, an ASP application can provide the same level of security one would expect from any other n-tier client-server application. In addition, a security evaluation matrix, which could be used to evaluate not only the security of ASP applications but the security of any n-tier application, was developed by the author. This thesis shows that perceptions with regard to fears of inadequate security of ASP solutions and solution data are misguided. Finally, based on the research conducted, the author recommends that ASP solutions should be developed and deployed on tried, tested and trusted infrastructure. Existing Application Programming Interfaces (APIs) should be used where possible and security best practices should be adhered to where feasible.

FUNDAMENTO: A ecocardiografia de estresse com dobutamina de baixa dose é um teste específico para predizer disfunção de contratilidade reversível, mas mesmo assim, sua sensibilidade é menor do que ideal. OBJETIVO: Avaliar os preditores de recuperação miocárdica contrátil após a revascularização, em pacientes sem viabilidade na ecocardiografia de estresse com dobutamina de baixa dose. MÉTODOS: Trinta pacientes consecutivos foram selecionados consecutivamente, que apresentavam estenose coronária/oclusão significantes, tratáveis através de revascularização, anormalidade de motilidade de parede regional na distribuição da artéria afetada e ausência de viabilidade na ecocardiografia de estresse com dobutamina de baixa dose. Os pacientes foram submetidos a estudo de imagem com 99mTc-sestamibi em repouso e então submetidos à revascularização coronária bem sucedida. A ecocardiografia de seguimento foi realizada três meses depois. Os pacientes foram classificados em 2 grupos: grupo 1: com evidência de recuperação miocárdica contrátil após a revascularização na ecocardiografia de seguimento e grupo 2: sem evidência de recuperação miocárdica. Os dois grupos foram comparados em relação aos dados clínicos, ecocardiográficos e cintilográficos. RESULTADOS: A média da idade era 52,3 ± 5,9 anos e 97% eram do sexo masculino. A porcentagem de captação total de 99mTc-sestamibi foi significantemente mais alta no grupo 1 quando comparado ao grupo 2 (p < 0,01) e foi o preditor independente mais forte de recuperação miocárdica contrátil no seguimento de 3 meses na análise de regressão multivariada. A curva ROC (Receiver Operating Characteristic) mostrou que um valor de corte da porcentagem de captação total do 99mTc-sestamibi uptake de 72%, foi o melhor preditor da recuperação miocárdica contrátil, com uma sensibilidade de 100% e especificidade de 95.7%. CONCLUSÃO: Em pacientes sem evidência de viabilidade na ecocardiografia de estresse com dobutamina de baixa dose, a porcentagem de captação total do 99mTc-sestamibi prediz, de forma independente, a recuperação miocárdica contrátil após a revascularização coronária.

Background: The use of three-dimensional rotational angiography (3D-RA) to assess patients with congenital heart diseases appears to be a promising technique despite the scarce literature available. Objectives: The objective of this study was to describe our initial experience with 3D-RA and to compare its radiation dose to that of standard two-dimensional angiography (2D-SA). Methods: Between September 2011 and April 2012, 18 patients underwent simultaneous 3D-RA and 2D-SA during diagnostic cardiac catheterization. Radiation dose was assessed using the dose-area-product (DAP). Results: The median patient age and weight were 12.5 years and 47.5 Kg, respectively. The median DAP of each 3D-RA acquisition was 1093µGy.m2 and 190µGy.m2 for each 2D-SA acquisition (p<0.01). In patients weighing more than 45Kg (n=7), this difference was attenuated but still significant (1525 µGy.m2 vs.413µGy.m2, p=0.01). No difference was found between one 3D-RA and three 2D-SA (1525µGy.m2 vs.1238 µGy.m2, p = 0.575) in this population. This difference was significantly higher in patients weighing less than 45Kg (n=9) (713µGy.m2 vs.81µGy.m2, P = 0.008), even when comparing one 3D-RA with three 2D-SA (242µGy.m2, respectively, p<0.008). 3D-RA was extremely useful for the assessment of conduits of univentricular hearts, tortuous branches of the pulmonary artery, and aorta relative to 2D-SA acquisitions. Conclusions: The radiation dose of 3D-RA used in our institution was higher than those previously reported in the literature and this difference was more evident in children. This type of assessment is of paramount importance when starting to perform 3D-RA.

Background:Statins have proven efficacy in the reduction of cardiovascular events, but the financial impact of its widespread use can be substantial.Objective:To conduct a cost-effectiveness analysis of three statin dosing schemes in the Brazilian Unified National Health System (SUS) perspective.Methods:We developed a Markov model to evaluate the incremental cost-effectiveness ratios (ICERs) of low, intermediate and high intensity dose regimens in secondary and four primary scenarios (5%, 10%, 15% and 20% ten-year risk) of prevention of cardiovascular events. Regimens with expected low-density lipoprotein cholesterol reduction below 30% (e.g. simvastatin 10mg) were considered as low dose; between 30-40%, (atorvastatin 10mg, simvastatin 40mg), intermediate dose; and above 40% (atorvastatin 20-80mg, rosuvastatin 20mg), high-dose statins. Effectiveness data were obtained from a systematic review with 136,000 patients. National data were used to estimate utilities and costs (expressed as International Dollars - Int$). A willingness-to-pay (WTP) threshold equal to the Brazilian gross domestic product per capita (circa Int$11,770) was applied.Results:Low dose was dominated by extension in the primary prevention scenarios. In the five scenarios, the ICER of intermediate dose was below Int$10,000 per QALY. The ICER of the high versus intermediate dose comparison was above Int$27,000 per QALY in all scenarios. In the cost-effectiveness acceptability curves, intermediate dose had a probability above 50% of being cost-effective with ICERs between Int$ 9,000-20,000 per QALY in all scenarios.Conclusions:Considering a reasonable WTP threshold, intermediate dose statin therapy is economically attractive, and should be a priority intervention in prevention of cardiovascular events in Brazil.

Abstract Background: The effect of statins on the endothelial function in humans remains under discussion. Particularly, it is still unclear if the improvement in endothelial function is due to a reduction in LDL-cholesterol or to an arterial pleiotropic effect. Objective: To test the hypothesis that modulation of the endothelial function promoted by statins is primarily mediated by the degree of reduction in LDL-cholesterol, independent of the dose of statin administered. Methods: Randomized clinical trial with two groups of lipid-lowering treatment (16 patients/each) and one placebo group (14 patients). The two active groups were designed to promote a similar degree of reduction in LDL-cholesterol: the first used statin at a high dose (80 mg, simvastatin 80 group) and the second used statin at a low dose (10 mg) associated with ezetimibe (10 mg, simvastatin 10/ezetimibe group) to optimize the hypolipidemic effect. The endothelial function was assessed by flow-mediated vasodilation (FMV) before and 8 weeks after treatment. Results: The decrease in LDL-cholesterol was similar between the groups simvastatin 80 and simvastatin 10/ezetimibe (27% ± 31% and 30% ± 29%, respectively, p = 0.75). The simvastatin 80 group presented an increase in FMV from 8.4% ± 4.3% at baseline to 11% ± 4.2% after 8 weeks (p = 0.02). Similarly, the group simvastatin 10/ezetimibe showed improvement in FMV from 7.3% ± 3.9% to 12% ± 4.4% (p = 0.001). The placebo group showed no variation in LDL-cholesterol level or endothelial function. Conclusion: The improvement in endothelial function with statin seems to depend more on a reduction in LDL-cholesterol levels, independent of the dose of statin administered, than on pleiotropic mechanisms.

The authors discuss a formula for the determination of the most profitable level of fertilization (x*). This formula is a modification of one of the formulas of PIMENTEL GOMES (1953). One of its advantages is that is does not depend on A, the theoretical maximum harvest, which is not directly given by experimental data. But another advantage, proved in this. paper, is that the first term on the right hand side K= 1(/c) log cx u L 10 ____________ 1 - 10-cx u is practically independent of c, and approximately equivalent to (1/2) x u. This is an easy way to determine the most profitable level of fertilization when experimental data on the response u to a dressing x u are available. Tables for the calculation of Y are included, for nitrogen, phosphorus, potash, and manure.

This report belongs to the series of works carried out Oswaldo Cruz Ins¬titute, on the treatment of treponematosis with penicillin. The present report deals with investigations performed in order to ascer¬tain the following points: 1) the mininal curative dosis for the initial lesions of yaws; 2) the effect of reduction of the number of injections each day, to verifie the possibility of application of penicillin in the prophylaxis of yaws in rural zones; 3) reduction of the time of treatment by application of high dosis. 1) With dosis of 150 and 100 Oxford units each four hours, clinical reco¬very was obtained after 17 days of treatment. With 50 O.u. during 40 days clinical recovery was not obtained. 2) a) With 3 injections of 400 O.u. each day (6,12 and 18 hoórs clocks) clinical recovery was obtained after 14 to 16 days; b) with 2 injections of 400 O.u. each day (6 and 18 hoors clocks), clinical recovery was obtained after 16 to 23 days; c) with 1 injection of 1.600 and 3.200 each day, clinical recovery was obtained after 30 and 20 days. 3) With dosis of 33.3 and 46.7 O.u. by each kilo of weight each four hours, during 15 days, clinical recovery was obtained more or less in 25 days. The same result was obtained with the dosis of 61.5 and 166.7 O.u. by each kilo of weight, each four hours, during 4 days. But with 100.000 O.u. in fine dosis of 20.000 in a day ou by, clinical recovery was not obtained.

Actualment, la resposta de la majoria d’instrumentació operacional i dels dosímetres personals utilitzats en radioprotecció per a la dosimetria neutrònica és altament dependent de l’energia dels espectres neutrònics a analitzar, especialment amb camps neutrònics amb una important component intermitja. En conseqüència, la interpretació de les lectures d’aquests aparells es complicada si no es té un coneixement previ de la distribució espectral de la fluència neutrònica en els punts d’interès. El Grup de Física de les Radiacions de la Universitat Autònoma de Barcelona (GFR-UAB) ha desenvolupat en els últims anys un espectròmetre de neutrons basat en un Sistema d’Esferes Bonner (BSS) amb un contador proporcional d’3He com a detector actiu. Els principals avantatges dels espectròmetres de neutrons per BSS són: la seva resposta isotròpica, la possibilitat de discriminar la component neutrònica de la gamma en camps mixtos, i la seva alta sensibilitat neutrònica als nivells de dosi analitzats. Amb aquestes característiques, els espectròmetres neutrònics per BSS compleixen amb els estándards de les últimes recomanacions de la ICRP i poden ser utilitzats també en el camp de la dosimetria neutrònica per a la mesura de dosis en el rang d’energia que va dels tèrmics fins als 20 MeV, en nou ordres de magnitud. En el marc de la col•laboració entre el GFR - UAB i el Laboratorio Nazionale di Frascati – Istituto Nazionale di Fisica Nucleare (LNF-INFN), ha tingut lloc una experiència comparativa d’espectrometria per BSS amb els feixos quasi monoenergètics de 2.5 MeV i 14 MeV del Fast Neutron Generator de l’ENEA. En l’exercici s’ha determinat l’espectre neutrònic a diferents distàncies del blanc de l’accelerador, aprofitant el codi FRUIT recentment desenvolupat pel grup LNF. Els resultats obtinguts mostren una bona coherència entre els dos espectròmetres i les dades mesurades i simulades.

Introduction: We previously reported the results of a phase II study for patients with newly diagnosed primary CNS lymphoma (PCNSL) treated with autologous peripheral blood stem-cell transplantation (aPBSCT) and responseadapted whole brain radiotherapy (WBRT). The purpose of this report is to update the initial results and provide long-term data regarding overall survival, prognostic factors, and the risk of treatment-related neurotoxicity.Methods: A long-term follow-up was conducted on surviving primary central nervous system lymphoma patients having been treated according to the ,,OSHO-53 study", which was initiated by the Ostdeutsche Studiengruppe Hamatologie-Onkologie. Between August 1999 and October 2004 twentythree patients with an average age of 55 and median Karnofsky performance score of 70% were enrolled and received high-dose mthotrexate (HD-MTX) on days 1 and 10. In case of at least a partial remission (PR), high-dose busulfan/ thiotepa (HD-BuTT) followed by aPBSCT was performed. Patients without response to induction or without complete remission (CR) after HD-BuTT received WBRT. All patients (n=8), who are alive in 2011, were contacted and Mini Mental State examination (MMSE) and the EORTC QLQ-C30 were performed.Results: Eight patients are still alive with a median follow-up of 116,9 months (79 - 141, range). One of them suffered from a late relapse eight and a half years after initial diagnosis of PCNSL, another one suffers from a gall bladder carcinoma. Both patients are alive, the one with the relapse of PCNSL has finished rescue therapy and is further observed, the one with gall baldder carcinoma is still under therapy. MMSE and QlQ-C30 showed impressive results in the patients, who were not irradiated. Only one of the irradiated patients is still alive with a clear neurologic deficit but acceptable quality of life.Conclusions: Long-term follow-up of our patients, who were included in the OSHO-53 study show an overall survival of 30 percent. If WBRT can be avoided no long-term neurotoxicity has been observed and the patients benefit from excellent Quality of Life. Induction chemotherapy with two cycles of HD-MTX should be intensified to improve the unsatisfactory OAS of 30 percent.

Where directly applicable, the work of others is included, but no complete review of the subject has been attempted.

INTRODUCTION: Solid tumors are known to have an abnormal vasculature that limits the distribution of chemotherapy. We have recently shown that tumor vessel modulation by low-dose photodynamic therapy (L-PDT) could improve the uptake of macromolecular chemotherapeutic agents such as liposomal doxorubicin (Liporubicin) administered subsequently. However, how this occurs is unknown. Convection, the main mechanism for drug transport between the intravascular and extravascular spaces, is mostly related to interstitial fluid pressure (IFP) and tumor blood flow (TBF). Here, we determined the changes of tumor and surrounding lung IFP and TBF before, during, and after vascular L-PDT. We also evaluated the effect of these changes on the distribution of Liporubicin administered intravenously (IV) in a lung sarcoma metastasis model. MATERIALS AND METHODS: A syngeneic methylcholanthrene-induced sarcoma cell line was implanted subpleurally in the lung of Fischer rats. Tumor/surrounding lung IFP and TBF changes induced by L-PDT were determined using the wick-in-needle technique and laser Doppler flowmetry, respectively. The spatial distribution of Liporubicin in tumor and lung tissues following IV drug administration was then assessed in L-PDT-pretreated animals and controls (no L-PDT) by epifluorescence microscopy. RESULTS: L-PDT significantly decreased tumor but not lung IFP compared to controls (no L-PDT) without affecting TBF. These conditions were associated with a significant improvement in Liporubicin distribution in tumor tissues compared to controls (P < .05). DISCUSSION: L-PDT specifically enhanced convection in blood vessels of tumor but not of normal lung tissue, which was associated with a significant improvement of Liporubicin distribution in tumors compared to controls.
