PURPOSE: To compare two single-agent chemotherapy (ChT) regimens evaluating, in first-line treatment, response and side effects and, in final single-agent treatment, the outcomes, among Brazilian patients with low-risk gestational trophoblastic neoplasia (GTN), according to International Federation of Gynecology and Obstetrics (FIGO) 2002. METHODS: Retrospective analysis of two concurrent cohorts with 194 low-risk GTN patients: from 1992 to 2012, as first-line treatment, 115 patients received 4 intramuscular doses of methotrexate alternated with 4 oral doses of folinic acid (MTX/FA) repetead every 14 days and, since 1996, 79 patients received an endovenous bolus-dose of actinomycin D (Act-D), biweekly. At GTN diagnosis, patient opinion was taken into consideration when defining the initial single-agent ChT regimen, and when there was resistance or toxicity to one regimen, the other drug was used preferentially. This study was approved by the Irmandade da Santa Casa de Misericórdia de Porto Alegre Ethical Committee. RESULTS: Both groups were clinically similar (p>0.05). In first-line treatments, frequency of complete response was similar (75.7% with MTX/FA and 67.1% with bolus Act-D); the number of ChT courses -median 3 (range: 1-10) with MTX/FA and 2 (range: 1-6) with bolus Act-D - and the time to remission -median 9 weeks (range: 2-16) with MTX/FA and 10 weeks (range: 2-16) with bolus Act-D) - were not different between the groups. In both groups, first-line side effects frequency were high but intensity was low; stomatitis was higher with MTX/FA (p<0.01) and nausea and vomit with Act-D (p<0.01). Final single-agent ChT responses were high in both groups (94.8% with MTX/FA and 83.5% with bolus Act-D; p<0.01) and 13% higher in the group initially treated with MTX/FA. Rates of hysterectomy and of GTN recurrence were low and similar. No patient died due to GTN. CONCLUSION: The two regimens had similar first-line ChT response. Final single-agent response rates were high and similar in both groups but the final single-agent remission rate was higher in the MTX/FA group.

Changes in the electroencephalography (EEG) signal have been used to study the effects of anesthetic agents on the brain function. Several commercial EEG based anesthesia depth monitors have been developed to measure the level of the hypnotic component of anesthesia. Specific anesthetic related changes can be seen in the EEG, but still it remains difficult to determine whether the subject is consciousness or not during anesthesia. EEG reactivity to external stimuli may be seen in unconsciousness subjects, in anesthesia or even in coma. Changes in regional cerebral blood flow, which can be measured with positron emission tomography (PET), can be used as a surrogate for changes in neuronal activity. The aim of this study was to investigate the effects of dexmedetomidine, propofol, sevoflurane and xenon on the EEG and the behavior of two commercial anesthesia depth monitors, Bispectral Index (BIS) and Entropy. Slowly escalating drug concentrations were used with dexmedetomidine, propofol and sevoflurane. EEG reactivity at clinically determined similar level of consciousness was studied and the performance of BIS and Entropy in differentiating consciousness form unconsciousness was evaluated. Changes in brain activity during emergence from dexmedetomidine and propofol induced unconsciousness were studied using PET imaging. Additionally, the effects of normobaric hyperoxia, induced during denitrogenation prior to xenon anesthesia induction, on the EEG were studied. Dexmedetomidine and propofol caused increases in the low frequency, high amplitude (delta 0.5-4 Hz and theta 4.1-8 Hz) EEG activity during stepwise increased drug concentrations from the awake state to unconsciousness. With sevoflurane, an increase in delta activity was also seen, and an increase in alpha- slow beta (8.1-15 Hz) band power was seen in both propofol and sevoflurane. EEG reactivity to a verbal command in the unconsciousness state was best retained with propofol, and almost disappeared with sevoflurane. The ability of BIS and Entropy to differentiate consciousness from unconsciousness was poor. At the emergence from dexmedetomidine and propofol induced unconsciousness, activation was detected in deep brain structures, but not within the cortex. In xenon anesthesia, EEG band powers increased in delta, theta and alpha (8-12Hz) frequencies. In steady state xenon anesthesia, BIS and Entropy indices were low and these monitors seemed to work well in xenon anesthesia. Normobaric hyperoxia alone did not cause changes in the EEG. All of these results are based on studies in healthy volunteers and their application to clinical practice should be considered carefully.

The increasing use of nanotechnologies in advanced therapies has allowed the observation of specific adverse reactions related to nanostructures. The toxicity of a novel liposome formulation of meglumine antimoniate in dogs with visceral leishmaniasis after single dose has been investigated. Groups of 12 animals received by the intravenous route a single dose of liposomal meglumine antimoniate (group I [GI], 6.5 mg Sb/kg), empty liposomes (GII) or isotonic saline (GIII). Evaluation of hematological and biochemical parameters showed no significant changes 4 days after administration. No undesired effects were registered in the GIII. However, adverse reactions were observed in 67.7% of dogs from both groups that received liposomal formulations. The side effects began moments after bolus administration and disappeared during the first 15 minutes after treatment. Prostation, sialorrhea and defecation were the most frequent clinical signs, registered in 33.3% and 41.6 % of animals from the groups GI and GII, respectively. Tachypnea, mydriasis, miosis, vomiting and cyanosis were also registered in both groups. The adverse reactions observed in this study were attributed to the activation of the complement system by lipid vesicles in a phenomenon known as Complement Activation-Related Pseudoallergy (CARPA). The influence of the physical-chemical characteristics of liposomal formulation in the triggering of CARPA is discussed.

A single electroconvulsive shock (ECS) or a sham ECS was administered to male 3-4-month-old Wistar rats 1, 2, and 4 h before training in an inhibitory avoidance test and in cued classical fear conditioning (measured by means of freezing time in a new environment). ECS impaired inhibitory avoidance at all times and, at 1 or 2 h before training, reduced freezing time before and after re-presentation of the ECS. These results are interpreted as a transient conditioned stimulus (CS)-induced anxiolytic or analgesic effect lasting about 2 h after a single treatment, in addition to the known amnesic effect of the stimulus. This suggests that the effect of anterograde learning impairment is demonstrated unequivocally only when the analgesic/anxiolytic effect is over (about 4 h after ECS administration) and that this impairment of learning is selective, affecting inhibitory avoidance but not classical fear conditioning to a discrete stimulus.

The aim of the present study was to evaluate the impact of a multiple dose regimen of a liposomal formulation of meglumine antimoniate (LMA) on the pharmacokinetics of antimony in the bone marrow of dogs with visceral leishmaniasis and on the ability of LMA to eliminate parasites from this tissue. Dogs naturally infected with Leishmania chagasi received 4 intravenous doses of either LMA (6.5 mg antimony/kg body weight, N = 9), or empty liposomes (at the same lipid dose as LMA, N = 9) at 4-day intervals. A third group of animals was untreated (N = 8). Before each administration and at different times after treatment, bone marrow was obtained and analyzed for antimony level (LMA group) by electrothermal atomic absorption spectrometry, and for the presence of Leishmania parasites (all groups). There was a significant increase of antimony concentration from 0.76 µg/kg wet organ (4 days after the first dose) to 2.07 µg/kg (4 days after the fourth dose) and a half-life of 4 days for antimony elimination from the bone marrow. Treatment with LMA significantly reduced the number of dogs positive for parasites (with at least one amastigote per 1000 host cells) compared to controls (positive dogs 30 days after treatment: 0 of 9 in the LMA group, 3 of 9 in the group treated with empty liposomes and 3 of 8 in the untreated group). However, complete elimination of parasites was not achieved. In conclusion, the present study showed that multiple dose treatment with LMA was effective in improving antimony levels in the bone marrow of dogs with visceral leishmaniasis and in reducing the number of positive animals, even though it was not sufficient to achieve complete elimination of parasites.

The pharmacokinetics of some β-blockers are altered by cardiopulmonary bypass (CPB). The objective of this study was to compare the effect of coronary artery bypass graft (CABG) surgery employing CPB on the pharmacokinetics of propranolol and atenolol. We studied patients receiving oral propranolol with doses ranging from 80 to 240 mg (N = 11) or atenolol with doses ranging from 25 to 100 mg (N = 8) in the pre- and postoperative period of CABG with moderately hypothermic CPB (32°C). On the day before and on the first day after surgery, blood samples were collected before β-blocker administration and every 2 h thereafter. Plasma levels were determined using high-performance liquid chromatography and data were treated by pharmacokinetics-modelling. Statistical analysis was performed using ANOVA or the Friedman test, as appropriate, and P < 0.05 was considered to be significant. A prolongation of propranolol biological half-life from 5.41 ± 0.75 to 11.46 ± 1.66 h (P = 0.0028) and an increase in propranolol volume of distribution from 8.70 ± 2.83 to 19.33 ± 6.52 L/kg (P = 0.0032) were observed after CABG with CPB. No significant changes were observed in either atenolol biological half-life (from 11.20 ± 1.60 to 11.44 ± 2.89 h) or atenolol volume of distribution (from 2.90 ± 0.36 to 3.83 ± 0.72 L/kg). Total clearance was not changed by surgery. These CPB-induced alterations in propranolol pharmacokinetics may promote unexpected long-lasting effects in the postoperative period while the effects of atenolol were not modified by CPB surgery.

Ethanol abuse is linked to several acute and chronic injuries that can lead to health problems. Ethanol addiction is one of the most severe diseases linked to the abuse of this drug. Symptoms of ethanol addiction include compulsive substance intake and withdrawal syndrome. Stress exposure has an important role in addictive behavior for many drugs of abuse (including ethanol), but the consequences of stress and ethanol in the organism when these factors are concomitant results in a complex interaction. We investigated the effects of concomitant, chronic administration of ethanol and stress exposure on the withdrawal and consumption of, as well as the preference for, ethanol in mice. Male Swiss mice (30–35 g, 8-10 per group) were exposed to an ethanol liquid diet as the only source of food for 15 days. In the final 5 days, they were exposed to forced swimming stress. Twelve hours after removal of the ethanol liquid diet, animals were evaluated for ethanol withdrawal by measuring anxiety-related behaviors and locomotor activity. Twenty-four hours after evaluation of ethanol withdrawal, they were evaluated for voluntary consumption of ethanol in a “three-bottle choice” paradigm. Mice exposed to chronic consumption of ethanol had decreased locomotor activity during withdrawal. Contrary to our expectations, a concomitant forced swimming stress did not aggravate ethanol withdrawal. Nevertheless, simultaneous ethanol administration and stress exposure increased voluntary consumption of ethanol, mainly solutions containing high concentrations of ethanol. These results showed that stressful situations during ethanol intake may aggravate specific addiction-related behaviors.

Dent's disease type 1 is an X-linked tubular disease caused by mutations in the renal chloride channel CLCN-5, and it is characterized by low molecular weight proteinuria, hypercalciuria, nephrocalcinosis, and renal failure. Several cases have been described in which the only presenting symptoms were asymptomatic proteinuria, and focal segmental or global glomerulosclerosis. The renal failure in these patients may be caused by hypercalciuria and persistent proteinuria. Therefore, angiotensin converse enzyme inhibitor and thiazides could be useful. Our aim is to report the effects of these drugs in two novel mutations patients with Dent's disease type 1. In this report, no significant correlations between dosage of hydrochlorothiazide and calciuria and no significant correlations between proteinuria and dosage of enalapril were detected. This is important since these are polyuric patients and these drugs could be dangerous to their renal function.

ABSTRACTAfter more than twenty years of low housing construction output, the housing policy recovered its momentum in the country with the ascent of the Partido dos Trabalhadores (Workers' Party, PT) to the seat of the federal government. This article demonstrates - through the analysis of documents, interviews and research conducted with businessmen - that the impetus of such a state policy is a part of the PT electoral strategy, which is based on economic growth and the expansion of social programs. The research analyses the dovetailing of interests between the Lula (the Brazilian President from 2003 to 2010) administration and the civil construction business - the latter concerned with expanding its business, and the former with increasing the supply of jobs and the level of economic activity. This process culminated in the launching of the largest social housing program to be implemented in the country. Minha Casa, Minha Vida (My House, My Life), is a project in whose planning building companies played a key role, performing feasibility studies and carrying out social housing projects.

A looming attrition rate, a steady increase in the number of women in administration, and a lack of Canadian research all provided the rationale for this study. The problem in this study was to investigate the needs and challenges of new female administrators and to examine the role that mentors play in addressing these issues. This study also explored the perceived benefits of having a mentor. This study examined the inductive year of 33 female administrators from 3 Ontario school boards. It was a qualitative and quantitative design, using questionnaire and interview data. It was found that the majority felt that they struggled with biases and expectations that were gender specific. The challenges that were perceived to be most prevalent were categorized into 4 thematic areas: Maintaining Balance, Feeling Pressured, The Perceptions of Others, and Being Challenged by Others. Regarding the benefits of mentoring, the participants perceived mentoring to be most beneficial in terms of professional growth, followed by learning how to run a school, and then career advancement. The significance of this study was threefold: it had theoretical implications as well as implications for practice and future research. Suggestions included: facilitating longitudinal relationships, having the board become more actively involved in facilitating the relationship, and implementing an internship program. This study attempted to extend the current literature by theorizing that a mentorship is cyclical in nature. Future research could include program design and implementation, as well as providing consistent and accessible mentoring opportunities for all.

This is a study of the implementation and impact of formative assessment strategies on the motivation and self-efficacy of secondary school mathematics students. An explanatory sequential mixed methods design was implemented where quantitative and qualitative data were collected and analyzed sequentially in 2 different phases. The first phase involved quantitative data from student questionnaires and the second phase involved qualitative data from individual student and teacher interviews. The findings of the study suggest that formative assessment is implemented in practice in diverse ways and is a process where the strategies are interconnected. Teachers experience difficulty in incorporating peer and self-assessment and perceive a need for exemplars. Key factors described as influencing implementation include teaching philosophies, interpretation of ministry documents, teachers’ experiences, leadership in administration and department, teacher collaboration, misconceptions of teachers, and student understanding of formative assessment. Findings suggest that overall, formative assessment positively impacts student motivation and self-efficacy, because feedback is provided which offers encouragement and recognition by highlighting the progress that has been made and what steps need to be taken to improve. However, students are impacted differently with some considerations including how students perceive mistakes and if they fear judgement. Additionally, the impact of formative assessment is influenced by the connection between self-efficacy and motivation, namely how well a student is doing is a source of both concepts.

Partout dans le monde, la formation en gestion des cadres infirmiers est devenue une nécessité. Afin de doter la profession infirmière de chefs de file capables de faire face aux changements, plusieurs pays ont conçu des programmes de formation de deuxième cycle en administration des services de soins infirmiers. Plusieurs programmes sont développés en ce sens dont peu ou prou sont évalués. Plusieurs auteurs croient que l’évaluation de ces programmes permettrait de mieux apprécier l’atteinte des objectifs pédagogiques et ultérieurement, d’améliorer la formation et les pratiques de gestion. Afin de contribuer à ce projet, la présente étude vise à évaluer le programme de formation Administration et enseignement de l'École nationale de développement sanitaire et social (ENDSS) du Sénégal. Cette évaluation prend en compte les points de vue des diplômés, des enseignants, des concepteurs du programme et des directions d’hôpitaux. Il s’agit d’une recherche évaluative de type rétrospectif. Une enquête par questionnaire et par entrevue a été réalisée auprès des diplômés, des enseignants, des concepteurs de programme et des directions d'hôpitaux. Également, des bases de données sur les étudiants finissants entre les années 2003 et 2009 ont été consultées pour recueillir les notes obtenues à l’examen de certification, les effectifs d’admission et d’inscrits et les renseignements sociodémographiques. Cette évaluation porte sur les principales composantes du programme de formation Administration et enseignement de l’ENDSS qui sont examinées à l’aide du modèle systémique d’évaluation de programme éducatif de Stufflebeam (1981) communément appelé modèle CIPP. Les résultats de l’analyse s’appuient sur 70 questionnaires et 37 entrevues. En vue de répondre aux questions de recherche, les données recueillies sont analysées et interprétées à partir d’indicateurs et selon des critères définis. La démarche d’analyse des résultats s’est effectuée en deux étapes. D'abord, les données des questionnaires et des entrevues fournies par les diplômés, les enseignants et les concepteurs ont été analysées ainsi que celles du dossier scolaire des étudiants inscrits au programme. Ensuite, l’examen a porté sur les données des questionnaires recueillies auprès des directions d’hôpitaux. Les avis de tous les répondants sur les améliorations à apporter au programme sont analysés pour rendre compte de leurs recommandations à ce sujet. Tout au long de l’analyse des résultats, les données des questionnaires et des entrevues sont présentées à la fois. Le jugement sur l’efficacité du programme de formation Administration et enseignement de l’ENDSS est établi par l’analyse d’indicateurs reliés aux quatre types d'évaluation proposés par Stufflebeam (1981). Les diplômés et les concepteurs jugent que la formation est adéquate pour l’exercice de la fonction de chef de services des soins infirmiers. Par contre, les enseignants sont très partagés sur cette question, puisque près de la moitié d'entre eux expriment un avis favorable tandis que les autres ont une opinion contraire. Les diplômés apparaissent très partagés par rapport à la cohérence apparente entre les objectifs de stage d’administration hospitalière et les compétences à développer, alors que les enseignants et les concepteurs expriment plus largement une opinion négative. La cohérence entre les notions du module de Management des services de santé et l’exercice de la fonction de chef de services des soins infirmiers est jugée plus forte en ce qui concerne plusieurs notions, par les diplômés, les enseignants et les concepteurs. La majorité des diplômés et des enseignants jugent que la supervision offerte aux étudiants est insatisfaisante. Les diplômés et les enseignants croient que la disponibilité des ressources didactiques à la bibliothèque ne facilite pas les apprentissages. Tandis que les concepteurs, en proportion plus élevée, expriment le même jugement. Les diplômés et les enseignants sont divisés sur la question de l’équilibre entre le nombre d’heures alloué à la théorie et à la formation pratique. Tandis que la majorité des concepteurs croient qu’il n’y a pas d'équilibre entre le nombre d'heures consacré aux cours et à la formation pratique. Les diplômés et les enseignants expriment une opinion très positive sur la qualité des rapports sociaux des étudiants entre eux, avec les enseignants et le personnel administratif. Tandis que les concepteurs ont une opinion plutôt négative à ce sujet. Les diplômés, de même que les enseignants jugent en majorité, que la supervision des stages offerte aux étudiants est insatisfaisante. Ces opinions convergent avec celles de la grande majorité des concepteurs. Selon les diplômés, le travail de groupe, de même que les études de cas sont les méthodes les plus pertinentes dans la formation de chef de service des soins infirmiers. Pour les enseignants et les concepteurs c'est le travail de groupe qui est la méthode la plus pertinente pour faire acquérir des connaissances. Parmi l’ensemble des inscrits, incluant les étudiants sénégalais et étrangers, 104 d’entre eux ont obtenu un diplôme. Subséquemment, seuls 2 étudiants sur les 106 inscrits au programme ont échoué. Les enseignants, les diplômés et les concepteurs sont tous satisfaits du programme de formation Administration et enseignement de l’ENDSS. Les directions d'hôpitaux sont partagées quant à elles, à l’idée selon laquelle les étudiants sont préparés aux fonctions de chef de service des soins infirmiers. En plus, selon la grande majorité des directions le programme de formation ne répond pas à la stratégie de la réforme hospitalière. Elles affirment à l’unanimité qu’elles n’ont pas été associées au processus d’élaboration du programme et sont d’accord que, de manière générale, la formation reçue par le chef de services des soins infirmiers est de qualité. Unanimement, les directions d’hôpitaux jugent que des notions devraient être ajoutées ou améliorées au programme de formation Administration et enseignement de l’ENDSS. L’interprétation des données révèle la complexité qui entoure l’importance de l’équilibre du volume horaire à consacrer à la théorie et à la pratique dans un programme de formation. En plus, les résultats sur les connaissances à acquérir par les diplômés durant la formation montrent un besoin élevé de perfectionnement voire d’enseignement plus poussé du contenu des notions Système d’information sanitaire et Introduction à l’informatique, Introduction au management; Élaboration de programme et Administration hospitalière. Un choix approprié de terrain où l’étudiant pourrait réaliser les objectifs d’apprentissage, une sélection de la période adéquate pour le stage, de même qu’un encadrement signifiant, seraient le gage d’un stage profitable pour l’étudiant. Nous estimons que cette recherche a ouvert des pistes à des perspectives de recherches reliées au développement professionnel des futurs chefs de service des soins infirmiers.
