Há poucos dados sistematizados sobre eventos adversos da vacina contra influenza no Brasil. Este trabalho visou identificar estes eventos em população acima de 60 anos que compareceu à Campanha Nacional de Vacinação do Idoso, em Distrito de Campinas, SP, em 2000. Foi realizada entrevista para relato de sintomas gerais e locais, com nexo temporal após a aplicação do imunobiológico, em amostra aleatória sistemática da população (n=206). Registraram-se 20,38% (IC 14,87-25,88) dos indivíduos com um ou mais sintomas, sendo a dor no local da vacina, a mais freqüente 12,6% (IC 8,09-17,15). Ajustou-se um modelo de regressão logística múltipla, tendo como variável dependente, a ocorrência de pelo menos um evento adverso. A variável independente que se mostrou associada às reações adversas foi o sexo (feminino) (OR=5,89 e IC 2,08-16,68). Os achados deste estudo reafirmam a pequena reatogenicidade da vacina contra a influenza.

Este estudo objetivou descrever o perfil dos idosos e avaliar informações que eles possuem sobre a vacinação contra a influenza. A coleta de dados foi realizada através de questionário, aplicado a 162 idosos de um Centro de Saúde Escola do interior de São Paulo, no período de abril a junho de 2006. Através da análise estatística descritiva, verificou-se que a maioria dos idosos tinha idade entre 70-79 anos e prevaleceu o sexo feminino (66%). em relação à vacinação, 83,8% dos entrevistados receberam a vacina e referiram não apresentar reação pós-vacinal. Os idosos obtiveram informação da vacinação por meio de rádio e televisão, e mencionaram que sua importância está relacionada com a prevenção. Concluiu-se que os idosos possuíam informações sobre a vacinação, porém ainda existe a necessidade de enfermeiros e outros profissionais da área, se responsabilizarem por uma orientação mais clara sobre a importância da vacinação contra influenza e suas complicações.

Objective. To investigate the epidemiologic profile of elderly persons who do or do not participate in influenza vaccination campaigns and to identify the variables that bear an influence on participation. Method. A cross-sectional population-based study was performed using data on individuals aged 60 years or older who were living in the municipalities of São Paulo, Itapecerica da Serra, Embu, Taboão da Serra, Campinas and Botucatu, Brazil, in 2001 and 2002. A stratified random sample of 1 908 elderly individuals was selected by means of two-stage cluster sampling. Exploratory data analysis was performed, including bivariate analysis and multiple logistic regression. Results. Sixty-six percent of the elderly subjects reported having received vaccination against influenza. After adjustment, the following factors were found to be associated with having received vaccination, based on self-report: age (OR = 1.47; 95% CI = 1.09 to 1.99), self-reported hypertension (OR = 1.39; 95% CI = 1.03 to 1.87) and educational level (OR = 0.64; 95% CI = 0.41 to 0.98). The highest number of vaccinated individuals was observed in the group = 70 years of age and in the hypertension group. Individuals with 9 or more years of schooling reported less adherence to influenza vaccination. Conclusions. The results suggest the need for campaigns to make information on the benefits of influenza vaccination more easily accessible to the elderly and health professionals.

Influenza exacts a heavy burden on the elderly, a segment of the population that is estimated to experience rapid growth in the near future. In the past decade most developed and several developing countries have recommended influenza vaccination for those > 65 years of age. The World Health Organization (WHO) set a goal of 75% influenza vaccination coverage among the elderly by 2010, but it was not achieved. In 2011, the Technical Advisory Group at the Pan American Health Organization, Regional Office of WHO for the Americas, reiterated the influenza vaccine recommendation for older adults. Relatively little information has been compiled on the immunological aspect of aging or on reducing its impact, information particularly relevant for clinicians and gerontologist with firsthand experience confronting its effects. To fill this data gap, in 2012 the Americas Health Foundation (Washington, D.C., United States) and the nonprofit, Fighting Infectious Diseases in Emerging Countries (Miami, Florida, United States), convened a panel of Latin American clinicians and gerontologists with expertise in influenza to discuss key issues and develop a consensus statement. The major recommendations were to improve influenza surveillance throughout Latin America so that its impact can be quantified; and to conduct laboratory confirmation of influenza for all patients who have flu-like symptoms and are frail, immunosuppressed, have comorbidities, are respiratory compromised, or have been admitted to a hospital. The panel also noted that: since evidence for antivirals in the elderly is unclear, their use should be handled on a case-by-case basis; despite decreased immunological response, influenza vaccination in older adults is still crucial; indirect immunization strategies should be encouraged; and traditional infection control measures are essential in long-term care facilities.

Although the natural reservoirs of the avian influenza (AI) virus have been extensively studied in many countries, there is a clear lack of information on this subject in South America, particularly in Brazil. The objective of this study was to conduct a serological survey for H5, H7 and H9 antibodies to AI-subtype viruses in wild birds in the state of São Paulo, Brazil. Serum samples were tested using the hemagglutination-inhibition assay. Out of the 31 wild birds sampled between January and December of 2006, seven (22.58%), were seropositive for H5, H7 and H9; four (12.90%) were seropositive for H5 and H7; 13 (41.94%), were seropositive only for H7; three (9.7%), were seropositive only for H9; and four (12.90%) were negative for all three hemagglutinin subtypes. These results indicate that AI viruses belonging to H5, H7 and H9 subtypes circulate among wild birds in the state of São Paulo in the form of either concurrent or consecutive infections. This study contributes to the knowledge of AI epidemiology in Brazil, and stresses the need of further detailed and long-term epidemiological and ecological investigation to determine the current status of this virus.

Vários estudos comparativos entre os sexos masculino e feminino comprovaram o fato de que o primeiro é mais vulnerável às doenças, sobretudo às enfermidades graves e crônicas e que morre mais precocemente. A despeito da maior vulnerabilidade e das altas taxas de morbimortalidade, os homens não buscam, como as mulheres, os serviços de atenção básica. O presente estudo buscou compreender como os homens freqüentadores de uma Unidade Básica de Saúde (UBS) se comportam em relação aos cuidados com a sua saúde. Foi realizado em um Centro de Saúde Escola (CSE), Unidade Auxiliar de uma Universidade de uma cidade do interior do Estado de São Paulo, inserido na rede Municipal de Saúde do município. Trata-se de uma pesquisa descritiva, qualitativa e quantitativa. A organização dos dados qualitativos foi baseada na Análise de Discurso do Sujeito Coletivo (DSC), sendo que a obtenção dos discursos ocorreu através de uma entrevista semi-estruturada. Na parte qualitativa, a constituição da amostra foi por conveniência e foram selecionados 15 sujeitos do gênero masculino, maiores de 18 anos, que compareceram na UBS para um atendimento eventual ou para um atendimento pré-agendado. Na parte quantitativa, a constituição da amostra foi ao acaso, sendo sorteados 459 prontuários de usuários do sexo masculino, com 18 anos ou mais, que foram atendidos no período de Janeiro a Dezembro de 2009 para serem analisados através da estatística descritiva. Além disso, foram utilizadas as planilhas referentes às campanhas de vacinação contra a Gripe Pandêmica (H1N1) e Gripe Sazonal (Influenza) para a obtenção do número dos usuários do sexo masculino, de 20 anos ou mais, que participaram dessas campanhas no período de Janeiro a Julho de 2010. EQUINE influenza A virus (EIV) is a highly infectious respiratory pathogen of horses (Hannant and Mumford 1996, Palese and Shaw 2007). The illness is characterized by an abrupt onset of fever, depression, coughing and nasal discharge, and is often complicated by secondary bacterial infections that can lead to pneumonia and death. Two subtypes of EIV, H3N8 and H7N7, have been isolated. The H7N7 subtype was first isolated from a horse in Czechoslovakia in 1956 (Prague/56), and the H3N8 subtype was first isolated from a horse in Miami in 1963 (Sovinova and others 1958, Waddell and others 1963). The last confirmed outbreak of H7N7 occurred in 1979, and this subtype is now considered to be either extinct or circulating at low levels in a few geographical areas (Ismail and others 1990, Webster 1993, Singh 1994, Madic and others 1996, van Maanen and Cullinane 2002). The H3N8 subtype is a common cause of disease in horses worldwide, particularly in areas where vaccination is not routinely performed (Paillot and others 2006).

INFLUENZA A virus (IAV) (family Orthomyxoviridae) is a highly infectious respiratory pathogen of birds and mammals, including human beings and horses (Palese and Shaw 2007). The virus is classified into different subtypes based on the antigenic properties of the haemagglutinin (HA) and neuraminidase (NA) proteins. Sixteen HA subtypes (H1 to H16) and nine NA subtypes (N1 to N9) have been identified (Fouchier and others 2005). Two subtypes, H3N8 and H7N7, have been isolated from horses. The H7N7 subtype was first isolated from a horse in Czechoslovakia in 1956 (Prague/56) (Sovinova and others 1958), and the H3N8 subtype was first isolated from a horse in Miami, USA, in 1963 (Waddell and others 1963). The H7N7 subtype has not been isolated from horses for three decades and is presumed to be extinct (Webster 1993). The H3N8 subtype is currently a common cause of disease in horses worldwide. In horses, influenza is characterized by an abrupt onset of pyrexia, depression, coughing and nasal discharge, and is often complicated by secondary bacteria infections that can lead to pneumonia and death (Hannant and Mumford 1996). Although H3N8 is a major cause of morbidity in horses throughout the world, information on the seroprevalence of IAV in horses and other domestic animals in Mexico is limited.

Risks of the introduction of highly pathogenic avian influenza (HPAI) H5N1 through migratory birds to the main wintering site for wild birds in southern Brazil and its consequences were assessed. Likelihoods were estimated by a qualitative scale ranging from negligible to high. Northern migrants that breed in Alaska and regularly migrate to South America (primary Charadriiformes) can have contact with birds from affected areas in Asia. The likelihood of the introduction of HPAI H5N1 through migratory birds was found to be very low as it is a probability conditioned to successful transmission in breeding areas and the probabilities of an infected bird migrating and shedding the virus as far as southern Brazil. The probability of wild species becoming exposed to H5N1-infected birds is high as they nest with northern migrants from Alaska, whereas for backyard poultry it is moderate to high depending on proximity to wetlands and the presence of species that could increase the likelihood of contact with wild birds such as domestic duck. The magnitude of the biological and economic consequences of successful transmission to poultry or wild birds would be low to severe depending on the probability of the occurrence of outbreak scenarios described. As a result, the risk estimate is greater than negligible, and HPAI H5N1 prevention strategy in the region should always be carefully considered by the veterinary services in Brazil.

Introduction: The flu, a condition that can affect the elderly by increasing the risk of serious complications can be prevented through vaccination. Estimate the prevalence of signs and symptoms suggestive of influenza in a group of elderly either vaccinated or unvaccinated against influenza was the objective this study. Methods: This is a cross-sectional study performed in a Brazilian City. A structured questionnaire was employed to identify the presence of signs and symptoms of influenza in individuals aged 60 years or over. For analysis of associations between variables the prevalence ratio (PR) and its 95% confidence interval (95% CI) were used. Results: One hundred ninety-six participants were interviewed, of whom 57.7% were female. The average age was 69.7 years. About 25% of the vaccinated and 20% of the unvaccinated in 2009, and 25% of the vaccinated and 22.5% of the unvaccinated in 2010 reported having the flu. Among the vaccinated and unvaccinated in 2009 and 2010, there was no verified association between vaccination and influenza (PR=1.24; [95% CI: 0.63-2.43] and PR=1.11; [95% CI: 0.59-2.09], respectively). Conclusions: This study suggests that, among the elderly selected, the vaccination coverage for influenza is below the ideal, even with projection of the best indices for 2011 ((similar to)84%). The data on vaccination and disease protection needs further research; however, the results point to the need for measures to better clarify to this population about the disease, its complications and the benefits of vaccination, in addition to combatting the stigma related to low adherence.
