Chronic Obstructive Pulmonary Disease (COPD) phenotypes have become increasingly recognized as important for grouping patients with similar presentation and/or behavior, within the heterogeneity of the disease. The primary aim of identifying phenotypes is to provide patients with the best health care possible, tailoring the therapeutic approach to each patient. However, the identification of specific phenotypes has been hindered by several factors such as which specific attributes are relevant, which discriminant features should be used for assigning patients to specific phenotypes, and how relevant are they to the therapeutic approach, prognostic and clinical outcome. Moreover, the definition of phenotype is still not consensual. Comorbidities, risk factors, modifiable risk factors and disease severity, although not phenotypes, have impact across all COPD phenotypes. Although there are some identified phenotypes that are fairly consensual, many others have been proposed, but currently lack validation. The on-going debate about which instruments and tests should be used in the identification and definition of phenotypes has contributed to this uncertainty. In this paper, the authors review present knowledge regarding COPD phenotyping, discuss the role of phenotypes and comorbidities on the severity of COPD, propose new phenotypes and suggest a phenotype-based pharmacological therapeutic approach. The authors conclude that a patient-tailored treatment approach, which takes into account each patient's specific attributes and specificities, should be pursued.

La transplantation de cellules souches hématopoïétiques (CSH) constitue une avenue thérapeutique potentiellement curative pour plusieurs cancers hématologiques comme la leucémie. L’utilisation d’une thérapie immunosuppressive pour prévenir la maladie du greffon contre l’hôte (GvHD) est un déterminant majeur du succès de la greffe. Malgré tout, cette complication survient chez 25 à 50% des transplantés et est une cause majeure de mortalité. L’optimisation du régime d’immunosuppression est un facteur facilement modifiable qui pourrait améliorer le pronostic des patients. Particulièrement, les polymorphismes du génome du donneur ou du receveur dans les voies pharmacogénomiques des immunosuppresseurs pourraient influencer l’exposition et l’action de ces médicaments, de même que le pronostic du patient. Le profilage de 20 pharmacogènes prioritaires chez des paires de donneurs-receveurs en greffe de CSH a permis d’identifier des variations génétiques liées au risque de la GvHD aiguë. Principalement, le statut génétique du receveur pour les protéines ABCC1 et ABCC2, impliquées dans le transport du méthotrexate (MTX), ainsi que des cibles moléculaires de ce médicament (ATIC et MTHFR) ont été associées au risque de GvHD aiguë. Similairement, le NFATc1, codant pour une cible moléculaire de la cyclosporine, augmentait lui aussi le risque de la maladie. Les porteurs de deux génotypes à risque et plus étaient particulièrement prédisposés à développer cette complication. Par surcroît, le statut génétique du donneur influençait également le pronostic du receveur après la greffe. Entre autres, des allèles protecteurs ont été identifiés dans les voies liées au transport (SLC19A1) et à l’action du MTX (DHFR). Inversement, NFATc2 a été associé à une augmentation du risque de GvHD aiguë. Afin de mieux comprendre les associations observées entre ces marqueurs génétiques et le risque de GvHD aiguë, une étude prospective innovante est en cours chez des greffés de CSH. Cette étude permettra d’étudier comment la génétique du patient ou du donneur peut influencer la pharmacocinétique et la pharmacodynamie des immunosuppresseurs, de même que leurs liens avec la GvHD aiguë. Ces paramètres sont quantifiés grâce à des approches analytiques que nous avons mises au point afin de répondre aux besoins spécifiques et uniques de cette étude. Les approches proposées dans cette thèse sont complémentaires aux méthodes classiques de suivi des immunosuppresseurs et pourraient aider à optimiser la pharmacothérapie du patient. Une meilleure identification des patients à haut risque de GvHD aiguë avant la greffe, basée sur des marqueurs pharmacogénomiques identitaires, pourrait guider le choix de la prophylaxie immunosuppressive, et ainsi améliorer l’issue clinique de la greffe.

When teaching students with visual impairments educators generally rely on tactile tools to depict visual mathematical topics. Tactile media, such as embossed paper and simple manipulable materials, are typically used to convey graphical information. Although these tools are easy to use and relatively inexpensive, they are solely tactile and are not modifiable. Dynamic and interactive technologies such as pin matrices and haptic pens are also commercially available, but tend to be more expensive and less intuitive. This study aims to bridge the gap between easy-to-use tactile tools and dynamic, interactive technologies in order to facilitate the haptic learning of mathematical concepts. We developed an haptic assistive device using a Tanvas electrostatic touchscreen that provides the user with multimodal (haptic, auditory, and visual) output. Three methodological steps comprise this research: 1) a systematic literature review of the state of the art in the design and testing of tactile and haptic assistive devices, 2) a user-centered system design, and 3) testing of the system’s effectiveness via a usability study. The electrostatic touchscreen exhibits promise as an assistive device for displaying visual mathematical elements via the haptic modality.

Dans le cadre du présent travail, nous examinons les présentations de sensibilisation effectuées à l’ASTROLab, par l’entremise d’une enquête terrain. Nous étudions ces présentations sous divers angles qui s’entrecroisent : les stratégies de communication existantes (tant de vulgarisation que de persuasion ou de communication environnementale), les processus de l’impact persuasif et de la modification comportementale et les caractéristiques du comportement modifiable. Les questions auxquelles nous tentons de répondre avec cette étude concernent les stratégies de vulgarisation scientifique utilisées à l’ASTROLab contribuant efficacement à la modification comportementale ainsi que la corrélation entre la promotion de l’information et l’action. L’observation des groupes ainsi que deux sondages, en deux temps, ont permis de ressortir des résultats qualitatifs et quantitatifs. Nous avons constaté que la principale stratégie de vulgarisation scientifique efficace pour la modification comportementale est celle qui repose sur les caractéristiques du comportement modifiable. L’exemplification semble aussi avoir porté fruit, mais de façon moins évidente. Le visuel et la clarté des termes ainsi que la crédibilité et l’humour ont participé à la compréhension et à la persuasion, tout comme la stratégie de l’appel à l’émotion, qui a aussi contribué à l’attention. Bien sûr, la persuasion, et par ricochet la rétention de l’information, est en grande partie responsable de la modification comportementale. La rétention a été consolidée grâce au visuel, à l’exemplarité et au récit, ainsi qu’un peu grâce à l’appel à l’émotion et à la comparaison. L’humour y a également joué un rôle, en aidant à conserver l’attention. Nos résultats démontrent que seulement 32 % des participantes et participants en sont à l’étape de l’action et qu’environ 90 % des gens étudiés affirment avoir transmis l’information acquise. Ce constat porte à croire que l’étape de la promotion de l’information n’est pas nécessairement précédée d’actions tangibles, mais minimalement d’intention d’action. À partir de l’étude menée, nous formulons cinq recommandations à l’intention des vulgarisateurs afin de pallier les facteurs limitants. Un des principaux facteurs limitants à l’action a été l’impuissance des participantes et participants. Nous constatons aussi que, dans le processus de l’impact persuasif, l’étape de la rétention est celle qui a le plus posé problème. Nos recherches théoriques n’étaient pas précisément tournées vers les stratégies de rétention de l’information, à part pour les méthodes comme le visuel, l’expérience ou l’humour. Il serait pertinent de refaire une étude qui permettrait d’approfondir les assises théoriques reliées à la mémorisation d’information, afin de diriger autrement la méthodologie ou l’analyse des résultats, et d’en arriver à des recommandations utiles à ce sujet.

In health and epidemiological research, the Healthy Lifestyle (HLS) is often invoked as an explanation for inconsistent effects. Modifiable components of the HLS are advocated as a panacea for the most common threats to public health. Biases resulting from the HLS are theorized to result from covariance among its components. This covariance has not yet been formally modeled. Furthermore, no mechanism has been proposed to explain this covariance among these factors. Using three large nationally representative samples, I evaluated the HLS as a latent variable. Using structural equation modeling (SEM) I evaluated the degree to which the shared variance of HLS components is accounted for by personality traits, and tested the HLS as a mediator of the personality health relationship. Across all three samples, the HLS fits well as a latent variable, is partially accounted for by personality traits, and mediates the effects of personality traits on health. In all cases personality traits have direct effects on health independent of the HLS. These results suggest that the utility of personality traits as predictors of health exceeds that provided by commonly used lifestyle predictors.

Cardiovascular disease (CVD), particularly coronary heart disease (CHD) and stroke, are the leading cause of morbidity and mortality worldwide. The common forms of CVD have a complex aetiology in which interactions between multiple genetic and environmental factors play an important roles. The incidence rates of these diseases are increasing in developing countries as a result of the modification of lifestyles and increased prevalence of cardiovascular risk factors. Many independent cardiovascular risk factors could be modifiable, in contrast to the genetic risk factors. However, the associated risk of the genetic factors can be prevented if early identified, making genetic studies a priority in cardiovascular genetics research.

South Asians migrating to the Western world have a 3 to 5-fold higher risk of developing type 2 diabetes and double the risk of cardiovascular disease (CVD) than the background population of White European descent, without exhibiting a proportional higher prevalence of conventional cardiometabolic risk factors. Notably, women of South Asian descent are more likely to be diagnosed with type 2 diabetes as they grow older compared with South Asian men and, in addition, they have lost the cardio-protective effects of being females. Despite South Asian women in Western countries being a high risk group for developing future type 2 diabetes and CVD, they have been largely overlooked. The aims of this thesis were to compare lifestyle factors, body composition and cardiometabolic risk factors in healthy South Asian and European women who reside in Scotland, to examine whether ethnicity modifies the associations between modifiable environmental factors and cardiometabolic risks and to assess whether vascular reactivity is altered by ethnicity or other conventional and novel CVD risks. I conducted a cross-sectional study and recruited 92 women of South Asian and 87 women of White European descent without diagnosed diabetes or CVD. Women on hormone replacement therapy or hormonal contraceptives were excluded too. Age and body mass index (BMI) did not differ between the two ethnic groups. Physical activity was assessed and with self-reported questionnaires and objectively with the use of accelerometers. Cardiorespiratory fitness was quantified with the predicted maximal oxygen uptake (VO2 max) during a submaximal test (Chester step test). Body composition was assessed with skinfolds measured at seven body sites, five body circumferences, measurement of abdominal subcutaneous (SAT) and visceral adipose tissue (VAT) with the use of magnetic resonance imaging (MRI) and liver fat with the use MR spectroscopy. Dietary density was assessed with food frequency questionnaires. Vascular response was assessed by measuring the response to acetylcholine and sodium nitroprusside with the use of Laser Doppler Imaging with Iontophoresis (LDI-ION) and the response to shear stress with the use of Peripheral Arterial Tonometry (EndoPAT). The South Asian women exhibited a metabolic profile consistent with the insulin resistant phenotype, characterised by greater levels of fasting insulin, lower levels of high density lipoprotein (HDL) and higher levels of triglycerides (TG) compared with their European counterparts. In addition, the South Asians had greater levels of glycated haemoglobin (HbA1c) for any given level of fasting glucose. The South Asian women engaged less time weekly with moderate to vigorous physical activity (MVPA) and had lower levels of cardiorespiratory fitness for any given level of physical activity than the women of White descent. In addition, they accumulated more fat centrally for any given BMI. Notably, the South Asians had equivalent SAT with the European women but greater VAT and hepatic fat for any given BMI. Dietary density did not differ among the groups. Increasing central adiposity had the largest effect on insulin resistance in both ethic groups compared with physical inactivity or decreased cardiorespiratory fitness. Interestingly, ethnicity modified the association between central adiposity and insulin resistance index with a similar increase in central adiposity having a substantially larger effect on insulin resistance index in the South Asian women than in the Europeans. I subsequently examined whether ethnic specific thresholds are required for lifestyle modifications and demonstrated that South Asian women need to engage with MVPA for around 195 min.week-1 in order to equate their cardiometabolic risk with that of the Europeans exercising 150 min.week-1. In addition, lower thresholds of abdominal adiposity and BMI should apply for the South Asians compared with the conventional thresholds. Although the South Asians displayed an adverse metabolic profile, vascular reactivity measured with both methods did not differ among the two groups. An additional finding was that menopausal women with hot flushing of both ethnic groups showed a paradoxical vascular profile with enhanced skin perfusion (measured with LDI-ION) but decreased reactive hyperaemia index (measured with EndoPAT) compared with asymptomatic menopausal women. The latter association was independent of conventional CVD risk factors. To conclude, South Asian women without overt disease who live in Scotland display an adverse metabolic profile with steeper associations between lifestyle risk factors and adverse cardiometabolic outcomes compared with their White counterparts. Further work in exploring ethnic specific thresholds in lifestyle interventions or in disease diagnosis is warranted.

Este trabalho visou a prevenção das doenças cardiovasculares através da promoção de estilos de vida saudáveis em adultos em idade ativa. Objetivos: Avaliar o Risco Cardiovascular; identificar os conhecimentos sobre fatores de risco modificáveis; promover a criação de um grupo dinamizador de atividades promotoras de estilos de vida saudáveis. Metodologia: Planeamento em Saúde. Resultados: Dos 71 indivíduos avaliados, 33,8 % apresentaram evidência de risco cardiovascular, sendo que 6 apresentaram um Risco Cardiovascular muito alto, 4 risco alto e 14 risco moderado. Existiram diferenças significativas ao nível dos conhecimentos sobre doenças cardiovasculares e estilos de vida referidos pelos 2 grupos de funcionários avaliados. Conclusões: A adesão a estilos de vida saudáveis contribui para a redução dos fatores de risco modificáveis, cuja avaliação só é possível a médio e longo prazo. O investimento na criação de um grupo dinamizador no local de trabalho e a formalização de parcerias contribuiu para garantir a sustentabilidade do projeto; ABSTRACT: This work aimed the prevention of cardiovascular disease by promoting healthy lifestyles in adults of working age. Aims: Evaluate cardiovascular risk; identify the knowledge of modifiable risk factors; promote the creation of a dynamic group of activities that promote healthy lifestyles. Methodology: Planning in Health. Results: 71 workers evaluated, 33,8% had evidence of Cardiovascular Risk, and 6 showed a very high cardiovascular risk, 4 high risk and 14 moderate risk. There were significant differences in the knowledge of cardiovascular diseases and lifestyles reported by the two groups of evaluated employees. Conclusions: Following healthy lifestyles contributes to the reduction of modifiable risk factors, whose assessment is possible only in the medium and long term. The investment in creating a dynamic group in the workplace and the formalization of partnerships contributed to ensure the sustainability of the project.

Aim There is a high burden of oesophageal cancer in Malawi with dismal outcomes. It is not known whether environmental factors are associated with oesophageal cancer. Without knowing this critical information, prevention interventions are not possible. The purpose of this analysis was to explore environmental factors associated with oesophageal cancer in the Malawian context. Methods A hospital-based case-control study of the association between environmental risk factors and oesophageal cancer was conducted at Kamuzu Central Hospital in Lilongwe, Malawi and Queen Elizabeth Central Hospital in Blantyre, Malawi. Ninety-six persons with squamous cell carcinoma and 180 controls were enrolled and analyzed. These two groups were compared for a range of environmental risk factors, using logistic regression models. Unadjusted and adjusted odds ratios and 95% confidence intervals (CI) were calculated. Results Firewood cooking, cigarette smoking, and use of white maize flour all had strong associations with squamous cell carcinoma of the oesophagus, with adjusted odds ratios of 12.6 (95% CI: 4.2-37.7), 5.4 (95% CI: 2.0-15.2) and 6.6 (95% CI: 2.3-19.3), respectively. Conclusions Several modifiable risk factors were found to be strongly associated with squamous cell carcinoma. Research is needed to confirm these associations and then determine how to intervene on these modifiable risk factors in the Malawian context.

Transition to diets that are high in saturated fat and sugar has caused a global public health concern as the pattern of food consumption is a mayor modifiable risk factor for chronic non-communicable diseases Although agri food systems are intimately associated with this transition, agriculture and health sectors are largely disconnected in their priorities policy, and analysis with neither side considering the complex inter relation between agri trade patterns of food consumption health, and development We show the importance of connection of these perspectives through estimation of the effect of adopting a healthy diet on population health, agricultural production trade the economy and livelihoods, with a computable general equilibrium approach on the basis of case studies from the UK and Brazil we suggest that benefits of a healthy diet policy will vary substantially between different populations, not only because of population dietary intake but also because of agricultural production trade and other economic factors

Background: Over the last 10 years, Australia's spontaneous vaginal birth rate has decreased approximately 1% each year and the caesarean section rate has increased approximately 1% each year. This trend has serious implications for the health of women and babies. As midwives we are the caretakers of normal birth and therefore partly responsible for its decline and the solution to its decline. Although antenatal education is in a potentially powerful position to promote normal birth, a structured review conducted as part of this thesis found that it does not realise that potential. Currently framed in pathogenesis, antenatal education in particular and maternity services in general, are in need of reframing. The theory of salutogenesis may offer a new lens as it focuses on health rather than illness. Sense of coherence is the cornerstone of salutogenesis and is a predictive indicator of health. What is unclear is the role pregnant women's sense of coherence plays in their birthing outcomes. This study explored associations between pregnant women's sense of coherence, their pregnancy choices, their anticipated labour choices, their labour and birthing outcomes as well as factors associated with modification to sense of coherence from the antenatal to postnatal periods. Methods: After a comprehensive review of the literature, questionnaire development and psychometric tool testing and modification, a longitudinal survey was conducted where eligible women completed a questionnaire before the 30th week of pregnancy (Phase One) and approximately 8 weeks after birth (Phase Two). Eligible women were less than 30 weeks pregnant with a single fetus, could read and write in English and lived in the Australian Capital Territory in Australia. Phase One provided information on women's sense of coherence scores, Edinburgh Postnatal Depression Scale (EPDS) scores, Support Behaviour Inventory (SBI) scores, pregnancy choices including care proVider, planned place of birth, planned birth type and anticipated epidural use and demographics. Phase Two provided information on women's sense of coherence scores, EPDS scores and their labour and birthing outcomes. Findings: 1074 women completed Phase One representing a 61.3% response rate. 753 women completed Phase Two representing a 70.1% retention rate between phases. Compared to women with low sense of coherence, women with high sense of coherence were older, reported fewer pregnancy conditions such as diabetes or hypertension, were less likely to have depressive symptoms, were more likely to feel well supported, were less likely to experience a caesarean section and more likely to experience an assisted vaginal birth. Sense of coherence was not associated with women's pregnancy choices. Higher EPDS scores, lower sense of coherence and greater satisfaction with birth were associated with an increase in women's sense of coherence from the antenatal to the postnatal period. Decreased birth satisfaction and experiencing epidural anaesthesia in labour and assisted vaginal birth were associated with a decrease in sense of coherence from the antenatal to the postnatal period. Conclusion: Strong sense of coherence in pregnant women halved the likelihood of experiencing caesarean section compared to women with low sense of coherence. Sense of coherence is a modifiable predictor of women's childbearing health and was found to be raised by birth satisfaction and lowered by birth dissatisfaction and labour interventions. These important findings add to the limited body of knowledge about sense of coherence and childbearing.

The foundations for cardiovascular disease (CVD) in adults are laid in childhood and accelerated by the presence of comorbid conditions. Early detection of manifestations of cardiovascular pathology is an important clinical objective to identify those at risk for subsequent cardiovascular morbidity and events, and to initiate behavioral and medical interventions to reduce risk. Children were once considered to be at low risk, but with the growing health concerns related to lifestyle, cardiovascular screening may be needed earlier. Several noninvasive procedures are available to assess the cumulative effect of these exposures. These include carotid ultrasound, flow-mediated dilation, pulse wave velocity and measures left ventricular mass. This dissertation analyzes the comorbid conditions that increase cardiovascular risk in youth, namely obesity and low physical fitness, using carotid intima-media thickness to objectively detect early manifestations of cardiovascular pathology. Until recently researchers have not used surrogate markers of subclinical atherosclerosis to examine the role of a single bout of exercise. Utilizing the acute exercise model can be advantageous as it allows for an efficient manipulation of exercise variables and permits greater experimental control of confounding variables. It is possible that the effects of a bout of exercise can predict the effects of chronic exercise. We analyze the physiological factors pertinent to arterial stiffness using arterial distensibility and pulse wave velocity in the context of acute exercise in children and adults. In some instances, those who amend their trajectory by not maintaining risk factors into adulthood experience reductions in subclinical markers to levels associated with never having had the risk factor. Though avoidance of risk factors in youth is ideal, there is still a window for intervention where long-lasting cardiovascular effects might be avoided. In this dissertation we present preliminary findings linking modifiable youth risk factors to subclinical markers of CVD in adulthood.

Partindo do princípio que a cognição é modificável e alterável, surge a educação cognitiva. Este repensar da educação converge com a necessidade de uma educação para a diversidade e inclusão. Ao mesmo tempo surgem as novas tecnologias, cada vez mais presentes. Focando os processos de criatividade e resolução de problemas, construímos um programa e-learning de desenvolvimento cognitivo através da matemática (PEDCM) e estudamos os efeitos da implementação do mesmo. A metodologia utilizada foi a quasi-experimental, com pré e pós-teste e sem grupo de controlo. A amostra foi constituída por 1O alunos de uma turma do 8° ano da Escola EB 2,3 Conde de Vilalva, em Évora. Os instrumentos utilizados foram: o Teste de Auto-conceito de Susan Harter; Matrizes Standard de Raven; Bateria de Provas de Raciocínio; Teste de Pensamento Criativo de Torrance e o PEDCM. Este último foi implementado durante 10 sessões (com 1h30m cada). Os resultados obtidos demonstram efeitos positivos evidentes da implementação PEDCM, ao nível do desempenho cognitivo, desempenho criativo, no auto-conceito e principalmente no rendimento escolar. ABSTRACT: Starting from the principle that cognition is alterable and modifiable, rises the cognitive education. This rethinking of education converges with an education for diversity and inclusion. Parallelly the new technologies rise, more and more present in education. Focusing on the processes of creativity and problem resolution and we've built an e-learning program of cognitive development through mathematics (PEDCM) and studied the effects of its implementation. The used methodology was the almost experimental, with pre and post test without a group of control. The show was built by 1O students of a class of 8th grade of the school EB2,3 Conde de Vilalva in Évora. The instruments used were: the Test of SelfConcept by Susan Harter; Standard Registries by Raven; Set of Reasoning Tests; Test of Creative Thought from Torrance and the PEDCM, implemented for 10 sessions (of 1.30h each). The results attained show the positive effects evident from the implementation of the PEDCM, to the level of cognitive performance, creative performance, in the self concept and particularly in the school results.
Représentations identitaires chez un homme ayant purgé une longue peine : aspects stables et mobiles

Nombre de recherches portent sur la question de l’abandon de la délinquance et du processus de désistance qui la précède. Bien que les angles d’approche soient diversifiés, elles s’entendent pour dire que ce processus implique des changements sociaux autant que personnels. Ce mémoire s’est intéressé à la question des changements identitaires chez des individus qui avaient été condamnés à une longue peine d’emprisonnement et qui ont obtenu leur libération conditionnelle totale. Le principal objectif de ce mémoire est de comprendre en quoi les aspects permanents de l’identité et ceux qui sont modifiables sont à l’œuvre dans la trajectoire de changement d’un homme condamné à perpétuité pour meurtre et qui a obtenu sa libération conditionnelle totale. La méthode qualitative qu’est le récit de vie, et selon une perspective phénoménologique, a été utilisée afin d’atteindre les objectifs de cette recherche. Un homme condamné à une sentence de prison à vie, mais ayant obtenu sa libération totale a été rencontré en dix entretiens en profondeur d’une durée d’une heure à une heure et demie. Nous avons choisi de procéder à un nombre élevé d’entretiens de type semi-directifs afin de permettre l’approfondissement des propos de l’individu rencontré. Les résultats suggèrent que certaines représentations de l’identité restent stables dans le temps alors que d’autres se transforment au fil de la trajectoire de vie. En effet, de l’analyse du récit de vie de l’individu se dégagent deux représentations stables, qui marquent durablement l’identité de ce dernier dans sa trajectoire de vie, et quatre modifiables, qui se sont développées au cours de sa détention. Les résultats montrent aussi que les représentations stables semblent intervenir autant dans le processus criminogène que dans celui de la désistance.

Les maladies cardiovasculaires ont un impact considérable sur la vie des Canadiens, et de nombreux efforts ont permis d’identifier différents facteurs de risque associés à cette condition. L’hypertension artérielle représente un de ces facteurs modifiables les plus importants. Quoique l’hypertension est définie à l’aide de mesures de pression artérielle en périphérie, il devient de plus en plus apparent que la mesure de pression centrale et de ses composantes auraient des avantages au niveau de la prédiction de la survenue d’événements cardiovasculaires. Le présent mémoire vise à mieux caractériser deux déterminants de cette pression centrale, le traitement antihypertenseur à base de bêtabloqueurs et l’insuffisance rénale chronique précoce. En utilisant les données recueillies dans la banque de données populationnelle CARTaGENE, il a été possible à l’aide d’analyses statistiques par appariement basé sur le coefficient de propension de démontrer que l’utilisation d’agents antihypertensifs de type bêtabloqueurs était associée à un profil hémodynamique central défavorable. Ainsi, les individus recevant ces agents avaient une pression centrale et une amplification artérielle plus élevées que des individus du groupe contrôle apparié et ce, malgré une pression périphérique identique. Cet effet semblait être incomplètement expliqué par la réduction du rythme cardiaque associé à l’utilisation de bêtabloqueurs. Aussi, il a été démontré que l’insuffisance rénale chronique de stade 3 (débit de filtration glomérulaire estimé entre 30 et 60 mL/min/1.73m2) n’était pas associée à une élévation des paramètres hémodynamiques centraux, contrairement à ce qui avait déjà été décrit chez des individus avec insuffisance rénale chronique plus avancée. De plus, le niveau d’albuminurie ne serait également pas associé à un changement du profil central dans un sous-groupe de la cohorte CARTaGENE.
