This is an experience report on clinical pharmacy in New York, United States of America, in a teaching hospital, describing the results of drug therapy monitoring in critically ill patients, as well as interventions to solve or prevent identified drug therapy problems. The cross-sectional study was conducted by the clinical staff at the Surgical Intensive Care Unit during August 20th to 24th, 2012. Blood counts, serum levels of certain antibiotics, microbiological cultures and their antibiotic susceptibility, possible drug interactions, dosage of each drug prescribed and the compatibility between the route of administration and pharmaceutical form were assessed daily through review of electronic medical records. Twenty seven patients were followed up and 16 drug therapy problems were identified: Unnecessary drug therapy (seven), adverse drug reaction (four), needs additional drug therapy (two), noncompliance (two) and dosage too low (one). After evaluation, the drug therapy problems and their pharmaceutical interventions were reported to clinical pharmaceutical responsible for the Surgical ICU, as well as the multidisciplinary team. Further, the clinical outcomes were monitored and interventions were classified as to its acceptance. Data demonstrate that clinical pharmacists can contribute to the security and proper use of medications, as the trigger tools for intensive monitoring helps in early detection of drug therapy problems and patient safety.

Pharmacological treatments currently used in ethanol addiction are inefficient, requiring new drugs for this purpose. The pro-drug N-Acetylcysteine (N-Ac) has shown efficacy in the treatment of addiction to cocaine and nicotine in preclinical research and clinical pilot studies. When administered, N-Ac is subsequently converted to cysteine, and cystine, which has an action on the central nervous system. However, there are few data about the possible application of N-Ac in the ethanol addiction. Behavioral sensitization is the gradual increase of the psychostimulant effects induced by repeated administration of drugs of abuse, including ethanol, and its development has been linked to important neuroadaptations in addiction. These neuroadaptations occur in neural circuits that mediate the reinforcing properties of these drugs and may involve several proteins. The ΔFosB protein accumulates in neurons after repeated activation and mediates long lasting changes in response to drugs of abuse. These changes are manifested mainly in the nucleus accumbens (Acb) and prefrontal cortex (PFC) neurons. The aim of the study was to evaluate the effects of N-Ac treatment in the development of ethanol behavioral sensitization and in alterations in the ΔFosB protein in mice. Swiss male mice were exposed to a standardized behavioral sensitization protocol to the experimental conditions of the laboratory and treated with N-Ac. At the end of behavioral sensitization procedure, animals were euthanized and their brains removed for ΔFosB quantification by Western blotting. Two experiments of behavioral sensitization were performed to the standardization of the protocol. The first, although effective in demonstrating the development of behavioral sensitization, was not effective in allowing the evaluation of the expression of the behavioral sensitization. This study compared acid-base and biochemical changes and quality of recovery in male cats with experimentally induced urethral obstruction and anesthetized with either propofol or a combination of ketamine and diazepam for urethral catheterization. Ten male cats with urethral obstruction were enrolled for urethral catheterization and anesthetized with either ketamine-diazepam (KD) or propofol (P). Lactated Ringer's solution was administered by intravenous (IV) beginning 15 min before and continuing for 48 h after relief of urethral obstruction. Quality of recovery and time to standing were evaluated. The urethral catheter was maintained to measure urinary output. Hematocrit (Hct), total plasma protein (TPP), albumin, total protein (TP), blood urea nitrogen (BUN), creatinine, pH, bicarbonate (HCO3-), chloride, base excess, anion gap, sodium, potassium, and partial pressure of carbon dioxide in mixed venous blood (pvCO(2)) were measured before urethral obstruction, at start of fluid therapy (0 h), and at subsequent intervals. The quality of recovery and time to standing were respectively 4 and 75 min in the KD group and 5 and 16 min in the P group. The blood urea nitrogen values were increased at 0, 2, and 8 h in both groups. Serum creatinine increased at 0 and 2 h in cats administered KD and at 0, 2, and 8 h in cats receiving P, although the values were above the reference range in both groups until 8 h. Acidosis occurred for up to 2 h in both groups. Acid-base and biochemical stabilization were similar in cats anesthetized with propofol or with ketamine-diazepam. Cats that received propofol recovered much faster, but the ketamine-diazepam combination was shown to be more advantageous when treating uncooperative cats as it can be administered by intramuscular (IM) injection.

Für die Entwicklung des zerebralen Kortex ist die radiale Migration von Neuronen von elementarer Bedeutung. Für diese radiale Migration sind extrazelluläre Signale, die mit den Neuronen interagieren und eine Umgestaltung des Zytoskeletts vermitteln, notwendig. Zu den extrazellulären Signalen gehört auch der Neurotransmitter GABA, der über Depolarisation der Neurone einen Ca2+-Einstrom vermittelt und dadurch die Modulation der Migration über Ca2+-abhängige Signalwege ermöglicht. Auch von Taurin ist bekannt, dass es die neuronale Migration beeinflusst. Frühere Studien zeigten, dass die Depolarisation von GABAA-Rezeptoren durch GABA zu einem Migrationsstop führt, wohingegen Picrotoxin-sensitive Rezeptoren die Migration von der Ventrikulären Zone in die Intermediäre Zone des pränatalen Kortex vermitteln. Obwohl zu den Picrotoxin-sensitiven Rezeptoren GABAA-, GABAC- und bestimmte Glyzinrezeptoren gehören, wurde die Rolle von GABAC- und Glyzinrezeptoren während der radialen Migration nie überprüft. Ziel dieser Dissertation war deshalb, den Einfluss von GABAC- und Glyzinrezeptoren auf die radiale Migration zu untersuchen. Unter Verwendung von Migrationsanalysen, Fluoreszenzmessungen, molekularbiologischen und histologischen Methoden wurde gezeigt, dass GABAC-Rezeptoren im unteren Bereiche des präfrontalen Kortex exprimiert werden, ihre Aktivierung durch GABA in der Intermediären Zone zu einer Depolarisation führt, dass GABAC-Rezeptoren die Migration fördern und dieser Effekt über den migrationsstoppenden Effekt der GABAA-Rezeptoren dominiert. Durch Aktivierung der Glyzinrezeptoren fördert Taurin die Migration.

L’estimation du stock de carbone contenu dans les forêts peut être effectuée de plusieurs manières. Les méthodes les plus connues sont destructives et nécessitent l’abattage d’un grand nombre représentatif d’arbres. Cette représentativité est difficilement atteinte dans les forêts tropicales, présentant une diversité d’espèces exceptionnelles, comme à Madagascar. Afin d’évaluer le niveau de dégradation des forêts, une étude d'images par télédétection est effectuée au moyen de l’analyse du signal radiométrique, combinée à un inventaire non destructif de biomasse. L’étude de la dynamique du paysage proposé est alors basée sur une correction atmosphérique d’une image SPOT 5, de l’année 2009, et sur une classification semi supervisée de l’occupation des sols, combinant une classification préliminaire non supervisée, un échantillonnage aléatoire des classes et une classification supervisée avec un maximum de vraisemblance. La validation est effectuée à l’aide de points indépendants relevés lors des inventaires de biomasse avec des valeurs du stock de carbone bien précises. La classification non supervisée a permis de ressortir deux classes de forêt dénommées « peu dégradée » et « dégradée ». La première désigne l’état climax (le stock de carbone a atteint une valeur qui varie peu) alors que la seconde est caractérisée par un taux de carbone plus faible que le niveau climax, mais qui peut être atteint sans perturbation. Cette première classification permet alors de répartir les placettes d’inventaire dans chaque classe. La méthode d’inventaire recueille à la fois des données dendrométriques classiques (espèce, densité, hauteur totale, hauteur fût, diamètre) et des échantillons représentatifs de branches et de feuilles sur un arbre. Ces différents paramètres avec la densité de bois permettent d’établir une équation allométrique de laquelle est estimée la biomasse totale d’un arbre et conséquemment de la formation forestière. Par la suite, la classification supervisée a été effectuée à partir d’échantillons aléatoires donnant la valeur de séparabilité des classes, de la classification finale. De plus, les valeurs de stocks de carbone à l’hectare, estimées de chaque placette, ont permis de valider cette classification et d’avoir une évaluation de la précision. La connaissance de ce niveau de dégradation issue de données satellitaires à haute résolution spatiale, combinées à des données d’inventaire, ouvre le champ du suivi interannuel du stock de carbone et subséquemment de la modélisation de la situation future du stock de carbone dans différents types de forêts.
